Psychosocial correlates of short stature and delayed puberty.

Studies relating to the psychosocial aspects of short stature and pubertal delay are reviewed. Although IQ scores show a statistical correlation with stature, significant intellectual, psychological, or academic deficits have not been consistently demonstrated for short children. However, hypopituitary growth failure is associated with poor social adjustment as an adult despite growth hormone therapy. The reasons for this remain unclear. Constitutional pubertal delay in boys can lead to social and academic problems. Treatment with a short course of testosterone can be beneficial in selected ca

Current Challenges and Future Innovation in Rural Heart Failure Care: A Qualitative Analysis.

BACKGROUND: Individuals living with heart failure experience challenges managing frequent hospital admissions and life-altering symptoms, including dyspnea, fatigue, and depression. These challenges are exacerbated by the unique obstacles associated with receiving healthcare in a rural community. Their care partners often do not receive sufficient support to care for someone with heart failure and experience fatigue, social isolation, and anxiety. OBJECTIVE: Explore the insights of individuals involved in and/or receiving heart failure care at a rural academic medical center with the aim of i

Heterogeneity of Academic Stress Profiles and Depression-Anxiety Symptom Networks in Left-Behind Children: A Latent Profile and Simulated Intervention Network Analysis.

Compared with non-left-behind children (NLBC), left-behind children (LBC) face a higher risk of academic stress, depression, and anxiety symptoms due to separation from their parents; however, the heterogeneity of academic stress profiles and their relationships with the symptom network remain insufficiently explored. To address this gap, a cross-sectional survey of 10,524 Chinese children compared LBC (n = 2487) and NLBC. Latent profile analysis (LPA) was first conducted to identify academic stress subgroups among LBC. Subsequently, depression-anxiety symptom networks were estimated using Isi

Sepsis, the Administration of IV Fluids, and Respiratory Failure: A Retrospective Analysis-SAIFR Study.

BACKGROUND: Although resuscitation with IV fluids is the cornerstone of sepsis management, consensus regarding their association with improvement in clinical outcomes is lacking. RESEARCH QUESTION: Is there a difference in the incidence of respiratory failure in patients with sepsis who received guideline-recommended initial IV fluid bolus of 30 mL/kg or more conservative resuscitation of less than 30 mL/kg? STUDY DESIGN AND METHODS: This was a retrospective analysis of prospectively collected clinical data conducted at an academic medical center in Omaha, Nebraska. We abstracted data from 2

Effects of comprehensive educational reforms on academic success in a diverse student body.

CONTEXT: Calls for medical curriculum reform and increased student diversity in the USA have seen mixed success: performance outcomes following curriculum revisions have been inconsistent and national matriculation of under-represented minority (URM) students has not met aspirations. Published innovations in curricula, academic support and pipeline programmes usually describe isolated interventions that fail to affect curriculum-level outcomes. METHODS: United States Medical Licensing Examination (USMLE) Step 1 performance and graduation rates were analysed for three classes of medical studen

A design-thinking approach to therapeutic translation: tympanic regeneration.

PURPOSE OF REVIEW: Clinician researchers face the pressures of meeting academic benchmarks combined with advancing new therapies to patients. The vast majority of drug discoveries fail in translation. A new method of meeting the challenges of preclinical therapeutic translation is presented using the example of tympanic regeneration. RECENT FINDINGS: The key to a design-thinking approach to therapeutic translation is to 'begin with the end in mind' by widening the scope of the problem, with multiple points of view, to not only understand the disease but the context for the patient and the hea

Bipolar disorder in school-age children.

This article examines the individual components of bipolar disorder in children and the behaviors that can escalate as a result of misdiagnosis and treatment. The brain/behavior relationship in bipolar disorders can be affected by genetics, developmental failure, or environmental influences, which can cause an onset of dramatic mood swings and dysfunctional behavior. School is often the site where mental health disorders are observed when comparing behaviors with other children. Assessing the emotional, academic, and health needs of a student with a bipolar disorder is a critical step in desig

Autistic Traits and College Adjustment.

This study evaluated the association of autistic traits (RAADS-14) with academic and social outcomes among college students using data from an online survey (N = 2,736). In the academic domain, the total trait score and all subscale scores (mentalizing deficits, social anxiety, sensory reactivity) were associated with course failure and academic difficulties independent of an autism diagnosis; the total score and mentalizing deficits also predicted lower grade point average (GPA). In the social domain, the total trait score and subscale scores were associated with lower odds of having a confid

The "immigrant paradox" phenomenon: assessing problem behaviors and risk factors among immigrant and native adolescents.

We investigated the "immigrant paradox" phenomenon by examining differences in problem behavior engagement and exposure to risk factors across four adolescent groups: 1,157 first-generation, 1,498 second-generation, and 3,316 White and minority third or higher generations. Latent mean differences in problem behavior engagement (i.e., academic failure, aggression, and substance use) and risk factors (i.e., low socioeconomic status, poor family relationship, and low sense of school belonging) were associated with significant differences across adolescent groups. Results supported the generationa

Inattention, hyperactivity, oppositional-defiant symptoms and school failure.

BACKGROUND: Attention-deficit hyperactivity disorder (ADHD) is associated with school failure. Inattention has been mainly implicated for this association. Oppositional-defiant disorder's (ODD) impact on academic performance remains controversial, because of the high comorbidity between ODD and ADHD. OBJECTIVE: To understand the role of inattention (IN), hyperactivity (H/I) and ODD in school failure. METHOD: Parents and teachers filled out SNAP-IV questionnaires for 241 / 6th grade students. The associations of the scores of oppositional-defiant (OP), H/I and IN symptoms with school year fai

[Advances in clinical features, diagnosis, and treatment of pre-acute-on-chronic liver failure].

Acute-on-chronic liver failure is a severe syndrome characterized by a rapid decline in liver function and multiple organ failure, with rapid progression often already in the mid-to-late stage by the time it is clinically diagnosed in patients with chronic liver disease. Currently, there are no therapeutic drugs that are efficacious, and the short-to medium-term mortality rate is high. Some academics have proposed establishing a term, "pre-acute-on-chronic liver failure," to denote a transitional stage for patients prior to their progress to acute-on-chronic liver failure. Early-stage interven

Concurrent manifestation of clinical hypodontia and blindness: a case report.

A case is reported of a 26-year-old blind man with hypodontia and multiple apparently underdeveloped impacted teeth. The patient reported that he had progressively developed visual impairment at the age of 11 years whence he became totally blind when he turned 12 years. The aim of this report is to open an academic and professional debate on the challenges of its definitive diagnosis and appropriate intervention.Blindness is not reported in any of the previously described syndromes; therefore, concurrent manifestation of "hypodontia, blindness, failure of eruption and digital lesions" can be p
