Position Statement: Medical Autonomy and Incident Reporting.

Retropharyngeal calcific tendonitis is an inflammatory process of the superior oblique tendons of the longus colli muscle, a neck flexor in the upper cervical spine, caused by deposition of calcium hydroxyapatite crystals; the definitive diagnostic test is computed tomography (CT). Presented in this article are two cases seen at our institution. Patients typically present with acute onset of neck pain/spasm, odynophagia, dysphagia, and/or low grade fevers. Leukocytosis and elevated erythrocyte sedimentation rate may be noted. It is important to understand this entity because its signs and symp

Atlantoaxial Rotatory Subluxation: A Review for the Pediatric Emergency Physician.

Pediatric emergency physicians must have a high clinical suspicion for atlantoaxial rotatory subluxation (AARS), particularly when a child presents with neck pain and an abnormal head posture without the ability to return to a neutral position. As shown in the neurosurgical literature, timely diagnosis and swift initiation of treatment have a greater chance of treatment success for the patient. However, timely treatment is complicated because torticollis can result from a variety of maladies, including: congenital abnormalities involving the C1-C2 joint or the surrounding supporting muscles an

Malignant Rhabdoid Tumor with Cervical Vertebra Involvement in a Teenage Child: Case Report and Review of the Literature.

Malignant rhabdoid tumor (MRT) is a highly malignant and aggressive tumor associated with the kidney that is mainly seen in children under 5 years old, resulting in a rapid fatal outcome. MRT invasion of the cervical vertebra is an extremely rare form of this malignancy. This study presents a case of MRT involving the C4 vertebra and its appendix. A 13-year-old girl was found to have vague neck pain and a mass in the left neck area of the mid-cervical spine. Percutaneous needle biopsy was performed and the histological and immunohistochemical analysis pointed towards a rhabdoid tumor. Chemothe

Spontaneous pneumomediastinum with a rare presentation.

Spontaneous pneumomediastinum is an unusual and benign condition in which air is present in mediastinum. A 20-year-old male patient presented to ED with complaint of hoarseness and odynophagia from the day before, after weightlifting. The patient was nonsmoker and denied history of other diseases. On physical examination he had no dyspnea with normal vital signs. Throat examination and pulmonary auscultation were normal and no crepitation was palpable. We could not find subcutaneous emphysema in neck and chest examination. In neck and chest X-ray we found that air is present around the trachea

[Hemicape-like sensory disturbance caused by cortical infarction in the postcentral gyrus].

We report a case of a small cortical infarction in the postcentral gyrus that presented an isolated hemicape-like sensory disturbance. A 47-year-old man suddenly developed numbness and paresthesia in the left neck, shoulder, arm, and upper trunk. Examination revealed hypoesthesia to touch and pain in these areas along with a hemicape-like distribution. The sensitivity to cold and vibration was normal, and two-point discrimination and graphesthesia were preserved. The patient had a normal visual field, muscle strength, and reflexes, and there were no neuropsychological deficits. Magnetic resona

Lateral medullary syndrome in a boy with hereditary dysfibrinogenemia.

A 9-year-old boy presented with sudden onset of nausea, vomiting and unsteady gait after a bread-eating game, which possibly caused neck hyperextension. Neurological examination revealed hemisensory loss of pain and temperature sensation in the right trunk and limbs along with left Horner's syndrome, suggesting lateral medullary syndrome (LMS). Magnetic resonance (MR) imaging of the brain revealed infarction at the left lateral medulla. MR angiography showed no sign of arterial dissection and no occlusion or stenosis of the intracranial, basilar or vertebral arteries or their branches. No evid

Transcervical video-assisted thymectomy: preliminary results of a modified surgical approach.

PURPOSE: A number of surgical approaches have been reported for thymectomy, including transsternal, transcervical, a combination of complete transsternal and transcervical, and various video-assisted thoracoscopic surgery techniques. A modified video-assisted transcervical approach to thymectomy is here described. METHODS: A video-assisted total thymectomy was performed through a 30-mm cervical incision. No hyperextension of the patient's neck or sternal retractor was used. The surgical instruments utilized for the resection were the ones created for the minimally invasive video-assisted thyr

Pyrolytic carbon endoprosthetic replacement for osteonecrosis and femoral fracture of the hip: a pilot study.

UNLABELLED: Hemiarthroplasty in young patients has the potential for eliminating bearing wear, but has the disadvantage of cartilage wear. Low-temperature isotropic (LTI) pyrolytic carbon reportedly reduces cartilage wear in canine hemiarthroplasties. We therefore initiated a study in humans when it was released for human use. However, we observed failures in some patients. We therefore document and report the high failure rate observed in a subset of patients treated with an LTI pyrolytic carbon femoral head for osteonecrosis. We conducted a prospective pilot study of 17 patients treated with

Cattle with thymic lymphoma and haematoma of the ventral neck: a comparison of findings.

