Homelessness experiences, sexual orientation, and sexual risk taking among high school students in Los Angeles.

PURPOSE: Prior studies reported homeless adolescents engage in more sexual risk than their housed peers. However, these comparisons are typically made post hoc by comparing homeless adolescent community-based samples with high school probability samples. This study uses a random sample of high school students to examine homelessness experiences and sexual risk behaviors. METHODS: A supplemental survey to the Youth Risk Behavior Survey containing questions regarding homelessness and sexual health was administered to Los Angeles high school students (N = 1,839). Multivariate logistic regression

Investigating suspected cancer clusters and responding to community concerns: guidelines from CDC and the Council of State and Territorial Epidemiologists.

This report augments guidelines published in 1990 for investigating clusters of health events (CDC. Guidelines for investigating clusters of health events. MMWR 1990;39[No. RR-11]). The 1990 Guidelines considered any noninfectious disease cluster, injuries, birth defects, and previously unrecognized syndromes or illnesses. These new guidelines focus on cancer clusters. State and local health departments can use these guidelines to develop a systematic approach to responding to community concerns regarding cancer clusters. The guidelines are intended to apply to situations in which a health dep

Qualitative assessment of the challenges to the treatment of idiopathic clubfoot by the Ponseti method in urban India.

The Ponseti method of clubfoot treatment has been shown to be a very safe, effective and minimally invasive technique when performed in medical centers in Europe and North America. However, only a limited number of studies have helped identify the challenges for effective treatment with the Ponseti method in India. In this study a qualitative approach was used through distribution of questionnaires, personal interviews and focus groups with orthopedic surgeons (in urban centers) and parents of patients with clubfoot. The following factors were evaluated: (i) physician education, (ii) alternati

Achievements during the Bone and Joint Decade 2000-2010.

Musculoskeletal diseases continue to produce major disability around the world. Advances in therapy - particularly for the inflammatory diseases - have the potential to eradicate the inflammation and thus prevent joint destruction. Surgical advances include minimally invasive and computer-assisted robotic surgery, and advances in arthroscopic surgery. The development of new musculoskeletal tissues - tendons, cartilage and bone using nanotechnology and stem cells - has the potential to revolutionise the way we approach these chronic destructive diseases as well as major trauma. With the rapid i

Herpes simplex epithelial and stromal keratitis: an epidemiologic update.

Herpes simplex virus (HSV) is associated with a variety of ocular diseases, including epithelial and stromal keratitis. HSV can cause stromal opacification and is believed to be the leading cause of infectious blindness in the developed world. An improved understanding of the global burden of HSV keratitis, including the incidence of severe vision loss, could have a significant effect on prevention and treatment and place it in perspective among causes of corneal ulceration. We found that the global incidence of HSV keratitis is roughly 1.5 million, including 40,000 new cases of severe monocul

Zoonotic diseases associated with free-roaming cats.

Free-roaming cat populations have been identified as a significant public health threat and are a source for several zoonotic diseases including rabies, toxoplasmosis, cutaneous larval migrans because of various nematode parasites, plague, tularemia and murine typhus. Several of these diseases are reported to cause mortality in humans and can cause other important health issues including abortion, blindness, pruritic skin rashes and other various symptoms. A recent case of rabies in a young girl from California that likely was transmitted by a free-roaming cat underscores that free-roaming cat

The impact of health education on the prevalence of faecal-orally transmitted parasitic infections among school children in a rural community in Cameroon.

BACKGROUND: Faecal-orally transmitted parasites are those parasites which are spread through faecal contamination of food and drinks. Infections with these parasites are among the most common in the world being responsible for considerable morbidity and mortality, especially in children. This study was carried out to determine the impact of health education on the prevalence of faecal-orally transmitted parasitic infections among primary school children in a typical African rural community. METHODS: An intervention study was conducted in two villages in the South-West Region of Cameroon. A to

Sarcoptes-World Molecular Network (Sarcoptes-WMN): integrating research on scabies.

Parasites threaten human and animal health globally. It is estimated that more than 60% of people on planet Earth carry at least one parasite, many of them several different species. Unfortunately, parasite studies suffer from duplications and inconsistencies between different investigator groups. Hence, groups need to collaborate in an integrated manner in areas including parasite control, improved therapy strategies, diagnostic and surveillance tools, and public awareness. Parasite studies will be better served if there is coordinated management of field data and samples across multidiscipli

HIV epidemiology and breakthroughs in prevention 30 years into the AIDS epidemic.

