An algorithm for diagnosis and therapy of management and complications of urolithiasis during pregnancy.

From 1 January 1986 to 30 June 1989, 15 pregnant women were diagnosed as having urolithiasis. Patients presented in the last two trimesters with an infection of the lower part of the urinary tract (60 per cent), flank and abdominal pain (27 per cent) and hematuria (13 per cent). Ultrasonographic findings confirmed the diagnosis in 47 per cent of the patients. Other roentgenologic procedures were required in the remaining patients. Initially, therapy was conservative in all, and in 67 per cent of patients, no further intervention was necessary. Only 33 per cent required invasive measures; cysto

Should women be tested for gonococcal infection of the cervix during routine gynecologic visits? An economic appraisal.

PURPOSE, PATIENTS, AND METHODS: We used decision analysis to calculate the economic implications of routine testing for Neisseria gonorrhoeae infection of the cervix. This study compared performing endocervical cultures in all women seeking routine gynecologic care to a no-test strategy. Estimates derived from the medical literature were varied in a sensitivity analysis, and a Monte Carlo technique was used to incorporate the estimates of experts into the analysis. RESULTS: We found that routinely testing for gonorrhea reduced overall medical costs when the prevalence of infection exceeded 1.

Setting up a decision support system with decision tables.

The aim of our study was to develop a decision support system using a conventional method which can be used as a shell for different applications. So it was necessary to find a method which allows separation of decision principles and decision algorithms. In addition, documentation of the patient records should be simplified. This could be attained by using the decision table technique and the programming language MUMPS. The general system developed was applied to the therapy decision for patients with liver metastases. The application system was clinically evaluated in a randomized group of p

[Tick-borne encephalitis in a northern zone of coniferous and broad-leaved forests of a focal region of the Amur River basin].

The geographical variability of the population of tick-borne encephalitis (TBE) virus in the northern part of the zone of combined coniferous and broad-leaved forests was established; this variability was manifested by higher virulence and homogeneity of TBE virus strains, as shown in experiments on white mice receiving the virus extraneurally, in comparison with the southern part of the zone and by higher virus carriership of the ticks Ixodes persulcatus. With the epizootic situation remaining tense and the danger of TBE virus infection still present, TBE morbidity and mortality rates decreas

Digestion and metabolism of high-tannin Eucalyptus foliage by the brushtail possum (Trichosurus vulpecula) (Marsupialia: Phalangeridae).

The digestion and metabolism of Eucalyptus melliodora foliage was studied in captive brushtail possums (Trichosurus vulpecula). The foliage was low in nitrogen and silica but high in lignified fibre and phenolics compared with diets consumed by most other herbivores. The high lignin content was suggested as the main cause of the low digestibility of E. melliodora cell walls (24%); microscopic observations of plant fragments in the caecum and faeces revealed few bacteria attached to lignified tissues. The conversion of digestible energy (0.34 MJ X kg-0.75 X d-1) to metabolizable energy (0.26 MJ

[Effect of forest fires on the population count and spatial structure of the taiga tick (Ixodidae)].

Results of two large-scale surveys of Ixodes persulcatus distribution were compared. Surveys were conducted in a south taiga forest area in the Khabarovsk region two and ten years after a great fire. The direct inhibiting effect of the fire on the population of I. persulcatus was not continuous though apparent. By the moment of pyrogenic small forests formation on slash fires, i.e. during the period not exceeding 2-3 complete developmental cycles of I. persulcatus, the abundance and spatial structure of its population are restored completely.

Nodular Fasciitis of the Lung: A Novel Entity.

Pulmonary tumors in young individuals may differ in their causes from older individuals, with a more broad and potentially benign process in the differential diagnosis. Inflammatory myofibroblastic tumors (IMTs) are well described in the literature, and here, we describe a rare previously unreported case of nodular fasciitis of the bronchial tree, initially mimicking a typical IMT, and which spontaneously regressed, avoiding the need for potentially life-altering surgery for the patient. This case highlights the importance of careful evaluation of pseudotumors of the lung and specialist multid

Decoding mitochondrial proteomes of diverse eukaryotes.

Our textbook view of mitochondria, which has been shaped by studies in animals and yeast, fails to do justice to the organelle's broader eukaryotic diversity. While most mitochondria retain a tiny genome, the overwhelming majority of their proteins are nuclear-encoded and vary extensively across lineages. To map this diversity, the MitoCarta Tree of Life Consortium developed experimental and computational workflows to generate high-accuracy mitochondrial proteomes across diverse eukaryotes-helping to lay a foundation for comparative mitochondrial biology with broad implications for physiology,

The genome sequence of Schoenoplectus triqueter (L.) Palla, 1888 (Poales: Cyperaceae).

We present a genome assembly of Schoenoplectus triqueter (Triangular Club-rush; Streptophyta; Magnoliopsida; Poales; Cyperaceae). The genome sequence has a total length of 580.26 megabases. Most of the assembly (98.75%) is scaffolded into 21 chromosomal pseudomolecules. Five mitochondrial sequences and the plastid genome were also assembled. Gene annotation of this assembly on Ensembl identified 31 746 protein-coding genes. This assembly was generated as part of the Darwin Tree of Life project, which produces reference genomes for eukaryotic species found in Britain and Ireland.

[Endohepatology in 2026 : innovations and perspectives].

