Impact of recombinant erythropoietin on anemia, physical function, and cognitive assessment in hospitalized older adults.

Anemia is frequent in older adults. The essential mechanisms underlying its pathogenesis and relationship with clinical outcomes have been investigated. Erythroid-stimulating agents, particularly recombinant erythropoietin (EPO), corrected anemia in older adults and improved their clinical outcomes. Notwithstanding, many questions remain unanswered. The aim of this study was to correlate a few components of the comprehensive geriatric assessment, such as frailty index, time to up and go test, and mini mental state examination with hemoglobin levels (Hb) after treatment with EPO and blood trans

Psychometric properties of the Spanish Multidimensional Fatigue Inventory in people with heart diseases.

BACKGROUND: Fatigue is one of the most limiting symptoms in individuals with heart disease (HD). However, valid and reliable instruments for assessing fatigue in clinical practice still need to be improved. OBJECTIVE: To assess the dimensional structure of the self-reported Spanish Multidimensional Fatigue Inventory (MFI) and analyze its psychometric properties in individuals with HD. METHODS: A longitudinal observational study included 247 participants (age 57.71 years; 23.9 % women; 89.9 % Caucasian). Test-retest reliability was assessed 10-14 days after the first evaluation. Measures eval

A Multimodal Prediction Model for Diagnosing Pulmonary Hypertension in Systemic Sclerosis.

OBJECTIVE: Diagnosis of pulmonary hypertension (PH) in systemic sclerosis (SSc) requires an invasive right heart catheterization (RHC), often based on an elevated estimated pulmonary artery systolic pressure on screening echocardiography. However, because of the poor specificity of echocardiography, a greater number of patients undergo RHC than necessary, exposing patients to potentially avoidable complication risks. The development of improved prediction models for PH in SSc may inform decision-making for RHC in these patients. METHODS: We conducted a retrospective study of 130 patients with

Altered Low Frequency Heart Rate Variability Associated with Agoraphobia in Panic Disorder: A Retrospective Study.

PURPOSE: This study aimed to compare the clinical features of panic disorder (PD) with comorbid agoraphobia to those of PD alone. We focused on autonomic nervous system (ANS) alterations reflected in heart rate variability (HRV) and executive function deficits reflected in the Stroop test. MATERIALS AND METHODS: We retrospectively compared psychometric features, Stroop test results, and resting-state HRV across three groups: a subclinical group with anxiety attack history, a PD group without agoraphobia, and a PD group with agoraphobia. The subclinical group included 10 male and 34 female, th

Autonomic dysfunction and the development of hypertension in patients treated with recombinant human erythropoietin (r-HuEPO).

Hypertension is the most common complication of r-HuEPO therapy in dialysis patients. The aim of this study was to test the hypothesis that hypertension develops in patients who fail to autoregulate adequately their hemodynamic response to correction of anemia. Twenty-five dialysis patients (17-71 yrs, 13 male, 13 CAPD) initially received r-HuEPO 50 U/kg 3 times/week intravenously or subcutaneously. Hypertension, defined as a rise in mean blood pressure (BP) of greater than 15 mmHg during therapy developed in 44% (Group 1: stable BP; Group 2: rise in BP). There was no difference in sex, age, m

Functional and anatomic correlates of markedly abnormal stress tests.

The functional state and coronary anatomy of 120 patients evaluated primarily because of a markedly positive ischemic exercise stress test (greater than 2 mm ST depression) is presented. Twenty-seven patients were asymptomatic (group A), 36 patients (group B) had type I angina (Canadian classification) and 57 patients (group C) had angina with only minor limitations (type II angina). All patients underwent exercise stress testing (Bruce protocol) within 2 months of cardiac catheterization. No significant intergroup differences were observed in exercise variables including time of onset of isch

Diagnostic Utility of NT-proBNP in Tracking Cardiovascular Functional and Structural Changes in Advanced CKD and Post-Transplantation.

INTRODUCTION: NT-proBNP is a critical biomarker in the management of heart failure. However, its role in tracking cardiovascular (CV) function and structure in patients with advanced chronic kidney disease (CKD) and after kidney transplantation remains unclear. Herein, we investigated the association between NT-proBNP with CV function and structure in advanced CKD and longitudinally post-kidney transplantation. METHODS: We assessed 159 stage 5 CKD patients from the Cardiopulmonary Exercise Testing in Renal Failure and After Kidney Transplantation (CAPER) cohort, including kidney transplant re

Machine Learning Reconstruction of Left Ventricular Pressure From Peripheral Waveforms.

BACKGROUND: Left ventricular (LV) pressure measurement is the clinical gold standard for assessing cardiac function; however, its reliance on invasive catheterization limits accessibility and widespread use. OBJECTIVES: This study aimed to develop a cuff-based machine learning (cuff-ML) approach for reconstructing LV pressure from noninvasive brachial waveforms as a bedside assessment of cardiac function. METHODS: Subjects referred for nonemergent left heart catheterization were recruited for LV pressure and brachial cuff waveform measurement. The cuff-ML method was trained using brachial wa

Utility of Heart Rate Variability and Exertional Task Completion During Recovery of Mild Traumatic Brain Injury in Active Duty Service Members.

INTRODUCTION: Primary care exertional testing is recommended for mild traumatic brain injury (mTBI) management in active duty service members (ADSM) before return-to-duty (RTD), yet there are no validated standardized tasks with objective physiological measures. Impairments to the automatic nervous system (ANS) after mTBI may last beyond symptom resolution and impact readiness for duty. Assessment of ANS functioning (heart rate variability, HRV) could provide an objective measure of mTBI recovery. MATERIALS AND METHODS: Eighteen ADSM with mTBI (2 weeks or less post-injury) and 18 age-matched

Prognostic Value of Left Ventricular Global Longitudinal Strain for Major Adverse Cardiovascular Events in Patients with Aortic Valve Disease: A Meta-Analysis.

INTRODUCTION: Valvular heart disease is one of the most common heart diseases. It is characterized by abnormal function or structure of the heart valves. There may be no clinical symptoms in the early stages. Clinical symptoms of arrhythmia, heart failure, or thromboembolic events may occur in the late stages of the disease, such as palpitation after activities, breathing difficulties, fatigue, and so on. Aortic valve disease is a major part of valvular heart disease. The main treatment for aortic valve disease is valve replacement or repair surgery, but it is extremely risky. Therefore, a rig
