PURPOSE OF REVIEW: In this review, we intend to define the main characteristics of the elite controller reservoir, exploring into its heterogeneity, which goes hand in hand with the different progression phenotypes within elite controllers.
RECENT FINDINGS: Show that the viral reservoir measured by high-resolution techniques in elite controllers is heterogeneous, exhibiting distinct characteristics in elite controllers who ultimately lose control compared to those with persistent viral remission. Among the latter, we find exceptional individuals with a reservoir consistent with a phenomenon o

The naming of HIV-1 circulating recombinant forms (CRFs)-descendent viruses from the same intersubtype recombination events, is along with the designation of 'subtypes' and 'groups', routinely used to track HIV-1 diversity. However, we argue that continuing to designate all detected CRFs as distinct entities is biologically unjustified, as many represent recombinants of limited epidemiological significance. Indeed, the mechanistic underpinning of HIV-1 recombination highlights the arbitrary nature of naming these incidental recombinants, the majority of which are rarely detected again. This un

Sex hormones and reproductive transitions profoundly shape immune, metabolic, neuropsychological, and mucosal biology, as well as HIV persistence, across women's lives. Although women constitute the majority of people with HIV worldwide, hormonal and reproductive biology are still not sufficiently incorporated into HIV cure research. Cisgender women, transgender women on gender-affirming hormone therapy (GAHT), and gender-diverse individuals assigned female at birth are underrepresented in cure studies, despite experiencing unique and changing hormonal states that influence estrogen signaling,

Inflammation is a crucial component of the immune response essential for host defense and tissue repair. However, when the immune response becomes dysregulated, it can contribute to the pathogenesis of chronic diseases. While acute inflammation is a short-lived, protective response, chronic inflammation is sustained over time and can lead to immune dysfunction, tissue damage, and disease progression. The chronic inflammation theory of disease suggests that persistent immune activation/inflammation underlies both infectious and non-infectious conditions and serves as a unifying mechanism across

PURPOSE OF REVIEW: Mechanisms of HIV persistence in tissues are distinct from that in the blood. Spatial transcriptomic profiling examines HIV-infected cells, surrounding neighborhoods, and tissue microenvironment in unprecedented resolution. Spatial profiling captures cytokine gradients, distances between HIV-infected cells and immune effectors (and their function versus exhaustion), and cell-cell interactions. We present an overview of spatial transcriptomic platforms and a workflow of quality controls, sanity check, and bioinformatic analysis.
RECENT FINDINGS: The selection of spatial prof

PURPOSE OF REVIEW: While post-treatment control following interruption of standard-of-care antiretroviral therapy (ART) is well described, post-intervention control following immunotherapy in HIV cure-related clinical trials is less well understood. We provide an overview of recent studies that have identified post-intervention controllers and review the mechanisms that may drive this biologically important phenotype.
RECENT FINDINGS: Post-intervention controllers have been identified in recent immunotherapy trials testing broadly neutralizing antibodies, immune modulators, modified T cells,

To date, the only intervention that has cured HIV infection has been bone marrow transplants from HIV-resistant donors to HIV-infected recipients. This approach has been used to both cure hematological malignancies and HIV infection, but it cannot be widely adopted due to the high risk of mortality associated with cell transplants between individuals. To overcome this limitation, several approaches have been developed to generate HIV resistance using gene therapy in an infected individual's own cells. With the growing arsenal of effective methods to generate HIV-resistant cells, a safe and eff

INTRODUCTION: HIV testing and counselling (HTC) interventions are key to controlling the HIV epidemic in East and Southern Africa where HTC is primarily delivered through voluntary counselling and testing (VCT), provider initiated testing and counselling (PITC), and home-based counselling and testing (HBVCT). Decision making processes around uptake of HTC models must be taken into account when designing new interventions. Counselling in HTC aims to reduce post-test risk taking behaviour and to link individuals to care but its efficacy is unclear. This meta-ethnography aims to understand the co

OBJECTIVE: To review the incidence, signs, symptoms, and mechanisms of adverse drug reactions (ADRs) to sulfonamides, anticonvulsants, and antimycobacterial medications among people with HIV.
DATA SOURCES: Searches of MEDLINE/PubMed (1980-November 2005) and National Library of Medicine Meeting Abstracts (1989-November 2005), as well as hand searches of journals and abstracts, were conducted to identify primary literature. Reference lists were reviewed to identify additional relevant reports.

