Intraoperative fat embolism during core decompression and bone grafting for osteonecrosis of the hip: report of 3 cases and literature review.

Osteonecrosis (ON) of the femoral head, without timely intervention, often progresses to debilitating hip arthritis. Core decompression (CD) with bone grafting was used to treat patients with early-stage ON. In 3 cases, intraoperative oxygen saturation, end-tidal carbon dioxide fluctuations, and/or vital sign fluctuations were observed during insertion of the graft, a mixture of bone marrow and demineralized bone matrix. In 1 case, continued postoperative pulmonary symptoms required admission to intensive care. In this article, we describe these cases and provide supporting evidence that they

Chapter 3: Impact of primary hyperparathyroidism.

At present, primary hyperparathyroidism is most often discovered in an asymptomatic patient, but can sometimes be revealed by a renal or bone complications. In all cases, a full work-up is recommended, with assessment of renal function (glomerular filtration rate), 24-hour calciuria, screening for risk factors for lithiasis, and renal and urinary tract imaging (ultrasound or CT scan) to look for stones or nephrocalcinosis. Bone densitometry, with measurements of the spine, femur and radius, is the recommended reference test for demineralization. Standard X-rays of the spine or other imaging te

Clinical evaluation of a composite bone graft with a calcium sulfate barrier.

BACKGROUND: The goal of this study was to evaluate the effectiveness of a composite bone graft (demineralized freeze-dried bone allograft, calcium sulfate, tetracycline, and porous hydroxyapatite) and calcium sulfate barrier to treat non-furcation osseous defects. METHODS: One hundred patients were treated with the technique. The preoperative measurements were compared to the postoperative measurements (mean 5.2 months) to determine whether the technique had a statistically significant effect. Various factors were evaluated to determine whether they could be related to improved or diminished

Ridge preservation utilizing an acellular dermal allograft and demineralized freeze-dried bone allograft: Part I. A report of 2 cases.

Alveolar ridge deformities are usually the result of trauma, periodontal disease, surgical insult, or developmental defects. Preventing ridge collapse with the extraction of maxillary anterior teeth is vital to an esthetic restorative result. Several techniques are available to prevent ridge collapse. In these case presentations, ridge preservation was achieved utilizing an acellular dermal matrix as a barrier membrane with a demineralized freeze-dried bone allograft. This report demonstrated an acceptable esthetic result with no loss of ridge height or width. Soft tissue dimensions were also

Immunostaining of Skeletal Tissues.

Immunohistochemistry (IHC) is a routinely used technique in clinical diagnosis of pathological conditions and in basic and translational research. It combines anatomical, immunological, and biochemical methods and relies on the specific binding of an antibody to an antigen. Using the technique with mineralized tissues is more challenging than with soft tissues. Demineralizing the samples allows for embedding in paraffin wax, and also facilitates cryosectioning. This chapter describes methods for IHC on formaldehyde-fixed, demineralized, paraffin-embedded, or frozen sections to detect antigens

Surgical problems in space: an overview.

Terrestrial experience indicates that surgical emergencies may cause significant morbidity in space, and may present unique problems. Microgravity physiologic changes that may increase susceptibility to, and complications from, injury include fluid redistribution and diuresis, bone demineralization, and immune system alterations. Conventional diagnostic tests may be unavailable or unreliable--e.g. the radiologic finding of "air under the diaphragm", which conventionally indicates perforation of an air-containing hollow viscus, such as the stomach, with a gravity-dependent rising of gas in the

New Classification of Autologous Tooth-Derived Grafting Materials: Fundamental Concepts.

The use of dentin grafts is relatively recent, and their efficacy remains a topic of debate. Various techniques and devices are available for dentin grafting; however, their application has been inconsistent, as each method yields a distinct product with unique biological properties and potential uses. One of the challenges that arises with the introduction of a new biomaterial is the potential confusion between different preparations. Specifically, generalization may significantly impact the understanding of unique qualities and/or potential limitations. The various materials that make up the

Digital Evaluation of Vertical Ridge Augmentation with the Modified Shell Technique Using a Xenogeneic Bone Lamina: A Case Series.

Vertical ridge augmentation is a demanding and technique-sensitive surgical procedure. In the present case series, cone beam CT (CBCT) scans from the clinical routine of patients treated using a novel approach for vertical bone augmentation were assessed. All patients showed a single-tooth class 5 defect and were treated using a modification of the original shell technique. Cortical bone plates were replaced with a lamina composed of a partially demineralized porcine xenograft. CBCT scans of six consecutive patients were treated with the lamina and particulate bone from the mandibular ramus pr

Secondary bone grafting for alveolar clefts: surgical timing, graft materials, and evaluation methods.

