Preoperative caudal block prevents emergence agitation in children following sevoflurane anesthesia.

BACKGROUND: The frequency of emergence agitation in children is increased following sevoflurane anesthesia. However, controversies still exist concerning the exact etiology of this postanesthetic problem. Although this phenomenon is present with adequate pain relief or even following pain-free procedures, pain is still regarded as a major contributing factor. METHODS: In a prospective, randomized, double-blind study, we enrolled 48 premedicated and calm 2-6-year-old children undergoing inguinal hernia repair. We assigned children to one of two groups: children assigned to the caudal group (n

Carotid endarterectomy: a retrospective analysis. Microendarterectomy and transcranial Doppler ultrasound monitoring.

BACKGROUND: Analyses of the results obtained following our protocol for the surgical therapy of carotid artery stenosis is the aim of this study. METHODS: Ninety-one procedures were performed on 84 patients by the same surgical team. Indications for carotid endarterectomy (CEA) were 50-70% internal carotid artery (ICA) stenosis in 8% of the patients (100% symptomatic), 70-90% stenosis in 67% (64% symptomatic) and 90-99% ICA stenosis in 25% of our patients (33% symptomatic). Carotid shunt during arterial cross-clamp was only used in 4 cases (4.4%) following trans-cranial Doppler ultrasound (TC

Predictors and clinical outcomes of acute kidney injury after thoracic surgery: a retrospective cohort study.

BACKGROUND: Acute kidney injury (AKI), defined as a rapid decline of renal function, is a frequent complication observed after major surgical procedures. In patients undergoing thoracic surgery, the reported incidence of AKI ranges from 5% to 15%. The development of AKI is associated with an elevated risk of postoperative complications, including the need for reintubation, longer hospital admissions, and higher mortality rates. During one-lung ventilation (OLV), intraoperative hypoxemia and hypercarbia, along with systemic inflammation induced by alveolar over-distension, can contribute to dec

Seeing Beyond the Tract: The Superiority of 3 Tesla (3T) MRI Over X-ray Fistulography in Perianal Fistula Evaluation.

Background Perianal fistulas are a prevalent anorectal condition, often resulting from cryptoglandular infections. They cause significant morbidity due to chronic discharge, pain, and recurrent infections, affecting patient's quality of life. Accurate preoperative imaging is crucial for defining the fistulous tract, identifying internal and external openings, and planning surgical intervention to prevent recurrence. X-ray fistulography is outdated due to its poor visualization of soft tissues and inability to adequately image complex anatomical structures, making it a less preferred method in

Effect of intravenous lidocaine infusion on postoperative pulmonary complications in patients undergoing minimally invasive esophagectomy: a study protocol of double-center, double-blind, randomized controlled trial.

INTRODUCTION: Esophageal cancer is the eighth most prevalent malignant tumor in the world and has the sixth highest mortality rate. Postoperative pulmonary complications (PPCs) are one of the most common complications of minimally invasive esophagectomy (MIE). The non-local anesthetic effects of lidocaine have been widely reported, but only a few studies have focused on its effects on lung protection in MIE. This study is designed to test the hypothesis that intraoperative intravenous lidocaine infusion can reduce the incidence of PPCs in patients undergoing MIE. METHODS AND ANALYSIS: In this

Reducing surgical site infections after spine surgery: the optimal amount of normal saline for intra-wound irrigation.

BACKGROUND CONTEXT: Surgical site infection (SSI) following lumbar surgery can increase healthcare costs and lead to poor clinical outcome. Irrigation of wounds with saline solution is widely accepted globally and safe for nearly all kinds of surgery. However, the efficacy of different volumes of wound irrigation has not been addressed in elective spine surgery. The role and the optimal amount of intraoperative wound saline irrigation in preventing SSI in clean spinal surgery remain unclear. PURPOSE: We aimed to investigate if insufficient intraoperative irrigation may be a risk factor for po

Intraoperative Computed Tomography-Guided Navigation for Pediatric Spine Patients Reduced Return to Operating Room for Screw Malposition Compared With Freehand/Fluoroscopic Techniques.

PURPOSE: Placement of pedicle screws can be performed using freehand/fluoroscopic technique or intraoperative computed tomography (CT)-guided navigation. We sought to compare screw malposition and return to operating room (OR) for pedicle screw malposition for screws placed with and without CT-guided navigation. METHODS: This study was a single-center retrospective comparative study. All patients younger than 18 years with minimum two-year follow-up who underwent pedicle screw instrumentation between 2009 and 2015 were included. Institutional review board approval was obtained and patient cha

Avoiding extinction: The importance of safety and timeouts in the OR.

