[Pulmonary vein isolation with multipolar ablation catheters for the treatment of paroxysmal atrial fibrillation].

Atrial fibrillation (AFIB) is the most common atrial rhythm disease and is associated with an increased risk of thromboembolic and cardiac complications. Different therapies are used in clinical routine: frequency control with anticoagulants and rhythm control. In patients with paroxysmal AFIB but without structural heart disease, antiarrhythmic drug therapy was previously first priority; however, pulmonary vein catheter ablation is becoming more important. "Single tip" systems, ultrasound, and various balloon techniques have been used in clinical routine. New radiofrequency systems with multi

Pressure-guided cryoballoon isolation of the pulmonary veins for the treatment of paroxysmal atrial fibrillation.

BACKGROUND: Pulmonary vein (PV) isolation using a balloon-mounted cryoablation system is a new technology for the percutaneous treatment of atrial fibrillation (AF). Complete PV occlusion during balloon ablation has been shown to predict successful electrical isolation. The aim of this study was to correlate mechanical PV occlusion with changes in a pressure curve recorded at the distal tip of the cryoballoon catheter. METHODS AND RESULTS: We analyzed 51 PVs in 12 patients (61 +/- 6 years old) with paroxysmal AF. At first, PV occlusion via the cryoballoon was documented by changes in the pres

Utility of brain natriuretic peptide as a predictor of atrial fibrillation after cardiac operations.

BACKGROUND: Atrial fibrillation (AF) occurs frequently after coronary bypass grafting and valve operations. Brain natriuretic peptide (BNP) has been shown to predict recurrence of AF in congestive heart failure. It is a potential biomarker for preoperative risk stratification for development of AF in at-risk patients. METHODS: A total of 398 consecutive patients were prospectively evaluated for new-onset AF after heart operations. Patients with a history of AF and presence of permanent pacemaker were excluded. BNP levels were measured before and immediately after the operation. RESULTS: AF o

[Perspectives in cardiology: evidence of efficacy in atrial fibrillation and hopes in acute coronary syndrome].

New antithrombotic drugs, antithrombin or anti Xa, will probably be very useful in cardiology. Two directions are interesting: in one hand atrial fibrillation, in which the unmet need concern drugs as effective as vitamine K antagonists but easier to use. On the other hand, in acute coronary syndrome the situation is different, there are many antithrombotic drugs available but there is still a place for innovative drugs which could provide a gain in terms of efficacy, but the hemorrhagic risk must remain acceptable. In atrial fibrillation, the RELY trial, performed in 18,113 patients has demon

The effect and safety of the antithrombotic therapies in patients with atrial fibrillation and CHADS score 1.

BACKGROUND: The revised ACC/AHA/ESC 2006 guideline recommends either aspirin or warfarin for the prevention of ischemic stroke in patients with atrial fibrillation (AF) in CHADS(2) score 1. We hypothesized that warfarin is superior to aspirin therapy for the prevention of stroke without increasing bleeding complication in AF patients with CHADS(2) score 1. METHODS AND RESULTS: Among 1,502 patients (mean 62.4 +/- 13.8 years old, male 65.4%) who were treated for nonvalvular AF without previous stroke, the number of patients with CHADS(2) score 1 was 422 (62.9 +/- 10.7 years old, male 290 [68.7%

Is there an evidence of pulmonary vein stenosis following epicardial microwave ablation of atrial fibrillation?

BACKGROUND: Surgical ablation techniques using microwave energy are an alternative to catheter based ablation therapy in the treatment of atrial fibrillation (AF). However, little is known about potential procedure-related complications. We investigate, whether there is evidence of pulmonary vein stenosis (PVST) in patients with atrial fibrillation undergoing epicardial microwave ablation. METHODS: 14 patients (ten males and four females) with AF and structural heart disease underwent cardiac surgery for the underlying disease and concomitant ablation of AF using microwave energy. [New method for the treatment of atrial fibrillation: circumferential cryoballoon ablation of the pulmonary vein].

Atrial fibrillation is the most frequent arrhythmia with increasing prevalence. Given a limited success rate of drug therapy for atrial fibrillation, interventional treatment options have been developed during the last years. Catheter ablation of atrial fibrillation (until recently the mostly used energy source was radiofrequency energy) has been established as an important therapeutic alternative. Depending on interpersonal (both on patient and operator side) and technical variabilities using radiofrequency energy potentially life-threatening complications such as pulmonary vein stenosis or a

Non-invasive organization variation assessment in the onset and termination of paroxysmal atrial fibrillation.

Atrial Fibrillation (AF) is the most common supraventricular tachyarrhythmia. Recently, it has been suggested that AF is partially organized on its onset and termination, thus being more suitable for antiarrhythmia and to avoid unnecessary therapy. Although several invasive and non-invasive AF organization estimators have been proposed, the organization time course in the first and last minutes of AF has not been quantified yet. The aim of this work is to study non-invasively the organization variation within the first and last minutes of paroxysmal AF. The organization was evaluated making us

Increased augmentation index of the radial pressure waveform in patients with paroxysmal atrial fibrillation.

OBJECTIVE: The augmentation index, a marker of wave reflection, has been reported to reflect vascular properties and to determine left ventricular (LV) characteristics. We investigated the relationship between the augmentation index and paroxysmal atrial fibrillation (AF). METHODS: A total of 244 outpatients (122 patients with paroxysmal AF and 122 age-, and gender-matched controls without paroxysmal AF) were examined during sinus rhythm. The augmentation index was calculated from the radial arterial waveform using applanation tonometry methods. RESULTS: After adjusting for age, gender, hear

Is atrial fibrillation with very short cycle length suitable for ablation? A case report.

