Shunt-related internal carotid artery dissection during carotid endarterectomy: illustrative cases.

BACKGROUND: Intraoperative shunt use during carotid endarterectomy (CEA), although critical for cerebral protection, may cause internal carotid artery (ICA) dissection in approximately 0.5% of cases. Shunt-induced dissections represent a distinct pathophysiological mechanism, differing from surgical technique-related dissection, and typically occur distal to the arteriotomy site due to trauma from shunt tips or balloon inflation. OBSERVATIONS: The authors present 2 cases of ICA dissection occurring during CEA with shunt use, both successfully managed with emergency endovascular stent insertio

Distal internal carotid artery dissection due to the carotid shunt during carotid endarterectomy.

We described two cases of shunt-related distal internal carotid artery (ICA) dissection from high cervical ICA stenosis. These cases suggest that for high cervical internal carotid endarterectomy, surgeons should reconsider using a carotid shunt to reduce the risk of ICA dissection.

Reversible cerebral vasoconstriction, internal carotid artery dissection and renal artery stenosis.

Reversible cerebral vasoconstriction is a rare and poorly understood syndrome, without clear diagnostic criteria. It has been described in association with multiple disorders, but has only been reported rarely in the setting of carotid artery dissection and, to our knowledge, never before in association with renal artery stenosis.

Internal Carotid Artery Dissection Treated with C-Guard Stent.

Internal carotid dissection is the most common cause of stroke in the young population. Stroke, the presence of a pseudoaneurysm, and decreased artery lumen with cerebral flow impairment are indications for treatment. Medical therapy with antithrombotic drugs and endovascular therapy with stenting are the main available options. The C-Guard stent is an open cell mesh-covered dual layer stent that has been mainly used in cases of internal carotid artery stenosis with a post-operative reduction in stroke incidence. Thus, we present two cases of internal carotid artery dissections of the cervical

Bilateral Traumatic Internal Carotid Artery Dissection after CrossFit Training.

Bilateral traumatic internal carotid artery (ICA) dissection after CrossFit training is an extremely rare and life-threatening condition associated with high risk of cerebral ischemia. Several imaging modalities are involved in its clinical evaluation and clinical decision-making. Anticoagulation and antiplatelet management, endovascular therapy, and open surgical intervention are the available options for ICA dissection treatment. This report details a bilateral traumatic ICA dissection after CrossFit training treated with antiplatelet and anticoagulation therapy.

Rare case of bilateral traumatic internal carotid artery dissection.

A 55-year-old man was working in a trench when the wall collapsed in on him, pinning him to the wall. On arrival in the emergency department the patient began reporting of right-sided headache. Neurological examination revealed left-sided reduced sensation with weakness. Whole-body CT scan showed right-sided flail chest and bilateral haemothorax as well as loss of flow and thinning of the distal right internal carotid artery (ICA) and loss of grey white matter differentiation in keeping with traumatic ICA dissection with a right middle cerebral artery (MCA) infarct. He was started on aspirin 3

Internal carotid artery dissection secondary to excessive vocal output.

A previously healthy 61-year-old male presented to eye casualty with a left-sided Horner's syndrome. He reported that while offering strong vocal support at a football match 5 days previously, he had suddenly noticed an unusual sensation behind his left eye, accompanied by a left hemifacial headache. He had noted pupillary asymmetry soon after this. Radiological imaging revealed a left internal carotid artery dissection. Anticoagulant therapy was commenced, and all symptoms and signs had fully resolved at 1-month follow-up, with no further complications.

Spontaneous dissection of internal carotid artery masquerading as angioedema.

Spontaneous dissection of the internal carotid artery usually presents with stroke-like symptoms secondary to ischemia in its vascular territory, as well as local signs and symptoms, which may include head, face or neck pain, Horner's syndrome, pulsatile tinnitus, and cranial nerve palsies. We report a case of a 44-year-old healthy white male who presented with tongue swelling mimicking angioedema as an unusual manifestation of spontaneous dissection of the internal carotid artery. Two weeks after the initial presentation, the patient returned with similar symptoms and slurred speech. Upon phy

Association of internal carotid artery dissection and chiropractic manipulation.

BACKGROUND: To determine the relationship between chiropractic manipulative therapy and internal carotid artery dissection, a MEDLINE literature search was performed for the years 1966 through 2000 using the terms internal carotid dissection. Literature that included information concerning causation of ICAD, as well as all case studies and series, was selected for review. REVIEW SUMMARY: In reviewing the cases of internal carotid dissection potentially related to CMT, there were many confounding factors, such as connective tissue aberrations, underlying arteriopathy, or coexistent infection,

Bilateral internal carotid artery dissection mimicking inflammatory demyelinating disease.

