Pericardial release for early LVAD malalignment a less invasive approach.

In a previously well-functioning LVAD, pericardial release via thoracotomy may improve inflow angle and correct malpositioning to ultimately restore LVAD function and patient hemodynamics. To prevent this, we recommend that implantations include a longitudinal pericardiotomy and anchoring sutures.

Reverse Knot Technique for Face Lifting With Polydioxanone Thread.

BACKGROUND: The use of polydioxanone thread has grown substantially for facial soft-tissue repositioning and collagen biostimulation, with a well-established safety profile and favorable tissue integration. AIMS: To describe the reverse knot technique, a novel method for the implantation of polydioxanone suspension threads. PATIENTS/METHODS: We present an original polydioxanone thread-lifting technique designed primarily for patients with a "heavy face." The reverse knot technique positions the knot in the anterior portion of the face, below the zygomatic cutaneous ligament, whereas anchorin

Biomechanical characteristics of different methods of neo-chordal fixation to the papillary muscles.

BACKGROUND AND AIM OF THE STUDY: Several techniques have been described for neo-chordal fixation to the papillary muscles without any reported clinical differences. The objective of this study is to compare in vitro the biomechanical properties of four of these common techniques. METHODS: We studied the biomechanical properties of expanded polytetrafluoroethylene neo-chordal fixation using four techniques: nonknotted simple stitch, nonknotted figure-of-eight stitch, knotted pledgeted mattress stitch, and knotted pledgeted stitch using commercially available prefabricated loops. Neo-chordae we

Twofold technique for iridodialysis repair.

BACKGROUND: Disinsertion of iris leads to debilitating sequelae like diplopia and glare. Hence, iridodialysis repair is essential to optimize the visual quality. Iris base repair often leads to corectopia that necessitates an additional procedure to optimize the pupil shape and size. Twofold technique helps to achieve both the aspects. PURPOSE: To highlight the technique of twofold iridodialysis repair. SYNOPSIS: The video highlights the method of twofold iridodialysis repair, wherein nonappositional repair is followed by single-pass four-throw (SFT) pupilloplasty that allows adequate closur

Iridodialysis repair - A simplified approach.

BACKGROUND: Iris root is the thinnest and weakest portion of the iris stroma. It can detach easily due to blunt trauma or accidental engagement of the iris during intraocular surgery resulting in glare, photophobia and monocular diplopia. Multiple techniques described for iridodialysis repair such as hang back technique, stroke and dock technique and sewing machine technique are technically challenging. PURPOSE: To describe an simplified approach of iridodialysis repair using 9-0 prolene suture. SYNOPSIS: We demonstrate the technique of iridodialysis repair using animation for better underst

Nonsyndromic craniosynostosis: current treatment options.

The significance and etiology of abnormal skull shape have been under investigation since ancient times. Nonsyndromic, or isolated, craniosynostosis predominates and is defined as suture fusion that creates functional impairments related to local effects of the fusion. The purpose of this article is to present our current approach to patients with nonsyndromic craniosynostosis, outlining the place of both open, conventional approaches and newer, minimally invasive, endoscopic assisted craniosynostosis correction.

[The Indian pouch as an orthotopic reservoir].

Patients undergoing this procedure should be strongly motivated and should have normal or near-normal bowel function. A reservoir is made using an 18-20 cm segment of terminal ileum, caecum and ascending colon. The ureters are implanted along the taenia of the ascending colon. The result is a neo-bladder that is close to the ideal ellipsoid shape. The stapling technique used decreases operating time and the detubularisation maintains low pressure in the neo-bladder. Although the operation has so far been performed on only 10 patients results have been very favourable and it has proved to be a

Abnormal Cranial Shape Preceding Radiographic Evidence of Craniosynostosis.

Premature fusion of a cranial suture is known to result in characteristic changes in the head shape, even when the synostosis involves only part of the suture. We report an unusual case of a patient seen at the age of 2 months for an abnormal head shape that was present at birth. The phenotype was consistent with an isolated fusion of the frontosphenoidal suture, but the suture was open on a high-resolution computed tomography scan finding. There was no improvement in cranial form after 6 months of helmet therapy, and a follow-up computed tomography scan image taken at age 10 months showed the

[Severe cartilage loss caused by metallic anchors in surgical treatment of a Bankart lesion: report of three cases].

