Inhaled sevoflurane in critically ill COVID-19 patients: A retrospective cohort study.

BACKGROUND: Managing sedation in critically ill COVID-19 patients is challenging due to high sedative requirements and organ dysfunction that alters drug metabolism. Inhaled sevoflurane offers a lung-eliminated alternative that may mitigate drug accumulation. METHODS: This single-center, retrospective cohort study analyzed 43 mechanically ventilated COVID-19 patients (March-November 2020). Patients received inhaled sevoflurane adjunctive to IV sedation (n=30) or IV sedation alone (n=13). The primary outcome was the cumulative dose of IV sedatives over 7 days. Secondary outcomes included time

Beating the Odds: Survival After Extracorporeal Membrane Oxygenation Decannulation Due to Poor Prognosis in Children-An Extracorporeal Life Support Organization Registry Study.

Extracorporeal membrane oxygenation (ECMO) is a life-saving intervention for critically ill children. However, decisions regarding discontinuation due to poor prognosis remain ethically complex. While outcomes after successful weaning are well described, survival following decannulation for perceived futility is poorly understood. We conducted a retrospective cohort study using the Extracorporeal Life Support Organization (ELSO) registry, including pediatric ECMO runs from 2013 to 2023. Among 6,662 cases decannulated for poor prognosis, we identified patients who survived until hospital discha

Case Report: Successful treatment of steroid-refractory severe immunotherapy-induced pneumonitis with equine antithymocyte globulin.

Immune checkpoint inhibitors (ICI) have revolutionized melanoma treatment but are associated with autoimmune toxicities. Immunotherapy-induced pneumonitis (IIP) is a potentially fatal immune-related adverse event. Current management of IIP involves corticosteroids, mycophenolate mofetil (MMF), intravenous immunoglobulin, or infliximab for severe cases. Limited data exist for corticosteroid-refractory pneumonitis. This case report is the first to describe successful treatment of refractory grade 4 IIP with equine antithymocyte globulin (eATG) after failure of corticosteroids and MMF. A 50-year-

Metabolic imprinting due to small litter size preserves gut microbiota diversity and SCFA output despite high-sucrose diet.

According to the Developmental Origins of Health and Disease (DOHaD), early-life nutritional exposure can affect long-term metabolic health. Gut microbiota development during lactation plays a pivotal role in regulating metabolism, with short-chain fatty acids (SCFAs) serving as key mediators connecting early microbial colonization to future metabolic outcomes. Diet composition strongly affects gut microbiome, and high-sucrose diets (HSD) promote dysbiosis and metabolic dysfunction. We investigated whether small litter size (SL) modulates susceptibility to HSD-induced metabolic disruptions by

Impact of a Short-Term High-Fat Diet on Lifespan and Tumorigenesis After Early-Life Exposure to Radiation.

INTRODUCTION: Children are highly susceptible to radiation's carcinogenic effects. Lifestyle, in particular, diet-induced obesity (DIO) is closely linked to cancer risk factors. Most prior studies, however, have concentrated on the impact of long-term lifestyle factors. OBJECTIVE: We assessed the impact of a short-term, high-fat diet (HFD) during young or adult on lifespan shortening and tumor development after infant radiation exposure. METHODOLOGY: B6C3F1 male mice were irradiated at 1 week of age with 4 Gy of gamma rays or did not undergo irradiation (0 Gy). After weaning, they were assig

Paternal physical inactivity alters offspring sex ratio and is associated with heritable impairments in reproductive success in rats.

Paternal lifestyle factors are increasingly recognized as determinants of offspring health, yet the effects of physical inactivity on reproduction remain unclear. Here, we demonstrated that paternal physical inactivity caused a bias in offspring sex ratio and reduced fertility across generations in rats. Male rats subjected to eight weeks of restricted activity produced offspring with a female-biased sex ratio (female : male = 39 : 16; p = 0.011; odds ratio = 2.84) compared with controls (24 : 28). Sperm motility was markedly reduced in inactive fathers (p < 0.001 versus control) but was fully

Cardiovascular outcomes following intrauterine and lactational exposure to cyantraniliprole in male Wistar rats.

Cyantraniliprole, a systemic insecticide that modulates ryanodine receptors, may impact mammalian physiological systems. Due to its ability to cross biological barriers, the offspring of exposed pregnant females could also be affected. This study evaluated the cardiovascular toxicity following the intrauterine and lactational exposure to cyantraniliprole in pubertal and adult male offspring of Wistar rats. Pregnant rats were treated with water (control), 1 mg/kg (CYA1), or 10 mg/kg (CYA10) of the insecticide by gavage from gestational day 5 until weaning. Biometric parameters in dams and offsp

Early-Life Dietary Protein Source Reshapes Gut Microbiota but Fails to Curtail Diet-Induced Metabolic Syndrome in Young Rats.

OBJECTIVE: Dairy consumption plays a protective role in obesity that may be mediated through the gut microbiota. This study examined the role of dairy proteins, casein, whey, and complete dairy (skim milk powder), and a plant-based alternative, soy, on gut microbiota composition, obesity susceptibility, and insulin sensitivity in early development. METHODS: Following weaning, rat pups were randomized into four isocaloric diets (n = 8-11/sex/diet) for 4 weeks: (1) complete dairy; (2) casein; (3) whey; and (4) soy. Primary outcomes included food intake, growth, body composition, glucose toleran

JNK and p53 cause human and mouse β cell death during excessive unfolded protein response.

