Upper airway and soft tissue structural changes induced by CPAP in normal subjects.

Nasal continuous positive airway pressure (CPAP) is the treatment of choice for adults with obstructive sleep apnea. CPAP is known to increase upper airway size; however, the direct effects of CPAP on soft tissue structures surrounding the upper airway are less well understood. Magnetic resonance imaging was used to study the effect of incremental levels (0, 5, 10, and 15 cm H2O) of CPAP on the upper airway and surrounding soft tissue structures in 10 normal subjects. Progressive increases in CPAP resulted in the following major findings: (1) airway volume and airway area (measured at several

Critical analysis of the results of surgery in the treatment of snoring, upper airway resistance syndrome (UARS), and obstructive sleep apnea (OSA).

Surgery is widely considered to be the first-line therapy for "simple" snoring and moderate sleep apnea syndrome. Surgical treatment of severe obstructive sleep apnea (OSA) is currently generally considered to be second-line therapy after continuous positive airway pressure (CPAP) treatment. Many patients, however, find difficulty in complying with the continued long-term demands of CPAP treatment and seek a more definitive one-off solution to their sleep-related problem. Surgical therapy has been found useful for snoring, at least on subjective grounds; this has incited patient demand and led

Dynamic upper airway imaging during awake respiration in normal subjects and patients with sleep disordered breathing.

The effects of respiration on upper airway caliber were studied using cine computed tomography (CT) in 15 normal subjects, 14 snorer/mildly apneic subjects, and 13 patients with obstructive sleep apnea. All subjects were scanned in the supine position during awake nasal breathing. Eight-millimeter-thick axial slices were obtained at four anatomic levels from the nasopharynx to the retroglossal region every 0.4 s during a respiratory cycle. Tidal volume measured from an integrated pneumotachograph signal was correlated with slice acquisition during inspiration and expiration to generate loops c

Obstructive sleep apnea treatment: peripheral and central effects on plasma renin activity and aldosterone.

To assess the effect of obstructive sleep apnea treatment on plasma renin activity (PRA) and plasma aldosterone seven male patients were studied under two conditions: untreated and treated with nasal continuous positive airways pressure (CPAP). PRA and plasma aldosterone were measured at 10-min intervals for both nights. CPAP treatment diminished the urinary and Na+ excretion, whereas plasma volume increased. The mean levels of PRA and aldosterone were significantly enhanced by the treatment, increasing respectively from 1.5 +/- 0.3 to 3.0 +/- 0.7 ngAI ml-1.hr-1 (p less than 0.05) and from 8.0

[Snoring: physiopathology, surgical treatment and a modified uvulo-palato-pharyngoplasty].

Sleep apnea, now recognized as a specific clinicopathological entity, had received little attention until late 1960 when described by Gastaud. Since that time, following the works of Ikematsu, Guillemineault and Fujita, there has been rapid extension in the knowledge and understanding of sleep problems and associated surgery. Snoring, whether part of the sleep apnea syndrome or alone, may be explained with the help of an aerodynamic model comparing the pharynx to a Starling resistance. The factors nasal obstruction, abnormal mass in the upper respiratory airways and transmural resistance are d

Skin blood flow changes during apneic spells in preterm infants.

Changes in skin blood flow during apneic spells were determined in 18 preterm infants using a diode laser Doppler flow meter without light conducting fibres. Heart rate, nasal air flow, impedance pneumography, skin and incubator temperature and laser Doppler skin blood flow were recorded simultaneously in each infant. During 212 apneic spells with a duration of 11.6 +/- 7.5 s (mean +/- S.D.) (range 6.0-48.0 s), the laser Doppler skin blood flow was measured. In all children except one, the majority of the apneic spells was associated with a decrease in skin blood flow. During 155 apneic spells

Clinical complications of obesity.

The development of important respiratory disorders and significant hypertension in association with increasing body weight is not widely recognized. Altered respiratory function results from a combination of mechanical impedance to breathing exerted by thoracic and abdominal fat and a ventilation-perfusion mismatch. Sleep-disordered breathing with periods of hypoventilation, with or without apnoeic episodes, may commonly occur in patients with extreme obesity. Nocturnal hypercapnia and hypoxia in such patients may lead to a decrease in ventilatory drive, abnormal central respiratory control an

Respiratory pauses in normal prematurely born infants. A comparison with full-term newborns.

