Atrial Cardiomyopathy with Impaired Functional Reserve in Patients with Paroxysmal Atrial Fibrillation.

BACKGROUND: Impaired atrial functional reserve during exercise may represent an early stage of atrial cardiomyopathy. To test this hypothesis, the authors evaluated left atrial (LA) and left ventricular (LV) function reserve during exercise in patients with paroxysmal atrial fibrillation (PAF). METHODS: Sixty-one patients with PAF undergoing radiofrequency catheter ablation and 38 healthy control subjects were prospectively enrolled. LV global longitudinal strain and LA reservoir strain (RS) were measured at rest and during supine bicycle exercise. To identify the early stage of atrial cardio

Efficacy of pulmonary vein isolation on left atrial function in paroxysmal and persistent atrial fibrillation and the dependency on its baseline function.

BACKGROUND: The effects of pulmonary vein (PV) isolation in atrial fibrillation (AF) on left atrial (LA) function or PV flow have not been well documented. METHODS: We examined the LA function and PV flow before and 3-6 months after PV isolation in 67 AF patients (34 paroxysmal [PAF] and 33 persistent [CAF]) using transesophageal echocardiography. RESULTS: AF recurred in 6/34 patients with PAF and in 6/33 patients with CAF 6 months after PV isolation. A larger LA dimension, a lower systolic PV flow velocity, and a lower ratio of systolic to diastolic PV flow velocity were related to a higher

[Atrial natriuretic hormone and endocrine functions].

The atrial natriuretic hormone (ANP) is a cardiac hormone which gene and receptors are widely present in the body. Its main function is to lower blood pressure and to control electrolyte homeostasis. Its main targets are the kidney and the cardiovascular system but ANP interacts with many other hormones in order to regulate their secretion. The adrenal glands are the first endocrine target. Steroidogenesis, especially mineralocorticoid synthesis, is inhibited by ANP, but glucocorticoid production seems to be depressed too. As ANP synthesis is enhanced by the latter, it suggests a regulatory lo

Left atrial function during exercise stress echocardiography as a sign of paroxysmal/persistent atrial fibrillation.

OBJECTIVE: Atrial cardiomyopathy is closely associated with atrial fibrillation (AF), and some patients exhibit no dysfunction at rest but demonstrate evident changes in left atrial (LA) function and LA volume during exercise. This study aimed to identify distinguishing signs during exercise stress echocardiography (ESE) among patients in sinus rhythm (SR), with and without history of paroxysmal/persistent AF (PAF). METHODS: A prospective cohort of 1055 patients in SR was enrolled across 12 centers. The main study cohort was divided into two groups: the modeling group (n = 513) and the verifi

Effects of Left Atrial Appendage Closure on Neuroendocrine Function in Patients with Nonvalvular Atrial Fibrillation.

BACKGROUND The left atrial appendage (LAA) is an organ with neuroendocrine function. It remains unclear whether left atrial appendage closure (LAAC) has physiological effects on neuroendocrine function in patients with nonvalvular atrial fibrillation (NVAF). In the present study, we aimed to investigate the effects of LAAC on neuroendocrine function in patients with NVAF. MATERIAL AND METHODS We enrolled 20 patients with NVAF treated by LAAC in Jiangsu Taizhou People's Hospital from October 2019 to October 2020. Blood samples were collected 1 day before LAAC and 12 months after LAAC. Plasma co

Impact of atrial fibrillation ablation on biatrial strain: Differences between left and right atrial function.

BACKGROUND: Catheter ablation for atrial fibrillation (AF) can improve atrial function; however, little is known about chamber-specific remodeling between the left (LA) and right atria (RA). OBJECTIVE: We investigated the differences in biatrial strain and its predictive value for atrial arrhythmia (AF/atrial tachycardia [AT]) recurrence. METHODS: We retrospectively analyzed patients undergoing initial AF ablation who underwent pre- and postprocedural echocardiography. Atrial strain was quantified by 2-dimensional speckle-tracking echocardiography. RESULTS: 81 patients were analyzed. Both t

Comparison of atrial arrhythmias and autonomic function parameters according to atrial septal defect closure techniques.

BACKGROUND: Atrial septal defects (ASDs) are associated with an increased risk of atrial arrhythmias due to right atrial dilation, electrical remodeling, and conduction abnormalities. In addition to arrhythmias, autonomic dysfunction may also occur. Although several studies have investigated the impact of transcatheter ASD closure on arrhythmia risk in pediatric patients, direct comparative analyses between surgical and transcatheter closure techniques remain limited in the current literature. METHODS: This study included patients who underwent ASD closure via surgical or transcatheter method

Predicting Atrial Fibrillation Risk in Hypertrophic Cardiomyopathy Patients: Left Atrial Phasic Function and Strain Mechanics.