Endohepatology is a rapidly expanding field that combines hepatological expertise with advanced interventional endoscopy. It refers to the use of endoscopic techniques for the diagnosis and management of hepatobiliary diseases and, in particular, complications related to portal hypertension. This approach primarily relies on endoscopic ultrasound and endoscopic retrograde cholangiopancreatography, which allow for a direct and precise access to the liver, the biliary tree, and the portal venous system. Endohepatology thus exemplifies the shift toward an integrated and minimally invasive managem

The Human Respiratory Virome in Health and Disease: Interactions, Dysbiosis, and Methodological Challenges (Advanced Genetics 4/06).

The Dual Landscape of the Respiratory Virome The bronchial tree is the lung's forest, hosting a complex microbial world. This cover art captures the delicate balance of the respiratory virome. On one side, lush foliage signifies a diverse, healthy ecosystem in homeostasis. On the other, withered branches reveal the impact of viral proliferation and dysbiosis. In article 202500022, Yao et al. comprehensively review the respiratory virome, covering its healthy composition, host-microbiota interactions, and disease-associated alterations, while addressing methodological challenges and future clin

A Curious Cyst Beyond the Thorax: A Rare Case of Extrathoracic Bronchogenic Cyst Over the Shoulder in a Child.

Bronchogenic cysts are rare congenital malformations originating from abnormal budding of the tracheobronchial tree during embryogenesis. While typically found in the mediastinum or lungs, extrathoracic presentations, particularly in the scapular or shoulder region, are exceedingly rare. We report the case of a three-year-old female with a cystic lesion over the left supraclavicular area that was histopathologically diagnosed as a bronchogenic cyst. This case underscores the importance of including bronchogenic cysts in the differential diagnosis of pediatric cystic masses outside the thorax.

Management of Tracheoesophageal Fistula and Tracheoinnominate Fistula.

Fistulization between the tracheobronchial tree and gastrointestinal tract is a challenging clinical situation that leads to life-threatening pulmonary contamination and inadequate nutrition. Operative management requires repair of both airway and esophageal defects with or without tracheal resection and reconstruction to restore a functional airway and maintain gastrointestinal continuity. Tracheoinnominate fistula is a rare communication between the airway and innominate artery that can result in fatal hemorrhage. Prompt operative intervention is lifesaving and can be performed by division o

Asymptomatic Bradycardia and Normal Chest Radiography in a Patient With Spontaneous Pneumomediastinum: A Case Report of a Rare Entity.

Spontaneous pneumomediastinum (SPM) is an uncommon condition caused by alveolar rupture due to increased intra-alveolar pressure resulting in air tracking along the tracheobronchial tree. While chest pain, neck pain, and dyspnea are the most commonly described symptoms, bradycardia could be an associated manifestation occasionally. In the majority of cases, pneumomediastinum is usually diagnosed on chest X-ray. We present an atypical case of chest X-ray-negative SPM complicated by marked bradycardia, which has been sparsely documented in published literature.

Diagnosis and Endoscopic Treatment of Hemobilia Due to Biliary Angiodysplasia: A Case Report and Literature Review.

Hemobilia is described as bleeding from the intra- or extrahepatic biliary tree expressed through the major duodenal papilla into the duodenum, with angiodysplasia of the major biliary duct as a rare etiological factor with few cases reported in the literature. Cholangioscopy plays a pivotal role in diagnosing and making therapeutic decisions regarding biliary tract lesions. We report a case of the diagnosis and treatment of hemobilia secondary to bleeding from angiodysplasia of the major biliary duct, which was resolved after the placement of a fully covered metallic stent, with a review of t

MR Imaging in Primary Sclerosing Cholangitis and Other Cholangitis.

Sclerosing cholangitis is characterized by irregular and ill-defined inflammation, fibrosis, and stricturing of the bile ducts and may be primary or secondary in cause. It causes progressive destruction of the biliary tree, progressive parenchymal cirrhosis, hepatic failure, and malignancy such as cholangiocarcinoma. The subtypes of sclerosing cholangitis can be broken down into primary sclerosing cholangitis (PSC), immune-mediated sclerosing cholangitis, infectious cholangitis, and ischemic cholangitis with other causes including metastatic disease and chemotherapy change. MR imaging is the g

Fatal Choking Due to a Slug: A Death Driven by Folklore.

The use of animals for therapeutic purposes is based on traditional beliefs, family pressure, and watching others appear to be cured. We report an unusual case of a 47-y-old man who choked to death after consuming a live slug. During autopsy, a 9.5-cm slug was found impacted in the tracheobronchial tree. History provided by the family revealed that the deceased consumed slugs in the belief that this would relieve his symptoms of chronic arthritis. We report this rare case to highlight the possible ill effects of such practices.

Acute Cholangitis: Causes, Diagnosis, and Management.

Acute cholangitis, also referred to as ascending cholangitis, is an infection of the biliary tree characterized by fever, jaundice, and abdominal pain, which in most cases is the consequence of biliary obstruction. Diagnosis is commonly made by the presence of clinical features, laboratory tests, and imaging studies. The treatment modalities include administration of intravenous fluids, antimicrobial therapy, and prompt drainage of the bile duct. Early diagnosis and treatment of acute cholangitis are crucial to prevent unwanted clinical outcome of the disease. This article provides an update o

CT Findings of Central Airway Lesions Causing Airway Stenosis-Visualization and Quantification: A Pictorial Essay.

The tracheobronchial tree is a system of airways that allows the passage of air to aerate the lungs and entire body. Several pathological conditions can affect this anatomical region. Multidetector CT (MDCT) helps identify and characterize various large airway diseases. Post-processing tools, such as virtual bronchoscopy and automatic lung analysis, can help enhance the performance of imaging studies. In this pictorial essay review, we provide imaging findings of various bronchial lesions manifested as wall thickening and endoluminal nodules on conventional MDCT and advanced image visualizatio