Safety in the operating room is fundamental to ensuring optimal surgical outcomes. Surgical timeouts, established by the World Health Organization Surgical Safety Checklist and supported by the Joint Commission's Universal Protocol, have played a pivotal role in reducing morbidity and mortality. Originating from aviation safety principles, this standardized cognitive aid demonstrated significant reductions in surgical mortality, infections, and other complications. By formalizing timeouts at critical points-before induction, incision, and departure from the operating room-these checklists coll

External fixator frames as interim damage control for limb injuries: experience in the 2010 Haiti earthquake.

BACKGROUND: An earthquake occurred in Haiti on January 12, 2010. The center of earthquake and the most extensive damage occurred near the capital Port-au-Prince. There were an estimated 230,000 deaths with more than 250,000 others injured. The Israeli Defense Forces Field Hospital (IDF hospital) is a military unit composed of army-recruited (volunteer) medical personnel that was sent to Haiti to serve as a stand-alone center for early response until larger medical missions could become functional and take on the task of more sophisticated and long-lasting medical support. This study describes

Macular Laceration Following Intravitreal Injection in a Nanophthalmic Eye: Management with Pars Plana Vitrectomy.

PURPOSE: To report a case of macular laceration following intravitreal injection in a nanophthalmic eye and its surgical management. METHODS: A nanophthalmic patient presented with visual decline after intravitreal bevacizumab injection. Multimodal imaging demonstrated a full-thickness macular defect, vitreous hemorrhage, and epiretinal membrane (ERM), consistent with suspected needle-induced injury. Following failed medical therapy, 27-gauge pars plana vitrectomy with internal limiting membrane peeling and gas tamponade was performed using modified sclerotomy placement. RESULTS: Surgery was

Commentary on "A pilot study of a digital workflow for navigated tumor bed marking to reduce clinical target volume during adjuvant radiotherapy for oral squamous cell carcinoma".

The accurate identification of the Clinical Target Volume (CTV) remains a challenge in providing postoperative adjuvant radiation therapy for patients with OSCC, due to postoperative changes in anatomy, along with the inability to define the exact location of the CTV during surgery. Holdorf et al. report on a pilot study assessing the utility of a digital navigation intraoperatively to identify and outline the tumor bed to improve planning of postoperative radiation therapy. Using navigated margins traced on imaging postoperatively, the authors demonstrate a reduction in CTV volume compared wi

Cavernous Hemangioma Masquerading as a Ganglion Cyst of the Flexors of the Index Finger: A Case Report.

INTRODUCTION: Vascular lesions, such as cavernous hemangiomas of the finger's flexor tendon sheath are uncommon and can easily be mistaken for more common masses, such as ganglion cysts. Early recognition is important to plan appropriate imaging and excision with correct surgical technique. CASE REPORT: We report a 60-year-old female who presented with a painless, slowly enlarging subcutaneous swelling over the palmar aspect of her right hand. The initial clinical impression was a ganglion cyst of the flexor sheath. Ultrasound suggested a hypoechoic lesion without any internal vascular flow o

Ureteral Preservation Using Fluorescent Ureteral Catheter during Laparoscopic Resection of an Appendiceal Inflammatory Pseudotumor: A Case Report.

INTRODUCTION: When a bulky intra-abdominal tumor cannot be clearly distinguished from a malignancy, careful surgical planning is required to preserve adjacent organs. CASE PRESENTATION: A 31-year-old female presented to our hospital with lower abdominal pain. Contrast-enhanced CT revealed a 67-mm mass in the cecum. Colonoscopy revealed a raised lesion; however, biopsy results were inconclusive. Laparoscopic ileocecal resection with D2 lymphadenectomy was performed because of a suspected malignancy. Given the proximity of the tumor to the right ureter on preoperative imaging, a fluorescent ure

Glottic narrowing secondary to prior mosquito repellent ingestion presenting as a difficult airway during craniovertebral junction surgery.

Airway involvement following chemical ingestion is rare but may pose life-threatening challenges to the anaesthesiologist. We report an Asian woman, in her 50s, with craniovertebral junction anomaly scheduled for posterior instrumentation comprising occiput-bilateral C1-C2 laminar screw and rod fixation along with foramen magnum decompression. Intraoperatively, she developed unexpected glottic narrowing which was later attributed to prior mosquito repellent ingestion which posed significant airway challenges. Initial intubation was uneventful with a video laryngoscope; however, attempts at tra

Robotic-Assisted Anatomical Caudate Lobe Resection with Glissonean Pedicle Approach. A Step-by-Step Surgical Technique (with Video).

Minimally invasive surgery (MIS) has transformed hepatobiliary procedures, providing significant advantages over traditional open techniques. However, challenges remain, especially in major hepatectomies and posterior segment resections due to the inherent limitations of laparoscopy. The caudate lobe (segment I), located deep within the liver and adjacent to critical vascular structures, presents unique technical difficulties, making it one of the most challenging segments to access laparoscopically. Robot-assisted surgery has emerged as a promising alternative, offering enhanced precision and