We present a case of a 36-year-old woman with highly symptomatic persistent atrial fibrillation (AF) refractory to sotalol, flecainide, and external direct current (DC) cardioversion. The patient underwent biatrial mapping and ablation procedure for AF. Both atria were characterized by refractory properties which were much shorter than reported previously. Global fibrillatory activity was present with a median cycle length of 120 ms (range: 62-143). Extensive map-guided ablation sets had to be delivered to both left and right sides before effective DC cardioversion enabled sinus rhythm (SR) re

Effect of cardiac resynchronization therapy on conversion of persistent atrial fibrillation to sinus rhythm.

BACKGROUND: Spontaneous conversion of persistent atrial fibrillation to sinus rhythm (SR) has anecdotally been reported following cardiac resynchronisation therapy. OBJECTIVE: This monocenter observational study was designed to estimate the incidence of spontaneous conversion of persistent atrial fibrillation to SR in consecutive patients implanted with a cardiac resynchronisation device. METHODS AND RESULTS: A total of 46 patients with persistent atrial fibrillation (> or =4 weeks pre-implant), left bundle branch block (QRS > 130 ms), left ventricular ejection fraction <0.35 and NYHA III or

Prevalence and characteristics of continuous electrical activity in patients with paroxysmal and persistent atrial fibrillation.

BACKGROUND: Complex fractionated atrial electrograms (CFAEs) may play a role in the genesis of atrial fibrillation (AF). One type of CFAE is continuous electrical activity (CEA). The prevalence and characteristics of CEA in patients with paroxysmal and persistent AF are unclear. METHODS AND RESULTS: In 44 patients (age = 59 +/- 8 years) with paroxysmal (25) or persistent (19) AF, bipolar electrograms were systematically recorded for > or =5 seconds at 24 left atrial (LA) sites, including 8 antral sites, and 2 sites within the coronary sinus (CS). CEA was defined as continuous depolarization f

Efficacy of pulmonary vein isolation by cryoballoon ablation in patients with paroxysmal atrial fibrillation.

BACKGROUND: Radiofrequency ablation of pulmonary veins (PVs) has emerged as an effective treatment for patients with paroxysmal atrial fibrillation (AF). However, serious complications raise concern about an even wider application. In terms of safety, cryoenergy has advantages compared with radiofrequency. A new cryoenergy balloon catheter has been recently developed to make AF ablation shorter and safer. OBJECTIVE: The purpose of this study was to test the 6-month efficacy of this new device for ablation of paroxysmal AF. METHODS: Twenty-one patients with highly symptomatic paroxysmal AF, n

Role of the calpain system in pulmonary vein connexin remodeling in dogs with atrial fibrillation.

OBJECTIVES: Changes in connexins and calpains of the myocardial sleeve of the pulmonary vein and the left atrium were investigated in chronic atrial fibrillation (AF) animal models. BACKGROUND: There are no reports of changes in the calpain system and connexins in the pulmonary vein where AF is initiated. METHODS: An AF animal model was prepared by rapid pacing of the right atrium for 8 weeks. Histological changes of pulmonary veins were analyzed by Masson trichrome staining, and mRNA as well as protein expression of connexins and calpains were measured by real-time fluorescence quantitative

Staged circumferential and ostial pulmonary vein ablation for the treatment of paroxysmal atrial fibrillation.

BACKGROUND: Comparisons between segmental ostial disconnection of the pulmonary veins (PV) and circumferential ablation have produced conflicting results in patients with paroxysmal atrial fibrillation (AF). The aim of this study was to evaluate a staged ablation procedure, every step of which was assessed by means of AF inducibility. METHODS: Twenty-two patients with paroxysmal AF were subjected to three ablation stages during one session: (1) circumferential ablation around the PV ostia, (2) segmental ostial PV isolation, and (3) ablation of areas within the circumferential lines with fract

Is there an anatomical substrate for idiopathic paroxysmal atrial fibrillation? A case-control echocardiographic study.

AIM: Idiopathic paroxysmal atrial fibrillation (AF) occurs in patients with apparently normal heart. Its mechanisms may be complex and are poorly understood. The aim of the study was to evaluate whether patients with idiopathic AF have any structural abnormality that may explain the occurrence of AF. METHODS AND RESULTS: A case-control study was undertaken including 60 consecutive patients (mean age 48 +/- 12 years; 75% men) with idiopathic AF admitted to the emergency department. Sixty sex- and age-matched healthy volunteers made up the control group. An echocardiogram was performed in all p

Screening device for identification of patients with paroxysmal atrial fibrillation to prevent ischemic strokes.

The most frequently occurring cardiac arrhythmia in the adulthood is atrial fibrillation. In Germany, the number of sick people is estimated at 800,000. Patients who suffer from atrial fibrillation often do not sense any symptoms of the illness. Through the uncontrolled blood flow in the atrium, a blood clot, which can lead to the blood circulation for an embolism or for a stroke, can form itself. These persons must be recognized, because of their increased stroke risk in order to be able to attend it. In this work, the chest strap CorBelt, developed by the company Corscience GmbH&Co.KG, is eq
