Imaging of infections of the central nervous system.

Acquired immunodeficiency syndrome (AIDS) and AIDS-related infection of the brain constitute a large fraction of the most recent research into central nervous system infection. Two major areas of controversy discussed in the past year's literature were the neuroradiologic and neurologic manifestations of early human immunodeficiency virus infection in the central nervous system and differentiation of toxoplasmosis from lymphoma. Other non-AIDS related infections discussed included infectious aneurysms, bacterial abscesses, tuberculosis, cysticercosis, fungal infections, and zoster myelitis.

Imaging of Congenital/Childhood Central Nervous System Infections.

This article highlights the changing profile of the pediatric patient with central nervous system infection as countries develop and the roles of different imaging modalities such as cranial ultrasound, MR imaging, and computed tomography. It discusses the commonly encountered congenital toxoplasmosis, rubella, cytomegalovirus, herpes simplex (TORCH) infections, Group B Streptococcal and Escherichia coli infections in the neonatal period, and disease outbreaks affecting children. Iatrogenic, opportunistic, and immune-mediated changes as well as long-term effects of infection and mimics of infe

[Infections of the central nervous system in immunodeficiency].

Treatments in oncology, transplantation medicine and immunology frequently lead to immunodeficiency. This review presents the most important opportunistic neurologic infections, mostly of the central nervous system (CNS). Signs and symptoms, diagnostic procedures and therapeutic options are presented. The most frequent infections are due to varicella zoster virus (VZV), Cryptococcus neoformans and Toxoplasma gondii; JC virus (JCV) and cytomegalovirus (CMV) are rare causes of encephalitis. Differential diagnoses include infection by non-opportunistic causatives, therapy associated neurotoxicity

Central Nervous System Infections Complicating Immunosuppression and Transplantation.

PURPOSE OF REVIEW: This article reviews infections associated with cancer treatments and immunosuppressive/immunomodulatory therapies used in both neoplastic and non-neoplastic conditions, including hematopoietic cell transplantation and solid organ transplantation. It provides a clinical approach to the most commonly affected patient groups based on clinicoanatomic presentation and disease-specific risks resulting from immune deficits and drugs received. RECENT FINDINGS: The clinical presentations, associated neuroimaging findings, and CSF abnormalities of patients with central nervous syste

Staphylococcus aureus Central Nervous System Infections in Children.

BACKGROUND: Central nervous system (CNS) infections caused by Staphylococcus aureus are uncommon in pediatric patients. We review the epidemiology, clinical features and treatment in 68 patients with a S. aureus CNS infection evaluated at Texas Children's Hospital. METHODS: Cases of CNS infection in children with positive cerebrospinal fluid cultures or spinal epidural abscess (SEA) for S. aureus at Texas Children's Hospital from 2001 to 2013 were reviewed. RESULTS: Seventy cases of S. aureus CNS infection occurred in 68 patients. Forty-nine cases (70%) were secondary to a CNS device, 5 (7.1

Neuroimaging of infections of the central nervous system.

Neuroimaging plays a crucial role in the diagnosis and therapeutic management of neurologic infections. This article summarizes imaging findings in brain abscesses, ventriculitis, viral diseases, and opportunistic infections. In cases of uncomplicated meningitis, cranial computed tomography is sufficient to exclude brain edema, hydrocephalus, and skull base pathology. Magnetic resonance imaging (MRI) is superior in depicting complications (e.g., empyema, vasculitis). Diffusion-weighted imaging (DWI) shows parenchymal complications of meningitis earlier and is of help in differentiation of pyog

Central nervous system infections in the systemic vasculitides.

PURPOSE OF REVIEW: Central nervous system infections are not uncommon in systemic vasculitides. Being vigilant of central nervous system infections as an alternative consideration, instead of systemic vasculitides per se, could prove to be lifesaving in certain circumstances. RECENT FINDINGS: Common pathogens, such as tuberculosis, Cryptococcus, and Listeria, still occur commonly as the major causes of central nervous system infection in systemic vasculitides. However, rare pathogens could be responsible among those immunocompromised hosts, and they should not be overlooked. The manifestation

Central nervous system manifestations of Mycoplasma pneumoniae infections.

Mycoplasma pneumoniae infection is associated with several manifestations from the central nervous system (CNS) such as encephalitis, aseptic meningitis, acute transverse myelitis, stroke, and polyradiculopathy. In the current paper epidemiologic, clinical, laboratory and treatment data on these manifestations are reviewed. The M. pneumoniae induced immune dysregulation and its contributing role in the pathogenesis of neurological insult is discussed. The recent introduction in clinical practice of newer molecular diagnostic techniques has helped in establishing a firmer association between M.

Central nervous system infections in injection drug users.

Central nervous system infections in injection drug users are often devastating in terms of excess morbidity and mortality. In injection drug users with infective endocarditis, embolization from infected valvular vegetations may cause cerebral infarction, intracranial hemorrhage, and the formation of brain abscess. Focal intracranial infections (i.e., brain abscess and spinal epidural abscess) may occur in the absence of infective endocarditis, resulting from bacteremia that seeds the brain or epidural space. Antimicrobial therapy, combined with surgical intervention, may be essential to impro

Neuroimaging of postnatal pediatric central nervous system infections.

Pediatric neuroimaging plays an important role in the timely diagnosis of postnatal central nervous system (CNS) infections, and in some patients the imaging findings are sufficiently specific to suggest a cause. The sequela of CNS infection, including hydrocephalus, subdural collections, ischemia/infarction, cerebral abscess, demyelination, and vascular thrombosis, can be accurately depicted and serially followed with current imaging techniques. The purpose of this review is to describe the spectrum of pediatric postnatal CNS infections, emphasizing key pathological and neuroimaging features,

Central nervous system infections in the immunocompromised host.

Central nervous system (CNS) infections in the immunocompromised host are predictable according to the arm of the immune defense which is most defective. A limited number of organisms with a tendency to cause CNS infections will take advantage of the different types of immune defects. Signs and symptoms of CNS infection are often quite subtle in the immunocompromised host because of the diminished inflammatory response. Serologic responses may not be as reliable as in a normal host. Aggressive diagnostic approaches are often necessary, including early brain biopsy and abscess drainage. Aggress

[Slow virus infections of the central nervous system].

The major slow viral infections discovered in man manifest themselves in chronic neurological disease. Studies of these diseases have allowed a better understanding of the mechanisms of virus persistence in the central nervous system. Findings are summarized from recent investigations on such slow viral infections as subacute sclerosing panencephalitis, progressive multifocal leukoencephalopathy and the spongiform encephalopathies. From the examples presented it is evident that there is a great diversity of virus-host interactions in slow viral infections of the central nervous system.
