A Comparative Analysis of Standard vs Jelonet/Gauze/Gamgee Dressing for Lower Limb Free Flap Reconstruction.

UNLABELLED: Lower limb free flaps require regular monitoring in the immediate days postoperatively and often involve prolonged outpatient wound care. Post-operative dressings are complicated when a circular external fixator is used. Circumferential post-operative dressings are the standard option, such as Jelonet/gauze/wool/crepe. An alternative option is the use of Jelonet and gauze, with Gamgee (JGG) wrapped loosely over the frame instead. This allows easier monitoring with complete flap visualisation, avoids constriction and allows much quicker dressing application. This dressing option has

The future of fungal diagnostics: What's next in laboratory testing for invasive fungal infection?

Invasive fungal infection (IFI) is responsible for millions of deaths globally each year. Prompt diagnosis of IFI is critical, with delay associated with increased mortality. Traditional, culture-based diagnostics are relatively slow and insensitive. Novel molecular techniques, biomarkers and point-of-care tests hope to overcome these limitations. However, these newer methods come with issues of cost, accessibility, and standardisation. This review briefly lays out the current diagnostic landscape, highlights important changes to standards of care, then explores new developments and the future

Achieving age-friendly cancer care through the NICHE practice model.

More than 50% of cancers occur in adults aged 70 years and older, and this population accounts for 60% of all cancer deaths. Cancer care for older adults is complex and influenced by comorbidities, age-related physiologic changes, social needs, and cognitive or functional limitations. This NICHE column addresses the alignment of the NICHE practice model and resources to advance evidence-based care of older adults with cancer in conjunction with the Institute for Healthcare Improvement's Age-Friendly 4Ms framework and international professional oncology nursing practice standards.

Conservative management of spontaneous pneumothorax: A review of evidence and guidelines.

The role of conservative management in spontaneous pneumothorax, an increasingly recognized alternative to invasive procedures is of interest. Recent evidence suggests that observation, oxygen therapy and symptom-based care provide comparable outcomes to invasive methods in stable primary spontaneous pneumothorax. Conservative strategies are associated with reduced complications, shorter hospital stays and improved patient satisfaction. However, controversies exist regarding secondary spontaneous pneumothorax, recurrence risks and the need for standardized guidelines. Further research is neede

Is There a Role for Comfort Care in Neonates With Severe Craniofacial Anomalies? Case Report and Review of Quality-of-Life Literature.

This article aims to determine how quality of life (QoL) is defined and assessed in cases of severe craniofacial anomalies, as well as the impact such considerations may have on the treatment of a neonate with these conditions with respect to palliative neonatal care. Our literature review found insufficient evidence to suggest that craniofacial anomalies result in consistently poor QoL. Based on these findings and in line with the current acceptable standards for the ethical care of neonates, with the exception of rare cases, resuscitative efforts should always be performed on patients with i

Innovations in Care of the Elderly Hip Fracture Patient; a Nightmare No More.

Hip fractures that occur in the elderly population continue to be a growing problem in communities. In this particular population, people 65 years and older, patients often have multiple comorbidities. These comorbidities cause challenges for a patient's optimization preoperatively and throughout the continuum of care. Although unplanned and traumatic, hip fracture patients can benefit from care organized in a comprehensive and standardized way. Having a hip fracture program that is driven by a multidisciplinary team approach has proved to decrease length of stay and promote positive outcomes

Group B Streptococcal Bacteriuria in Pregnancy: An Evidence-Based, Patient-Centered Approach to Care.

Screening and management of group B streptococcus (GBS) bacteriuria in pregnancy aims to reduce the incidence of pyelonephritis and GBS-related neonatal morbidity and mortality. Universal screening and management of GBS bacteriuria in pregnancy are standards of care in the United States; however, some women may decline guideline-based recommendations for screening, treatment, or intrapartum antibiotic prophylaxis. This article uses a case study approach to discuss evidence-based, patient-centered care for GBS bacteriuria in pregnancy as well as ethical incorporation of individual patient prefe

Hypertensive Emergencies in the Emergency Department.

