Sepsis and osteomyelitis about the ankle joint.

Sepsis and osteomyelitis about the ankle joint present a challenging clinical problem. Osteomyelitis usually follows open fracture of the distal tibia, often with a pilon fracture component. This article outlines the prevention of osteomyelitis in these difficult fractures. Treatment of subsequent osteomyelitis and sepsis, including the authors' experiences, is discussed. Septic ankle arthritis can occur hematogenously. In some patients, the optimal treatment for concomitant osteomyelitis and sepsis is a below knee amputation.

[Biomechanics of the ankle joint--injury mechanisms].

The complex mechanics of the ankle joint complex depends highly on the integrity of the ankle ligaments. An incomplete restoration of the antero-fibular ligament after trauma may result in a mechanical dissociation of the talus from the ankle mortise. The force that is needed to induce a non-physiological movement of the foot is determined by the individual configuration of the bony and articular structures of the hindfoot. A pes cavus is less protected to suffer of an ankle sprain than a flatfoot. As the leverarm of the peroneal muscles decreases with increasing plantarflexion of the foot, th

The ankle joint: the evaluation and treatment of sprains.

Nurse practitioners in a variety of settings may frequently be called upon to examine a patient with an ankle sprain. These sprains are extremely common and are easily and successfully treated conservatively. However, a chronically unstable ankle may result if certain treatments are not instituted almost immediately after the injury. For the purpose of this article, the anatomy of the ankle and its examination are reviewed, including the ligaments injured with inversion, eversion, dorsiflexion, and plantar flexion injuries. Key studies and laxity tests are described. Principles of treatment ar

The influence of orthoses on the proprioception of the ankle joint.

The ankle joints of 14 healthy volunteers and 16 patients with unstable ankle joints were tested regarding their functional and proprioceptive capabilities. All of them were active athletes. Three tests were used of the study: single-leg stance test, single-leg jumping course test, angle-reproduction test. The influence of three stabilization devices (lace-on-brace/"Mikros", stirrup-brace/"Aircast", taping) on the proprioceptivity of stable and unstable ankle joints was evaluated. The scores of the single-leg jumping course without any stabilizing device (category "standard") ranged between 8.

[Magnetic resonance tomography of the ankle joint].

To define the clinical role of MRI of the ankle joint, a total of 88 patients was investigated. In the evaluation of ligamentous injury, MRI was inferior to established imaging methods. By contrast, it provided additional therapy-relevant information in the assessment of hemophilic arthropathy, osteochondritis dissecans, and inflammatory and neoplastic diseases of the ankle joint. In the latter conditions, MRI may make other more conventional methods of examining the ankle joint unnecessary.

Arthrodesis of the diabetic neuropathic ankle joint.

At the authors' institution from 1964 to 1984, ankle arthrodesis was performed in 13 patients with insulin-dependent diabetes mellitus who had a history of ankle sprain or fracture. Nine patients were diagnosed by clinical exam as having a peripheral neuropathy; nine patients had roentgenographic evidence of neuropathic arthropathy prior to surgery. Follow-up study with examination and roentgenograms averaged 42 months. Clinical and roentgenographic union was achieved in seven ankles at an average of 16 weeks. Two patients developed a nonunion, three had an amputation, and one died at two mont

Posterior block of the ankle joint in dancers.

The presence of a bony block at the back of the ankle should be recognized as occurring in patterns of symptoms which are not themselves directly related to the ankle joint. When careful examination and observation of the patient indicates that the source of the trouble is in fact at the back of the ankle joint and it has been determined that there is some extra bone at this site, then surgical treatment will give excellent results. It must be emphasized that the presence of an os trigonum or other bony prominence does not mean that it is the source of recurrent symptoms. Surgery should certai

Chronic Ankle Instability-Related Outcomes Associate With Ankle-Joint Loading During Walking.