To assess the influence of gestational age (GA) and sleep states on respiratory pauses (RP), apnea index (AI) and periodic breathing (PB), we made polygraphic recordings on 40 normal newborns with GA of 31-41 weeks. Only 2 out of a total of 2,406 RP were obstructive. We never recorded greater than 15-second duration central RP. 10-15 s RP were rare, 2% of the total number observed in newborns with a GA of 31-38 weeks. Selective flattening of thoracic respiratory movements (while abdominal and nasal respirations persisted) were observed in all age-groups. In all GA groups, RP frequency, AI and

Craniofacial Ossifying Fibromas in Children: Clinical Variability and Surgical Outcomes in a Case Series.

Juvenile ossifying fibroma (JOF) is a rare benign fibro-osseous tumor affecting the craniofacial skeleton in children and adolescents. Although nonmalignant, it may behave aggressively and extend into adjacent structures such as the orbit or anterior skull base. Early symptoms are often subtle, delaying diagnosis. Two histologic variants exist - trabecular and psammomatoid - each with distinct clinical behavior and recurrence potential. We describe a case series of three pediatric patients (an 8-year-old male, a 13-year-old female, and a 16-year-old male) diagnosed with craniofacial JOF betwee

Bone-Preserving Management of Maxillary Cemento-Ossifying Fibroma With Orbital Floor Thinning: A Case Report.

Cemento-ossifying fibroma (COF) is a benign fibro-osseous neoplasm that most often affects the mandible and occurs predominantly in women during the third and fourth decades of life. Maxillary involvement is less common and may remain minimally symptomatic while expanding into the maxillary sinus. We present a 26-year-old woman with a six-month history of progressive right midface enlargement without pain, nasal obstruction, diplopia, visual symptoms, or dental mobility. Computed tomography demonstrated a well-circumscribed expansile right maxillary lesion measuring 44.9 × 42.5 × 31.9 mm, occu

Dynamic Supraglottic Airway Collapse Diagnosed Using Airway Point-of-care Ultrasound: A Case Report.

INTRODUCTION: Acquired idiopathic laryngomalacia causing dynamic airway collapse is rare in adolescents and may be overlooked because it mimics more common causes of acute dyspnoea and stridor in the emergency department (ED). Airway point-of-care ultrasound (POCUS) provides a rapid, non-invasive means to visualize dynamic supraglottic obstruction when laryngoscopy is not immediately feasible or tolerated. CASE REPORT: A 13-year-old boy presented with sudden-onset respiratory distress and dyspnoea that worsened on lying flat and improved when sitting upright, without history of fever, trauma,

Application of Fixed Implant Superstructures Following Multistage Maxillary Reconstruction in Osteosarcoma Patients: A Case Report.

Extensive resections of the maxillary bone often result in significant defects that affect oral functions, such as speech and chewing. Although dentomaxillary prostheses are common, they frequently lead to instability and reduced chewing ability. Vascularized bone grafts, including the fibula, are increasingly used to address these challenges due to their anatomical suitability and functional restoration benefits. Despite advances, problems remain, including insufficient bone height for stable implantation. A 60-year-old Japanese woman presented with swelling of the right maxilla and nasal obs

Increased LEF1 protein levels and isoform switching drive cell proliferation in chronic lymphocytic leukemia.

INTRODUCTION: Whooping cough (pertussis) is an acute respiratory infection primarily caused by Bordetella pertussis, which predominantly affects children. While the disease commonly presents with upper respiratory symptoms, it can lead to severe complications such as pneumonia, encephalopathy, and even death. The association between respiratory infections and renal injury, specifically acute interstitial nephritis (AIN), remains poorly understood. This report describes a rare case of AIN in a patient with whooping cough. CASE PRESENTATIONS: A 73-year-old male with a history of systemic arteri

Evaluation of Airway Changes Following Isolated Unilateral mandibular Fracture: A Prospective Quasi-Experimental.

BACKGROUND: Maxillofacial trauma presents unique airway management challenges due to anatomical disruptions, soft tissue compromise, edema, hemorrhage, and emesis. While isolated mandibular fractures are typically non-emergent, specific fracture patterns can precipitate airway obstruction secondary to bony fragment displacement. There is a paucity of data on airway changes after unilateral mandibular fractures. This prompted us to conduct the present study. METHODS: This prospective, quasi-experimental study enrolled patients with isolated unilateral mandibular fractures requiring surgical re

Detection of Aberrant Carotid Arteries Prior to an Adenoidectomy: A Case Report.

Aberrant carotid arteries are rare vascular anomalies that can significantly complicate head and neck surgeries, particularly in pediatric patients. These anomalies may be asymptomatic and are often discovered incidentally on imaging studies performed for unrelated conditions. The failure to recognize these anomalies preoperatively can result in life-threatening complications, such as catastrophic hemorrhage. This case report highlights the critical importance of thorough imaging and cautious surgical planning in preventing such outcomes. A female pediatric patient presented with nasal obstruc
