Biliary tract cancer and stones in relation to chronic liver conditions: A population-based study in Shanghai, China.

Biliary tract cancers are relatively rare but fatal tumors. Apart from a close link with gallstones and cholangitis, risk factors for biliary tract cancer are obscure. Chronic liver conditions, including liver cirrhosis, have been linked to a higher risk of biliary tract cancer. In a population-based case-control study conducted in Shanghai, China, we investigated the relationships of a history of chronic hepatitis and liver cirrhosis as well as a family history of liver cancer with biliary tract cancer risk. The study included 627 patients with biliary tract cancers (368 gallbladder, 191 bile

Role of intercellular adhesion molecule-1 and lymphocyte function-associated antigen-1 during nonsuppurative destructive cholangitis in a mouse graft-versus-host disease model.

Intercellular adhesion molecule-1 (ICAM-1) is expressed abnormally on the bile duct epithelium during the course of primary biliary cirrhosis (PBC), but the importance of ICAM-1 and its lymphocyte function-associated antigen-1 (LFA-1) receptor during the course of nonsuppurative destructive cholangitis (NSDC) has not been defined. To address this question, we defined the relationship between ICAM-1 on the intrahepatic bile duct epithelium and the evolution of NSDC lesions in a mouse graft-versus-host disease (GVHD) model. We also determined the effects of anti-ICAM-1 and anti-LFA-1 treatments

Complications of endoscopic biliary sphincterotomy.

BACKGROUND: Endoscopic sphincterotomy is commonly used to remove bile-duct stones and to treat other problems. We prospectively investigated risk factors for complications of this procedure and their outcomes. METHODS: We studied complications that occurred within 30 days of endoscopic biliary sphincterotomy in consecutive patients treated at 17 institutions in the United States and Canada from 1992 through 1994. RESULTS: Of 2347 patients, 229 (9.8 percent) had a complication, including pancreatitis in 127 (5.4 percent) and hemorrhage in 48 (2.0 Percent). There were 55 deaths from all causes

Cholangitis resembling Caroli's syndrome in a patient with autosomal dominant polycystic kidney disease: Case report.

Caroli's syndrome is a rare autosomal recessive disorder characterized by segmental cystic dilation of the intrahepatic bile ducts and congenital hepatic fibrosis. It is associated with autosomal recessive polycystic kidney disease. Here, we report a 63-year-old male known with polycystic kidney disease, who was admitted to our hospital with intermittent fever, accompanied by weight loss. Magnetic resonance cholangiopancreatography suggested the presence of Caroli's syndrome, characterized by cystic dilation of the intrahepatic proximal bile ducts on both the right and left lobes of the liver.

[Thrombosis of portal cavernoma].

Portal cavernoma thrombosis is a complication of portal cavernoma. We describe the case of a 74-year-old patient who presented to the emergency department with abdominal pain. The computed tomography scan showed a mass from the head of the pancreas to the hepatic hilum not enhanced after injection of iodinated contrast. There was no dilatation of the bile ducts. Abdominal magnetic resonance ruled out a tumour and confirmed a portal cavernoma thrombosis. In 50 % of cases the etiology of the portal cavernoma is unknown. It is often asymptomatic. It may be discovered in case of complications of p

Current status of and future issues related to endoscopic papillectomy.

Endoscopic papillectomy is widely performed to treat duodenal papillary tumors, particularly at high-volume centers. It is indicated for adenomas without intraductal extension of the bile or pancreatic ducts. However, despite numerous reports of carcinomas that expand the indications to include well-differentiated adenocarcinomas that do not invade the sphincter of Oddi, the low agreement between biopsy and final pathological diagnosis, as well as the current inability of imaging modalities to diagnose sphincter of Oddi invasion, makes it difficult to consider expanding indications. Although c

Duplicated gallbladder with double cystic duct: hidden in plain sight.

