We describe a boy who developed precocious puberty resulting from chorionic gonadotropin produced by a mediastinal germ cell tumor. Following tumor removal he began spontaneous precocious sexual development which was treated and then arrested spontaneously. Investigation of this arrested puberty established that he had Klinefelter syndrome (KS) mosaicism. The alpha and beta subunits of human chorionic gonadotropin are secreted both as a combined, noncovalently linked dimer form as well as uncombined, free forms by human trophoblastic cells. A case of choriocarcinoma is presented which developed during close follow-up for hydatidiform mole. Serum levels of beta subunits of human chorionic gonadotropin (beta-HCG) were assessed by radioimmunoassay (RIA) before, during, and after diagnosis of choriocarcinoma and remained below the sensitivity level of the assay. To our knowledge, this is the fourth reported case where choriocarcinoma was not associated with the presence of elevated beta-HCG levels.

Three clones of ectopic hormone-producing cells from a single human neoplasm (bronchogenic carcinoma) show different rates of synthesis and secretion of chorionic gonadotropin and its alpha and beta subunits. For each clone, the amount of one or the other subunit always exceeded that of the complete hormone molecule. These findings may be analogous to unbalanced immunoglobulin chain synthesis in certain forms of myeloma.

Of 22 consecutive cases of squamous cell carcinomas of the urinary bladder, 4 expressed the beta subunit of human chorionic gonadotropin as demonstrated by immunohistochemistry. The significance of this finding is compared to similar observations made in urothelial carcinomas by others and its value as a possible marker in diagnosing and monitoring the progress of these tumors is discussed.

Of the various sensitive assays for human chorionic gonadotropin, the radioreceptor assay is now the most practical and should be used for the early diagnosis of pregnancy (35 days or less after the last normal menstrual period). Serial measurement of this hormone by the beta subunit radioimmunoassay provides a valuable guide to the management of trophoblastic disease.

Corpus luteum activity was monitored in 20 women undergoing nonsurgical management of ectopic pregnancy with methotrexate and citrovorum factor (N = 15) or observation (N = 5). Seven clonal human placental cell lines were established by transformation of human first-trimester placental cells with simian virus 40.