Alveolar cleft belongs to the spectrum of cleft lip and/or palate, affecting 75% of cleft lip/palate patients. The goals of alveolar cleft treatment are stabilizing the maxillary arch, separating the nasal and oral cavities, and providing bony support for both erupting teeth and the nasal base via the piriform aperture. Secondary alveolar bone grafting is a well-established treatment option for alveolar cleft. Secondary alveolar bone grafting is performed during the period of mixed dentition using autologous bone from various donor sites. There are several issues relevant to maximizing the suc

Periodontal and peri-implant bone regeneration: clinical and histologic observations.

The principle of guided tissue regeneration by barrier membranes to restore lost periodontal tissue around natural teeth has also been used around osseointegrated implants in an attempt to restore alveolar ridge defects. While most periodontal procedures in the literature describe root coverage by mucogingival surgery, which achieves healing through soft tissue attachment, regeneration of denuded root surfaces is performed by guided tissue regeneration using expanded polytetrafluoroethylene barrier membranes and demineralized freeze-dried bone allografts as inductive/conductive materials. Case reports: management of large segmental tibial defects using a cylindrical mesh cage.

We report a case series of three patients who sustained open Gustilo-Anderson Type IIIB tibia fractures associated with extensive segmental bone and soft tissue loss. The patients initially were treated with serial wound irrigations, débridements, and external fixation. After the soft tissue envelope was reconstructed successfully, each large segmental bone defect was reconstructed with a cylindrical titanium mesh cage packed with a composite of cancellous allograft and demineralized bone matrix putty and stabilized with a statically locked intramedullary nail. The mean segmental bone loss was

Efficacy of different sterilization protocols and its impact on osteogenic potential of autogenous tooth bone graft: A comparative in vitro study.

BACKGROUND: The efficacy of autogenous tooth bone graft (auto-TBG) is affected by many determinants, such as sterilization protocols and the expression of bone-inducing proteins such as bone morphogenetic protein (BMP) present in the graft. AIM: This in vitro study was conducted to compare the efficacy of different sterilization protocols and their impact on the osteogenic potential of auto-TBG. MATERIALS AND METHODS: Ninety-eight auto-TBG samples were obtained from 10 extracted teeth that were prepared, ground, and sorted with the help of a dentin grinder. Seventy samples were partially dem

Bone graft substitutes and allografts for reconstruction of the foot and ankle.

In reconstructive foot and ankle surgery, the use of bone graft is common. Whether for trauma, acquired or congenital deformities, arthrodeses, joint replacement, bone loss from infection, or bone tumor resection, the foot and ankle surgeon must be knowledgeable about current bone grafting options to make informed decisions. Innovation and technologic advances have produced an impressive and exciting array of options, advancing us closer to mimicking the gold standard: autograft. However, the sheer volume of available products makes it challenging for the foot and ankle surgeon to stay abreast

Ligand-modified aminobisphosphonate for linking proteins to hydroxyapatite and bone surface.

An increase in bone resorption is one of the main symptoms of osteoporosis, a disease that affects more and more individuals every day. Bisphosphonates are known to inhibit bone resorption and thus are being used as a treatment for osteoporosis. Aminobisphosphonates present a functionality that can be easily used for conjugation to other molecules, such as peptides, proteins, and ligands for protein recognition. In this study, an aminobisphosphonate conjugated with biotin was used as a model linker for protein attachment to bone. With this system, the interaction of biotinylated aminobisphosph

Improved osteoinduction of cortical bone allografts: a study of the effects of laser perforation and partial demineralization.

Massive cortical bone allografts have been found to incorporate slowly into host bone and thus are subject to complications such as nonunion, fatigue fracture, and infection. To better understand and improve the process of osteoinduction in these types of bone grafts, a new experimental model was developed with use of diaphyseal cortical bone grafts from rat tibiae that were prepared by partial demineralization and drilling of 0.33 mm diameter holes with a pulsed, 2.94 microns wavelength, erbium:yttrium-aluminum-garnet laser. Six types of grafts were analyzed: untreated (Type I), demineralized

Prefabricated microvascular autograft in tracheal reconstruction.

Tracheal reconstruction continues to be a challenge in head and neck surgery. Numerous techniques, including the use of alloplasts, composite grafts, and staged laryngotracheal troughs, have met with limited success because of implant exposure, infection, persistent granulation tissue, and eventual restenosis. With recently introduced techniques for soft-tissue molding, bone induction with bone morphogenetic protein, and microvascular free tissue transfer, a rodent model was developed to create a well-vascularized tracheal autograft. In this model, a rigid tube having the same dimensions and f
