Autoimmune Optic Neuropathy: Pathogenesis, Diagnosis, and Therapeutic Advances.

Autoimmune optic neuropathy (AON) encompasses a heterogeneous group of immune-mediated optic nerve inflammatory disorders, characterized by progressive or recurrent visual loss with or without optic disc edema, and distinct autoantibody profiles and neuroimaging features. This review synthesizes current knowledge on AON, highlighting key advances in the discovery of disease-specific biomarkers [eg, aquaporin-4 (AQP4)-IgG for NMOSD, myelin oligodendrocyte glycoprotein (MOG)-IgG for MOG-associated disease (MOGAD), and glial fibrillary acidic protein (GFAP)-IgG for astrocytopathy], while neuroima

Optical imaging of the optic nerve: beyond demonstration of retinal nerve fiber layer loss.

Although we are still early in the evolution of optical imaging of the optic nerve, the available techniques already play an important role in clinical decision making. I would summarize our findings to date as follows: For acute ON: Presentation: OCT shows RNFL swelling, normal GCL + IPL by OCT; 1 month: OCT and SLP show RNFL thinning and swelling, GCL + IPL thinning by OCT; 3 months or later: OCT and SLP show RNFL thinning, further mild GCL thinning by OCT; 6 months: RNFL and GCL + IPL thinning finished. For acute NAION: Presentation: OCT shows RNFL swelling and SLP shows loss of birefringen

Sudden-onset Blindness from a Spontaneous Carotid-cavernous Fistula with Secondary Central Retinal Artery Occlusion and Posterior Ischemic Optic Neuropathy.

Our case describes a patient diagnosed with a carotid-cavernous fistula (CCF) secondary to a spontaneously ruptured cavernous carotid aneurysm, presenting with sudden vision loss, and a concomitant central retinal artery occlusion as visualized by a cherry-red spot in the macula and posterior ischemic optic neuropathy. Computed tomography of the brain and orbits showed mild hydrocephalus, orbital fat haziness, and proptosis with concern for fluid in the basal cisterns. Cerebral angiography confirmed the suspected diagnosis of CCF. After angiography, a Magnetic resonance imaging of the brain de

Retinal degeneration in mice and humans with neuronal ceroid lipofuscinosis type 8.

BACKGROUND: Ceroid lipofuscinosis type 8 belongs to a heterogenous group of vision and life-threatening neurodegenerative diseases, neuronal ceroid lipofuscinosis (NCL). Effective therapy is limited to a single drug for treatment of ceroid lipofuscinosis type 2, necessitating animal disease models to facilitate further therapeutic development. Murine models are advantageous for therapeutic development due to easy genetic manipulation and rapid breeding, however appropriate genetic models need to be identified and characterized before being used for therapy testing. To date, murine models of oc

Unilateral blindness and complete ophthalmoplegia following middle cerebral artery aneurysm clipping: a case report.

BACKGROUND: Middle cerebral artery (MCA) aneurysms rarely cause ophthalmic complications due to their anatomical distance from ocular structures. We report an exceedingly rare case of acute unilateral blindness and complete ophthalmoplegia following MCA aneurysm clipping via a pterional/trans-sylvian approach, a complication not previously documented in the literature with concurrent optical coherence tomography (OCT) findings. CASE PRESENTATION: A 49-year-old man with a history of hypertension presented with a severe headache. Computed tomography (CT) and angiography confirmed a subarachnoid

Carotid dissection and central serous chorioretinopathy related to sarcoidosis-antiphospholipid syndrome: a case report.

Sarcoidosis is a chronic multisystemic disease, which can be rarely associated with autoimmune disorders, such as antiphospholipid syndrome (APS). Although amaurosis fugax is an uncommon complication, its presentation can unmask a carotid artery dissection (CAD) in these diseases. In addition, central serous chorioretinopathy (CSC) has been related to vascular disorders too. We presented a case of a Caucasian middle-aged man, who developed CAD symptoms, such as amaurosis fugax in the right eye (RE) and headache. His medical history included arterial hypertension, hypothyroidism, and Lofgren's

Retinal curvature as a regional morphologic phenotype of the posterior pole in high myopia: A two-center OCTA study.

PURPOSE: To evaluate retinal curvature across macular regions in eyes with high myopia and to examine its relationship with retinal thickness and optical coherence tomography angiography-derived retinal flow-density metrics within a unified spatial framework. METHODS: In this dual-center observational study, participants underwent swept-source optical coherence tomography angiography imaging using a 24 × 20 mm macular scan. Retinal curvature (RC) was derived from Bruch's membrane-based surface reconstruction and analyzed across six concentric macular rings. Corresponding regional measurements

Microcystic macular oedema in multiple sclerosis is associated with disease severity.

Macular oedema typically results from blood-retinal barrier disruption. It has recently been reported that patients with multiple sclerosis treated with FTY-720 (fingolimod) may exhibit macular oedema. Multiple sclerosis is not otherwise thought to be associated with macular oedema except in the context of comorbid clinical uveitis. Despite a lack of myelin, the retina is a site of inflammation and microglial activation in multiple sclerosis and demonstrates significant neuronal and axonal loss. We unexpectedly observed microcystic macular oedema using spectral domain optical coherence tomogra

Three-dimensional optical coherence digital-null deformography of multi-refractive-surface optics with nanometer sensitivity.

Knowledge of the lens deformation during the reliability test is critical for lens design and fabrication. Refractive surface distorts the optical path of probing light, and poses a great challenge to measuring the test-induced nanoscale changes of all refractive lens surfaces simultaneously. In this work, we present an optical coherence digital-null deformography (ODD). A digital null, i.e., the interference signals (including intensity and phase) of the backscattered probing light from each lens surface, was recorded prior to the test with a phase-sensitive optical coherence tomography (OCT)

Multicolor Imaging of Optic Disc Drusen.

Optic disc drusen (ODD) are calcified deposits at the anterior optic nerve that are often detectable by ophthalmic imaging, including optical coherence tomography and fundus autofluorescence imaging. Multicolor (MC) imaging is a novel modality that captures reflectance of blue, green, and near-infrared laser lights with confocal scanning laser ophthalmoscopy to rapidly acquire high-resolution reflectance images of the optic disc and retina. Here, we show an eye with 3 MC imaging features of ODD, including prominent green hyperreflectance of the optic disc, green sheathing of the papillary and

Multilayered retinoschisis associated with optic disc pit.

BACKGROUND: Recently, optical coherence tomography (OCT) analysis has contributed greatly to the detection of posterior retinoschisis associated with optic disc pits. We report an unusual case of optic disc pit maculopathy. CASE: A 37-year-old woman with a multilayered structure of posterior retinoschisis associated with a very small, shallow optic disc pit was treated. OBSERVATIONS: The patient came to our clinic after a few weeks of decreased vision in her right eye. Fundus examination showed a shallow macular detachment with a tiny, shallow pit at the temporal edge of the optic disc. OCT