Thirty years after the first AIDS cases were reported in the United States, the HIV epidemic continues to be heavily concentrated among men who have sex with men (MSM) in the United States. MSM are heavily impacted throughout most of the world and are the predominant risk group throughout the Americas and Western Europe; heterosexuals are the predominant risk group in sub-Saharan Africa; and injection drug users predominate throughout Eastern Europe and Southeast Asia. In the United States, blacks and Latinos continue to be disproportionately affected, despite overall advances in HIV testing a

'Slowing down when you should': initiators and influences of the transition from the routine to the effortful.

BACKGROUND: 'Slowing down when you should' has been described as marking the transition from 'automatic' to 'effortful' functioning in professional practice. The ability to 'slow down' is hypothesized as an important factor in expert judgment. This study explored the nature of the 'slowing down' phenomenon intraoperatively and its link to surgical judgment. METHODS: Twenty-eight surgeons across different surgical specialties were interviewed from four hospitals affiliated with a large urban university. In grounded theory tradition, data were collected and analyzed in an iterative design, usin

Lack of evidence of transfusion transmission of Creutzfeldt-Jakob disease in a US surveillance study.

BACKGROUND: Since 2004, several reported transfusion transmissions of variant Creutzfeldt-Jakob disease (vCJD) in the United Kingdom have reawakened concerns about the possible risk of similar transmissions of nonvariant or classic forms of CJD. STUDY DESIGN AND METHODS: Patients with a CJD diagnosis and a history of donating blood were reported to the study coordinator. Through review of blood distribution and hospital records, the recipients of blood components from these donors were identified. We then determined each recipient's vital status and, if deceased, the cause(s) of death identif

Arthritis in Aboriginal Manitobans: evidence for a high burden of disease.

OBJECTIVE: To evaluate the relative burden of arthritis and patterns of care in Aboriginal Manitobans, using multiple data sets to ensure a representative picture. METHODS: Arthritis burden and healthcare utilization was ascertained using 3 separate data sources. Physician claims for 3 common ICD-9 musculoskeletal diagnoses were abstracted from the Population Health Research Data Repository for First Nations (FN) Manitobans and compared to all other Manitobans. Self-reported arthritis rates were obtained from the Manitoba First Nations Regional Longitudinal Health Survey (MFN Survey), which s

Does deprivation of area of residence influence the incidence, tumour site or T stage of cutaneous malignant melanoma? A population-based and clinical database study.

This study aimed to document the incidence of malignant melanoma at specific subsites in men and women, stratified by deprivation of area of residence in southeast England, and to explore the association between deprivation and tumour thickness at diagnosis. Data were extracted on 6468 cases from the Thames Cancer Registry for the years 1998 to 2002, and data on, and 508 cases were extracted from the clinical database of the Skin Tumour Unit, St Thomas' Hospital, for the years 1996 to 2004. The postcode of residence was used to assign quintiles of deprivation based on the income domain stated

Laboratory animal allergy: an occupational hazard.

Laboratory animal allergy is a relatively common work-related condition occurring in an estimated one-third of laboratory animal workers. More than 10% of these workers develop occupational asthma. Sensitization often occurs in the first 3 years of employment. Risk factors include a personal or family history of atopy, other preexisting non-work-related allergies, and a significant exposure to laboratory animals. Inhalation is the most common route of exposure, followed by skin and eye exposures. Preplacement testing and regular health surveillance screening may be used by institutions employi

Balancing prevention and screening among international migrants with tuberculosis: population mobility as the major epidemiological influence in low-incidence nations.

Tuberculosis infection and disease remain a significant cause of global morbidity and mortality. The burden of tuberculosis disease is greatest in the developing nations of the world, although the effect of imported disease is observed in low-incidence tuberculosis regions, represented predominantly by high-income countries. In these regions, national tuberculosis control and elimination programmes are increasingly challenged to address disease in foreign-born residents. Immigration policies and shifting migration patterns over the past 5 decades have brought larger numbers of permanent and te
