Cochlear initiation sites of the frequency following potential.

This study used a series of non-simultaneous masking stimuli set at subjects' pulsation threshold (PT) level to deduce frequency following potential (FFP) tuning curves for 500-Hz tone bursts at 80 and 100 dB SPL. PT and FFP tuning curves were determined in the presence and absence of continuous high-pass masking noise. Results from study demonstrated differences in FFP tuning curves, depending on the high-pass masking condition. High-frequency end of FFP tuning curve was diminished in the presence of high-pass masking noise. These findings suggested that higher frequency tone bursts were less

Hearing loss in multiple sclerosis: localization of the auditory pathway lesion according to electrocochleographic findings.

Multiple sclerosis is known to affect the myelin of the auditory pathway resulting in acute hearing loss. Two cases of sudden deafness due to multiple sclerosis have been evaluated by conventional audiometry, brainstem auditory evoked response audiometry and transtympanic electrocochleography. The abnormalities of the compound action potential in both patients (enhanced latency, abnormal adaptation using fast stimulus rate) and the normal receptor potentials (cochlear microphonic, summating potential), as well as the absence of brainstem responses suggest a disturbance of synchronization at th

[Evoked potentials. Indications and diagnostic significance].

Visual, auditory and somatosensory evoked potentials are useful in the detection and localization of demyelinating lesions, which mainly cause a slowing of conduction and therefore a latency increase in the evoked potential. Primarily amplitude reductions or loss of the response are seen in partial or complete impulse conduction blocks due to a tumor e.g. Acoustic nerve tumors can be detected by AEP, orbit tumors or processes of the chiasm by VEP and spinal cord tumors by SEP. The use of evoked potentials for the evaluation of ophthalmologic disorders, for audiometry in children and for monito

A comparison of two methods of using transtympanic electrocochleography for the diagnosis of Meniere's disease: click summating potential/action potential ratio measurements and tone burst summating potential measurements.

CONCLUSION: Transtympanic electrocochleography (TT ECochG) is helpful for the confirmation of Meniere's disease using tone burst stimuli to measure the amplitude of the frequency specific summating potentials (SP) but not when using the click evoked summating potential versus action potential ratio (SP/AP). OBJECTIVES: To evaluate the effectiveness of TT ECochG as a means of confirming the clinical diagnosis of Meniere's disease. PATIENTS AND METHODS: A total of 2,717 patients were referred for TT ECochG between August 1998 and September 2008. Each patient was assessed prospectively on clini

Effects of glycerol on cochlear frequency selectivity in patients with Menière's disease.

During transtympanic electrocochleography, action potential (AP) tuning curves were derived as a measure of cochlear frequency selectivity. In 18 out of 21 patients with symptoms of Menière's disease, a poor frequency selectivity was observed. In 12 subjects the effects on frequency selectivity of oral glycerol were measured, and in 5 patients AP tuning curves indicated an improvement in sharpness of tuning by a factor of 1.5-2.5. These changes started approximately 30 min after oral glycerol administration. In 2 subjects there was a deterioration in AP tuning. In 5 patients no significant cha

[Use of a body acoustic probe and the conduction of brain stem potentials for the determination of sound conduction during tympanoplasty].

A new procedure of evoking brain stem potentials is introduced. Contrary to evoking brain stem potentials by acoustic stimulation via earphone or loudspeaker, we touch the middle ear apparatus at various locations using a piezoelectric transducer with a screwed-on probe, thus causing physiological movements within the sound conducting structures (Fig. 3). As a reaction to this kind of stimulation--1000 clicks with a duration of 0.1 ms were given--far field brain stem potentials can be recorded. The calibration of this special sound probe--not bigger than a pencil (Fig. 1)--was made by equalizi

[On possible causes of discrepancy between the results of auditory brainstem responses (ABR) and auditory steady-state responses (ASSR) recordings].

UNLABELLED: Auditory brainstem responses (ABR) and auditory steady-state responses (ASSR) are the most widely used tests for objective hearing assessment. These tests are included in clinical guidelines for sensorineural hearing loss and other protocols. The literature sources describe examples of inconsistency between the results of these two tests, which are suggested to be interpreted in favor of ABR as the test with the highest values of sensitivity and specificity. Also, in a number of cases, discrepancy between the results of ABR and ASSR recordings in patients with neurological patholog

A Rare Case of Hearing Impairment due to Cerebello-Pontine Angle Lesion: Trigeminal Schwannoma.

Schwannoma of the trigeminal nerve is a rare condition. Even rarer is hearing loss occurring as a result of this lesion. The aim of this study is to highlight this rare cause of hearing impairment. Here we report the clinical features and findings of the imaging and audiological investigations of a case of trigeminal schwannoma diagnosed at our institution. Our patient presented with headache, giddiness, tinnitus, left-sided facial weakness, left-sided hearing loss, right-sided hemiplegia, and unintelligible speech. Radiological studies revealed a large well-defined mass lesion in the left cer

Evidence for hair cell degeneration as the primary lesion in hearing loss of the elderly.

All patients in a geriatric unit showed a hearing loss with a mean value of approximately 60 dB HL. Auditory brain stem evoked responses had normal amplitudes but, in most cases, waves I and II were absent. Extratympanic electrocochleograms produced well-marked action potentials with evidence of a type of recruitment. These results suggest a primary disturbance of cochlear function peripheral to the cochlear nerve. Examination of stained surface preparations of the organ of Corti showed complete atrophy of all hair cells at the end of the basal coil and severe outer hair cell loss in all three

Clinical neurophysiological examination of deafness associated with juvenile motor neurone disease.

Three patients with the syndrome of progressive severe deafness, associated with juvenile onset of motor neurone disease, were examined using conventional audiometry and brainstem auditory evoked response (BAER) recordings, and electrocochleographic recordings were made in two of them. Findings indicate that the deafness had resulted from loss of acoustic nerve fibers and/or sensory cells of the spiral ganglion.

Auditory-steady-state response reliability in the audiological diagnosis after neonatal hearing screening.

INTRODUCTION AND OBJECTIVES: Conventional audiometry is the gold standard for quantifying and describing hearing loss. Alternative methods become necessary to assess subjects who are too young to respond reliably. Auditory evoked potentials constitute the most widely used method for determining hearing thresholds objectively; however, this stimulus is not frequency specific. The advent of the auditory steady-state response (ASSR) leads to more specific threshold determination. The current study describes and compares ASSR, auditory brainstem response (ABR) and conventional behavioural tone aud

Automatic audiometry using auditory steady-state response and sequential test strategy applied to volunteers with normal hearing.

PURPOSE: In the present study, a new procedure to perform automatic audiometry using multifrequency Auditory Steady-State Response (ASSR) is proposed. METHODS: The automatic audiometry procedure consists of detecting the presence of multifrequency ASSR in real-time using the sequential test strategy and by adjusting the stimulus intensity independently. The ASSR audiometric thresholds of 18 adult volunteers with normal hearing were determined by automatically (four simultaneous frequencies per ear) at modulation frequencies in the 80 Hz range. The exam time and the difference between ASSR thr