CONTEXT: Recurrent trauma and altered biomechanics in those with chronic ankle instability (CAI) have been linked to altered joint loading. Previous studies revealed that patients with CAI exhibit altered joint contact force (JCF) profiles relative to uninjured individuals during walking and landing. Identifying more easily obtainable outcomes that are associated with ankle JCF in those with CAI would reduce the knowledge gap between loading profiles at the ankle joint and outcomes related to CAI. OBJECTIVE: To quantify how ankle JCF, structural measures, postural control, and walking biomech

Chronic Ankle Joint Instability Induces Ankle Sensorimotor Dysfunction: A Controlled Laboratory Study.

BACKGROUND: Chronic ankle instability (CAI) is a clinical sequela that causes the recurrence of ankle sprain by inducing ankle sensorimotor dysfunction. Animal models of CAI have recently shown that ankle ligament injuries mimicking an ankle sprain result in chronic loss of ankle sensorimotor function. However, the underlying mechanisms determining the pathogenesis of CAI remain unclear. HYPOTHESIS: Ankle instability after an ankle sprain leads to the degeneration of the mechanoreceptors, resulting in ankle sensorimotor dysfunction and the development of CAI. STUDY DESIGN: Controlled laborat

Radiotherapy for osteoarthritis of the ankle and tarsal joints-analysis of 66 joints.

PURPOSE: Osteoarthritis of the ankle and tarsal joints is less common than osteoarthritis of the knee or hip, but the associated disability is at least as severe as that of the other major joints of the lower limb. The results for total arthroplasty are still not satisfactory. For this reason, arthrodesis is still the gold standard of non-joint-conserving surgery. For the reason of functionality, joint-conserving therapies play a major role in treatment of ankle and tarsal osteoarthritis. Low-dose radiotherapy has a long history of treatment of osteoarthritis. The aim of this survey was to exa

Joint Preserving Surgery for Valgus Ankle Osteoarthritis.

Valgus ankle OA is a complex problem with multiple etiologies that can either be isolated or superimposed on top of other medical or musculoskeletal disorders. Proper medical history, physical, and preoperative radiological examinations are crucial in deciding on surgery and planning the surgical approach. JPS, especially the varisating medial closing-wedge SMOT with solid plate fixation, has been consistently associated with good outcomes for patients with valgus ankle OA. To further improve JPS for valgus ankle OA, further clinical and biomechanical studies are required to address the long-t

Joint-Preserving Surgery in Varus Ankle Osteoarthritis.

Ankle deformity is a disabling condition especially if concomitant with osteoarthritis (OA). Varus ankle OA is one of the most common ankle OA deformities. This deformity usually leads to unequal load distribution in the ankle joint and decreases joint contact surface area, leading to a progressive degenerative arthritic situation. Varus ankle OA might have multiple causative factors, which might present as a single isolated factor or encompassed together in a single patient. The etiologies can be classified as post-traumatic (e.g., after fractures and lateral ligament instability), degenerati

Joint-preserving surgery of valgus ankle osteoarthritis.

Patients with posttraumatic ankle osteoarthritis (OA) typically present with asymmetric involvement of the tibiotalar joint, resulting in valgus or varus deformity of the ankle and hindfoot. Without appropriate treatment, patients with asymmetric ankle OA typically develop full end-stage ankle OA. Ankles with valgus deformities suffer from a lateral joint overload with subsequent lateral tibiotalar joint degeneration, which causes further lateral load shift. In these cases patients may benefit from joint-preserving realignment surgery to unload the degenerated lateral area and normalize joint

[Surgical indication of ankle joint supination trauma?].

Even for severe ankle sprains the non-operative, functional treatment is the therapy of choice. The prognosis is good: persisting symptoms and instability are rare and the outcome seems to be independent of initial therapy. An indication for primary operation is an exception. In case of remaining symptoms the patient should be referred to an orthopedic surgeon for further diagnostic studies (MRI) and possibly secondary operative stabilization.