BACKGROUND: To evaluate whether the left atrium (LA) phasic function and myocardial deformation by cardiovascular magnetic resonance (CMR) in hypertrophic cardiomyopathy (HCM) patients is a predictor of atrial fibrillation (AF) development. METHODS: The baseline CMRs of HCM patients who later developed AF (n = 140; mean age = 61 ± 12 years; male = 86) were compared with those of HCM patients who did not develop AF (n = 70; mean ag e= 58 ± 11 years; male patients = 48). This case-control study was conducted at our quaternary referral centre between January 2001 and June 2021. The univariate an

Association Between Renal Function and Left Atrial Low-Voltage Area Burden in Paroxysmal Atrial Fibrillation.

BACKGROUND: Circumferential pulmonary vein isolation (CPVI) plus left atrial low-voltage areas (LVA) modification is a useful method to treat paroxysmal atrial fibrillation (PAF); however, it is hard to predict LVA burden before the procedure. The existence of chronic kidney disease (CKD) increases the prevalence of left atrial LVA. In this study, we explored the relationship between renal function and the LVA burden in elderly patients with PAF. METHODS: We retrospectively analyzed the 435 elderly patients with PAF who underwent ablation. Based on the LVA burden measured during the procedure

Left atrial appendage function in prediction of paroxysmal atrial fibrilation in patients with untreated hypertension.

Abstract The onset of AF results in a significant increase in mortality rates and morbidity in hypertensive patients and this rhythm disorder exposes patients to a significantly increased risk of cerebral or peripheral embolisms. Tissue Doppler imaging was found to be useful in early detection of myocardial dysfunction in several diseases. It was shown that tissue Doppler analysis of the walls of the left atrial appendage (LAA) can give accurate information about the function of the LAA in hypertensive patients. In this study, we aimed to investigate and identify the specific predictive parame

PRRX1 Loss-of-Function Mutations Underlying Familial Atrial Fibrillation.

Background Atrial fibrillation (AF) is the most common form of clinical cardiac dysrhythmia responsible for thromboembolic cerebral stroke, congestive heart failure, and death. Aggregating evidence highlights the strong genetic basis of AF. Nevertheless, AF is of pronounced genetic heterogeneity, and in an overwhelming majority of patients, the genetic determinants underpinning AF remain elusive. Methods and Results By genome-wide screening with polymorphic microsatellite markers and linkage analysis in a 4-generation Chinese family affected with autosomal-dominant AF, a novel locus for AF was

Left atrial function in patients with mitral valve regurgitation.

The purpose of this study was to assess left atrial (LA) function and myocardial mechanics in patients with degenerative mitral regurgitation (MR). Eighty patients with degenerative MR and twenty control subjects were included prospectively. LA volume (LAV) and right atrial (RA) volume (RAV) were measured with three-dimensional transthoracic echocardiography at three phases of the cardiac cycle as maximal volume (LAVmax and RAVmax), minimal volume (LAVmin and RAVmin), and volume before atrial contraction (LAVpre-A and RAVpre-A). From these volumes, active stroke volume (SV), distensibility, an

The left atrial appendage: anatomy, function, and noninvasive evaluation.

The left atrial appendage (LAA) is a finger-like extension originating from the main body of the left atrium. Atrial fibrillation (AF) is the most common clinically important cardiac arrhythmia, occurring in approximately 0.4% to 1% of the general population and increasing with age to >8% in those >80 years of age. In the presence of AF thrombus, formation often occurs within the LAA because of reduced contractility and stasis; thus, attention should be given to the LAA when evaluating and assessing patients with AF to determine the risk for cardioembolic complications. It is clinically import

Atrial natriuretic peptide preserves endothelial function during intimal hyperplasia.

BACKGROUND: Atrial and C-type natriuretic peptides (ANP and CNP), acting through different receptors, have antiproliferative effects in vitro. Beneficial effects of CNP in vivo on early atherosclerosis have been described, but it is not known if ANP is antiproliferative in vivo. In the present study, the effects of chronic in vivo ANP were tested and compared with CNP on endothelial dysfunction and intimal thickening caused by peri-arterial collars. METHODS: Non-occlusive collars were placed bilaterally around the common carotid arteries of rabbits. One collar was filled with saline vehicle.
