Facilitation of functional independence after neurotrauma: strategies for increasing physiologic readiness.

BACKGROUND: While acute rehabilitation following neurotrauma has evolved over the past 30 years to include the common use of pharmacologic agents to promote synaptogenesis and improve recovery potential, little guidance exists for a similar strategy in the post-acute or community re-entry phases of injury. Drawing upon the existing scientific literature, models of pharmacologic intervention in promoting stability in other disease states and the authors' collective clinical experience, this article provides a potential structure by which to implement methods to create a stable physiologic platf

Carbamazepine for prevention of chemotherapy-induced nausea and vomiting: a pilot study.

CONTEXT AND OBJECTIVE: Nausea and vomiting are major inconveniences for patients undergoing chemotherapy. Despite standard preventive treatment, chemotherapy-induced nausea and vomiting (CINV) still occurs in approximately 50% of these patients. In an attempt to optimize this treatment, we evaluated the possible effects of carbamazepine for prevention of CINV. DESIGN AND LOCATION: Prospective nonrandomized open-label phase II study carried out at a Brazilian public oncology service. METHODS: Patients allocated for their first cycle of highly emetogenic chemotherapy were continuously recruite

Complaints of Poor Sleep and Risk of Traffic Accidents: A Population-Based Case-Control Study.

INTRODUCTION: This study aimed to determine the sleepiness-related factors associated with road traffic accidents. METHODS: A population based case-control study was conducted in 2 French agglomerations. 272 road accident cases hospitalized in emergency units and 272 control drivers matched by time of day and randomly stopped by police forces were included in the study. Odds ratios were calculated for the risk of road traffic accidents. RESULTS: As expected, the main predictive factor for road traffic accidents was having a sleep episode at the wheel just before the accident (OR 9.97, CI 95%

Cognitive-behavioural factors that predict sleep disturbance 4 years later.

OBJECTIVE: Cognitive models have identified a number of factors that maintain sleep disturbance. It is unknown whether similar factors lead to initial onset. This study tested whether three cognitive-behavioural factors: ruminative style, somatic sensitivity, and behavioural avoidance predicted sleep disturbance onset at four-year follow-up. METHODS: Participants (n=4,042) were assessed using a) a composite measure of Somatic Sensitivity, b) the Ruminative Style Scale, c) the Behavioural Inhibition Scale of the BISBAS and d) sleep-related items from the Goldberg Depression and Anxiety Scale.

Sleep disruption and interstitial cystitis symptoms in women.

Four hundred and seven women with interstitial cystitis participated in a Web-based study. One hundred percent of participants had poor sleep, with a mean sleep quality score of 13.4. This indicates very poor sleep. The predictor variables (nocturia, pain, and urinary urgency) were significant predictors of sleep quality when controlling for confounding variables.

Drugs for the treatment of menopausal symptoms.

IMPORTANCE OF THE FIELD: Over the last decade, the management of the menopause has attracted extensive public and professional debate and has become one of the most controversial areas in clinical practice. AREAS COVERED IN THIS REVIEW: This review provides an overview of the field, primarily from a clinical practice perspective. However, as we have incorporated in this 'big-picture' snapshot of the field both conventional and complementary approaches to managing the menopause, it is not an exhaustive review of the literature. WHAT THE READER WILL GAIN: By reviewing menopausal management fro

The relationship between sleep problems and neuropsychological functioning in children with first recognized seizures.

Epilepsy is associated with sleep disturbance, but little is known about how early this relationship develops and how it affects neuropsychological functioning. This study documented the frequency and types of sleep problems and examined how sleep problems are associated with seizures and neuropsychological functioning in 332 children following their first recognized seizure (ages 6-14) and in 225 sibling controls. Formal neuropsychological batteries were administered to all subjects. Sleep was measured using the Sleep Behavior Questionnaire and the Child Behavior Checklist. Sleep problems wer

Nocturnal gastroesophageal reflux disease: issues, implications, and management strategies.

The consequences of nocturnal gastroesophageal reflux disease (GERD) may be greater than daytime GERD in terms of clinical complications such as increased risk of esophageal lesions and respiratory conditions, as well as issues of health-related quality of life, sleep, work productivity, and economics. Proton pump inhibitors (PPIs) are the most effective therapy for patients with GERD; however, treatment success is greater in the improvement of daytime symptoms and acid control. Acid suppression of most PPIs, which are administered once daily before breakfast, wanes during the nighttime hours.

