The activation of vitamin D to its hormonal form is mediated by cytochrome P450 enzymes. CYP2R1 catalyzes the initial step converting vitamin D into 25-hydroxyvitamin D. A CYP2R1 gene mutation causes an inherited form of rickets due to 25-hydroxylase deficiency. Post-absorption levels of 25-hydroxy vitamin D (25-OHD) after oral administration of 25-hydroxycholecalciferol (25-OHD3) were measured in 11 subjects. Five had presented with steatorrhoea of various causes while six had post-gastrectomy osteomalacia. The effect of vitamin-D deficiency and subsequent vitamin-D replacement on the metabolism of rat epiphyseal growth plate cartilage was studied. Biochemical analyses showed the presence of the two unique glyoxylate cycle enzymes isocitrate lyase and malate synthase in cartilage.
