[Prevention and prophylaxis in angiology].

Vascular diseases develop over decades. Avoidable exposures, such as tobacco use and insufficient implementation of evidence-based prevention strategies contribute to the progression. For angiology, prevention and prophylaxis therefore means an integrated approach to structural prevention, primary prevention and secondary prevention. Structural prevention particularly addresses nicotine products, including e‑cigarettes/vapes and nutrition, such as reducing sugar in processed foods. Primary prevention includes risk-adapted early detection and risk reclassification, which do not rely on populati

Randomized pilot evaluation of a virtual standardized patient training for enhancing suicide safety planning skills among VA providers.

OBJECTIVE: The Suicide Safety Planning Intervention (SPI) is an evidence-based intervention associated with reduced suicidal behavior and improved treatment engagement, yet provider fidelity in delivering SPI remains inconsistent and opportunities for structured skills practice are limited. This study evaluated whether Virtual Standardized Patient (VSP) training improved SPI delivery skills among Veterans Health Administration (VA) behavioral health providers compared with training as usual (UT). METHODS: Thirty VA behavioral health professionals without prior formal SPI training were randomi

When Prevention is an Emergency: The Imperative of Contraception Initiation in the Emergency Department.

BACKGROUND: Millions of U.S. women live in contraceptive deserts, with limited or no access to comprehensive reproductive health services. Barriers are intensified by Medicaid restrictions, provider shortages, and growing reproductive health inequities. The emergency department (ED), as the healthcare safety net, is uniquely positioned to expand contraceptive access. OBJECTIVES: To highlight the public health need for ED-initiated contraception, describe feasible approaches for implementation, and address common challenges to integrating contraception into emergency care. DISCUSSION: Women o

Contemporary Landscape of Medical Technology in Gastroenterology Between 2013 and 2023.

Medical technology in gastroenterology spans devices, point-of-care diagnostics, digital health, and artificial intelligence. We conducted a narrative review of US Food and Drug Administration (FDA)-reviewed medical technology on the available evidence base to support uptake in clinical practice. Seven hundred thirteen new gastrointestinal devices were FDA approved or cleared between 2013 and 2023. The most frequent technologies had indications for use in general endoscopy (442 or 62.0% of all new technologies) with advanced endoscopy technologies being the second most common (172 or 24.1%). N

Best practice in the use of middle meningeal artery embolisation for chronic subdural haematoma.

Chronic subdural haematoma (cSDH) is a common neurosurgical disorder in older people and projected to become the most common cranial neurosurgical pathology by 2030. Incidence of cSDH has risen in recent years as a consequence of improved access to brain imaging, and the increased prescribing of antithrombotic medication for both primary and secondary prevention of vascular disease. Chronic SDH typically presents with an insidious onset of broad-ranging symptoms including impaired cognition, gait, balance and mobility, often with headache. It progresses to more fulminant symptoms of hemiplegia

Reevaluating the safety of chamomile poultices in ophthalmic care.

INTRODUCTION: German chamomile [Matricaria recutita L. (Asteraceae)] tea poultices are a common folk remedy in Central and Eastern Europe for treating eye inflammations such as blepharitis and conjunctivitis. This practice often precedes medical consultation and professional advice. Surprisingly, some healthcare professionals, including pharmacists, naturopaths, general practitioners, and pediatricians, recommend it as a primary treatment, particularly among the elderly. However, the ophthalmic use of chamomile lacks scientific validation, and the European Medicines Agency (EMA) and ophthalmic

A Practical Guide to Implementing an Evidenced-Based Hands-On Naloxone Training.

Synthetic opioids contribute to the majority of opioid overdose-related deaths in the United States. Expansion of naloxone training to community laypersons is one strategy to mitigate opioid overdose-related deaths. A hands-on naloxone training demonstrated efficacy in improving opioid knowledge and overdose response in baccalaureate nursing students, Greek-affiliated students, and rural clinicians and staff post-training. The purpose of this practical guide is to provide detailed steps to implement an evidence-based hands-on naloxone training for laypersons in community settings. The hands-on

Outside the Clinic Door: How Neighborhood Sociodemographic Disadvantage is Associated with Community Therapist Secondary Traumatic Stress and Emotional Exhaustion.

Therapists serving families with high rates of trauma exposure in community mental health clinics face the potential risk of experiencing secondary traumatic stress and emotional exhaustion, both of which pose barriers for the implementation and sustainment of evidence-based practices. Previous research documents negative effects of living in socioeconomic disadvantaged neighborhoods on child development but has not examined the effects of working in these neighborhoods on therapist well-being. The current study merges publicly available data, administrative claims data on mental health servic

The European Guidelines on Diagnosis and Management of Neutropenia in Adults and Children: A Consensus Between the European Hematology Association and the EuNet-INNOCHRON COST Action.

