Ambivalence: Motivational Interviewing Informed Communication for Serious Illness Care.

Patients facing serious illness may experience ambivalence about important treatment decisions, sharing information with loved ones, or, perhaps most consequentially, living in a way that is aligned with their values. Motivational interviewing (MI) is a collaborative, person-centered communication style that explores ambivalence by eliciting a patient's motivations, values, and goals. MI offers a practical framework for serious illness communication (SIC), when patients and families often experience competing priorities and uncertainty. Through a clinical case, this article explores the four f

Rapid changes in risk attitudes originate from Bayesian inference on parietal magnitude representations.

Risk attitudes-the willingness to accept uncertainty for larger potential rewards-are often treated as a stable individual trait, yet people's risky choices change markedly across contexts and even across repetitions of identical decisions. Here we show, using modelling and neuroimaging, that such rapid changes emerge from Bayesian inference on noisy magnitude representations in parietal cortex. Participants underwent fMRI while choosing between safe and risky options that are either held in working memory or shown on screen. Options held in working memory are noisier and more prone to central

Feature-temporal predictions dynamically modulate performance, feature-based attentional capture, and motor response activity during visual search.

Recent work has shown that temporal predictions linked to spatial expectations can guide visual search in dynamic environments, but less is known about feature-based temporal predictions. Here, we investigated whether behaviour is guided towards feature-temporally predictable targets in crowded, dynamic displays under spatial uncertainty. Participants searched for two distinct targets (non-overlapping colour-shape combinations) in a free-viewing visual-search task. During learning trials, each target appeared transiently at a predictable time (early or late); in testing, some appeared unpredic

Re-evaluation of lymph node dissection and surgical strategy for locally advanced gastric cancer in the era of neoadjuvant immunotherapy: a narrative review.

Gastric cancer is a highly prevalent malignancy with high mortality worldwide. Radical surgical resection remains the cornerstone for achieving long-term survival in patients with locally advanced gastric cancer. In recent years, immune checkpoint inhibitors have been increasingly introduced into the perioperative setting. In locally advanced gastric cancer, immunotherapy shows potential to improve pathological complete response rates and R0 resection rates. However, its overall survival benefit still requires confirmation from high-quality studies. Meanwhile, immune-related inflammatory respo

Enabling responsible industry-academic collaboration in mental health research and the involvement of people with lived experience.

There is an urgent need for more effective and better-tolerated treatments for mental illness. Achieving this requires responsible collaboration between academia, healthcare professionals, industry and people with lived experience. However, regulatory and compliance frameworks can make early-stage, non-promotional collaboration difficult, particularly when discussions are exploratory and multidisciplinary. Drawing on experience from the UK Research and Innovation Mental Health Platform's attempted collaboration with an international pharmaceutical company, this article describes how engagement

Hospitalist perspectives and practices in treating hospitalized patients with alcohol withdrawal syndrome in the era of phenobarbital re-emergence.

Medication treatment for moderate to severe alcohol withdrawal syndrome (AWS) is evolving, particularly with renewed phenobarbital use. However, little research has examined how hospitalists manage AWS. We conducted a multiple-methods study involving hospitalists from a national US Hospital Medicine research network. Thirty-three hospitalists from 27 tertiary hospitals completed surveys and participated in seven focus groups on August 8, 2025. Benzodiazepines remained the most common treatment, but phenobarbital use was reported in 67% of emergency departments and 44% of inpatient medical unit

Application of Artificial Intelligence in Breast Ultrasound Diagnosis.

Artificial intelligence (AI) is reshaping ultrasound diagnosis by converting operator-dependent grayscale, Doppler, elastography, contrast-enhanced, automated-volume, and video data into reproducible decision support. In breast ultrasound, the most mature evidence involves benign-malignant lesion classification, BI-RADS risk stratification, reduction in unnecessary biopsy in selected low-risk lesions, assistance for less experienced readers, automated breast volume scanning, video-based assessment, axillary staging, and prediction of biologic markers such as molecular subtype, HER2 status, Ki-

The spaces in between: silence as a witness to accompaniment in congenital heart disease.

Paediatric cardiology is often experienced as a noisy environment. Outpatient clinics, echocardiography laboratories, ICUs, catheterisation suites, and operating theatres are filled with alarms, conversations, machines, movement, and continual clinical activity. Beneath this activity, however, another dimension of CHD care exists: silence. These pauses are not simply absences within clinical work, but moments through which patients, families, nurses, physiologists, and clinicians encounter uncertainty, adaptation, fear, relief, and continuity across time. Across fetal diagnosis, infancy, child

Myocardial bridging: a practical guide for clinicians.

Myocardial bridging is the most common congenital coronary anomaly, characterized by an intramyocardial course of a segment of an epicardial coronary artery, with considerable variability in its depth and length. In most cases, MB is a benign anatomical variant without clinical significance, often asymptomatic and not requiring treatment. However, with the increasing use of coronary computed tomography angiography for coronary assessment, myocardial bridging is now more frequently identified during non-invasive imaging. It can also be diagnosed via invasive coronary angiography, where it appea

Diagnostic and therapeutic challenges of isolated spinal vasculitis: A case report.