Endoplasmic reticulum (ER) stress contributes to β cell death in both Type 1 and Type 2 diabetes (T1D and T2D). However, the molecular mechanisms driving β cell death during ER stress remain insufficiently defined, limiting development of protective therapies. GRP78, an ER chaperone, is the master regulator of unfolded protein response (UPR), suppressing UPR initiators during the unstressed state and releasing them to allow UPR activation during stress. To dissect the pathways leading to ER-stress response related β cell decompensation, we engineered mice genetically lacking GRP78 in pancreati

The Role of Selenium in the Antioxidant System of Cattle, Pigs, and Small Ruminants: Implications for Animal Health and Productivity.

Oxidative stress contributes to reproductive disorders, immune dysfunction, and reduced productivity in livestock during periods of high metabolic demand and environmental challenge. Selenium supports antioxidant defense systems because it is incorporated as selenocysteine into selenoproteins, including glutathione peroxidases and thioredoxin reductases that detoxify peroxides and sustain redox balance. The review summarizes selenium occurrence and chemical forms in feeds, as well as its absorption, transportation, and storage. The review also outlines the major features of selenoprotein biosy

Does the nature of solid feed influence rumen development? A comparative study of calves fed a high milk replacer with starter concentrate or alfalfa hay.

This study evaluated the effects of two milk replacer-based diets, one supplemented with starter feed and the other with alfalfa hay, on rumen development in preweaned Holstein calves. Twenty newborn male calves were fed 8 L/day of milk replacer plus either starter feed (group A) or alfalfa hay (group B) ad libitum for 10 weeks. Rumen morphometry and histological features were assessed via gross anatomical evaluation, histology, and scanning electron microscopy. The calves in group A consumed significantly more solid feed and exhibited greater papillae development, with higher papillae height,

Impact Of Transesophageal Echocardiography-guided Hemodynamic Optimization On Fast-track Recovery After Cardiac Surgery.

Perioperative volume misclassification and cardiac dysfunction often hinder fast-track recovery following cardiac surgery. This retrospective cohort study evaluated whether intraoperative transesophageal echocardiography (TEE)-guided hemodynamic optimization was associated with improved fast-track outcomes. Seventy-two adult patients undergoing cardiac surgery received either TEE-guided optimization (n = 34) or standard care (n = 38). The intervention involved goal-directed hemodynamic management based on TEE phenotyping of volume status and cardiac function at predefined intraoperative time p

Programming the Infant Gut: How Maternal and Early Life Nutrition Shape the Infant Microbiome and Long-term Health-A Narrative Review.

Childhood malnutrition, including undernutrition, obesity, and micronutrient deficiencies, remains a major global health burden. Emerging evidence points to the gut microbiome as a critical mediator linking maternal, prenatal, and early-life nutrition to long-term offspring health outcomes. From conception and through the first years of life, maternal diet, metabolic state, and environmental exposures shape offspring microbial colonization and maturation. Breastfeeding and consumption of fiber-rich and fermented foods (maternal and post-weaning) support beneficial microbiota, while high-fat, h

Postweaning diet affects offspring metabolic outcomes more than maternal diet in male and female mice.

The pervasiveness of obesity necessitates expanded pursuits to understand mechanistic drivers of the condition. Maternal diet during conception and gestation can program offspring towards excessive weight gain. Furthermore, the association between obesity and hypertension suggests that maternal programming may impact offspring vascular reactivity. We investigated whether a maternal high fat+sucrose diet (HFD) prior and during pregnancy and lactation affected offspring body composition, metabolic parameters, and vascular reactivity. Using C57BLJ/6 mice, male and female offspring from control (l

Immediate and Gradual Withdrawal of Immunosuppression After Kidney Graft Loss Lead to Similar Outcomes.

The management of immunosuppression in dialysis patients with a failed kidney transplant remains a pending question, and different approaches to immunosuppression weaning have been proposed. We conducted a retrospective study of patients who experienced a graft failure, and compared the rates of immune and non-immune events, according to different modalities of immunosuppression withdrawal. Two hundred and eighteen patients were included. During the follow-up (45 (20-80) months post-graft failure), 53 patients (24.3%) experienced an intolerance syndrome. The time between graft failure and the

Lactiplantibacillus plantarum promotes intestinal goblet cell differentiation via indole-3-lactic acid-AHR signaling in pigs.

The swine intestinal microbiota dynamically remodels during development and supports gut homeostasis. However, whether stage-specific microbial shifts, are associated with epithelial development remains poorly understood. Here, longitudinal metagenomic profiling of the swine gut microbiome identified Lactiplantibacillus plantarum as a transiently enriched nursery-stage bacterium positively associated with goblet cell numbers. Dietary supplementation with L. plantarum validated this association, showing increased goblet cell numbers and MUC2 expression in the ileum of nursery piglets. Co-cultur
