Quantification of relative afferent pupillary defects induced by posterior sub-Tenon's, peribulbar, and retrobulbar anaesthetics.

AIMS: The effect of local anaesthetics on optic nerve function can be investigated by quantifying the relative afferent pupillary defect (RAPD). METHODS: The study compared the depth of induced RAPD following posterior sub-Tenon's, retrobulbar, and peribulbar local anaesthetics using crossed polarising filters before cataract surgery (time 1 = 5 minutes), immediately after surgery (time 2 = 42 minutes (av)), and once again on the ward (time 3 = 107 minutes (av)). RESULTS: All patients developed a RAPD. There was no significant difference in the depth of RAPD between the groups at any one tim

Size of glaucomatous optic discs.

In normal eyes, the optic disc size shows a high interindividual variability. In the diseased eye, some optic nerve anomalies and diseases occur preferentially in small optic discs, and some are more common in large optic nerve heads. We conducted a study to determine whether glaucoma subtypes are correlated with the optic disc size or not. Stereo optic disc photographs of 997 eyes with primary open-angle glaucoma, 44 eyes with normal-tension glaucoma, 129 eyes with pseudoexfoliative glaucoma, and 850 normal eyes were examined. Compared to eyes with primary open-angle glaucoma (mean optic disc

Vascular factor in erectile failure among diabetics.

Clinical, sexual, and laboratory evaluations were made on 47 impotent diabetics. All patients were given a Doppler PAPS examination and a neurologic assessment of the penis (BCRL and/or CMG). Nerve conductivity tests of the legs were also administered to most patients. The groups were classified as to whether they were insulin or noninsulin dependent. Abnormalities of penile arterial flow were detected in 95.7 per cent of all the patients. Penile arterial occlusions without associated penile neuropathies occurred in 61.7 per cent. Penile neuropathies were found in 34 per cent, and all were ass

Progressive extrinsic ophthalmoplegia with peripheral neuropathy and storage of muscle glycogen.

A case of progressive extrinsic ophthalmoplegia associated with peripheral neuropathy and hypertriglyceridemia type IV is described. Motor and sensory conduction velocities of the spinal nerves were severely decreased, while the EMG of the facial muscles was more suggested of a myopathic disorder. Electron microscopic study of biopsies of biceps and peroneus brevis muscles disclosed many ragged red fibers, mainly type I, which contained typical abnormal mitochondria. Other fibers, all type II, contained increased amounts of glycogen between myofibrils or beneath the sarcolemmal membrane, but t

Combined Central and Peripheral Demyelination with a Favorable Outcome: A Case with 10-Year Follow-up.

INTRODUCTION: Combined central and peripheral demyelination (CCPD) is a rare and heterogeneous condition, often associated with a poor prognosis. Contrary to the popular reports, we present a case of CCPD with a favorable prognosis and a 10-year follow-up. CASE: A 29-year-old female patient presented with progressive limb weakness following a Brucella infection. She was subsequently diagnosed with motor type chronic inflammatory demyelinating polyneuropathy (CIDP), which was characterized by motor conduction blocks and demyelinating motor conduction abnormalities in a nerve conduction study (

Increased intracortical facilitation in patients with autosomal dominant pure spastic paraplegia linked to chromosome 2p.

There are at least seven clinically indistinguishable but genetically different types of autosomal dominant pure spastic paraplegia (ADPSP). In this study we investigated electrophysiological characteristics in patients with ADPSP linked to chromosome 2p (SPG4). Twelve patients from six different families with ADPSP linked to chromosome 2p and 15 control persons were included. Electromyography (EMG), motor and sensory nerve conduction, and motor evoked potentials using single and paired transcranial magnetic stimulation (PTMS) was performed. From the peripheral nervous system we found signs of

Untargeted Metabolomic Analysis in the Short-Lived Killifish Using HILIC Ultra Performance Liquid Chromatography Method Coupled to a Q Exactive Orbitrap Mass Spectrometer.

Over the past decade, advancements in liquid chromatography-mass spectrometry (LC-MS) have transformed untargeted metabolomic analysis. These improvements have led to biomarker discoveries, enabling scientists to deepen their understanding of biological systems in disease contexts. Achieving reproducibility, robustness, and reliable quantification is essential for confident metabolite identification. In this chapter, we present an untargeted metabolomic approach to analyze metabolites in the optic nerve tissue of adult Nothobranchius furzeri, an established biogerontology model that is increas

[Two elderly cases of transthyretin amyloid polyneuropathy without a family history].