BACKGROUND AND PURPOSE: Internal carotid artery (ICA) dissection (ICAD) may be extremely difficult to diagnose only on the basis of historical information and clinical signs, and even standard brain imaging (computed tomography [CT], T2-weighted magnetic resonance imaging [MRI]) may not be sufficient to delineate the underlying pathology clearly, as shown in this case. METHODS: The clinical presentation and parenchymal lesion pattern on CT were suggestive of inflammatory demyelinating disease, and additional multiparametric MRI was performed. RESULTS: Diffusion-weighted MRI, magnetic resonan

Subacute hypoglossal nerve paresis with internal carotid artery dissection.

OBJECTIVES: To describe a case of an isolated hypoglossal nerve palsy in a patient with a spontaneous internal carotid artery dissection (ICAD). This condition is a well-recognized cause of cerebral ischemic stroke in patients younger than 45 years of age. Isolated cranial nerve neuropathy is a rare presentation. More common manifestations include incomplete hemiparesis, hemicrania, Horner syndrome, cervical bruit, pulsatile tinnitus, and multiple cranial nerve palsies. METHODS: A comprehensive literature search (Ovid, MEDLINE) for the presentation, diagnostic evaluation, treatment, and outco

Bilateral internal carotid artery dissection due to trivial trauma.

Internal carotid artery dissection (ICAD) is a known cause of unilateral headache and focal cerebral ischemic symptoms. Other symptoms include oculosympathetic paresis, facial pain, neck pain, subjective carotid bruits, and cranial nerve deficits. Traumatic dissection has an obvious precipitating incident preceding the neurologic or visual symptoms. An ICAD that occurs spontaneously or from trivial trauma usually lacks an obvious incident and thus requires awareness of its possibility for accurate detection and treatment. Dissections arise from a defect in the internal elastic lamina allowing

Cerebral microembolism in acute spontaneous internal carotid artery dissection.

The rate and risk factors for early ischemic recurrence in patients with internal carotid artery dissection (ICAD) are largely unknown. Serial transcranial Doppler (TCD) monitoring of microembolic signals (MES) was performed in 28 consecutive patients with acute ICAD. MES were identified in 13 patients, and early ischemic recurrence occurred in 7. Six of 13 patients with MES and 1 of 15 without MES experienced early ischemic recurrence (p = 0.029). MES detection on serial TCD monitoring may be associated with an increased risk of early ischemic recurrence in patients with acute ICAD.

Ischemic optic neuropathy associated with internal carotid artery dissection.

BACKGROUND: Ischemic optic neuropathy (ION) is an infarction of the anterior or, less frequently, posterior part of the optic nerve, usually due to a disease of small arteries supplying the optic nerve. Carotid stenosis or occlusions are rare causes, and among them, carotid dissections have been so far reported in only 5 cases. METHODS: We describe 4 patients with ION (2 anterior and 2 posterior) due to internal carotid artery dissection of a consecutive series of 110 patients with internal carotid artery dissection (3.6%). RESULTS: None of the patients had signs of central retinal artery oc

[A case of spontaneous cervical internal carotid artery dissection].

Dissection of the extracranial carotid artery is a recognized cause of ischemia, particularly in young persons who present with acute neurologic deficits, both transient and permanent. We describe a patient with a spontaneous dissection of the cervical internal carotid artery (ICA). A previously healthy 24-year-old man was hospitalized because of a sudden onset of right hemiparesis and consciousness disturbance. In reality, right cervical pain preceded this attack. The first brain MRI revealed a cerebral infarction in the right cerebral hemisphere including basal ganglia. A conventional angiog

[Dissection of the internal carotid artery with subarachnoid hemorrhage].

INTRODUCTION: Dissection of the intracranial arteries is uncommon, forming less than 10% of all cranio-cervical dissections. Apart from the classical clinical findings of extracranial dissections, intracranial dissection may cause subarachnoid haemorrhage (SAH), mainly dissections involving the posterior circulation. CLINICAL CASE: We describe the case of a 49 year old man, a smoker, who had a sudden onset of headache followed by loss of consciousness. On CT there was SAH and multiple cerebral infarcts. Arteriography showed findings compatible with dissection of the extracranial and intracran
