Integrated Management of Malignant Arrhythmias in Pediatric LMNA-Related Cardiomyopathy Using Active-Fixation CRT-D.

BACKGROUND: Pediatric LMNA-related dilated cardiomyopathy (DCM) carries extreme risks of malignant arrhythmias and sudden cardiac death. While cardiac resynchronization therapy with defibrillator (CRT-D) is life-saving, small body size and coronary anatomy make lead stability technically challenging. CASE SUMMARY: An 11-year-old boy with LMNA-related DCM (c.1622G>A, p.R541H) experienced ventricular fibrillation arrest requiring extracorporeal membrane oxygenation support. A CRT-D was implanted using an active-fixation quadripolar left ventricular (LV) lead in the coronary vein. Subsequent ref

An explicative review on Diabetic Cardiac Autonomic Neuropathy: From pathophysiology to the era of gene therapy.

Diabetic Cardiac Autonomic Neuropathy (DCAN) is a prevalent and severe yet underdiagnosed complication of diabetes characterized by progressive injury to autonomic nerve fibers that regulate cardiovascular functions. This multifactorial disorder contributes significantly to morbidity and mortality due to arrhythmias, silent myocardial ischemia, and sudden cardiac death. DCAN onset is often insidious, with early stages marked by subclinical autonomic dysfunction detectable through heart rate variability and reflex testing. The pathogenesis involves chronic hyperglycemia-induced oxidative stress

D801N in ATP1A3-encoded Na/K-ATPase alpha 3 causes cardiac arrhythmogenesis through sodium-calcium exchanger-mediated calcium overload.

Short QT syndrome is a heritable arrhythmia disorder linked to sudden cardiac death. We recently identified that individuals with alternating hemiplegia of childhood (AHC), a rare neurodevelopmental disorder, can exhibit shortened corrected QT intervals and elevated risk for ventricular fibrillation. This is especially true for patients with AHC heterozygous for the recurrent ATP1A3-D801N variant, though the underlying cardiac mechanism remains unclear. We hypothesized that the D801N missense impairs Na+/K+-ATPase function, causing Ca2+ overload, shortened action potential duration (APD), and

Percutaneous coronary intervention for acute myocardial infarction in a patient with an anomalous origin of the right coronary artery from the ascending aorta: a case report.

BACKGROUND: Coronary artery anomalies represent morphological deviations from standard coronary anatomy, occurring in less than 1% of the general population. Although coronary artery anomalies are often asymptomatic, they can lead to serious complications, including sudden cardiac death and cardiac ischemia. Percutaneous coronary intervention in patients with coronary artery anomalies presents challenges, with an increased risk of ostial dissection, particularly in the setting of ST-segment elevation myocardial infarction. CASE PRESENTATION: This case report describes the successful managemen

Early Onset Heart Failure due to RBM20 Variant: A Case Report Emphasizing Genetic Diagnosis and Arrhythmic Risk Stratification.

The RBM20 gene, located on chromosome 10q25.2, encodes a serine/arginine-rich protein essential for post-transcriptional splicing of several cardiac genes, including titin. Pathogenic variants in RBM20 are increasingly recognized as causes of familial dilated cardiomyopathy (DCM) with a high risk of heart failure and sudden cardiac death. We describe a 23-year-old man who presented with heart failure with mildly reduced ejection fraction secondary to DCM caused by a heterozygous missense variant in the RBM20 gene (c.1907G>A; p.Arg636His). Comprehensive clinical evaluation excluded non-genetic

Pharmacological inhibition of neddylation by MLN4924 protects against Coxsackievirus B3-induced myocarditis.

Viral myocarditis (VMC), predominantly caused by Coxsackievirus B3 (CVB3), remains a leading cause of sudden cardiac death and heart failure in young individuals. VMC results from both direct viral cytopathic effects and subsequent immune-mediated myocardial injury, yet effective antiviral therapies remain limited. MLN4924 (pevonedistat), a first-in-class neural precursor cell expressed developmentally downregulated 8 (NEDD8)-activating enzyme inhibitor, has been extensively characterized for its anti-proliferative and anticancer activities, though its antiviral potential in vivo remains large

Precision medicine applications in dilated cardiomyopathy: Advancing personalized care.