Duplicated gallbladder with double cystic duct is a rare anomaly, with 62 cases reported including this case. We present a 76-year-old man who underwent interval laparoscopic cholecystectomy after previous conservative management of acute cholecystitis. Retrograde dissection of gallbladder was performed due to difficult access. Gallbladder was opened at Hartman's pouch which revealed two bile-flowing structures. Cholangiogram was only successful via one of the ducts with no evidence of leak. Subtotal cholecystectomy was performed after consultation with a Hepatobiliary surgeon. Preoperative co

Current understanding of primary biliary cholangitis.

Primary biliary cholangitis (PBC) causes chronic and persistent cholestasis in the liver, eventually resulting in cirrhosis and hepatic failure without appropriate treatment. PBC mainly develops in middle-aged women, but it is also common in young women and men. PBC is considered a model of autoimmune disease because of the presence of diseasespecific autoantibodies, that is, antimitochondrial antibodies (AMAs), intense infiltration of mononuclear cells into the bile ducts, and a high prevalence of autoimmune diseases such as comorbidities. Histologically, PBC is characterized by degeneration

Diagnosis and Treatment of Fasciola Hepatica With Endoscopic Retrograde Cholangiopancreatography in a Child Patient: Case Report.

Fasciola hepatica (FH) is a parasite that causes fever, hepatomegaly, abdominal pain, weight loss, anemia, and eosinophilia in the acute period, and jaundice, pancreatitis, and cholangitis in the chronic period by settling in the bile ducts. A 13-year-old girl admitted with abdominal pain, nausea, and jaundice. In her hemogram, the patient had leukocytosis and eosinophilia. The transaminase, bilirubin, amylase, and lipase values were high in the biochemistry of the patient. Abdominal ultrasonography revealed dilatation, and moving and hyperechogenic tubular structures in the intra- and extrahe

Presentation of gallbladder torsion at an abnormal position: A case report.

BACKGROUND: Gallbladder torsion is a rare acute abdominal condition that requires emergency surgery. It occurs more commonly in elderly people and in women in the adult population. Diagnosis is a challenge as non-specific symptoms and signs have been reported on ultrasonography, computed tomography and magnetic resonance imaging. Prompt cholecystectomy can decrease the mortality and morbidity of perforation due to gallbladder torsion. CASE SUMMARY: An 82-year-old woman with upper-right quadrant pain and associated nausea and vomiting was diagnosed with ectopic acute calculus cholecystitis. Ma

Hepatoduodenal lymph node metastasis mimicking Klatskin tumor in a patient with sigmoid colon mucinous cancer.

We report a case of a 48-year-old female patient, who presented with abdominal pain, jaundice, and lack of appetite. Ultrasound showed intrahepatic biliary dilatation with retroperitoneal lymphadenopathy. Further magnetic resonance cholangiopancreatography detected Klatskin tumor. Computed tomography (CT) confirmed the Klatskin tumor with liver metastases and retroperitoneal lymphadenopathy. Biopsy from the hepatic lesion identified mucinous adenocarcinoma, likely originating from bile ducts. Endoscopic retrograde cholangiopancreatography was performed 3 times with stents placed in the left an

Imaging of autoimmune biliary disease.

Autoimmune biliary diseases are poorly understood but important to recognize. Initially, autoimmune biliary diseases are asymptomatic but may lead to progressive cholestasis with associated ductopenia, portal hypertension, cirrhosis, and eventually liver failure. The three main forms of autoimmune biliary disease are primary biliary cirrhosis, primary sclerosing cholangitis, and IgG4-associated cholangitis. Although some overlap may occur between the three main autoimmune diseases of the bile ducts, each disease typically affects a distinct demographic group and requires a disease-specific dia

[Morphometric parameters of hepatic lobule vessels in mice during the restorative period after leg injury].

