Comparing the accuracy and safety of automated CO2 angiography to iodine angiography in peripheral arterial disease with chronic limb ischemia: a prospective cohort study.

INTRODUCTION: Diagnostic angiography of peripheral arteries using carbon dioxide (CO2) is feasible in nearly all areas below the diaphragm. Using carbon dioxide as a contrast material in angiography provides the highest quality diagnostic results in the vascular segments above the knee. However, its diagnostic reliability decreases as it moves toward the distal side of the vessels below the knee. This study investigated the diagnostic accuracy and consistency between CO2 and iodine contrast angiography in patients with peripheral vascular disease (PVD) with chronic limb ischemia (CLI). METHOD

Inter-Relations of Orthostatic Blood Pressure Change, Aortic Stiffness, and Brain Structure and Function in Young Adults.

BACKGROUND: Relations of orthostatic change in blood pressure with brain structure and function have not been studied thoroughly, particularly in younger, healthier individuals. Elucidation of factors that contribute to early changes in brain integrity may lead to development of interventions that delay or prevent cognitive impairment. METHODS AND RESULTS: In a sample of the Framingham Heart Study Third Generation (N=2119; 53% women; mean age±SD, 47±8 years), we assessed orthostatic change in mean arterial pressure (MAP), aortic stiffness (carotid-femoral pulse wave velocity), neuropsychologi

Differential effects of teriparatide on regional bone formation using (18)F-fluoride positron emission tomography.

Teriparatide increases skeletal mass, bone turnover markers, and bone strength, but local effects on bone tissue may vary between skeletal sites. We used positron emission tomography (PET) to study (18)F-fluoride plasma clearance (K(i)) at the spine and standardized uptake values (SUVs) at the spine, pelvis, total hip, and femoral shaft in 18 postmenopausal women with osteoporosis. Subjects underwent a 1-hour dynamic scan of the lumbar spine and a 10-minute static scan of the pelvis and femurs at baseline and after 6 months of treatment with 20 µg/day teriparatide. Blood samples were taken to

Osteonecrosis induced by a single administration of low-dose lipopolysaccharide in rabbits.

We succeeded in developing a novel rabbit model of nonsteroid and nontraumatic osteonecrosis (ON) by use of a single- and low-dose lipopolysaccharide (LPS) injection. This model is simple and highly reproducible for the frequent development of multifocal and widespread ON lesions. Male adult Japanese white rabbits intravenously injected with a single injection of 10 microg/kg body weight of LPS were histopathologically examined in the early phase (3 [n = 3], 5 [n = 3], and 24 h [n = 3]) and at 4 weeks (n = 22). Seventy-seven percent of the rabbits developed multifocal ON 4 weeks after LPS inje

Can surface landmarks help us identify the gibson interval during surgical hip dislocation: a study of 617 hips.

INTRODUCTION: The Gibson approach, used in hip-preserving surgery, is intermuscular and develops the space anteriorly to the gluteus maximus. Reliable anatomical landmarks for the development of this interval do not exist, but the interval is marked by perforating vessels (PV) of the inferior gluteal artery. The aim of this study was to provide reference values for the relationship between palpable anatomical landmarks on the femur/pelvis and the anterior border of the gluteus maximus using CT scans of the proximal femur. MATERIALS AND METHODS: Single center retrospective study of 617 hips wh

Midline peripheral catheters inserted in the superficial femoral vein at mid-thigh: Wise choice in COVID-19 acute hypoxemic respiratory failure patients with helmet continuous positive airway pressure.

BACKGROUND: During coronavirus disease 2019 (COVID-19) pandemic, Helmet Continuous Positive Airway Pressure (h-CPAP) has been widely used to treat Acute Hypoxemic Respiratory Failure (AHRF). In COVID-19 patients undergoing h-CPAP a simple short peripheral catheter could be insufficient. According to the European Recommendations for Proper Indication and Use of Peripheral venous access consensus, a stable peripheral Vascular Access Device is indicated for intravenous treatment compatible with the peripheral route scheduled for more than 1 week. Iliac transluminal angioplasty and distal surgical revascularisation can be performed in a one-step technique.

PURPOSE: A few authors have suggested treating double-level atherosclerotic lesions in high risk patients by an association of iliac transluminal angioplasty (ITA) and distal surgical restoration in a two-step technique with an interval of one to three weeks between the two procedures. Previous reports of ITA showed the influence of the quality of the outflow on early results, we therefore decided to perform the two procedures mentioned above during the same operation. METHODS: During a five-year period, 51 patients (42 men, 9 women) underwent an isolated ITA in 32 cases (group I) or associat

Extracranial giant cell arteritis: A narrative review.

A systematic literature search was performed to summarise current knowledge on extracranial giant cell arteritis (GCA), i.e. large-artery involvement in patients with or without clinically apparent temporal arteritis (cranial GCA). Extracranial GCA is increasingly recognised, both in patients with cranial GCA and with solitary extracranial GCA, due to increased awareness among physicians and development of modern imaging modalities. The literature on the pathogenesis and histopathology of extracranial GCA is scarce. It is considered to be similar to cranial GCA. Patients with solitary extracra

Epidemiology of Vascular Injury Within the U.S. Department of Defense Trauma Registry From 2007-2023.

