OBJECTIVE: To examine the efficacies and adverse events of low-dose tacrolimus in intractable myasthenia gravis (MG) patients during a long-term follow-up.
METHODS: Tacrolimus was administered at 0.1 mg×kg(-1)×d(-1) to 36 generalized or ocular MG patients at our department from November 2008 to December 2010. The efficacies of tacrolimus were assessed by the myasthenia gravis activities of daily living (MG-ADL) profile and the classification of Myasthenia Gravis Foundation of America (MGFA). And the adverse events of tacrolimus were monitored in each patient.
RESULTS: (1) All patients were f

We present a case of postural hypoxemia with a final diagnosis of myasthenia gravis (MG). A 62-year-old man experienced double vision in his left eye from December 2008 and received a diagnosis of diabetic neuropathy. From mid-December he began to experience breathing difficulties at night when in a supine position and was admitted to our hospital. Bilateral diaphragmatic elevation was observed on a chest X-ray film, and lower lung atelectasis and an anterior mediastinal tumor were observed on chest CT. However, his breathing difficulties only occurred when he was in a supine position. Therefo

We report a case of a patient with myasthenia gravis who presented with an atypically focal distribution of severe weakness in the triceps brachii and tibialis anterior muscles. Repetitive nerve stimulation studies demonstrated the typical decrement at rest, but also demonstrated marked facilitation after brief exercise. This case adds to the clinical and electrophysiological spectrum of variations of presentation of myasthenia gravis.

Hepatitis C virus (HCV) infection is a growing international health problem, and more than 170 million people are chronic carriers. Up to 50% of HCV-positive patients develop at least one extrahepatic manifestation during the course of disease. To varying degrees of certainty, there is evidence of an association between chronic HCV infection and a variety of neuromuscular diseases. The pathogenesis of most extrahepatic diseases remains unclear but possibly includes HCV lymphotropism and/or HCV-induced autoantibodies. The therapeutic approach to HCV-associated autoimmune disorders entails eradi

OBJECTIVE: To compare the short-term financial costs of treating a patient in myasthenia gravis crisis with intravenous immunoglobulin (IVIG) versus plasma exchange.
METHODS: An itemized comparative cost-minimization analysis of IVIG versus plasma exchange for myasthenia gravis crisis was performed. Calculations were based on each therapy's implementation cost, associated hospitalization times, and predicted cost to treat known complications. A cost superiority determination was proposed based on the total cost profile of each therapy.
RESULTS: The difference in total cost favored IVIG over

The initial presentation of myasthenia gravis as trismus is very rare and no previous reports have been found in the literature. A 35-year-old male presented to the outpatient unit of our department with inability to clench well and to open his mouth. Physical examination revealed that he had clinical findings consistent with the signs and symptoms of myasthenia gravis. He was immediately referred to a neurologist, who confirmed that he was in an advanced stage of myasthenia gravis with severe deficit to his respiratory muscles and he was promptly treated. He is presently on a maintenance drug

The role of antibodies to a fragment of neuronal acetylcholine receptor was studied by EIA in patients with myasthenia. Antibody levels were significantly higher in patients with generalized myasthenia. Enzyme immunoassay of antibodies by the reaction with acetylcholine receptor fragment can serve as an additional method for studies of autoimmune myasthenia pathogenesis.

In 12 patients with myasthenia, the content of β(2)-adrenoreceptors on the cell surface and activity of intracellular lymphocytic enzymes were determined by EIA and biochemical methods, respectively. In comparison with the normal, these patients demonstrated pronounced elevation in the content of β(2)-adrenoreceptors and significant changes in activity of lymphocytic enzymes. In 10 of 12 patients, administration of the agonists to β(2)-adrenoreceptors resulted in health improvement accompanied by normalization of EMG and immunobiochemical indices. Our findings suggest that intra- and intercell

OBJECTIVES: To determine whether patients with myasthenia gravis (MG) have serum antibodies to lipoprotein-related protein 4 (LRP4), a newly identified receptor for agrin that is essential for neuromuscular junction formation, and to establish whether such antibodies contribute to MG pathogenesis.
DESIGN: Serum samples from patients with MG with known status of serum antibodies to the acetylcholine receptor (AChR) and muscle-specific kinase (MuSK) and serum samples from control subjects (healthy individuals and individuals with other diseases) were tested for antibodies to LRP4. Serum samples

BACKGROUND: Soluble forms of CD28 (sCD28) and CTLA-4 (sCTLA-4) were associated with many autoimmune diseases like Sjögren's syndrome, systemic lupus erythematosus, asthma, and autoimmune myasthenia gravis. However, sCD28 and sCTLA-4 in neuromyelitis optica (NMO) and multiple sclerosis (MS) patients were less studied.
OBJECTIVE: To measure the plasma sCD28, sCTLA-4 in NMO and MS patients, and investigate whether sCD28 and sCTLA-4 possible use as sensitive biomarkers for diseases activity.
METHODS: Plasma concentrations of sCD28, sCTLA-4 were measured by an enzyme-linked immunosorbent assay (E

OBJECTIVE: To evaluate cardiac autonomic control in patients with myasthenia gravis (MG) and thymoma.
MATERIALS AND METHODS: The study was performed on 21 patients with MG and thymoma and the same number of matched healthy volunteers. Standard cardiovascular reflex tests according to Ewing and baroreflex sensitivity (BRS) at rest was applied. Spectral analysis of heart rate variability (HRV) at rest was assessed using a 20-minute ECG recording (normalized low- and high-frequency bands-LFnu-RRI, HFnu-RRI and LF/HF-RRI) Time-domain analysis of HRV was derived from 24-hour ECG monitoring.

PURPOSE OF REVIEW: The aim is to review the most relevant findings published during the last year concerning clinical, genetic, pathogenic, and therapeutic advances in motor neuron disease, neuropathies, and neuromuscular junction disorders.
RECENT FINDINGS: Studies on animal and cell models have improved the understanding of how mutated survival motor neuron protein in spinal muscular atrophy governs the pathogenetic processes. New phenotypes of SOD1 mutations have been described. Moreover, animal models enhanced the insight into the pathogenetic background of sporadic and familial amyotroph

Myasthenia gravis is a neuromuscular disorder, and the severe complication of myasthenic crisis is a life-threatening condition, usually induced by stress such as fever, trauma or a surgical procedure. Simultaneous heart surgery and thymectomy in a patient with concomitant heart disease and myasthenia gravis has been previously reported. We report a female patient with rheumatic valvular disease and myasthenia gravis who received triple heart valve surgery and thymectomy simultaneously. In her early recovery, two episodes of myasthenic crisis occurred which were treated promptly with a success
