An evaluation of the neuroendocrine response to sleep in pediatric burn patients.

INTRODUCTION: Previous work demonstrated reduced stage 3+4 and rapid eye movement (REM) sleep following burn injury. This study evaluated the hormonal effects of drug intervention on measures of endocrine status. A secondary objective examined the relationship between hormones and sleep stage distribution. METHODS: Forty patients 3-18 years of age with a mean percent total body surface area burn of 50.1 +/- 2.9 were randomly assigned to zolpidem or haloperidol utilizing a blinded crossover design. Polysomnography was performed 6 nights, 3/week over 2 weeks. Each week's first night of monitori

Effects of sleep fragmentation on glucose metabolism in normal subjects.

BACKGROUND: Sleep disorders are increasingly associated with insulin resistance, glucose intolerance, and type 2 diabetes mellitus. Whether the metabolic toll imposed by sleep-related disorders is caused by poor-quality sleep or due to other confounding factors is not known. The objective of this study was to examine whether experimental sleep fragmentation across all sleep stages would alter glucose metabolism, adrenocortical function, and sympathovagal balance. METHODS: Sleep was experimentally fragmented across all stages in 11 healthy, normal volunteers for two nights using auditory and m

Levodopa use and sleep in patients with dementia with Lewy bodies.

Sleep disturbance and excessive daytime somnolence (EDS) are features of Parkinson's disease (PD) and dementia with Lewy bodies (DLB) that may be influenced by dopamine replacement therapy. The effect of levodopa on sleep and EDS in DLB is unknown and unclear in PD. The aim of this study is to determine if levodopa treatment alters sleep symptoms and EDS in DLB. Dopamine naïve patients with DLB (n = 15; mean mini mental state examination (MMSE) score 17.7(4.6)) and PD (n = 9; mean MMSE 25.5(2.2)) were assessed using the Epworth sleep scale, Parkinson's disease sleep scale, and the neuropsychia

Evaluation of a school-based intervention for adolescent sleep problems.

STUDY OBJECTIVES: The present study investigated the effectiveness of a school-based intervention in increasing sleep knowledge and improving adolescent sleep problems. DESIGN: A randomized, controlled trial using 2 groups (program class, classes-as-usual: [CAU]) assessed over 3 time points (pre-program, post-program, 6-week follow-up). PARTICIPANTS/SETTING: Eighty-one students (mean age = 15.6 +/- 0.6 y; 33% male) from 2 schools in South Australia. Schools provided one class to participate in the sleep intervention program (N = 41) and a second class to act as a control class (N = 40). INT

Allergic and nonallergic rhinitis: the threat for obstructive sleep apnea.

BACKGROUND: Although allergic rhinitis (AR) is accepted as a risk factor for obstructive sleep apnea syndrome (OSAS), the role of nonallergic rhinitis (NAR) is unknown. OBJECTIVE: To compare OSAS in patients with AR vs NAR. METHODS: We performed an observational study in 48 adults with AR and NAR that included a review of rhinitis and sleep symptoms, skin prick test results, self-administered questionnaire (Epworth Sleepiness Scale and 36-Item Short Form Health Survey) findings, and all-night polysomnography records. RESULTS: The most frequent sleep symptom was snoring. Patients with AR had

[Observation of sleep disorders in patients with kinking of carotid artery].

Carotid artery redundancy with kinking and coiling can be the cause for acute, ischaemic transitory or even permanent, brain damage and cerebrovascular symptoms. The cerebral ischaemia typically associated to kinking of carotid artery is positional and can be provoked by some (extreme) movenets of the head. The awake patient is coscious of those typical head positions, and capable to avoid them. Prolonged uncontrolled turning of the head during sleep has been reported to cause a stroke in sleep or unpleasant phenomena during sleep. The parasomnias and central sleep apnea syndrome caused by isc

Adenotonsillectomy for sleep-disordered breathing in children with syndromic craniosynostosis.

Syndromic craniosynostosis (SCS) is known to be associated with sleep-disordered breathing (SDB) in childhood. Problems can occur at various levels, but midface hypoplasia is a major factor in affected infants.Adenotonsillectomy (AT) has been proven to be efficacious as a first-line treatment of SDB in healthy children. However, its role in the treatment of this issue in those with SCS has not been evaluated. Owing to the multiple possible levels of obstruction in such individuals, AT may have no benefit. This study therefore aimed to evaluate the effectiveness of AT in this group of patients.

Madelung's disease as a rare cause of obstructive sleep apnea.

Madelung's disease, or multiple symmetric lipomatosis, is a rare disease, characterized by accumulation of unencapsulated fat, generally located symmetrically around the neck and shoulders. Here, we present the case of a patient with diffuse lipomatosis accompanied by obstructive sleep apnea due to cervical involvement and facial deformity, which made it necessary to use nasal pillows for ventilation. The patient was hospitalized with a diagnosis of pneumonia and required noninvasive ventilation due to severe hypercapnia. A brief review of the literature was made, and we describe and discuss t

Daytime sleepiness and changes of sleep architecture in patients with epilepsy.

OBJECTIVE: The relationship between epilepsy and sleep has been known for many years. Yet is still not well understood because of it's reciprocal and intrinsic influences. Epileptic manifestations during sleep may lead to fragmentation of sleep stages. On the other hand insomnia or other sleep disorders may cause sleep deprivation and increase number of epileptic seizures in patients with epilepsy. The study is designed to compare daytime sleepiness and architecture of sleep in patients with epilepsy and control subjects. We tried to evaluate factors that have influence on sleep architecture o

Evaluation of ApneaGraph in the diagnosis of sleep-related breathing disorders.