Background: Since WWII, the incidence of vascular trauma in U.S. military casualties has steadily risen, with the highest rates having been reported from the recent conflicts in Iraq and Afghanistan. Within civilian populations, the incidence of vascular trauma is lower, but with disproportionately higher rates of mortality, morbidity, and resource use for these injuries. Analyses of trauma databases have informed technological advances that best help our U.S. Military Health System (MHS) manage vascular injuries. We sought to perform a retrospective cohort analysis in military casualties with

Passive muscle heating attenuates the decline in vascular function caused by limb disuse.

Limb disuse has profound negative consequences on both vascular and skeletal muscle health. The purpose of this investigation was to determine whether repeated application of passive heat, applied to the knee extensor muscles, could mitigate the detrimental effects of limb disuse on vascular function. This was a randomized, single-blinded placebo controlled trial. Twenty-one healthy volunteers (10 women, 11 men) underwent 10 days of unilateral lower limb immobilization and were randomized to receive either a daily 2 h sham (Imm) or heat treatment (Imm+H) using pulsed shortwave diathermy. Tibial access is associated with tibial intervention in claudicants.

OBJECTIVE: Although the Society for Vascular Surgery guidelines support revascularization in select patients with lifestyle-limiting intermittent claudication (IC), infrapopliteal intervention is discouraged owing to questionable benefit and potential harm. Retrograde tibial/pedal access is increasing in peripheral vascular interventions (PVIs) in general, their its use and appropriateness in IC have not been studied or codified. This study aimed to analyze national practice patterns of tibial access in PVI for IC, and its potential association with tibial interventions. METHODS: Elective IC

Percutaneous axillary vs femoral access for transcatheter aortic valve replacement: Insights from a 2164-patient study.

BACKGROUND: Transaxillary (TAx) access is increasingly adopted for transcatheter aortic valve implantation (TAVR) in patients with challenging iliofemoral anatomy. We aimed at evaluating the early clinical outcomes of TAx vs transfemoral (TF) access for TAVR. METHODS: We conducted a retrospective observational study stemming from the prospective RISPEVA registry, including all consecutive patients undergoing TAVR since 2013 at our Institution, where TAx is the only alternative access site whenever TF is contraindicated. RESULTS: A total of 2164 patients were included: 60 (2.8 %) in the TAx g

Development and validation of a nomogram for predicting fasciotomy requirement in lower extremity arterial injuries: a retrospective case-control study.

BACKGROUND: Acute compartment syndrome (ACS) following lower extremity arterial injuries necessitates urgent fasciotomy to prevent limb loss, yet current diagnostic tools lack specificity for ischemia-reperfusion pathophysiology. Our study aimed to develop a nomogram combining biomarkers and clinical indicators to predict fasciotomy risk, enhancing early risk stratification and optimizing surgical decision-making. MATERIALS AND METHODS: In this retrospective case-control study (2010-2024), data were sourced from a tertiary hospital in China. A total of 146 patients with traumatic femoral or p

Ageing attenuates regional vasoconstriction during acute lowering of upper and lower limbs.

The venoarteriolar and myogenic response (VMR) is a non-adrenergic, non-baroreflex-mediated mechanism that increases local vascular resistance and contributes to blood pressure (BP) regulation during orthostasis. Despite the importance of the VMR in human cardiovascular control, no information exists elucidating possible differences in the VMR with ageing and sex. We studied 26 healthy young adults [9 male; mean (SD) 28 (4) years old] and 18 healthy older adults [7 male; 71 (3) years old] during acute arm and leg dependency (i.e., limb lowering below heart level) to evoke the VMR. Brachial and

Transcatheter aortic valve replacement (TAVR): access planning and strategies.

Transcatheter aortic valve replacement (TAVR) has proven to be a viable tool for the high-surgical-risk population with severe aortic valve stenosis. Vascular access complications are not uncommon with TAVR and may increase early and late mortality. Avoiding these serious complications is the goal. With experience and careful screening, we are now able to risk-stratify patients who may be at increased risk of vascular complications. While the traditional iliofemoral access site remains the most common for TAVR, alternate access sites that have proven to be viable and safe alternatives include

Method analysis for optimal continuous imaging using intravascular optical coherence tomography.

OBJECTIVES: Optical coherence tomography(OCT)is a high-resolution imaging method that can clearly visualize vessels through the displacement of blood with flushing agents. Continuous imaging methods have not been established. This study investigated optimal methods for continuous OCT imaging. METHODS: Thirty-four arteries with stent implantation (24 peripheral and 10 coronary arteries)in 14 pigs were examined using OCT with a motorized pullback device. Two imaging methods (flush alone by liquid substance and flush with occlusion) were compared. Adequate image acquisition was defined as the en