The goal of the present paper was to establish criteria for the differentiation of thymic lymphoma and haematoma of the ventral neck in cattle. Four cattle with thymic lymphoma and seven with haematoma of the ventral neck underwent physical, haematological, biochemical and ultrasonographic examinations as well as histological evaluation of a biopsy specimen. All of the animals had firm non-painful swellings, which were no warmer than normal and extended from the pharyngeal region to the thoracic inlet. A diagnosis could not be established based on the results of physical examination and palpat

Arthroplasty for rheumatoid forefoot deformities by a shortening oblique osteotomy.

Seventy-five feet in 47 patients (46 women, 1 man) who had rheumatoid arthritis were observed for an average of 6 years (range, 4-11 years) after an operation on the forefoot that included a shortening oblique osteotomy of the metatarsal neck of the lateral toes. In addition, patients underwent either flexible hinge toe implant arthroplasty or Mitchell's osteotomy in the first metatarsophalangeal joint. Forty-two feet (56%) looked normal with no valgus or dorsal displacement of the toes. Recurrence of callosities occurred in 9 feet (12%) with moderate pain in 3 feet. Thirty-nine (83%) patients

Screw fixation versus hemiarthroplasty for undisplaced femoral neck fractures in the elderly: a meta-analysis.

INTRODUCTION: Screw fixation and hemiarthroplasty are commonly used surgical options for treating undisplaced femoral neck fractures in the elderly. While fixation preserves the native joint, arthroplasty offers immediate mechanical stability. The optimal treatment remains controversial. METHODS: A systematic review and meta-analysis was conducted across PubMed, Scopus, Cochrane Library, and Google Scholar through June 2025. Twelve studies (including randomised controlled trials and observational cohorts) encompassing 2190 patients treated with screw fixation and 2606 with hemiarthroplasty we

Heterotopic ossification, osteolysis and implant failure following cervical total disc replacement with the M6-C™ artificial disc.

INTRODUCTION: A recent study reported a 34% mid-term revision rate after M6-C™ cervical total disc replacement (CTDR) for wear-related osteolysis. Here, we aim to investigate the prevalence, risk factors, and radiographic characteristics of periprosthetic bony changes and implant failure of the M6-C™ artificial disc. METHODS: We retrospectively analysed radiographic (conventional X-ray, CT scan) and clinical outcomes (EQ-5D-5L, Neck Disability Index (NDI), and Visual Analog Scale (VAS) for neck and arm pain) data collected during routine follow-up of patients who underwent CTDR with the M6-C™

Posterior Release, Reduction, and Intra-Articular Fusion for Irreducible Type III Atlantoaxial Rotary Fixation.

BACKGROUND AND OBJECTIVES: For irreducible atlantoaxial rotary fixation (AARF), anterior or posterior release was often needed before posterior reduction and fusion. Anterior atlantoaxial joint release has potential complications such as retropharyngeal abscess, persistent hoarseness, and infection. This study aims to assess the efficacy of posterior release, reduction, and intra-articular fusion without resecting the C2 nerve root on irreducible type III AARF. METHODS: The data of 9 pediatric patients diagnosed with AARF who underwent posterior atlantoaxial release, reduction, and intra-arti

Assessment of subcutaneous fat tissue thickness as a biomarker for cervical intervertebral disc degeneration.

Cervical intervertebral disc degeneration (IVDD) is a chronic musculoskeletal disorder contributing to neck pain, often associated with mechanical instability of the disc. Obesity and increased subcutaneous fat tissue thickness (SFTT) have been identified as key risk factors for the development of IVDD. This study retrospectively analyzed 245 Turkish female patients aged 40-50 years, who presented with neck pain and underwent cervical MRI between 2022 and 2024. Patients with malignancy, prior spinal surgery, congenital anomalies, or artifacts on MRI were excluded. The severity of IVDD at the C

Transcranial Doppler in the Detection and Management of Arterial Vasospasm after Aneurysmal Subarachnoid Haemorrhage.

Delayed cerebral ischaemia (DCI) and cerebral infarction is a much-feared complication of aneurysmal subarachnoid haemorrhage (aSAH). It has been largely attributed to focal hypoperfusion from reversible cerebral arterial narrowing, "vasospasm," from the effects of prolonged exposure of the arteries to perivascular blood and oxy-haemoglobin. Transcranial Doppler (TCD) provides a non-invasive method for detecting and monitoring vasospasm. We report a 38-year-old lady who developed sudden dizziness and catastrophic generalised headache with neck pain (Pain Score 10/10) while voiding her bowels.

Reconstruction of the spinal accessory nerve with selective fascicular nerve transfer of the upper trunk.

OBJECTIVE: Spinal accessory nerve palsy is frequently caused by iatrogenic damage during neck surgery in the posterior triangle of the neck. Due to late presentation, treatment regularly necessitates nerve grafts, which often results in a poor outcome of trapezius function due to long regeneration distances. Here, the authors report a distal nerve transfer using fascicles of the upper trunk related to axillary nerve function for reinnervation of the trapezius muscle. METHODS: Five cases are presented in which accessory nerve lesions were reconstructed using selective fascicular nerve transfer