Dilated cardiomyopathy (DCM) is a prevalent cardiac disorder affecting 1 in 250-500 individuals, characterized by ventricular dilation and impaired systolic function, leading to heart failure and increased mortality, including sudden cardiac death. DCM arises from genetic and environmental factors, such as drug-induced, inflammatory, and viral causes, resulting in diverse yet overlapping phenotypes. Advances in precision medicine are revolutionizing DCM management by leveraging genetic and molecular profiling for tailored diagnostic and therapeutic approaches. This review highlights comprehens

Impact of Anabolic-Androgenic Steroid Abuse on the Cardiovascular System: Molecular Mechanisms and Clinical Implications.

Anabolic-androgenic steroids (AAS) are synthetic derivatives of testosterone that are used therapeutically but are frequently abused by athletes and individuals seeking to increase muscle mass. Their anabolic (promoting muscle growth) and androgenic (inducing masculine characteristics) effects result from androgen receptor activation in target tissues. However, chronic supraphysiological AAS exposure is associated with serious cardiovascular consequences, ranging from hypertension and lipid disorders to cardiomyopathy, atherosclerosis, and sudden cardiac death. This review provides an updated

Hypertrophic cardiomyopathy in identical twins: a case report with mismatch between genotype and phenotype.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is the most common genetic cardiomyopathy; however, it is a very rare condition among twins. A mismatch between the phenotype and clinical course has been identified, even in the presence of apparently identical genotypes. CASE SUMMARY: A pair of male monozygotic-diamniotic twins of 17 years old with a mismatch in HCM clinical expression is informed. Both twins have the same pathogenic variant [c.1816G>A (p.Val606Met)] located in MYH7, a gene that encodes an essential component in the contraction of cardiac muscle. The twins presented obstructive

The Role of Human-Induced Pluripotent Stem Cells in Studying Cardiac Channelopathies.

Cardiac channelopathies are inherited diseases that increase the risk of sudden cardiac death. While different genes have been associated with inherited channelopathies, there are still subtypes, e.g., catecholaminergic polymorphic ventricular tachycardia and Brugada syndrome, where the genetic cause remains unknown. Various models, including animal models, heterologous expression systems, and the human-induced pluripotent stem-cell-derived cardiomyocytes (hiPSCs-CMs) model, have been used to study the pathophysiological mechanisms of channelopathies. Recently, researchers have focused on usin

Sensor-Based Measurement Method to Support the Assessment of Robot-Assisted Radiofrequency Ablation.

Digital surgery technologies, such as interventional robotics and sensor systems, not only improve patient care but also aid in the development and optimization of traditional invasive treatments and methods. Atrial Fibrillation (AF) is the most common cardiac arrhythmia with critical clinical relevance today. Delayed intervention can lead to heart failure, stroke, or sudden cardiac death. Although many advances have been made in the field of radiofrequency (RF) catheter ablation (CA), it can be further developed by incorporating sensor technology to improve its efficacy and safety. Harnessing the therapeutic potential of mesenchymal stem cell-derived exosomes in cardiac arrest: Current advances and future perspectives.

BACKGROUND: Cardiac arrest (CA), characterized by sudden onset and high mortality rates, is one of the leading causes of death globally, with a survival rate of approximately 6-24%. Studies suggest that the restoration of spontaneous circulation (ROSC) hardly improved the mortality rate and prognosis of patients diagnosed with CA, largely due to ischemia-reperfusion injury. MAIN BODY: Mesenchymal stem cells (MSCs) exhibit self-renewal and strong potential for multilineage differentiation. Their effects are largely mediated by extracellular vesicles (EVs). Exosomes are the most extensively stu

Subvalvular Aortic Stenosis: Learning From Human and Canine Clinical Research.