'Office-hour' ambulatory blood pressure monitoring is sufficient for blood pressure diagnosis.

We conducted this study to determine whether 'office hour', defined as time period from 0800 to 1800 hours, ambulatory blood pressure monitoring (ABPM) predicts daytime ('waking-hour') and 24-h ABPM results, and to examine the impact of sleep disturbance on ABPM and nocturnal dip. Eighty-four patients (mean age 49+/-18 years, 47 males) were studied. Systolic, diastolic and mean 4-, 6-, 8-, 'office-hour' as well as 'waking-hour' blood pressures (BPs) were obtained from 24-h ABPM readings. Of these, no statistical differences were found between 8-h and 'office-hour' systolic, diastolic and mean

Neuroendocrine aspects of chronic fatigue syndrome.

Chronic fatigue syndrome (CFS) is a serious health concern affecting over 800000 Americans of all ages, races, socioeconomic groups and genders. The etiology and pathophysiology of CFS are unknown, yet studies have suggested an involvement of the neuroendocrine system. A symposium was organized in March 2001 to explore the possibility of an association between neuroendocrine dysfunction and CFS, with special emphasis on the interactions between neuroendocrine dysfunction and other abnormalities noted in the immune and autonomic nervous systems of individuals with CFS. This paper represents the

[Sleep in adolescents and its disorders. A survey in schools].

OBJECTIVES: Numerous studies emphasized the high prevalence of sleep problems in adolescents. However, it remains to be seen whether these problems are developmental or if they are related to pathological conditions. In order to try to answer this question, we conducted an epidemiological study investigating the prevalence and correlates of sleep disorders in a population of high school adolescents. METHOD: The sample for this investigation consisted of 652 high school students (344 males and 308 females), aged 13 to 19 years (mean age: 15.1 1.2 years), attending colleges in the North of Fran

Modafinil for excessive daytime sleepiness in myotonic dystrophy.

The authors conducted an open-label trial of modafinil for excessive daytime sleepiness in myotonic dystrophy. Eleven patients were evaluated: two were not treated because of obstructive sleep apnea, and nine received 200 to 400 mg modafinil/day for an average of 16.4 weeks. There were no major side effects. Average sleep latency as measured by the Multiple Sleep Latency Test increased from 7.3 to 22.7 minutes ( p = 0.00013), and average Epworth Sleepiness Scale score decreased from 13.25 to 7.75 (p = 0.01028). Modafinil shows evidence of effectiveness for excessive daytime somnolence in myoto

Psychometric evaluation of daytime sleepiness and nocturnal sleep onset scales in a representative community sample.

BACKGROUND: The public health importance of daytime sleepiness as a risk factor for accidents, interpersonal problems, and decreased productivity has been recognized. However, epidemiologic research on this topic has been limited by the reliance on laboratory measures (i.e., the Multiple Sleep Latency Test-MSLT). Two scales, daytime sleepiness and nocturnal sleep onset, have been identified from the self-report Sleep-Wake Activity Inventory (SWAI) in a clinic sample and validated against the MSLT. This study evaluates the replicability of the two scales in a population sample and assesses pote

INTRODUCTION: Recently a series of cases has been reported characterized by myoclonic crises similar to those occurring in benign myoclonic epilepsy of childhood. However, these crises only occurred after unexpected tactile or auditory stimuli. These clinical conditions represent a new epileptic syndrome, which is age-dependent and has been called benign myoclonic epilepsy of childhood. CLINICAL CASE: We present the case of a 12 month old girl with myoclonic crises which occurred only after auditory or tactile stimuli. The myoclonia could be set off whilst awake or asleep. Physiological sleep disturbance in children with atopic dermatitis: a case control study.

It is generally acknowledged that sleep disturbance commonly complicates atopic dermatitis (AD), but the nature of this disturbance, including its physiologic aspects, has been little studied, especially in children. The results of home polysomnography (PSG) were compared for 20 school-age children with AD and sex- and age-matched healthy controls. The findings provided objective confirmation of disruption of sleep by both brief and longer awakenings associated with scratching episodes. Sleep efficiency was reduced but other sleep variables were not significantly affected. Home sleep studies o
