Quantification of relative afferent pupillary defects induced by posterior sub-Tenon's, peribulbar, and retrobulbar anaesthetics.

AIMS: The effect of local anaesthetics on optic nerve function can be investigated by quantifying the relative afferent pupillary defect (RAPD). METHODS: The study compared the depth of induced RAPD following posterior sub-Tenon's, retrobulbar, and peribulbar local anaesthetics using crossed polarising filters before cataract surgery (time 1 = 5 minutes), immediately after surgery (time 2 = 42 minutes (av)), and once again on the ward (time 3 = 107 minutes (av)). RESULTS: All patients developed a RAPD. There was no significant difference in the depth of RAPD between the groups at any one tim

Size of glaucomatous optic discs.

In normal eyes, the optic disc size shows a high interindividual variability. In the diseased eye, some optic nerve anomalies and diseases occur preferentially in small optic discs, and some are more common in large optic nerve heads. We conducted a study to determine whether glaucoma subtypes are correlated with the optic disc size or not. Stereo optic disc photographs of 997 eyes with primary open-angle glaucoma, 44 eyes with normal-tension glaucoma, 129 eyes with pseudoexfoliative glaucoma, and 850 normal eyes were examined. Compared to eyes with primary open-angle glaucoma (mean optic disc

Vascular factor in erectile failure among diabetics.

Clinical, sexual, and laboratory evaluations were made on 47 impotent diabetics. All patients were given a Doppler PAPS examination and a neurologic assessment of the penis (BCRL and/or CMG). Nerve conductivity tests of the legs were also administered to most patients. The groups were classified as to whether they were insulin or noninsulin dependent. Abnormalities of penile arterial flow were detected in 95.7 per cent of all the patients. Penile arterial occlusions without associated penile neuropathies occurred in 61.7 per cent. Penile neuropathies were found in 34 per cent, and all were ass

Progressive extrinsic ophthalmoplegia with peripheral neuropathy and storage of muscle glycogen.

A case of progressive extrinsic ophthalmoplegia associated with peripheral neuropathy and hypertriglyceridemia type IV is described. Motor and sensory conduction velocities of the spinal nerves were severely decreased, while the EMG of the facial muscles was more suggested of a myopathic disorder. Electron microscopic study of biopsies of biceps and peroneus brevis muscles disclosed many ragged red fibers, mainly type I, which contained typical abnormal mitochondria. Other fibers, all type II, contained increased amounts of glycogen between myofibrils or beneath the sarcolemmal membrane, but t

Vagal nerve stimulator-associated sleep disordered breathing secondary to vagal-induced laryngospasm in pediatric populations: Case presentation and systematic review.

OBJECTIVES: Sleep disordered breathing (SDB) is a well-documented complication of vagus nerve stimulation (VNS) in the literature. Yet, a formal consensus on its management has not been established, particularly in the pediatric population. This study aims to evaluate the current literature on VNS-associated SDB in order to further characterize its presentation, pathogenesis, diagnosis, and treatment. METHODS: A literature review from 2001 to November 8, 2021 was conducted to search for studies on SDB during vagal nerve stimulation in pediatric populations. RESULTS: Of 277 studies screened,

Influence of psychological variables on the diagnosis of facet joint involvement in chronic spinal pain.

BACKGROUND: Facet or zygapophysial joint pain is one of the common conditions responsible for chronic spinal pain. Controlled diagnostic blocks are considered the only means of reliable diagnosis of facet joint pain, due to the inability of physical examination, clinical symptoms, radiologic evaluation, and nerve conduction studies to provide a reliable diagnosis. The prevalence of facet joint pain has been established to be 15% to 45% of patients with low back pain, 39% to 67% of patients with neck pain, and 34% to 48% of patients with thoracic pain. However, using only a single block, false-

Protocol for a pragmatic randomized controlled trial evaluating a care pathway incorporating ultrasound-guided platelet-rich plasma injection plus physiotherapy versus physiotherapy alone in patients with L5/S1 lumbar disc herniation.

BACKGROUND: Lumbar disc herniation (LDH) causes low back pain and lower-limb neurological symptoms. Conservative and surgical treatments have similar outcomes and recurrence rates within two years. Anti-inflammatory drugs or opioids reduce inflammation but are unsuitable for long-term use. Physiotherapy improves mobility and function, but has limited evidence for reversing disc herniation. Platelet-rich plasma (PRP) is emerging as a regenerative therapy for LDH. Combining ultrasound-guided PRP with McKenzie therapy may enhance disc repositioning, tissue repair, and sustained recovery. Same same, but different? The neurological presentation of wildtype transthyretin (ATTRwt) amyloidosis.

BACKGROUND: Both hereditary transthyretin (ATTRv) amyloidosis and wildtype transthyretin (ATTRwt) amyloidosis can be associated with neurological diseases such as carpal tunnel syndrome and polyneuropathy. While ATTRv amyloidosis has been extensively studied, to date little is known about neurological complications of ATTRwt amyloidosis. In particular, the prevalence, pattern and extent of polyneuropathy and autonomic dysfunction has not been adequately investigated in the context of ATTRwt amyloidosis. To tackle this issue, we aimed to characterise the neurological presentation of ATTRwt amyl

Neurologic and histologic outcome after intraneural injections of lidocaine in canine sciatic nerves.

BACKGROUND: Inadvertent intraneural injection of local anesthetics may result in neurologic injury. We hypothesized that an intraneural injection may be associated with higher injection pressures and an increase in the risk of neurologic injury. METHODS: The study was conducted in accordance with the principles of laboratory animal care, and was approved by the Laboratory Animal Care and Use Committee. Fifteen dogs of mixed breed (16-21 kg) were studied. After general endotracheal anesthesia, the sciatic nerves (n= 30) were exposed bilaterally. Under direct vision, a 25-gauge, long-beveled ne

An endogenous cholinergic system controls electrical conduction in the heart.

BACKGROUND AND AIMS: The cholinergic system is distributed in the nervous system, mediating electrical conduction through acetylcholine (ACh). This study aims to identify whether the heart possesses an intact endogenous cholinergic system and to explore its electrophysiological functions and relationship with arrhythmias in both humans and animals. METHODS: The components of the heart's endogenous cholinergic system were identified by a combination of multiple molecular cell biology techniques. The relationship of this system with cardiac electrical conduction and arrhythmias was analysed thr

Engineered neural tissue made using clinical-grade human neural stem cells supports regeneration in a long gap peripheral nerve injury model.

A surgical autograft remains the clinical gold-standard therapy for gap repair following peripheral nerve injury, however, challenges remain with achieving full recovery and reducing donor-site morbidity. Engineered Neural Tissue (EngNT) manufactured using differentiated CTX0E03 human stem cells (EngNT-CTX) has been developed as a potential 'off the shelf' allogeneic autograft replacement. Ensheathed within a collagen membrane developed to facilitate biomechanical integration, EngNT-CTX was used to bridge a critical-length (15 mm) sciatic nerve gap injury in athymic nude rats. The effectivenes

Intereye Retinal Asymmetry, Incident Cardiovascular Disease, and Circulating Proteomic Signatures.

BACKGROUND: Retinal imaging provides a noninvasive window into systemic microvascular health. Many optical coherence tomography studies of cardiovascular disease have focused on mean retinal thickness, which may be influenced by demographic and ocular factors. Whether intereye retinal asymmetry provides additional prognostic information beyond mean retinal thickness remains unclear. METHODS: We conducted a prospective cohort study in the UK Biobank including 46 836 participants with bilateral optical coherence tomography imaging. Across the 4 end point-specific analyses, the maximum follow-up
