[Naso-sinusal polyposis in children. Mid-term results of sinusal surgery].

Nasosinusal polyposis in the pediatric population is uncommon and its etiology is unclear. In this eleven-year retrospective study, we describe the etiologic features and evaluate the effectiveness of endoscopic sinus surgery in 49 children. Patients were divided into three groups according to whether nasosinusal polyposis was either isolated (n = 14), or associated with either asthma (n = 5) or cystic fibrosis (n = 30). An allergy was present in 10% of patients with isolated polyposis, 80% of patients with polyposis associated with asthma, and 19% of patients with polyposis associated with cy

[Diagnostic significance of phono-mechanographic medication of constrictive endocardial fibrosis (or endomyocardial fibrosis)].

The phonomecanographic investigation of 12 patients with FEC allow us to recognize diagnostics signs of great interest. 1. The ventricular aspect of the jugular veinous pulse involving a positive systolic wave and a diastolic aspect in "dip-plateau" is a chief indication of a right ventricular fibrosis. This indication is more significant of FEC when the nadir of the dip is far above the base line, when the dip-rising time is short, when the telediastolic ratio is high (more important than 0.40) when the electro-mechanical latency time is prolonged and when the isovolumetric relaxation time is

Efficacy of LGE-MRI-guided fibrosis ablation versus conventional catheter ablation of atrial fibrillation: The DECAAF II trial: Study design.

INTRODUCTION: Success rates of catheter ablation in persistent atrial fibrillation (AF) remain suboptimal. A better and more targeted ablation strategy is urgently needed to optimize outcomes of AF treatment. We sought to assess the safety and efficacy of targeting atrial fibrosis during ablation of persistent AF patients in improving procedural outcomes. METHODS: The DECAAF II trial (ClinicalTrials. gov identifier number NCT02529319) is a prospective, randomized, multicenter trial of patients with persistent AF. Patients with persistent AF undergoing a first-time ablation procedure were rand

Pulmonary Surfactant Nanoparticles for Lung-Targeted and Dose-Efficient Delivery.

Although pulmonary drug delivery enables lung-targeted therapy, its clinical efficacy is often limited by inflammation, heterogeneous deposition, and physiological barriers such as mucociliary clearance and phagocytic uptake. Here, we report a biomimetic platform based on pulmonary surfactant nanoparticles (PSNPs) that preserves native surfactant proteins and biophysical functionality, resulting in enhanced pulmonary delivery. Microfluidic synthesis yielded highly uniform PSNPs with improved encapsulation efficiency and colloidal stability, while markedly increasing surfactant protein retentio

Associations between leisure sedentary behaviors, physical activity, and chronic respiratory diseases: a Mendelian randomization study.

INTRODUCTION: Chronic respiratory diseases represent a significant global health burden, characterized by high incidence and mortality rates. However, the potential associations between physical activity, leisure sedentary behavior, and susceptibility to chronic respiratory diseases remain uncertain. MATERIAL AND METHODS: A two-sample Mendelian randomization approach was used in this investigation, using physical activity and leisure sedentary behavior as exposures, and common chronic respiratory diseases such as asthma, bronchiectasis, chronic obstructive pulmonary disease, idiopathic pulmon

Multimodal noninvasive prediction of pulmonary hypertension in IPF.

INTRODUCTION AND OBJECTIVE: Pulmonary hypertension (PH) complicating idiopathic pulmonary fibrosis (IPF) is challenging to diagnose given inaccuracy of transthoracic echocardiogram (TTE) measurements. However, it has significant prognostic implications and is therefore important to accurately identify. METHODS: We conducted a cross-sectional study of patients with IPF who underwent RHC as part of their evaluation. A variety of commonly available noninvasive variables were evaluated for their ability to predict pulmonary arterial pressure in a linear regression model, including the traditional

Cardiac uses of phosphodiesterase-5 inhibitors.

Phosphodiesterase-5 inhibitors (PDE5Is) improve erectile function by enhancing nitric oxide availability in the penis and its supplying vasculature, resulting in vasodilation and increased blood flow. PDE5Is might benefit cardiovascular diseases because phosphodiesterase-5 is also located elsewhere in the body, including the pulmonary and systemic vasculature and in hypertrophied myocardium. PDE5Is are approved for pulmonary arterial hypertension, given that they improved several hemodynamic and clinical parameters in large randomized trials. Initial evidence suggests that PDE5Is benefit patie

Unraveling Mycobacterium abscessus: Pathogenesis and host immune dynamics.

The worldwide trend shows a continuous increase in pulmonary infections caused by nontuberculous mycobacteria (NTM), with Mycobacterium abscessus (M. abscessus) emerging as a major concern as the most common rapidly growing opportunistic pathogen. It poses a substantial risk, causing severe respiratory, skin, and mucosal infections, especially among patients suffering from chronic pulmonary disease and cystic fibrosis (CF). The diverse virulence factors and complex resistance mechanisms of M. abscessus create formidable challenges in clinical management. As pivotal effectors in innate immunity

Zafirlukast ameliorates lipopolysaccharide and bleomycin-induced lung inflammation in mice.

Acute lung injury (ALI) is the result of damage to the capillary endothelia and the alveolar epithelial cell caused by various direct and indirect factors, leading to significant pulmonary interstitial and alveolar edema and acute hypoxic respiratory insufficiency. A subset of ALI cases progresses to irreversible pulmonary fibrosis, a condition with fatal implications. Zafirlukast is a leukotriene receptor antagonist licensed for asthma prevention and long-term treatment. This study demonstrated a significant improvement in lung tissue pathology and a reduction in inflammatory cell infiltratio

Transient Receptor Potential (TRP) Channels in Airway Toxicity and Disease: An Update.

