Finite element models of human crystalline capsular bag and zonular fibers are used to estimate the mechanical response of the capsule to the presence of a C-loop Intra-Ocular Lens (IOL) after cataract surgery, to assess the influence of capsulorhexis size, shape and location on IOL decentration and tilt. The model includes, in the anterior capsule, a hole with variable size, shape and position, which represents the rhexis obtained in a manual or laser-assisted manner. The IOL is not explicitly modelled, but its action is reproduced by means of a set of elastic ties, connecting the opposite si

PURPOSE: Capsulorhexis is a critical and technically demanding step in cataract surgery where slight errors can cause serious complications, making repeated simulation-based training essential. Limited access to hands-on surgical training has increased reliance on simulators, but existing models are often costly and limited. To address this, the Farra Eye Model was developed as a low-cost, reusable 3D-printed simulator offering realistic eye simulation. This study aims to assess its construct validity in distinguishing between different levels of surgical expertise.
METHODS: A cross-sectional

PURPOSE: The intraocular lens blocking technique described for the removal of intraocular foreign bodies emerged as a result of an effort to prevent the foreign body from being retracted into the posterior segment because of the resistance encountered while removing it from the corneal incision. However, in the technique described, in addition to the difficulty of the surgical procedure, the new concern is to create a posterior capsulorhexis according to the size of the foreign body and to pass the foreign body through the capsulorhexis.
METHODS: Here, the authors describe a new approach to t

PURPOSE: Posterior capsular opacification is the most common surgically related cause of reduced vision after cataract surgery. We studied the effect of capsulorhexis size on the pattern and severity of posterior capsular opacification.
METHODS: In this prospective study 75 patients underwent standardized phacoemulsification with capsulorhexis and in-the-bag placement of a 5.5-mm polymethylmethacrylate intraocular lens implant. The patients were randomly assigned to receive either a small capsulorhexis of 4.5 to 5 mm to lie completely on the intraocular lens optic or a large capsulorhexis of

PURPOSE: To observe the posterior continuous curvilinear capsulorhexis (PCCC) after cataract surgery in control eyes and eyes with an increased risk for postoperative inflammation.
SETTING: Department of Ophthalmology, University Hospital Antwerp, Belgium.
METHODS: After phacoemulsification, a PCCC was performed before intraocular lens (IOL) implantation in 20 eyes of 18 patients with ocular or systemic conditions that predisposed them for increased postoperative inflammation; e.g., diabetes, uveitis, retinitis pigmentosa (inflammation group). These eyes were compared with 20 eyes of 16 pati

IMPORTANCE: Appropriately sized and well-centered capsulorhexis is key for high-quality phacoemulsification cataract surgery and can be achieved with digital-guidance devices. However, such devices are not readily available, warranting the need for a device-free manual auxiliary capsulorhexis technique.
OBJECTIVES: To determine if manual marking-assisted capsulorhexis (MMAC) is noninferior to a digital guidance-assisted capsulorhexis (DGAC) in capsulorhexis accuracy following phacoemulsification for age-related cataract.
DESIGN, SETTING, AND PARTICIPANTS: This noninferiority randomized clini

BACKGROUND: The study focuses on developing a cataract capsulorhexis training system incorporating 3D printing and machine vision technologies. This system aims to improve surgical skills and proficiency in cataract surgery among medical students and interns. The initiative utilizes an eye model that closely simulates the capsulorhexis phase of real cataract surgeries, thereby providing a realistic training environment.
METHOD: A comparative study was conducted involving two cohorts of junior ophthalmologists, each comprising 18 participants. Group A, serving as the control group, did not rec

This surgical technique describes a modification of the continuous curvilinear capsulorhexis (CCC) to achieve an adequate-sized capsulorhexis in pediatric cataracts with high intralenticular pressure. Performing CCC in pediatric cataracts is challenging, especially when the intralenticular pressure is high. This technique involves 30 G needle decompression of the lens to reduce positive intralenticular pressure and subsequent flattening of the anterior capsule. This minimizes the chances of extension of CCC without using any special equipment. This technique was used in two eyes of two patient

Pediatric ophthalmologists increasingly recognize that the ideal site for intraocular lens (IOL) implantation is in the bag for aphakic eyes, but it is always very difficult via conventional technique. We conducted a prospective case series study to investigate the success rate and clinical outcomes of capsular bag reestablishment and in-the-bag IOL implantation via secondary capsulorhexis with radiofrequency diathermy (RFD) in pediatric aphakic eyes, in which twenty-two consecutive aphakic pediatric patients (43 aphakic eyes) enrolled in the Childhood Cataract Program of the Chinese Ministry

Negative dysphotopsia (ND) is a known cause of patient dissatisfaction following cataract surgery with intraocular lens (IOL) implantation. While IOL explantation is often considered in persistent cases, identifying reversible anatomical factors is essential to minimize surgical risk. Capsulorhexis size and configuration have been implicated in the pathogenesis of ND but remain underrecognized as a direct therapeutic target. A 73-year-old woman underwent uncomplicated phacoemulsification with implantation of an extended depth of focus (EDOF) IOL in the left eye. Postoperatively, she experience