Blood banks have intensified their efforts to discourage donations from individuals at risk for the human immunodeficiency virus (HIV-1). Since the onset of HIV-1 donor screening in April 1985, a marked reduction in seroprevalence has been seen at the authors' institutions: from 51 cases per 100,000 donors in 1985 to 13 per 100,000 in the first 6 months of 1988. Data from 3.5 years have been analyzed for temporal trends in the association of HIV-1 seroprevalence with donation site (urban vs. non-urban) and donor gender. The association of HIV-1 seropositivity with an urban donation site decrea

HIV can take several mutational pathways to become resistant to lenacapavir, each with distinct resistance and fitness profiles (Pennetzdorfer et al., this issue).

The improved efficacy and tolerability of newer antiretroviral drugs, as well as the introduction of long-acting regimens, have prompted more frequent therapy switches in individuals on suppressive antiretroviral therapy (ART). For these individuals, the assessment of HIV drug resistance using DNA from peripheral blood lymphocytes has become increasingly popular. However, compared with HIV RNA-based analyses, implementation of HIV DNA testing as an alternative approach in clinical care requires new documented quality-assessment procedures and clinical validation. Furthermore, the use of HIV DN

BACKGROUND: Lenacapavir (LEN) is a first-in-class inhibitor of human immunodeficiency virus type 1 (HIV-1) capsid function for the treatment of heavily treatment-experienced people with HIV (PWH) harbouring multidrug resistance in combination with an optimized background regimen (OBR). Here, we describe in vitro analysis of the interplay between entry inhibitors (EI; enfuvirtide, fostemsavir, ibalizumab, and maraviroc) susceptibility and LEN susceptibility in samples from 72 participants in the phase 2/3 CAPELLA study, as well as the emergence of resistance in CAPELLA through 52 weeks.

BACKGROUND: Antiretroviral therapy (ART) has led to a decline in human immunodeficiency virus (HIV)-related mortality, but comorbidities, including organ dysfunction, are increasingly the focus of care. Heart transplant (HT) is a very effective therapeutic strategy for end-stage heart failure (HF); however, clinicians may be hesitant due to concerns of complex drug-drug interactions (DDIs) between ART and HT immunosuppressive regimens and the potential impact of ART on long-term HT outcomes. In this report, we describe long-term (76-month) follow-up of a patient with HIV-positive status who un

PURPOSE OF REVIEW: Close to 2 million individuals globally become infected with HIV-1 each year and just over two-thirds will have access to life-prolonging antivirals. However, the rapid development of drug resistance creates challenges, such that generation of more effective therapies is not only warranted but a necessary endeavour. This review discusses a group of HIV-1 entry inhibitors known as CD4 mimics which exploit the highly conserved relationship between the HIV-1 envelope glycoprotein and the receptor, CD4.
RECENT FINDINGS: We review the structure/function guided evolution of these

BACKGROUND: The detection of acute HIV infection (AHI) is vital for timely diagnosis and appropriate management, thus, in vitro diagnostic tests that accurately identify antigen and anti-bodies separately, have a short seroconversion window period, rapid turnaround time are essential. This study aimed to evaluate the analytical and clinical performance of the RapiSafe rapid test, with a particular focus on its efficacy in detecting AHI in serum samples.
METHODS: Antigen sensitivity was evaluated in 1st WHO International Standard for HIV-1 p24 Antigen (NIBSC 22/230) and Reference Panel (NIBSC

HIV testing is the essential entry point for both treatment and prevention. The need to identify acute HIV infection (the period immediately after HIV acquisition, when persons are most infectious) and HIV-2 infection, which does not respond to many first-line antiretroviral agents, poses challenges for the traditional algorithm of Western blot confirmation after a repeatedly reactive antibody screening test. Immunoassays that detect antibodies earlier, tests for HIV RNA, and combination assays that screen simultaneously for both p24 antigen and HIV antibody are now approved for HIV diagnosis

The 2022 annual meeting of the HTLV & HIV-2 Spanish Network was held in Madrid on December 14. We summarize here the main information presented and discussed at the workshop and review time trends for human retroviral infections in Spain. As transmissible agents, infections by human retroviruses are of obligatory declaration. Until the end of 2022, the Spanish national registry had recorded 451 cases of HTLV-1, 821 of HTLV-2, and 416 of HIV-2. For HIV-1, estimates are of 150 000 people currently living with HIV-1 and 60 000 cumulative deaths due to AIDS. During year 2022, new diagnoses in Spai
