[Splenectomy in Werlhof disease without thrombocyte substitution. Prospective study of 7 consecutive cases].

As a prophylaxis against the risk of postoperative bleeding platelet mass transfusions or transfusions of whole fresh blood are recommended for splenectomy in chronic idiopathic thrombocytopenic purpura. As the efficacy of this treatment is uncertain, seven consecutive patients with platelet counts of 4000/mm3 (2000-16000) were splenectomized without this prophylaxis. Platelet counts rose to 152500/mm3 (70000--305000) 18 h after operation, in every case the critical lower limit of 50000 was exceeded. Apart from one case of transient fever of undetected origin and one small pleural effusion the

Successful Heart Transplantation Despite Rhesus Blood Type Mismatch: A Case Report.

Matching for the rhesus (Rh) blood group is currently not taken into account in the organ allocation system. However, in Rh-mismatched transplantation, the primary concern is the potential for RhD-negative recipients to develop sensitization and produce anti-D anti-bodies if they receive a transfusion of RhD-positive blood. It is estimated that over 80% of RhD-negative recipients may experience Rh allosensitization when exposed to RhD-positive blood, although this occurrence is less common in recipients of solid organs. In theory, RhD-negative recipients who receive organs from RhD-positive do

Audit of Blood Component Utilization and Request Appropriateness in a Tertiary Care Hospital: A Cross-Sectional Study.

BACKGROUND: Blood components, packed red blood cells (PRBCs), fresh frozen plasma (FFP), and platelet concentrates (PCs), are finite resources whose inappropriate use compromises patient safety and depletes inventory. Structured audit of transfusion practice underpins sound hemovigilance, yet its application across Indian tertiary centers remains inconsistent. This study quantified the appropriateness of blood component requests and mapped component-wise and departmental variation. METHODS: A six-month retrospective cross-sectional audit (July-December 2025) was conducted at a tertiary care h

Transfusion of adult platelets triggers inflammatory responses in newborn mice through both P-selectin-dependent and -independent mechanisms.

BACKGROUND: Liberal platelet transfusion practices increase neonatal morbidity and mortality. The mechanisms underlying this harm are unknown but may involve the immune rather than hemostatic functions of platelets, as well as the significant differences between adult (transfused) and neonatal platelets, particularly the higher P-selectin surface expression on activated adult platelets. In this study, we investigated the immune/inflammatory effects of transfusing adult platelets into newborn mice. STUDY DESIGN AND METHODS: Washed platelets from wild-type (WT) or P-selectin-/- adult donors or

Transfusion reactions associated with COVID-19 convalescent plasma therapy for SARS-CoV-2.

BACKGROUND: Convalescent plasma (CP) for treatment of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has shown preliminary signs of effectiveness in moderate to severely ill patients in reducing mortality. While studies have demonstrated a low risk of serious adverse events, the comprehensive incidence and nature of the spectrum of transfusion reactions to CP is unknown. We retrospectively examined 427 adult inpatient CP transfusions to determine incidence and types of reactions, as well as clinical parameters and risk factors associated with transfusion reactions. Effect of clopidogrel on bleeding and transfusions after off-pump coronary artery bypass graft surgery: impact of discontinuation prior to surgery.

OBJECTIVE: The use of antiplatelet drugs to treat acute myocardial infarction, unstable angina, acute coronary syndrome and secondary prevention following percutaneous coronary interventions is well accepted. However, it constitutes a serious risk of bleeding for patients undergoing coronary artery bypass grafting surgery (CABG). We evaluated the effect of aspirin and clopidogrel (CPDG), both irreversible platelet aggregation inhibitors, on operative bleeding and determined the optimal timing for their discontinuation before surgery. METHOD: Between July 2001 and December 2004, we reviewed ou

[Epidemiological trend of chronic hepatitis C].

Though patients with chronic hepatitis C after blood transfusion hepatitis decrease dramatically during the past decade, patients with it after non-transfused acute hepatitis C are still in existence. This means that new patients with chronic hepatitis C are on the decrease but not diminish. To the contrary, the number of patients with chronic hepatitis C is increasing. This depends on mainly dig up the undiscovered patients. These result in increasing the patients with hepatocellular carcinoma.

Inhibition of red blood cell-derived small extracellular vesicles release by calpeptin attenuates neutrophil inflammation.

Transfusion of aged red blood cells (RBCs) is associated with adverse outcomes in critically ill patients. Extracellular vesicles (EVs), as potential risk factors for transfusion-related immunomodulation (TRIM), play a critical role in the development of sepsis. However, little is known about the effects and mechanisms of red blood cell-derived small extracellular vesicles (RBC-sEVs) that aggravate inflammation and cause poor prognosis in septic patients after RBC transfusion. This study characterized RBC-sEVs from human and mouse leukoreduced packed RBC units and their role in promoting neutr

Haemoglobin and transfusions in elderly patients with hip fractures: the effect of a dedicated orthogeriatrician.

BACKGROUND: Hip fractures are common in elderly patients, in whom it is important to monitor blood loss; however, unnecessary transfusions should be avoided. The primary objective of this study was to assess whether the employment of a dedicated orthogeriatrician in an Orthopaedic Department allows to optimise the clinical conditions of patients, influencing blood loss and consequently the number of transfusions. The secondary objective was to determine whether the influence of the orthogeriatrician differs according to the type of surgical treatment. METHODS: A total of 620 elderly patients

Neutrophils Derived from Genetically Modified Human Induced Pluripotent Stem Cells Circulate and Phagocytose Bacteria In Vivo.

Bacterial and fungal infections are a major cause of morbidity and mortality in neutropenic patients. Donor-derived neutrophil transfusions have been used for prophylaxis or treatment for infection in neutropenic patients. However, the short half-life and the limited availability of large numbers of donor-derived neutrophils for transfusion remain a significant hurdle in the implementation of neutrophil transfusion therapy. Here, we investigate the in vitro and in vivo activity of neutrophils generated from human induced pluripotent stem cells (iPSC), a potentially unlimited resource to produc

Buffy coat transfusions in neonates with sepsis and neutrophil storage pool depletion.

A randomized study was initiated in neonates with neutropenia (absolute peripheral neutrophil count less than 1,500/microL) and suspected bacterial infection. Twenty infants with proven infection were enrolled, nine of whom had depletion of bone marrow stores of maturing neutrophils (less than or equal to 7% metamyelocyte, band and mature forms per 100 nucleated cells). These nine were randomized to receive 15 mL/kg of either buffy coat transfusions (group 2) or plasma and blood products (group 3). The remaining 11 (group 1) were observed. Peripheral neutrophil counts were monitored to determi
