Cerebrospinal fluid hydrocele caused by scrotal migration of a ventriculoperitoneal shunt.

A 6-month-old boy presented with painless right hemiscrotal swelling. The scrotal ultrasound revealed a cerebrospinal fluid hydrocele caused by the migration of a ventriculoperitoneal shunt (VPS) catheter tip into the right hemiscrotum and associated undescended left testis. Earlier, he underwent a VPS placement for hydrocephalus secondary to neonatal bacterial meningitis and ventriculitis. The patient was treated with bilateral herniotomy, left-sided orchidopexy and repositioning of VPS into the peritoneal cavity.

Female gender predisposes for cerebrospinal fluid overdrainage in ventriculoperitoneal shunting.

BACKGROUND: Gravitational valves (GVs) prevent overdrainage in ventriculoperitoneal shunting (VPS). However, there are no data available on the appropriate opening pressure in the shunt system when implementing a GV. We performed a retrospective analysis of hydrocephalic patients who were successfully treated with VPS which included one or more GV. METHOD: In this retrospective study in adult VPS patients with GVs, we analysed all available data, including the most recent computed tomography (CT) scans, to determine the best adjustments for alleviating any symptoms of overdrainage and underdr

Prenatal surgery for myelomeningocele and the need for cerebrospinal fluid shunt placement.

OBJECT: The Management of Myelomeningocele Study (MOMS) was a multicenter randomized trial comparing the safety and efficacy of prenatal and postnatal closure of myelomeningocele. The trial was stopped early because of the demonstrated efficacy of prenatal surgery, and outcomes on 158 of 183 pregnancies were reported. Here, the authors update the 1-year outcomes for the complete trial, analyze the primary and related outcomes, and evaluate whether specific prerandomization risk factors are associated with prenatal surgery benefit. METHODS: The primary outcome was a composite of fetal loss or

Cerebrospinal fluid shunting and hearing loss in patients treated for medulloblastoma.

OBJECT: Cerebrospinal fluid shunting has previously been associated with hearing loss. Although the mechanism for this is unclear, it is thought that changes in CSF pressure can affect cochlear physiology via endolymph expansion in the setting of a patent cochlear aqueduct. Patients undergoing radiation and cisplatin chemotherapy are at risk for hearing loss. The authors hypothesized that the incidence and severity of hearing loss in patients undergoing radiation and chemotherapy for medulloblastoma would be greater in those with shunts than in those without shunts. METHODS: Baseline and long

Effectiveness of a clinical pathway for patients with cerebrospinal fluid shunt malfunction.

OBJECT: Patients with CSF shunts often present to the emergency department (ED) with suspected shunt malfunction. Timely assessment and treatment are important factors affecting patient outcomes. A protocol was implemented at a tertiary children's hospital ED to expedite the care of these patients. This study evaluated the effectiveness of this protocol. METHODS: The protocol assigned all patients with CSF shunts into 1 of 3 pathways. If a patient presented with altered mental status, the Cushing triad, acute focal neurological deficit, ongoing seizure activity, or severe dehydration due to e

Cerebrospinal fluid shunt failure: late is different from early.

Six hundred and seventy-one operations to insert or revise simple, linear cerebrospinal fluid shunts were performed in a single institution from 1976 through 1989. The results were analyzed using actuarial techniques. Failures within the first 8 months were defined as 'early', and subsequent failures were 'late'. Patient age did not quite attain significance as a determinant of the risk of late shunt failure (p = 0.054). The following factors had no effect on the risk of late failure: interval since last operation, etiology of hydrocephalus, identity of surgeon, burr hole site, type of valve,

Cerebrospinal fluid galactorrhea after ventriculopleural shunting: case report.

A complication of ventriculopleural shunting in which the pleural end of the catheter retracted out of the pleural cavity is described. Continued drainage of the cerebrospinal fluid into the subcutaneous and breast tissue led to the development of breast enlargement and drainage via the nipple. The mechanism of production and guidelines to avoid this complication are discussed, and a brief review of ventriculopleural shunting complications is presented.

Cerebrospinal fluid oedema: a rare complication of the ventriculo-subgaleal shunt.

