Patient-Specific Electric Field Simulation In Spinal Metastasis Electrochemotherapy.

Electrochemotherapy (ECT) combines the administration of cytotoxic agents with high-voltage electric pulses that transiently permeabilize tumor cell membranes and enhance intracellular drug uptake. This minimally invasive, nonthermal technique is particularly suitable for tumors located near critical structures, where surgery, radiotherapy, or percutaneous thermal ablation may be limited. In the spine, ECT can provide pain relief, neural decompression, and local tumor control while preserving neural structures. However, its implementation remains challenging due to complex vertebral anatomy, l

A Personalized Multimodal BCI-Soft Robotics System for Rehabilitating Upper Limb Function in Chronic Stroke Patients.

Multimodal brain-computer interfaces (BCIs) that combine electrical features from electroencephalography (EEG) and hemodynamic features from functional near-infrared spectroscopy (fNIRS) have the potential to improve performance. In this paper, we propose a multimodal EEG- and fNIRS-based BCI system with soft robotic (BCI-SR) components for personalized stroke rehabilitation. We propose a novel method of personalizing rehabilitation by aligning each patient's specific abilities with the treatment options available. We collected 160 single trials of motor imagery using the multimodal BCI from 1

Redox properties of quercetin iron Ii complex with enhanced antioxidant and antiviral activities.

Quercetin is a naturally occurring flavonoid widely recognized for its potent antioxidant, anti-inflammatory, and anticancer effects. However, its clinical application is limited by low water solubility and poor bioavailability. In this study, the complexation of quercetin with iron (II) ions was systematically investigated through electrochemical, spectroscopic, biological, and computational techniques to improve its physicochemical and pharmacological performance. Cyclic voltammetry revealed that quercetin undergoes irreversible oxidation due to semiquinone and para-quinonemethide formation,

Using a single penetrating interfascicular electrode to improve spatial selectivity of an extraneural polymeric cuff array.

BACKGROUND: Damage to the peripheral nervous system severely disrupts motor control, sensory perception, and organ function, often resulting in long-term disability. To restore such impairments, peripheral nerve interfaces (PNIs) aim to achieve precise stimulation selectivity, yet current approaches face several limitations. Most PNIs rely on metal-based electrodes, which introduce a mechanical mismatch with soft neural tissue and are limited by low charge-injection capacity. The device design of these PNIs also suffers from a fundamental trade-off: highly invasive approaches enable high selec

Treatment Strategy for Subaxial Minimal Facet/Lateral Mass Fractures: A Comprehensive Clinical Review.

Minimal facet and lateral mass fractures of the subaxial cervical spine (C3-C7) are a distinct subset of spinal injuries that present diagnostic and therapeutic challenges. These fractures often result from low-energy trauma or hyperextension mechanisms. They are frequently stable. However, subtle fracture instability and associated soft tissue injuries may lead to delayed instability, neurological compromise, and/or chronic severe pain if not properly identified. Accurate diagnosis relies on a combination of plain radiography, high-resolution computed tomography (CT), and magnetic resonance i

A patient with Eradheim-Chester disease presenting with progressive cystic lung lesions and confirmed pulmonary artery hypertension: a case report.

BACKGROUND: Erdheim-Chester disease (ECD), a rare type of non-Langerhans cell histiocytosis, was classified as a haematopoietic tumour by the World Health Organization (WHO) in 2016. It involves multiple systems and is challenging to diagnose due to its broad spectrum of clinical manifestations. The pulmonary manifestations of ECD lack specificity. We present a case of ECD with pronounced cystic lung abnormalities to increase awareness of the disease among pulmonologists and expedite diagnosis and treatment. CASE PRESENTATION: We report the case of a 44-year-old male who presented with interm

Utility of ABI and API Versus CTA to Identify Surgically Significant Arterial Injury After Lower Extremity Trauma in a LMIC.

BACKGROUND: South Africa faces a high burden of trauma-related vascular injury. Prompt diagnosis and management are crucial to limit morbidity and mortality. Literature recommends a thorough vascular examination of at-risk patients. Ankle brachial index (ABI) and arterial pressure index (API) are considered reliable screening tools for lower extremity vascular injury (LEVI) in the correct clinical scenario. Patients with an abnormal ABI/API warrant diagnostic imaging with computed tomography angiography (CTA). However, recent international literature demonstrates a trend toward potential CTA o

From Scan to Surgery: Evaluating High-Resolution Computed Tomography (HRCT) and Diffusion-Weighted Magnetic Resonance Imaging (DWI) Accuracy in Detecting Cholesteatoma.

