[Orthognathic surgery and temporomandibular joint].

Orthognathic surgery changes the jaw position and occlusion, and also affects the original structure and function of the temporomandibular joint (TMJ). With the widespread development of orthognathic surgery, the impact of orthognathic surgery on the structure and function of the TMJ is increasingly valued, and the importance of the TMJ in orthognathic surgery is gradually recognized. Proper understanding the relationship between orthognathic surgery and TMJ not only helps to elucidate how the orthognathic surgery affects the condyle and causes temporomandibular disorders (TMD), but also has s

Temporomandibular Joint: Review of Anatomy and Clinical Implications.

Temporomandibular joints (TMJ) are one of the most complex joints. Each one is located on one side of the face, and are composed of mandibular fossa, joint tubercle, and condylar process of mandible, separated by an articular disk. To these structures are attached ligaments and muscles, which will provide stability and movement. When TMJs work properly, jaw movements can be performed without pain or discomfort. It is important to mention that the complex formed by both TMJs will confront the maxillary with the mandibular bone and therefore will be related to the occlusion, linking these struct

Gnathic Bones and Hyperparathyroidism: A Review on the Metabolic Bony Changes Affecting the Mandible and Maxilla in case of Hyperparathyroidism.

Parathyroid glands secrete the parathyroid hormone that plays an essential role in bone remodeling. Excessive production of parathyroid hormone causes a common metabolic bone disorder known as hyperparathyroidism that is classified into primary, secondary, or tertiary. In hyperparathyroidism, the late bony complication is manifested as a giant cell osteolytic lesion called "brown tumor." Primary hyperparathyroidism is usually a sporadic disorder, but in minority of cases it occurs in inherited forms, and one of these forms is the hyperparathyroidism-jaw tumor syndrome, which is characterized b

Solitary Hard Palate Langerhans Cell Histiocytosis Demonstrated on PET/CT Scan in an Adult.

Langerhans cell histiocytosis (LCH) is rare in adults. Oral and maxillofacial LCH commonly affects the bones of the skull and jaws. Here we report a case of a 57-year-old man with a painless oral mass evaluated by F-FDG PET/CT. PET/CT showed a solitary F-FDG-avid tumor in his left hard palate. LCH was confirmed after surgery. This case reminds us that LCH should be considered in an adult with hypermetabolic hard palate lesion on F-FDG PET/CT.

Adjuvant Alendronic Acid in the Management of Severe Cherubism: A Case Report and Literature Review.

Cherubism is a rare disease of the jaws characterized by bilateral symmetrical painless expansion of the mandible and maxilla. In extreme cases, larger lesions can become exophytic and have profound functional and esthetic implications. Several pharmacologic agents have been trialed in the treatment of cherubism with variable success reported. Bisphosphonates have not been significantly studied in this setting. We present a case where oral alendronic acid was used as an adjuvant treatment after surgical debulking of the maxilla in a 13-year-old boy with a severe case of cherubism.

Evolution and development of the cartilaginous skull: From a lancelet towards a human face.

Chrondrocranium, the cartilaginous skull, is one of the major innovations that underlie evolution of the vertebrate head. Control of the induction and shaping of the cartilage is a key for the formation of the facial bones and largely defines facial shape. The appearance of cartilage in the head enabled many new functions such as protection of central nervous system and sensory structures, support of the feeding apparatus and formation of muscle attachment points ensuring faster and coordinated jaw movements. Here we review the evolution of cartilage in the cranial region and discuss shaping o

Alveolar rhabdomyosarcoma of maxilla.

Rhabdomyosarcoma (RMS), a malignant neoplasm of skeletal muscle origin, is the most common soft tissue sarcoma seen in childhood and adolescence. The most frequent site is the head and neck accounting for 40% of all cases and other involved sites are genitourinary tract, retroperitoneum, and to a lesser extent, the extremities. RMS is relatively uncommon in the oral cavity and the involvement of the jaws is extremely rare. Here, we report a case of 50-year-old female with oral RMS involving maxillary alveolar region with clinical, radiological, histopathological and immunohistochemical finding

Solitary plasmacytoma of the mandible: A rare case report.

Plasmacytoma is a monoclonal, neoplastic proliferation of plasma cells that usually arises within bone marrow or soft tissue sites. It can involve either a single bone (solitary) or multiple bones. Solitary plasmacytoma has a predisposition for the red marrow-containing axial skeleton and is most frequently seen in the thoracic vertebrae, followed by the ribs, sternum, clavicle, or scapula. Its presence in the jaws is extremely rare. We present a case of a 54-year-old female with a well-defined radiolucency of the body region of the mandible later diagnosed as solitary plasmacytoma.

Growth modification in unilateral cleft lip and palate patients with face mask.

The intended aim of treatment in cleft lip and palate patients with growth modification is to improve the relation of jaws by advance the maxilla, restrict the mandible or combination of these. The appliances usually used are face mask or protraction headgear (Delaire and Petit types). Modification of growth is advocated to be applied before the end of adolescent growth spurt and long-term and permanent improvement cannot be guaranteed. Achieving the aim of growth modification is still controversial since most reported results of this treatment are dento-alveolar changes and backward rotation

Dentigerous cyst with an impacted third molar obliterating complete maxillary sinus.

Dentigerous cyst is the most commonly occurring odontogenic cyst after radicular cyst and accounts for 15% of all true cysts in the jaws. The cyst is more common in mandible than in maxilla. The dentigerous cyst is commonly associated with impacted mandibular third molars. In maxilla, the incidence is rare. Dentigerous cyst in maxillary sinus in association with an impacted third molar is an uncommon entity. We present a case of dentigerous cyst associated with an impacted third molar, completely obliterating the maxillary sinus in a 28-year-old female.