The ventriculo-subgaleal shunt is a useful technique for obtaining a temporary decompression of the ventricles in patients with obstructive hydrocephalus. A direct surgical approach to remove the obstruction can restore the CSF circulation. When the shunt tube is left behind after restoration of the CSF dynamics, an extensive CSF oedema can develop in the white matter along the drain, resulting in a severe clinical syndrome. A possible mechanism is discussed. Quick and adequate treatment proved that the cerebral damage is reversible.

Cerebrospinal Fluid Shunt Infections in Children: Do Hematologic and Cerebrospinal Fluid White Cells Examinations Correlate With the Type of Infection?

BACKGROUND: Cerebrospinal fluid (CSF) shunt infections in children represent an increasing problem in clinical practice. However, comprehensive clinical, laboratory and microbiologic data are scarce in pediatric age. METHODS: We conducted a 10-year retrospective study to (1) analyze clinical, laboratory and microbiologic parameters associated with infections in children; (2) analyze results according to the type of catheter (medicated or not), type of infection (first or relapses), type of hydrocephalus (acquired and congenital), presence or not of bacteriemia; (3) describe antibiotic suscept

Use of acetazolamide to decrease cerebrospinal fluid production in chronically ventilated patients with ventriculopleural shunts.

Acetazolamide (ACTZ), a carbonic anhydrase inhibitor, has been shown to decrease cerebrospinal fluid (CSF) production in both in vivo and in vitro animal models. We report two children with hydrocephalus who experienced multiple shunt failures, and who had externalised ventriculostomy drains (EVD) prior to ventriculopleural shunt placement. The effects of increasing doses of ACTZ on CSF production and subsequent tolerance to ventriculopleural shunts were evaluated. The patients had a 48% and a 39% decrease in their EVD CSF output when compared to baseline with maximum ACTZ dose of 75 mg/kg/day

CSFsim: A Simulation Framework for Cerebrospinal Fluid Dynamics and Hydrocephalus Shunt Systems.

This paper introduces a unified and open-source simulation framework for lumped-parameter models of cerebrospinal fluid (CSF) dynamics and hydrocephalus shunt systems. The framework enables the estimation of the underlying parameters of CSF models from physiological time series data to create a subject-specific model. A synthetic forcing generator is used to create customizable but realistic cardiovascular forcing signals as inputs to CSF models. Additionally, shunt systems with different physical properties and control strategies can be designed and integrated for closed-loop shunt simulation

Cerebrospinal Fluid Leakage from Scrotum Secondary to Ventriculoperitoneal Shunt Migration.

Ventriculoperitoneal (VP) shunts are a common neurosurgical procedure used to treat hydrocephalus. Despite their efficacy, many shunts fail and require revisions. The most common causes of shunt failure include obstruction, infection, migration, and perforation. Extraperitoneal migrations require urgent attention. We present a case of migration to the scrotum, a unique complication that may be present in young patients due to the presence of a patent processus vaginalis. Here, we discuss a case of a 16-month-old male patient with a VP shunt presenting with cerebrospinal fluid (CSF) drainage fr

Safety of laparoscopic cholecystectomy in patients with a cerebrospinal fluid shunt in the peritoneal cavity.

INTRODUCTION: The management of patients with a cerebrospinal fluid (CSF) shunt located in the peritoneal cavity undergoing laparoscopic surgery is an issue that has not yet been settled. These patients are at risk of increased intracranial pressure caused by peritoneal insufflation, shunt dysfunction, and shunt infection/retrograde meningitis. This study aimed to determine the need for perioperative shunt intervention in CSF shunt patients undergoing laparoscopic cholecystectomy. METHODS: We reviewed and analyzed five shunt patients who underwent laparoscopic cholecystectomy in our instituti

Outcomes following cerebrospinal fluid shunting in high-grade glioma patients.

OBJECTIVE: The clinical course of high-grade central nervous system gliomas is occasionally complicated by hydrocephalus. The risks of shunt placement and clinical outcome following CSF diversion in this population are not well defined. METHODS: The authors retrospectively reviewed the outcomes of patients with pathologically confirmed WHO grade III or IV gliomas with shunt-treated hydrocephalus at their institution. Outcomes of patients in this cohort were compared with those of patients who underwent shunt treatment for normal pressure hydrocephalus (NPH). Hospital-reported outcomes in a na