Subvalvular aortic stenosis (SAS) is the most common congenital heart disease (CHD) in dogs and is also prevalent in human children. A fibrous ridge below the aortic valve narrows the left ventricular outflow tract (LVOT) and increases blood flow velocity, leading to devastating side effects in diseased patients. Due to the similarities in presentation, anatomy, pathophysiology, cardiac development, genomics, and environment between humans and dogs, canine SAS patients represent a critical translational model of human SAS. Potential adverse outcomes of SAS include arrhythmias, left-sided conge

Personalized computational heart models with T1-mapped fibrotic remodeling predict sudden death risk in patients with hypertrophic cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) is associated with risk of sudden cardiac death (SCD) due to ventricular arrhythmias (VAs) arising from the proliferation of fibrosis in the heart. Current clinical risk stratification criteria inadequately identify at-risk patients in need of primary prevention of VA. Here, we use mechanistic computational modeling of the heart to analyze how HCM-specific remodeling promotes arrhythmogenesis and to develop a personalized strategy to forecast risk of VAs in these patients. We combine contrast-enhanced cardiac magnetic resonance imaging and T1 mapping data to c

Prevalence of anatomic variants in coronary arteries of Japanese Black cattle.

Coronary artery anomalies that can cause sudden cardiac death have been described in some mammals. However, few studies have investigated coronary anomalies and coronary artery branching patterns in cattle. Therefore, understanding of bovine coronary arteries is incomplete. The aim of this study was to clarify anatomic variants in coronary arteries of Japanese Black cattle. The gross anatomy of the coronary arteries was examined in the hearts of 100 cattle. A total of 14 of 100 hearts exhibited confirmed coronary anomalies in the left main trunk. These 14 hearts were classified into 5 types po

Bi-Allelic DES Gene Variants Causing Autosomal Recessive Myofibrillar Myopathies Affecting Both Skeletal Muscles and Cardiac Function.

Mutations in the human desmin gene (DES) may cause both autosomal dominant and recessive cardiomyopathies leading to heart failure, arrhythmias and atrio-ventricular blocks, or progressive myopathies. Cardiac conduction disorders, arrhythmias and cardiomyopathies usually associated with progressive myopathy are the main manifestations of autosomal dominant desminopathies, due to mono-allelic pathogenic variants. The recessive forms, due to bi-allelic variants, are very rare and exhibit variable phenotypes in which premature sudden cardiac death could also occur in the first or second decade of

Successful Resuscitation of Cardiac Arrest After Refeeding Syndrome Associated with Hiatal Hernia: A Case Report.

BACKGROUND Refeeding syndrome (RFS) is a life-threatening syndrome, which can cause sudden death. RFS has been reported frequently in young patients with anorexia without organic disease; however, there are few reports in elderly patients with organic disease. Herein, we report a case of cardiac arrest after refeeding syndrome associated with hiatal hernia. CASE REPORT We report the case of a 59-year-old woman who had a diagnosis of RFS during treatment for anorexia secondary to hiatal hernia. She was hospitalized with hypothermia, anemia, and hypovolemic shock and treated with electrolytes, h

A Rare Case of a Type IV Dual Left Anterior Descending Artery and Ectopic Left Anterior Descending and Circumflex Arteries Requiring Surgery.

Congenital anomalous coronary arteries (CACAs) comprise an important variant of the coronary vasculature. They are benign in the vast majority of cases, whereas a small minority may be affected by serious consequences such as myocardial infarction, arrhythmia, cardiac arrest, and even death. We herein describe a 62-year-old man with sudden and severe substernal chest pain; Q waves in electrocardiographic leads II, III, and aVF; and positive serum troponin I enzyme. Left heart cardiac catheterization revealed triple coronary vessel disease with a 60% to 70% occlusion in the left main coronary a

Coved and Saddleback ST-Segment Elevations: Brugada Phenocopy vs Brugada Syndrome.

We report a case of a middle-aged man who presented with near syncope, fever, and dysuria and was incidentally found to have coved ST-segment elevations in leads V1 and V2 confirming Brugada type 1 ECG (electrocardiogram) pattern. This ECG pattern morphed into saddleback ST-segment elevations in precordial leads consistent with type 2 Brugada the following day. Additionally, the patient reported a positive family history of sudden cardiac death. This initial presentation made it impossible to differentiate Brugada phenocopy (BrP) from Brugada syndrome (BrS). Continuous cardiac monitoring was i