Currently, arthroscopic modalities in the surgical treatment of shoulder instability using suture anchors are more popular than open surgery. However, there are some complications related to the metallic suture anchors used. One of the most common complications is cartilage loss due to shallow placement of the suture anchor. Herein, we report three cases with severe cartilage loss of the humeral head due to metallic proud anchors, including one of whom requiring total shoulder arthroplasty. In conclusion, it is essential to place the suture anchors in an appropriate position and deepness. In c

Arthroscopic hybrid fixation of a tibial eminence fracture in children.

The treatment of an anterior cruciate ligament (ACL) avulsion fracture is controversial, especially in skeletally immature patients, because of concerns about physeal damage. To reduce the risk of physeal injury, an arthroscopic technique was performed. A bioabsorbable suture anchor was inserted through anteromedial portals and fixed to a bioabsorbable suture anchor at the center of the fracture bed; it was then passed through the threads at the ACL avulsion fragment and tied with the ACL substance. After this, the avulsion fragment was repaired by an all-inside technique between the distal po

Arthroscopic all-inside repair for a tear of posterior root of the medial meniscus: a technical note.

This technical note describes a new arthroscopic technique to repair a tear of posterior root of the medial meniscus. Cartilage at the insertion area of the posterior horn of the medial meniscus (PHMM) was removed using a curved curette inserted through an anteromedial portal. A metal anchor loaded with two FiberWires (Arthrex, Naples, FL) was placed at the insertion area of the PHMM through a high posteromedial portal. A PDS suture was passed the PHMM by curved suture hook through the anteromedial portal. Two limbs of the PDS were then used to pass two limbs of the FiberWire through the menis

Simple arthroscopic technique for rotator interval closure.

The rotator interval is a recognized source of pathology; there are several different techniques described for its closure but all require special instruments and skills. We present a technique that is simple in terms of skills and instrumentation. Two spinal needles are used. One needle is used as a suture retriever and the other as a suture passer. Each needle is passed through the margins of the rotator interval and, under arthroscopic vision, the needles are approximated to pass and retrieve the suture. Several sutures can be used as required. The knots are finally tied in an outside-insid

[Internal ligatures of the pyloro-duodenal region after surgical treatment of bleeding ulcers].

1886 fibrogastroduodenoscopic examinations were performed in 820 patients within 7 years after operative treatment of bleeding pyloroduodenal ulcers. Catgut was used in 589 patients for internal sutures in pyloroplasty, the unresolving suture material--capron--was used in 221 patients. Clinico-endoscopic investigations have shown that the application of catgut is followed in the early postoperative period by the development of inflammatory reactions in the tissues along the suture line, while the use of the unresolving suture material is followed by inflammations of less degree, the rejection

Dolichocephaly without sagittal craniosynostosis.

Premature closure of the sagittal suture is thought to be fundamental to the etiopathology for the disease process called sagittal craniosynostosis. This process traditionally results in a well-known skull malformation termed dolichocephaly. Over recent decades, some authors have questioned the suture as the primary driving force for this pathology. This is a retrospective cases series of 4 patients from 2011 to 2012 who presented to the Yale Craniofacial Clinic for evaluation of dolichocephaly. The 4 children, 3 boys and 1 girl, had physical examinations and cranial indices concerning for sag

[Postoperative refraction and anterior chamber depth after complicated cataract operation with vitreous loss and transscleral posterior chamber lens fixation].

The accuracy of preoperative biometry was tested in patients with transscleral suture fixation of the posterior chamber lens. Postoperative refraction and anterior chamber depth of 34 eyes that had been subjected to complicated cataract extraction with vitreous loss were compared with those in 17 fellow-eyes in which uncomplicated cataract extraction had been performed. The anterior chamber depth was significantly deeper after transscleral suture fixation of the posterior chamber lens (mean 0.59 mm, P < 0.05). The refraction differed by +0.68 D between transscleral suture fixation and the cont