The present case report describes a 31-year-old male patient with isolated spinal vasculitis. The patient initially presented with right-hand numbness and weakness, which later extended to both lower limbs, causing gait instability. MRI scans performed after the symptoms had extended to both lower limbs revealed a cervical lesion with T1 hypointensity, T2 hyperintensity and heterogeneous enhancement, initially suspected to be a tumor. Histopathological analysis of the spinal cord biopsy specimen revealed lymphocytic infiltration surrounding the small vessels of spinal cord, supporting a diagno

The dynamics of hepatitis B virus transmission using a stochastic differential model and feed forward neural network approach.

Hepatitis B transmission is influenced by environmental variability, immune response and vaccination, making its spread inherently stochastic, while the integration of stochastic modeling with machine learning is particularly important for representing disease uncertainty in varied surroundings. We present a hybrid innovative framework that combines a stochastic epidemiological model with a feed-forward neural network to study hepatitis B virus (HBV) dynamics. We show the well-posedness by establishing the existence of solutions with uniqueness, and analyze extinction and persistence of the di

'I Will Never Succeed': An Inductive Thematic Analysis of Psychosocial Challenges Experienced Before Sleeve Gastrectomy Among Patients in Turkey.

Our study explored psychosocial experiences in the pre-surgery period that shaped the decision to undergo sleeve gastrectomy, with a focus on self-perception and internalised stigma. Nineteen participants from Turkey were purposively recruited. Semi-structured in-depth interviews were conducted to collect data, which were then analysed thematically with an inductive approach. Seven key themes emerged: body image, social stigma, family dynamics, emotional coping, psychological barriers to weight loss, surgical decision-making, and psychosocial impacts of weight. Participants reported feelings o

Bond Dissociation Energy of NO with Subwavenumber Precision via State-to-State Resolved Threshold Fragment Yield Spectroscopy.

We report the bond dissociation energy (BDE) of nitric oxide (NO) with subwavenumber precision (±0.20 cm-1), measured via state-to-state resolved threshold fragment yield spectroscopy. The ground-state dissociation limit, NO(X2Π1/2) → N(4S) + O(3P2), yields a BDE of 52396.92 ± 0.20 cm-1 (6.496390 ± 0.000025 eV; 626.8064 ± 0.0024 kJ/mol), reducing the uncertainty of prior work by a factor of 50. Combining this result with equally precise BDEs for N2, O2, CO, NO2, and CO2, we derive spectroscopically accurate thermochemical benchmarks, including ΔfH0(NO, 0 K), ΔfH0(NO2, 0 K), the atomization ene

Aortic Valve Myxoma: A Tiny Cardiac Mass With Major Neurologic Consequences.

BACKGROUND: Aortic valve myxomas are exceptionally rare benign cardiac tumors with substantial embolic potential. Their imaging appearance may overlap with more common valvular tumors, creating diagnostic uncertainty. CASE SUMMARY: A 42-year-old woman presented with aphasia and right arm weakness from recurrent embolic cerebral infarctions. Evaluation excluded hypercoagulable disorders, venous thromboembolism, and intracardiac shunting. Echocardiography identified a small mass on the right coronary cusp of the aortic valve, initially suspected to be papillary fibroelastoma. Surgical excision

Evidence for the Rare Decay B^{+}→Λ[over ¯]pμ^{+}μ^{-}.

A search for the rare decay B^{+}→Λ[over ¯]pμ^{+}μ^{-} is performed using proton-proton collision data recorded by the LHCb experiment at a center-of-mass energy of sqrt[s]=13  TeV, corresponding to an integrated luminosity of 5.4  fb^{-1}. An excess of events is found with respect to the background-only expectation, with a signal significance of 3.5 standard deviations, in the low invariant-mass region of m(Λ[over ¯]p)<2.8  GeV/c^{2}. The branching fraction is measured to be B_{low}(B^{+}→Λ[over ¯]pμ^{+}μ^{-})=(1.70_{-0.56}^{+0.65}(stat)±0.17(syst)±0.14(ext))×10^{-8}, where the last uncertain

Intracranial meningioma as a threat and a smoldering crisis: A qualitative interview study from diagnosis to surgery to postoperative surveillance.

BACKGROUND: Intracranial meningiomas are associated with substantial psychological morbidity. We aimed to characterize patients' subjective experience of their disease trajectories. METHODS: We conducted an exploratory interview study. Ten adults with meningioma participated in semistructured interviews 6-36 months post-diagnosis. Data were analyzed using Braun and Clarke's Reflexive Thematic Analysis. RESULTS: Patients consistently framed meningioma as a recurrent threat, with 3 temporal anchors: diagnosis, surgery, and long-term postoperative surveillance. Threat encompassed identity, dail

Unveiling the Aha! moment: A computational account of insight in active inference.

Insight, also referred to as the "Aha!" moment, is a distinctive cognitive phenomenon in which an idea or solution to a problem suddenly emerges into consciousness. Although the literature describes it extensively, the nature of its distinctive cognitive and affective mechanisms remains poorly understood. In this paper, we formalize a computational approach to insight grounded in the active inference framework. By modeling insight as Bayesian model reduction in a modified Wisconsin Card Sorting Task, we show that the model can reproduce a broader computational profile associated with insight,