Cemento-ossifying fibroma of maxillary antrum in a young female patient.

The cemento-ossifying fibroma is classified as a fibro-osseous lesion of the jaws. It commonly presents as a progressively growing lesion that can attain an enormous size with resultant deformity if left untreated. The cemento-ossifying fibroma is a central neoplasm of bone as well as periodontium which has caused considerable controversy because of controversy regarding terminology and the criteria for its diagnosis. This case report describes a female patient with cemento-ossifying fibroma involving maxillary antrum. The clinical, radiographic and histological features as well as the surgica

Non-syndromic bilateral dentigerous cysts associated with permanent second premolars.

The dentigerous cyst is one of the most common developmental odontogenic cysts in the jaw. Occurrence of the bilateral dentigerous cyst is uncommon, and frequently associated with syndromes like basal cell nevus syndrome or cleidocranial dysplasia. There are few reports on the presence of bilateral dentigerous cyst in nonsyndromic patients, and most of these are associated with first and third molars. The reported case in this paper is bilateral dentigerous cysts associated with mandibular permanent second premolars, in the absence of any signs of syndrome. To our knowledge bilateral dentigero

Management of a locally invasive central giant cell granuloma (CGCG) of mandible: report of an extraordinary large case.

Central Giant Cell Granuloma (CGCG) is a benign tumour of the jaws. The site most frequently involved is the anterior part of mandible, especially in females under 30 years of age. The traditional treatment of CGCG is curettage. Resection is indicated in locally aggressive cases. This report presents the management of a massive CGCG in a 13-year-old girl with its non-surgical and surgical treatments. The non-surgical treatment with corticosteroid therapy was difficult and unsuccessful. Surgical treatment was performed successfully and at 2-year follow-up showed no recurrence of the lesion.

Save-a-tooth: conservative surgical management of dentigerous cyst.

Dentigerous cysts are the most common bony lesions of the jaws in children. It is one of the most prevalent types of odontogenic cysts associated with an erupted or developing tooth, particularly the mandibular third molars; the other teeth that are commonly affected are, in order of frequency, the maxillary canines, the maxillary third molars, and rarely the central incisors. Radiographically, the cyst appears as ovoid well-demarcated unilocular radiolucency with a sclerotic border. Careful evaluation of the history and the clinical and radiographical findings help clinicians to currently dia

Mesiodens.

The presence of an extra supernumerary tooth in the central position of the upper or lower jaw is called mesiodens. These teeth can be present as part of a syndrome or can be found as an isolated finding. Mesiodens is the most frequently found extra tooth, with a prevalence in the general population of 0.15-1.9%, and with a higher frequency in males than females. Usually the shape is conical and mesiodens is smaller than the neighbour incisor. Several hypotheses have been suggested for the formation of supernumerary teeth. Early diagnosis and treatment are needed to support right dental occlus

[Prosthetic treatment of a six-year-old child with hypodontia. A case report].

The prosthetic treatment of young patients with congenital missing of several teeth is a challenge for the dentist. An early prosthetic management with regard to function, phonetics, esthetics is important to ensure a normal psychological development of these children and to promote a normal tooth and jaw development. On the basis of this case report about the prosthetic rehabilitation of a 6-year-old patient with congenital hypodontia of the primary and the permanent dentition, we give an overview of the literature available about this topic. Spectrum of PTCH mutations in Italian nevoid basal cell-carcinoma syndrome patients: identification of thirteen novel alleles.

The nevoid basal cell carcinoma syndrome (NBCCS) is an autosomal dominant genetic disease characterized by numerous basal cell carcinomas, odontogenic keratocysts of the jaws, palmar and plantal pits, skeletal abnormalities, and calcification of the falx cerebri. The gene responsible for this syndrome is the PTCH tumor suppressor gene encoding for the sonic hedgehog receptor. In this paper, we report thirteen novel mutations identified in the first screening of NBCCS patients in Italy. Except for p.T230P and p.F505_L506delinsLR, all the other mutations are predicted to determine a premature tr

[Bilateral anophthalmia associated with further anomalies of the head in German Holstein calves].

Two female calves of the breed German Holsteins showing bilateral anophthalmia and deformations of the jaws such as brachygnathia superior and bilateral cleft of lips and noses, respectively, were born on two different farms. Similar congenital defects could be found neither in the relatives of the affected calves nor in other animals of the same herds. A monogenic autosomal recessive inheritance may have caused bilateral anophthalmia. Chromosomal aberrations could not be detected in the affected calves. The possible environmental causes such as infection by the BVD-virus or oversupply or defi

A new technique for the postmortem detection of tooth-coloured dental restorations.

Identification of unknown bodies is mainly made by dental examination and comparison with accurate dental records. Therefore it is necessary to examine the jaws carefully and to locate every tooth-coloured dental restoration. Overlooking dental fillings can make positive identification impossible. The technique described prepares the natural dental hard tissue by etching with 37% phosphoric acid. In the next step an indicator colours the roughened dental tissue but not the polished restoration material. In this way all 15 tested dental restoration materials could be detected with high sensitiv

[Syndromes 6. Crouzon's disease].

Morbus Crouzon is a syndrome with craniosynostosis. Besides a deviating form of the skull this causes a retropositioned middle third of the face. Therefore pseudo-exophthalmos, strabismus and an Angle Class III relation of the jaws are usually seen. The eyes are especially vulnerable. Since the symptoms are rather symmetrical, a good result of the corrections can usually be achieved. The form of the skull will be corrected in very early childhood, while the deformities of the face are by preference operated after completion of growth. However, different reasons can be found to start correction