We report two cases of transthyretin familial amyloid polyneuropathy (ATTR-FAP) from non-endemic areas. Both cases showed chronic progressive distal limb numbness and weakness. Due to nonspecific symptoms, they were not diagnosed for a long period of time. A nerve conduction study revealed axonal neuropathy in the lower limbs and carpal tunnel syndrome. An echo test showed thickness of the left ventricle, one of the red flag symptom clusters of ATTR-FAP. Genetic analysis revealed a mutation in the transthyretin gene. In cases with chronic progressive neuropathy, it is important to consider a d

[Ulnar neuropathy in a poultry worker].

Three months after he was employed as a poultry worker, a 48-year-old man developed involuntary jerks of his right first, fourth, and fifth fingers, paraesthesiae, weakness, and eventually wasting of the first dorsal interosseous muscle. His job entailed repetitive lifting of boxes weighing 10-25 kg with flexion of the elbow, pronation of the forearm, and ulnar deviation of the wrist. A nerve conduction study indicated ulnar neuropathy just distal to the elbow. Surgery at this level alleviated the symptoms, but shortly after his return to work, he changed jobs because of aggravation.

Brainstem auditory evoked potentials (BAEPs) in tobacco smokers.

Brainstem auditory evoked potentials (BAEPs) were studied in 10 chronic male tobacco smokes (average age 26.55 years) with history of smoking 3 to 14 years to detect early subclinical impairments in auditory pathway. The values of BAEPs were compared with those of 28 age matched normal male non-smokers (control). The results revealed significant prolongation of latencies of wave I and III indicating that conductivity of sensory impulse along acoustic nerve and pons is affected in chronic smokers which may be attributed to adverse effect on myelination by nicotine and toluene present in tobacco

Uhthoff and his symptom.

At the turn of the last century, Wilhelm Uhthoff was a renowned clinical neuro-ophthalmologist, and probably the first clinician whose entire career was devoted to this discipline. His achievements are among those that mark the commencement of contemporary neuro-ophthalmology. Uhthoff's symptom of visual loss with exercise is most frequently associated with optic neuritis. The symptom carries a major risk for recurrence of optic neuritis and development of multiple sclerosis. This study and its companion in the publication show that, independently, a metabolic byproduct of exercise or increase

Effect of Escherichia coli endotoxin on cochlear potentials following its application to the chinchilla middle ear.

The compound action potential (CAP) of the eighth nerve and the endocochlear potential (EP) were examined in the chinchilla as an animal model when Escherichia coli endotoxin (100 micrograms) was applied to the middle ear cavity. A significant elevation of the CAP threshold at 2, 3, and 4 kHz was observed 48 h after the instillation of endotoxin, but this hearing loss was thought to be caused by a conductive component. No significant change in the CAP threshold was recognized 30 days after instillation. The EP in either period showed no significant difference. These findings indicate that the

Paraesthesiae induced by prolonged high frequency stimulation of human cutaneous afferents.

1. The present study has explored the behaviour of human cutaneous afferents following conduction of prolonged trains of impulses at 200 Hz for 10-20 min, correlating the resultant changes in excitability with the perception of paraesthesiae. 2. Tetanization for 10 min resulted in activity-dependent changes in axonal excitability, with an initial period of hyperexcitability, followed by a long-lasting subexcitability. All subjects experienced paraesthesiae soon after cessation of the tetanic train, and these subsided gradually over 16 min. 3. Longer tetanic trains of 20 min duration resulted i

Impact of three-dimensional prostate models during robot-assisted radical prostatectomy on surgical margins and functional outcomes.

BACKGROUND: Robot-assisted radical prostatectomy (RARP) is the standard surgical procedure for the treatment of prostate cancer. RARP requires a trade-off between performing a wider resection in order to reduce the risk of positive surgical margins (PSMs) and performing minimal resection of the nerve bundles that determine functional outcomes, such as incontinence and potency, which affect patients' quality of life. In order to achieve favourable outcomes, a precise understanding of the three-dimensional (3D) anatomy of the prostate, nerve bundles and tumour lesion is needed. STUDY DESIGN: Th