Hypertension affects approximately one-third of Americans. An additional 30% are unaware that they harbor the disease. Significantly increased blood pressure constitutes a hypertensive emergency that could lead to end-organ damage. When organs such as the brain, heart, or kidney are affected, an intervention that will lower the blood pressure in several hours is indicated. Several pharmacologic options are available for treatment, with intravenous antihypertensive therapy being the cornerstone, but there is no standard of care. Careful consideration of each patient's specific complaint, histor

Emergency management of sickle cell disease complications: review and practice guidelines.

The prevalence of sickle cell disease (SCD) in Minnesota is increasing because of the influx of immigrants from Africa, India, and countries in Latin America. Clinicians, families, and individuals with the disease have expressed the need to educate health care professionals in emergency settings about the standards of care for treating pain and other complications of SCD. Late last year, the Minnesota Department of Health and the Minnesota Hemoglobinopathy Collaborative created guidelines for treating patients with complications of SCD in the emergency department. This article provides additio

Hypertensive disorders of pregnancy.

Hypertensive disorders occur in 6% to 8% of all pregnancies, are the second leading cause of maternal death, and contribute to significant neonatal morbidity and mortality. This is a problem not only in inpatient settings, as ambulatory and home-care nurses are increasingly being called upon to monitor women who are at high risk and may have hypertensive disorders. To prevent hypertension-induced problems in pregnant women, nurses must have strong assessment, advocacy, and counseling skills. Nurses also must provide care based on the latest national standards as described in this article.

[New methods of prevention and treatment of the stress-related disorders in children with burn disease].

An assessment of the key pathogenesis chains involved in triggering the vital complications was based on a prospective randomized controllable retrospective study of a burn-disease clinical course in 473 children. As a result of it, a standardized system was worked out, within a large industrial region, designed to render the medical care to children with severe thermal lesion; such system cut the therapy time by 12 days in the intensive care unit and by 2.6 time--in hospital; it also contributed to reducing the purulent-and-septic complications from 73.9% to 21% and the lethality rate--from 3

User-Centered Design Approach to Dynamic Clinical Decision Support in Primary Prevention of Cardiovascular Disease.

Background/Objectives: National guidelines include recommendations for cardiovascular disease (CVD) prevention, but gaps in prevention still exist. Clinical decision support (CDS) tools may be an avenue for delivering preventive care. The goal of this project was to understand how clinicians approach primary prevention of CVD and to determine barriers and facilitators in delivery of primary preventive care with and beyond a CDS tool. Methods: A user-centered design approach including a cross-sectional survey and qualitative interviews was deployed in a health system in Central and Northern Cal

Combat orthopedic trauma care: Challenges and innovations in Ukraine's wartime response.

Orthopedic trauma care in Ukraine, shaped by the ongoing conflict since the Donbas invasion in 2014 and further intensified by the 2022 Russian invasion, reflects the complexity of treating modern wartime injuries in the face of resource constraints and evacuation challenges. Ukrainian medical teams have to the greatest extent possible adhered to Western medical standards, using a tiered care system that spans from the point of injury, to forward surgical teams leading, finally to advanced procedures in tertiary facilities. However, resource shortages and delayed evacuations have led to signif

Society for Maternal-Fetal Medicine Special Statement: Checklists for management of pregnancies complicated by stillbirth.

Stillbirth is one of the most common adverse pregnancy outcomes, occurring in 1 in 160 deliveries in the United States. In an effort to provide a standardized approach to the management and evaluation of stillbirth, the American College of Obstetricians and Gynecologists and the Society for Maternal-Fetal Medicine developed an Obstetric Care Consensus published in March 2020, "Management of Stillbirth." The consensus document describes detailed considerations for the acute medical management of a newly diagnosed stillbirth, including recommended initial evaluation, delivery considerations, and