Our respiratory system is exposed to toxicants and pathogens from both sides: the airways and the vasculature. While tracheal, bronchial and alveolar epithelial cells form a natural barrier in the airways, endothelial cells protect the lung from perfused toxic compounds, particulate matter and invading microorganism in the vascular system. Damages induce inflammation by our immune response and wound healing by (myo)fibroblast proliferation. Members of the transient receptor potential (TRP) superfamily of ion channel are expressed in many cells of the respiratory tract and serve multiple functi

[FEATURES OF THE COMPLICATED COURSE OF NON-HOSPITAL VIRAL COVID-19 PNEUMONIA].

The purpose of this work was to demonstrate clinical observations of the features of the complicated course of community-based viral COVID-19 pneumonia. Patients were referred for treatment with a complicated course of pneumonia from other medical institutions, where they were treated 2-3 months ago. The most serious complications are given - the formation of bullosa pulmonary emphysema ("disappearing lung syndrome"), which can occur at different periods of the disease. In some cases, the morphological study of the postoperative material with complications in the post covid period is character

Assisted mobilisation in critical patients with COVID-19.

The therapeutic value of early physiotherapeutic treatment in critical respiratory settings has already been clearly outlined in the last fifteen years by several authors. However, there is still a controversial perception of mobilisation by healthcare professions. In-bed cycling has attracted increasing attention having been demonstrated as a feasible and safe intervention in critical settings. Patients with respiratory diseases are typically prone to fatigue and exertional dyspnoea, as we observe in COVID-19 pandemic; in fact, these patients manifest respiratory and motor damage that can eve

Targeting Neprilysin (NEP) pathways: A potential new hope to defeat COVID-19 ghost.

COVID-19 is an ongoing viral pandemic disease that is caused by SARS-CoV2, inducing severe pneumonia in humans. However, several classes of repurposed drugs have been recommended, no specific vaccines or effective therapeutic interventions for COVID-19 are developed till now. Viral dependence on ACE-2, as entry receptors, drove the researchers into RAS impact on COVID-19 pathogenesis. Several evidences have pointed at Neprilysin (NEP) as one of pulmonary RAS components. Considering the protective effect of NEP against pulmonary inflammatory reactions and fibrosis, it is suggested to direct the

Sarcoidosis from Head to Toe: What the Radiologist Needs to Know.

Sarcoidosis is a multisystem granulomatous disorder characterized by development of noncaseating granulomas in various organs. Although the etiology of this condition is unclear, environmental and genetic factors may be substantial in its pathogenesis. Clinical features are often nonspecific, and imaging is essential to diagnosis. Abnormalities may be seen on chest radiographs in more than 90% of patients with thoracic sarcoidosis. Symmetric hilar and mediastinal adenopathy and pulmonary micronodules in a perilymphatic distribution are characteristic features of sarcoidosis. Irreversible pulmo

The Immune Mechanisms of Lung Parenchymal Damage in Tuberculosis and the Role of Host-Directed Therapy.

Impaired lung function is common in people with a history of tuberculosis. Host-directed therapy added to tuberculosis treatment may reduce lung damage and result in improved lung function. An understanding of the pathogenesis of pulmonary damage in TB is fundamental to successfully predicting which interventions could be beneficial. In this review, we describe the different features of TB immunopathology that lead to impaired lung function, namely cavities, bronchiectasis, and fibrosis. We discuss the immunological processes that cause lung damage, focusing on studies performed in humans, and

[Limitations of active therapeutic and palliative care in chronic respiratory disease].

The issue of intensive and palliative care in patients with chronic disease frequently arises. This review aims to describe the prognostic factors of chronic respiratory diseases in stable and in acute situations in order to improve the management of these complex situations. The various laws on patients' rights provide a legal framework and define the concept of unreasonable obstinacy. For patients with chronic obstructive pulmonary disease, the most robust decision factors are good knowledge of the respiratory disease, the comorbidities, the history of previous exacerbations and patient pref

Clinical and in vitro evidence for the antimicrobial therapy in Burkholderia cepacia complex infections.

Treatment of infections caused by Burkholderia cepacia complex (Bcc) in cystic fibrosis (CF) patients poses a complex problem. Bcc is multidrug-resistant due to innate and acquired mechanisms of resistance. As CF patients receive multiple courses of antibiotics, susceptibility patterns of strains from CF patients may differ from those noted in strains from non-CF patients. Thus, there was a need for assessing in vitro and clinical data to guide antimicrobial therapy in these patients. A systematic search of literature, followed by extraction and analysis of available information from human and

A rare case of fibrostenotic endobronchial tuberculosis of trachea.

Endobronchial tuberculosis (EBTB) is a sequelae of pulmonary tuberculosis (TB) that extends to the endobronchial or endotracheal wall causing inflammation, edema, ulceration, granulation or fibrosis of mucosa and submucosa. This case depicts a 20 year old foreign-born woman with a history of active pulmonary TB on anti-TB chemotherapy, who presented with worsening stridor, dyspnea, cough and weight loss. The disease state was diagnosed with multiple modalities including, spirometry, CT scan of the neck, and bronchoscopy. The biopsies of the tracheal web revealed fibrotic tissue without any gra

[Phenotypic and genotypic diagnosis of bone and miliary tuberculosis in an HIV+ patient in Bogotá, Colombia].

Tuberculosis is the single most frequent cause of death by an infectious agent worldwide. Diagnosis of extra-pulmonary tuberculosis is not always possible through conventional methods, due to the long time required for cultures and the paucibacillary nature of samples; hence the need of rapid molecular methods. HIV infection increases the risk of tuberculosis, and HIV/tuberculosis coinfection is associated with higher mortality. We describe the case of a 56-year old mestizo male patient suspected of having tuberculosis who consulted the San Ignacio Hospital in Bogotá with a two-month history o