The diameters of the hepatic lobule vessels (interlobular veins, central veins, interlobular arteries, intralobular sinusoidal capillaries, interlobular bile ducts) were been studied 3, 7 and 28 days after shin bones fracture in CBA mice (n=30). Most pronounced changes of morphometric parameters indicative of hemodynamic disturbances, were found 3 days after the trauma. The increase of the diameter of central, interlobulat veins and sinusoidal capillaries took place, together with the decrease of the diameter of interlobular arteries, which, probably, promoted the reduction of arterial blood s

Diagnosis and therapeutic problems of primary sclerosing cholangitis.

Primary sclerosing cholangitis (PSC) leads to a progressive destruction of the intra- and extrahepatic bile ducts. The cause is unknown but genetic and immunological mechanisms may play a role. The median survival time from diagnosis to death is about 12 years. MRCP is almost equal to ERCP for diagnosing PSC and shows the typical localised or multifocal strictures and interfering segments of ectatic bile ducts. Liver histology can be helpful in making the diagnosis but is often unspecific and there is a large sampling variability. The treatment of PSC is disappointing. The combination of ursod

The role of cholangiocytes in the development of chronic inflammatory liver disease.

Cholangiocytes constitute the biliary epithelium, an important barrier to infection entering the liver via the gastrointestinal tract. These cells have developed mechanisms to respond to infection by recruiting and interacting with effector leukocytes to clear bacterial or viral pathogens. Cholangiocytes are also targets of immune mediated damage in several liver diseases and under these circumstances protective mechanisms, for example the secretion of chemokines and expression of adhesion molecules, become harmful and promote the inappropriate recruitment and retention of effector cells withi

[Secondary biliary cirrhosis in a patient with histiocytosis X].

Histiocitose X or Histiocytosis of the Langerhans cells represents a complex spectrum of clinical alterations, resulting from infiltration by anomalous histiocytes of various organs, including the skin, bones, lungs, lymphatic ganglia and liver. Liver disease is rare and the mechanism by which lesions appear is unknown. Cholestasis results from phenomena of sclerosant colangitis, which affects the intrahepatic ducts, or from proliferation of histiocytic cells in the periportal areas. Some patients develop biliar cyrrhosis. The authors present the clinical case of a 62-year-old female patient,

A case of intrahepatic stones with an anomalous intrahepatic portal venous system.

A 64-year-old woman who had undergone surgery for gallstones twice was admitted with epigastric pain and fever. Ultrasonography revealed gallstones in the dilated left lateral intrahepatic bile ducts. Abdominal computed tomography disclosed atrophy of the left lateral segment and the right lobe of the liver, and hypertrophy of the left medial segment and the caudate lobe. Percutaneous transhepatic cholangiography demonstrated that the stones were localized in the left lateral intrahepatic bile ducts, and that the choledochoduodenostomy previously established was patent. Pancreaticobiliary ductal union in cholangiocarcinoma.

Entry points of the pancreatic and bile ducts into the duodenum were adequately visualized on endoscopic retrograde cholangiopancreatography in 23 confirmed patients with cholangiocarcinoma. The group comprised 19 men and four women, and age ranged from 25 to 70 years. In 21 patients (91.3%) a common channel was present, whereas in 2 patients the pancreatic and bile ducts opened separately into the duodenum. The mean length +/- SD of the common channel in patients of cholangiocarcinoma was 6.43 +/- 5.23 mm, which was comparable to the length of the common channel in 25 normal subjects (mean +/

Intrahepatic versus extrahepatic cholestasis. Discrimination with biliary scintigraphy combined with ultrasound.

Biliary scintigraphy and ultrasound imaging were performed in 52 patients with suspected biliary tract pathology. Results were correlated with the findings of direct cholangiography. Several new innovations in scintigraphic technique were used. The combination of ultrasound imaging and scintigraphy correctly identified biliary tract obstruction in 17 of 19 patients, 12 of whom had dilated bile ducts on ultrasonography. Intrahepatic cholestasis was correctly diagnosed in 11 of 13 patients. Accurate discrimination between intrahepatic and extrahepatic cholestasis was achieved in 28 of 32 patient
