Time to initial glycopeptide therapy and 30-day mortality in methicillin-resistant Staphylococcus aureus bacteremia: a retrospective cohort study.

BACKGROUND: The optimal time cut-off for appropriate antibiotic therapy (AAT) in methicillin-resistant Staphylococcus aureus (MRSA) bacteremia remains uncertain. We assessed the effects of time to AAT on 30-day in-hospital mortality in patients with MRSA bacteremia who received glycopeptides as initial therapy, and applied landmark analyses to adjust for immortal-time bias. METHODS: We conducted a retrospective cohort study of adults with MRSA bacteremia who received an initial course of glycopeptide therapy at two university-affiliated hospitals between 2018 and 2023. Evaluation of gastroesophageal reflux in pediatric laparoscopic appendectomy procedures.

BACKGROUND: The increased intra-abdominal pressure during laparoscopic surgical procedures was reported to be a factor in the development of gastroesophageal reflux. This study evaluated the presence of gastroesophageal reflux and associated factors using 24-h pH monitoring in children undergoing laparoscopic appendectomy. METHODS: Children who underwent laparoscopic surgery for presumed acute appendicitis between June 2017 and June 2018 were included in the study. After pre-operative endotracheal intubation, pH catheters were placed for 24-h esophageal pH monitoring. Relationships between ga

Reiterative survival analyses of total therapy 2 for multiple myeloma elucidate follow-up time dependency of prognostic variables and treatment arms.

PurposeIn Total Therapy 2, after randomly assigning 323 patients with myeloma to thalidomide and 345 to a control arm, no difference was observed in overall survival, with a median follow-up of 42 months, although at 72 months, survival was superior on the thalidomide arm in the one third exhibiting cytogenetic abnormalities (CA). After further follow-up of 87 months, we examined, in reiterative analyses, the effect of increasing time intervals on clinical outcomes relevant to baseline prognostic variables and treatment randomization.Patients and methodsWe investigated clinical trial end point

[Psychosocial factors and long-term adherence to lifestyle interventions in individuals with type 2 diabetes mellitus].

Type 2 diabetes (T2D) is a complex chronic illness that requires continuous management across multiple domains: nutrition, physical activity, psychological wellbeing and pharmacological therapy. The long-term success of these interventions depends on whether they can be maintained over time. This article discusses systemic, psychosocial and lifestyle factors that influence adherence to dietary lifestyle interventions and their implications for routine clinical care. Current guidelines are patient-focused, emphasizing individualization and adaptation to patients' needs within a multidisciplinar

Higher Risk of Post-Endoscopic Retrograde Cholangiopancreatography Pancreatitis in Patients With Asymptomatic Common Bile Duct Stones Compared to Symptomatic Cases: A Propensity Score-Matched Cohort Analysis.

BACKGROUND AND AIMS: Endoscopic retrograde cholangiopancreatography (ERCP), the standard procedure for common bile duct (CBD) stones (CBDS) removal, carries a risk of post-ERCP pancreatitis (PEP). We compared PEP-related factors between asymptomatic and symptomatic CBDS cases. METHODS: A retrospective cohort study was conducted on 367 patients who underwent ERCP for CBDS. To adjust for baseline differences, propensity score matching (PSM) was performed, yielding 170 matched patients (85 asymptomatic and 85 symptomatic groups) for comparative analysis of PEP-related factors. RESULTS: Baseline

Risk of Ixodes ricinus Bites in a Population of Forestry Workers in an Endemic Region in France.

The progressing worldwide increases in tick occurrence and tick-borne diseases calls for the development of new prevention strategies to reduce their impact on human and animal health. Defining the risk of exposure to tick bites is therefore essential. Forestry workers are at high risk of tick bites. We set up an explorative study among forestry workers in the Alsace region in eastern France to measure the different factors affecting the risk of Ixodes ricinus tick bites during their activities in forests. For one year, forestry workers recorded the presence of ticks on their clothes and tick

The dynamics of scarlet fever in The Netherlands, 1906-1920: a historical analysis.

Background. Scarlet fever, an infectious disease caused by Streptococcus pyogenes, largely disappeared in developed countries during the twentieth century. In recent years, scarlet fever is on the rise again, and there is a need for a better understanding of possible factors driving transmission. Methods. Using historical case notification data from the three largest cities in The Netherlands (Amsterdam, Rotterdam and The Hague) from 1906 to 1920, we inferred the transmission rate for scarlet fever using time-series susceptible-infected-recovered (TSIR) methods. Through additive regression mod

Factors shaping at-risk individuals' decisions to undergo genetic testing for cancer in Asia.

This study aims to enrich our understanding of factors influencing medically indicated at-risk individuals' decisions to take genetic tests (or not) in the context of cancer treatment and prevention. While previous studies have explored this topic in communities in Europe and the United States, we know relatively little about the situation in Asia. In this study, we conducted in-depth interviews with 24 women who underwent genetic testing for hereditary breast and ovarian cancer syndromes in Singapore. Grounded theory with thematic analysis was applied. Six encouraging and three discouraging f