Neutropenia, as an isolated blood cell deficiency, is a feature of a wide spectrum of acquired or congenital, benign or premalignant disorders with a predisposition to develop myelodysplastic neoplasms/acute myeloid leukemia that may arise at any age. In recent years, advances in diagnostic methodologies, particularly in the field of genomics, have revealed novel genes and mechanisms responsible for etiology and disease evolution and opened new perspectives for tailored treatment. Despite the research and diagnostic advances in the field, real world evidence, arising from international neutrop

A blended approach to developing psychomotor skills in novice learners in a doctor of physical therapy curriculum.

PURPOSE: Psychomotor skill performance is central to effective clinical practice across health professions. These complex skills are challenging to teach, particularly in the novice learner. As many health professions programs have increased blended course offerings, educators must establish best practices for teaching psychomotor skills in this evolving learning environment. The purpose of this paper is to describe the innovative application of an evidence-based framework to teaching psychomotor skills to novice learners in a blended learning environment. MATERIALS AND METHODS: Using a modif

Enhancing member engagement in a Tier 3 personality disorder service during COVID-19: evaluation of a virtual group programme.

BACKGROUND: Drop-out rates from evidence-based interventions for people with a diagnosis of personality disorder (PD) are high. The COVID-19 pandemic has likely exacerbated barriers to engagement with the introduction of virtual working. Virtual therapy has a good evidence-base for Axis I disorders, but limited research for Axis II disorders. AIMS: To investigate facilitators and barriers to engagement in a Tier 3 PD service virtual group programme. METHOD: A virtual group programme was developed in collaboration with service members, and analysed members' attendance rates over a 5-month per

Textbook typologies: Challenging the myth of the perfect obstetric pelvis.

Medical education's treatment of obstetric-related anatomy exemplifies historical sex bias in medical curricula. Foundational obstetric and midwifery textbooks teach that clinical pelvimetry and the Caldwell-Moloy classification system are used to assess the pelvic capacity of a pregnant patient. We describe the history of these techniques-ostensibly developed to manage arrested labors-and offer the following criticisms. The sample on which these techniques were developed betrays the bias of the authors and does not represent the sample needed to address their interest in obstetric outcomes. C

Classification of stress urinary incontinence.

INTRODUCTION: The relevant terminology for stress urinary incontinence (SUI) is affected by the context, namely the clinical assessment (the symptom of SUI elicited on history taking and the sign of SUI observed during examination) or diagnostic investigations (urodynamic stress incontinence). In some cases, SUI may only be observed after the reduction in coexistent prolapse (occult SUI). Classifying SUI often relies on distinguishing between intrinsic sphincter deficiency (ISD), and urethral malposition or hypermobility, although this potentially an over-simplification. REVIEW: Classificatio

Blood audit evidenced-based cross-match requesting for lumbar spine surgery.

STUDY DESIGN: Complete audit cycle. OBJECTIVE: Radical overhaul of the blood ordering practices made by the spinal surgery department, in order to maximize the efficient use of blood products. SUMMARY OF BACKGROUND DATA: Spinal surgeons request blood-based on historical practice and not evidence. Blood is a scarce resource. Provision relies on voluntary donations from the general population. However, the donor population has been reduced because of the risks of variant Creutzfeldt-Jakob disease. Hence, the responsibility rests with clinicians to optimize their management of blood. METHODS:

Airway Safety During Mechanical Ventilation: Survey of ICU Clinicians Practices and Perceptions.

We report results from a survey of members of the Society of Critical Care Medicine to assess ICU clinicians' perceptions of artificial airway safety practices and unplanned extubation (UE) prevention. The survey was distributed between January and February 2024 and received 518 responses (68.5% response rate), with 87.5% from adult ICUs and 12.5% from Pediatric ICUs. Only 48% of adult ICU respondents tracked UE, compared with 73% tracking pressure injuries. Most respondents did not consider UE a "never event," with over half viewing it as unavoidable. In adult ICUs, delirium was ranked as the

Radiologist's Guide to the Ninth Edition TNM Staging System for Quantitative and Qualitative Assessment of Pleural Mesothelioma.

Pleural mesothelioma is a highly lethal, rare thoracic malignant neoplasm arising from mesothelial cells. Previous versions of the clinical TNM staging classification have relied on a qualitative radiologic assessment of tumor invasion. However, these qualitative assessments have not proved practical to implement, with poor interobserver agreement and lack of prognostic significance. The updated ninth edition of the International Association for the Study of Lung Cancer TNM staging system for pleural mesothelioma, jointly published by the American Joint Committee on Cancer and Union for Intern

Reducing Allostatic Load in Depression and Anxiety Disorders: Physical Activity and Yoga Practice as Add-On Therapies.

The allostatic load (AL) index constitutes a useful tool to objectively assess the biological aspects of chronic stress in clinical practice. AL index has been positively correlated with cumulative chronic stress (physical and psychosocial stressors) and with a high risk to develop pathological conditions (e.g., metabolic syndrome, cardiovascular pathology, inflammatory disorders) and the so-called stress-related psychiatric disorders, including anxiety and depressive disorders. Chronic stress has negative effects on brain neuroplasticity, especially on hippocampal neurogenesis and these effec
