Severe left kidney laceration after minor fall in a patient with lumbar spine scoliosis.

BACKGROUND: The kidneys are one of the organs most commonly affected by blunt trauma; however, the anatomical variations that predispose kidney injury are poorly understood. CASE PRESENTATION: An 80-year-old Japanese woman with lumber scoliosis was brought to our hospital because of severe flank pain after a minor fall at home. Contrast-enhanced computed tomography showed a severe left kidney laceration at the level of an acute angle in the vertebral column caused by scoliosis. Extravasation of contrast medium from the left renal artery branches was also noted. Selective arterial embolization

Intracranial solitary fibrous tumor with delayed symptomatic metastasis to the lumbar spine: illustrative case.

BACKGROUND: Intracranial solitary fibrous tumors (SFTs), formerly known as hemangiopericytomas, are rare, aggressive mesenchymal extra-axial tumors typically treated via resection, often with preoperative embolization and postoperative radiation and/or antiangiogenic therapy. Although surgery confers a significant survival benefit, local recurrence and distant metastasis are not uncommon and may occur in a delayed fashion. OBSERVATIONS: The authors describe the case of a 29-year-old male who initially presented with headache, visual disturbance, and ataxia, and was found to have a large right

Cervical Spine Instability Screening Tool Thai Version: Assessment of Convergent Validity and Rater Reliability.

Neck pain, dizziness, difficulty supporting the head for an extended period, and impaired movement are all symptoms of cervical spine instability, which may produce cervical spondylolisthesis in patients who have more severe symptoms. To avoid problems and consequences, early detection of cervical spine instability is required. A previous study created a Thai-language version of a cervical spine instability screening tool, named the CSI-TH, and evaluated its content validity. However, other characteristics of the CSI-TH still needed to be evaluated. The objective of the current study was to as

Association Between Industry Sponsorship of Spine-Related Clinical Trials, Publication Status, and Research Outcomes.

STUDY DESIGN: Observational Database Study. OBJECTIVES: Prospective clinical trials in spinal surgery are expensive to conduct, especially when randomized, appropriately powered, and/or multicentered. Industry collaborations generate symbiotic relationships promoting technological advancement; however, they also allow for bias. To the authors' knowledge, there is no known analysis of correlations between industry sponsorship and publication rates of spine-related clinical trials. This observational work evaluates such potential associations. METHODS: The ClinicalTrials.gov database was queri

[Our experience with minimally invasive decompression spine surgeries performed with microscopic and endoscopic visualization].

Introduction: The rapid development of minimally invasive techniques did not leave neurosurgery untouched either. New surgical interventions, both skull and spine surgery, are gaining more and more popularity. Due to the aging society, previously inoperable degenerative abnormalities are also receiving more and more attention. In the treatment of spinal canal stenosis caused by degenerative changes, the initially used laminectomy was replaced by unilateral “over-the-top” surgery, and in recent years, by techniques that ensure the least possible collateral tissue damage, such as tubular retract

Vertebral Compression Fracture After Spine Stereotactic Body Radiotherapy: The Role of Vertebral Endplate Disruption.

BACKGROUND AND OBJECTIVES: Vertebral compression fracture (VCF) is a common, but serious toxicity of spinal stereotactic body radiotherapy (SBRT). Several variables that place patients at high risk of VCF have previously been identified, including advanced Spinal Instability Neoplastic Score (SINS), a widely adopted clinical decision criterion to assess spinal instability. We examine the role of tumoral endplate (EP) disruption in the risk of VCF and attempt to incorporate it into a simple risk stratification system. METHODS: This study was a retrospective cohort study from a single instituti

Advancing the Role of Proton Therapy for Spine Metastases Through Diagnostic Scan-Based Planning.

PURPOSE: Many patients with metastatic cancer live years beyond diagnosis, and there remains a need to improve the therapeutic ratio of metastasis-directed radiation for these patients. This study aimed to assess a process for delivering cost-effective palliative proton therapy to the spine using diagnostic scan-based planning (DSBP) and prefabricated treatment delivery devices. MATERIALS AND METHODS: We designed and characterized a reusable proton aperture system that adjusts to multiple lengths for spine treatment. Next, we retrospectively identified 10 patients scan treated with thoracic p

Changes in dendritic complexity and spine morphology following BCG immunization in APP/PS1 mice.

Bacillus Calmette - Guerin (BCG) is an immune regulator that can enhance hippocampal synaptic plasticity in rats; however, it is unclear whether it can improve synaptic function in a mouse model with Alzheimer's disease (AD). We hypothesized that BCG plays a protective role in AD mice and investigated its effect on dendritic morphology. The results obtained show that BCG immunization significantly increases dendritic complexity, as indicated by the increased number of dendritic intersections and branch points, as well as the increase in the fractal dimension. Furthermore, the number of primary

Human Exposure to Hand-Arm Vibration from a Surgical Drill During Simulated Spine Surgery.

OBJECTIVE: During spine surgery, surgeons are exposed to vibrations from surgical drills. Increased exposure to vibration can result in neurologic, vascular, and musculoskeletal impairments. To reduce these risks, occupational health standards have been implemented to limit exposure levels. The purpose of this study was to quantify human exposure to hand-arm vibration from a surgical drill during a simulation of a common spine procedure. METHODS: Vibration measurements were collected during three 30-second surgical trials on a fresh frozen cadaver torso specimen using a standard surgical dril

Value of Evoked Potential Changes Associated with Neck Extension Prior to Cervical Spine Surgery.

BACKGROUND: Multimodal intraoperative monitoring (MIOM) is a useful tool to warn surgeons to intervene for intraoperative spinal cord injury in cervical spine surgery. However, the value of MIOM remains controversial before cervical spine surgery. OBJECTIVE: To explore the value of MIOM in early detecting spinal cord injury associated with neck extension before cervical spine surgery. METHODS AND MATERIALS: Data of 191 patients receiving cervical spine surgery with the MIOM were enrolled from June 2014 to June 2020. The subjects were divided into a group of evoked potentials (EP) changes and

Spine Fragility Fracture Prediction Using TBS and BMD in Postmenopausal Women: A Bayesian Approach.

The trabecular bone score (TBS) estimates bone microarchitecture and can be used to evaluate the risk of osteoporotic fractures independently of bone mineral density (BMD). In this retrospective case-control study, we tested and compared the ability of TBS and lumbar spine BMD (LS-BMD) to predict vertebral fragility fractures. The inclusion criteria were female sex, age range 50-90 years, menopause, and clinical risk factors for osteoporosis. Patients with secondary osteoporosis were excluded. LS-BMD and TBS were measured at the L1-L4 vertebral level. The ability of the two diagnostic systems
