Consensus and difference among hospital professionals in evaluating child maltreatment.

The decision-making process in suspected cases of child maltreatment involves reaching interprofessional consensus. Interprofessional consensus in seriousness ratings of maltreatment incidents for the welfare of the child was examined by surveying 39 case vignette ratings by 295 pediatric hospital professionals from five occupations. The survey instrument was derived from research by Giovannoni and Becerrra (1979). An exploratory factor analysis yielded five categories of maltreatment: physical abuse, sexual abuse, general failures in care, minor neglect/discipline, and lifestyles/values. A si

A conventional content analysis of clinicians' perspectives on patient-based factors that influence surgical prioritization of abdominal trauma.

Operative triage of abdominal trauma, whether blunt or penetrating, remains a major challenge in emergency care. Although quantitative decision-support tools, such as Pediatric Emergency Care Applied Research Network (PECARN) rule, Focused Assessment with Sonography in Trauma (FAST), Shock Index, Pediatric Age-Adjusted (SIPA), and Blunt Abdominal Trauma Scoring System (BATSS), are widely used, they do not fully capture the complex clinical reasoning and experiential judgment required for real-world decision-making. This limitation is particularly important because blunt and penetrating abdomin

The sexual health consultation as a moral occasion.

Sexually transmitted infections (STIs) are socially constructed as more 'dirty' than other gynaecological conditions. This article analyses women's accounts of interactions with clinicians, subsequent to a diagnosis of genital herpes simplex virus or human papilloma virus. Women conceptualised consultations as a 'moral event,' different from other consultations. This moral component is highlighted drawing on Foucault's notion of 'the confessional.' Additionally, Douglas' anthropological construction of 'dirt' is used to consider why these consultations are 'confessional' experiences. Email int

A telephone- and text-message based telemedical care concept for patients with mental health disorders--study protocol for a randomized, controlled study design.

BACKGROUND: As in other countries worldwide, the prevalence of mental disorders in Germany is high. Although numerically a dense network of in- and outpatient psychiatric health services exists, the availability in rural and remote regions is insufficient. In rural regions, telemedical concepts can be a chance to unburden and complement the existing healthcare system. Telemedical concepts consisting of video or telephone consulting show first positive results, but there are only a few studies with a randomized controlled design. To improve the treatment of patients with mental disorders in rural

A rehabilitation hospital's experience with ionic silver Foley catheters.

A small rehabilitation hospital in northern Arkansas had a problem with catheter-associated urinary tract infections (CAUTI). A review of practices and policies provided no reason for the trend. The facility trialed an ionic silver Foley catheter finding that it dramatically reduced the incidence of CAUTI in their patient population.

Rh negative status and isoimmunization update: a case-based approach to care.

Prior to the 1970s and the advent of Rho (D) immune globulin (RIG) for Rh negative women, hemolytic disease of the newborn led to morbidity, long-term disabilities, and mortality. Antepartum RIG administration has been a standard of practice since 1983. Yet, Rh isoimmunization (sensitization) and its sequelae have not been completely eradicated. Rh-related issues remain clinical challenges facing perinatal and neonatal nurses. Evidence for the administration of RIG prenatally and during the postpartum period is presented including controversies and challenges. Current information about fetal a

Primary health care services provided by nurse practitioners and family physicians in shared practice.

BACKGROUND: Collaborative practice involving nurse practitioners (NPs) and family physicians (FPs) is undergoing a renaissance in Canada. However, it is not understood what services are delivered by FPs and NPs working collaboratively. One objective of this study was to determine what primary health care services are provided to patients by NPs and FPs working in the same rural practice setting. METHODS: Baseline data from 2 rural Ontario primary care practices that participated in a pilot study of an outreach intervention to improve structured collaborative practice between NPs and FPs were

Community health workers: examining the Helper Therapy principle.

A community approach to the integration of health and social services for low-income pregnant women is being addressed through Community Integrated Service System (CISS) initiatives of the Maternal Child Health Bureau. This particular CISS program model was designed to enable low-income mothers to function in a Community Health Worker (CHW) role providing social support for at-risk pregnant women. Using Riessman's notion of "helper therapy," the model was also developed to enhance the potential for CHWs to gain helper benefits. The purpose of this exploratory study was to describe perceived he

Development of a psychometrically valid training needs analysis instrument for use with primary health care teams.

The growing demand for professional updating and training within the health service has created a proliferation of post-registration courses, many of which fail to reach the appropriate personnel or the real training objectives of the participants and their managers. One reason behind this problem relates to the fact that many such courses are constructed and delivered in a haphazard way, without systematic reference to the direct and indirect consumers of the educational programmes. A more rational approach to post-experience provision of this sort might involve the methodical collection of i

Medical or Common Knowledge? Knowledge of Medical Professionals on Obesity Diagnosis Criteria and Treatment.

INTRODUCTION: Proper diagnosis of obesity, its severity, and complications and their effective treatment requires an interdisciplinary healthcare approach. Nevertheless, obesity remains under-identified and undertreated. Academic knowledge concerning obesity pathology, diagnosis, and treatment is advancing. It is not clear whether this translates into clinical practice. The goal of the study was to assess the knowledge of healthcare professionals (HCPs) on obesity and particularly on the criteria for diagnosis as well as for conservative and surgical treatment. METHODS: This cross-sectional s