Background Cholesteatoma is a destructive middle ear lesion that frequently leads to bone erosion and complications if not detected early. Accurate imaging is crucial for timely diagnosis and effective surgical planning. High-resolution computed tomography (HRCT) provides excellent anatomical detail, particularly for detecting bony erosions, but it is limited in differentiating cholesteatoma from granulation tissue. Diffusion-weighted magnetic resonance imaging (DWI) has emerged as a non-invasive modality with strong potential for improving diagnostic accuracy. This study aimed to assess the d

Lupus Enteritis as the Early Manifestation of Systemic Lupus Erythematosus Successfully Managed With Belimumab: A Case Report.

Lupus enteritis (LE) is an uncommon cause of abdominal pain in individuals with systemic lupus erythematosus (SLE). This condition is explained by the inflammation of the intestinal wall caused by the accumulation of immune complexes and activation of the complement system. A 19-year-old previously healthy Kuwaiti woman arrived at the emergency department (ED) complaining of diffuse abdominal pain, non-bloody diarrhea, and nausea. The symptoms started two days before presentation. The patient described the abdominal pain as sudden, sharp, and stabbing in nature, with a severity of 10 out of 10

Coronal plane alignment of the knee classification system does not reliably predict tibiofemoral joint gaps in arthritic knees undergoing total knee arthroplasty.

PURPOSE: Achieving optimal mediolateral balance in extension and flexion is critical for improving outcomes in total knee arthroplasty (TKA). Knee classification tools, like coronal plane alignment of the knee (CPAK), do not directly consider the soft tissue profile of the joint. This study evaluated the variation in tibiofemoral gap measurements across CPAK phenotypes and examined the relationships between preoperative tibiofemoral joint gaps and the arithmetic hip-knee-ankle angle (aHKA) and joint line obliquity (JLO), important for TKA planning and execution. METHODS: A retrospective analy

Multifrequency Magnetic Resonance Elastography Detects Small Abdominal Lymph Node Metastasis by High Stiffness.

OBJECTIVES: Response Evaluation Criteria in Solid Tumors (RECIST) 1.1 is a clinical and research standard for evaluating malignant tumors and lymph node metastasis. However, quantitative analysis of nodal status is limited to measurement of short axis diameter (SAD), and metastatic lymph nodes below 10 mm in SAD are often not detected. The purpose of this study was to evaluate the value of multifrequency magnetic resonance elastography (MRE) when added to RECIST 1.1 for detection of lymph node metastasis. MATERIALS AND METHODS: Twenty-five benign and 82 metastatic lymph nodes were prospective

"sCT-Feasibility" - a feasibility study for deep learning-based MRI-only brain radiotherapy.

BACKGROUND: Radiotherapy (RT) is an important treatment modality for patients with brain malignancies. Traditionally, computed tomography (CT) images are used for RT treatment planning whereas magnetic resonance imaging (MRI) images are used for tumor delineation. Therefore, MRI and CT need to be registered, which is an error prone process. The purpose of this clinical study is to investigate the clinical feasibility of a deep learning-based MRI-only workflow for brain radiotherapy, that eliminates the registration uncertainty through calculation of a synthetic CT (sCT) from MRI data. METHODS

Segmentation-based BI-RADS ensemble classification of breast tumours in ultrasound images.

BACKGROUND: The development of computer-aided diagnosis systems in breast cancer imaging is exponential. Since 2016, 81 papers have described the automated segmentation of breast lesions in ultrasound images using artificial intelligence. However, only two papers have dealt with complex BI-RADS classifications. PURPOSE: This study addresses the automatic classification of breast lesions into binary classes (benign vs. malignant) and multiple BI-RADS classes based on a single ultrasonographic image. Achieving this task should reduce the subjectivity of an individual operator's assessment. MAT

Benign intestinal pneumatosis associated with congenital Morgagni-Larrey diaphragmatic hernia.

Intestinal pneumatosis (IN) is an uncommon radiological finding defined as the accumulation of air in the gastrointestinal tract wall. Its clinical signs are nonspecific and include symptoms such as diarrhea or abdominal pain. It includes benign entities (with subtle symptoms and the accumulation of air in the form of cysts that appear as clustered nodular lesions on the endoscopy, collapsible and soft); or severe cases (symptoms indicative of general health compromise and linear accumulation of air or free fluid suggestive of hollow viscus perforation); which require different management. We