ApneaGraph relies on measuring pressure and airflow simultaneously at different levels in the pharynx identifying the segment of airway obstruction and providing baseline respiratory parameters. This study aims to evaluate ApneaGraph and correlate results with both sleep nasendoscopy and polysomnography. This was a prospective study of 49 patients with snoring and/or obstructive sleep apnoea. Thirty of the these patients underwent a PSG and an ApneaGraph study simultaneously in the Sleep Lab. Nineteen patients attended the day surgery unit and had a sleep nasendoscopy with a 10-min ApneaGraph

Obesity and the lung: 2. Obesity and sleep-disordered breathing.

As the prevalence of obesity increases in both the developed and the developing world, the respiratory consequences are often underappreciated. This review discusses the presentation, pathogenesis, diagnosis and management of the obstructive sleep apnoea, overlap and obesity hypoventilation syndromes. Patients with these conditions will commonly present to respiratory physicians, and recognition and effective treatment have important benefits in terms of patient quality of life and reduction in healthcare utilisation. Measures to curb the obesity epidemic are urgently required.

Is the aortic root dilated in obstructive sleep apnoea syndrome?

BACKGROUND: Obstructive sleep apnoea syndrome (OSAS) is associated with an increased risk of arterial hypertension (AH), coronary artery disease, atrial arrhythmias, heart failure, stroke and death. Whether OSAS influences aortic root size has not been fully investigated. The aim of our study was to investigate aortic root diameter and aortic stiffness in OSAS. METHODS: Using transthoracic Doppler echocardiography, we evaluated 76 patients with OSAS (mean age 52.7 +/- 9.5 years, 70 men [92%]) with no overt cardiovascular disease. The following parameters were measured offline: aortic diameter

The continuing problem of human African trypanosomiasis (sleeping sickness).

Human African trypanosomiasis, also known as sleeping sickness, is a neglected disease, and it continues to pose a major threat to 60 million people in 36 countries in sub-Saharan Africa. Transmitted by the bite of the tsetse fly, the disease is caused by protozoan parasites of the genus Trypanosoma and comes in two types: East African human African trypanosomiasis caused by Trypanosoma brucei rhodesiense and the West African form caused by Trypanosoma brucei gambiense. There is an early or hemolymphatic stage and a late or encephalitic stage, when the parasites cross the blood-brain barrier t

Is sleeping position related to asymmetry in bilateral Graves' ophthalmopathy?

BACKGROUND: In agreement with the systemic nature of Graves' disease, Graves' ophthalmopathy (GO) presents as a symmetric bilateral eye disease in the vast majority of patients. However, asymmetric involvement of both eyes is frequently observed. We hypothesized that sleeping position might be involved in asymmetric GO; when, for example, the preferred sleeping position is on the right side, retrobulbar pressure might be somewhat higher in the right than in the left orbit, resulting in more severe eye changes in the right eye. METHODS: A prospective study in 75 consecutive untreated patients

REM sleep behavior disorder in a patient with pontine stroke.

Rapid eye movement (REM) sleep behavior disorder (RBD) is frequently seen in patients with neurodegenerative diseases. It can also be seen in brainstem lesions involving the pons. However, RBD in patients with a pure pontine infarction has been rarely reported. A 68-year-old man had a two-month history of violent behavior during sleep. His nocturnal behavior was screaming, thrashing of arms, punching or kicking his spouse, and falling out of bed associated with nightmares of being attacked by what he described as, "terrible animals frightening me." No remarkable neurological signs were found.

Prazosin for the treatment of posttraumatic stress disorder sleep disturbances.

An estimated 70-87% of patients who suffer from posttraumatic stress disorder (PTSD) experience sleep disruption. These patients have distressing dreams or nightmares in which the traumatic event is reexperienced, and they also have difficulty in falling or staying asleep. Selective serotonin reuptake inhibitors are the treatment of choice for PTSD, but with the exception of fluvoxamine, they are often ineffective or only partially effective for sleep problems. Sedative-hypnotics may be helpful in the short term but are associated with tolerance and addiction potential. In the central nervous

Are sleep difficulties associated with migraine attributable to anxiety and depression?

OBJECTIVE: To examine whether sleep complaints reported by migraineurs can be attributed to comorbid anxiety and/or depression. BACKGROUND: A consistent association between migraine and sleep complaints has been reported in community and clinical studies. However, anxiety and depression are often comorbid with migraine. Thus, it may be possible that the increased prevalence of sleep problems in migraineurs is attributable to comorbid anxiety and depression. To our knowledge, no previous studies have demonstrated that the associations are not solely attributed to comorbid anxiety and depressio

Sleep duration and risk of ischemic stroke in postmenopausal women.

BACKGROUND AND PURPOSE: Many studies have shown a U-shape association between sleep duration and mortality, but epidemiological evidence linking cardiovascular diseases with habitual sleep patterns is limited and mixed. METHODS: We conducted a prospective study on 93 175 older women (aged 50 to 79 years) in the Women's Health Initiative Observational study cohort to examine the risk of ischemic stroke in relation to self-reported sleep duration. Cox models were used to investigate the putative associations, adjusting for multiple sociodemographic and lifestyle factors, depression, snoring, sl
