Current management of inflammatory bowel disease and colorectal cancer.

INFLAMMATORY BOWEL DISEASES (IBDS) CAN BE DIVIDED INTO TWO MAJOR DISORDERS: ulcerative colitis and Crohn's disease. Although IBD-associated colorectal cancer (IBD-CRC) accounts for only 1-2% of all cases of colorectal cancer, IBD with colon involvement is among the top three high-risk conditions for colorectal cancer. Today, colorectal cancer accounts for approximately 10-15% of all deaths among IBD patients. Indeed, patients with IBD colitis are six times more likely to develop colorectal cancer than the general population and have a higher frequency of multiple synchronous colorectal cancers

Diagnostic Performance and Reproducibility of Chromoendoscopic Classification (FOCUS Classification: Flat-Type Dysplasia Optical Assessment by Chromoendoscopy in Ulcerative ColitiS) for Flat-Type Dysplasia in Ulcerative Colitis-Associated Neoplasia.

OBJECTIVES: Ulcerative colitis-associated neoplasia (UCAN) often presents as flat-type dysplasia that is difficult to characterize endoscopically. We developed a FOCUS (Flat-type dysplasia Optical assessment by Chromoendoscopy in Ulcerative colitiS) classification and evaluated its diagnostic performance and reproducibility. METHODS: This single-center retrospective study included 33 UCAN lesions from 26 patients. Prospectively recorded FOCUS classifications at 230 biopsy sites were compared with histopathology. Chromoendoscopic patterns were categorized as Type I ("suggestive of non-dysplasi

Neuroendocrine carcinomas arising in ulcerative colitis: coincidences or possible correlations?

Patients with inflammatory bowel disease (IBD) are at increased risk of colorectal malignancies. Adenocarcinoma is the commonest type of colorectal neoplasm associated with ulcerative colitis (UC) and Crohn's disease, but other types of epithelial and non-epithelial tumors have also been described in inflamed bowel. With regards to non-epithelial malignancies, lymphomas and sarcomas represent the largest group of tumors reported in association with IBD, especially in immunosuppressed patients. Carcinoids and in particular neuroendocrine neoplasms other than carcinoids (NENs) are rare tumors an

Analysis of clinicopathological features and oncological outcomes of ulcerative colitis-associated colorectal cancer based on macroscopic classification.

PURPOSE: Few studies have reported the association of macroscopic classification with clinicopathological characteristics and prognosis of ulcerative colitis-associated colorectal cancer (UC-CRC), unlike sporadic CRC. In this study, we aimed to clarify the clinical significance of macroscopic classification of UC-CRC. METHODS: The cohort included 480 patients with UC-CRC with invasion beyond the muscularis propria treated at 43 Japanese institutions between 1983 and 2023. The patients were divided into six groups based on the macroscopic type (types 0-5), and clinicopathological features and

Clinical impact of combination of lesion-border findings and pit patterns for identifying invasive versus noninvasive neoplastic lesions in ulcerative colitis.

BACKGROUND AND AIMS: Patients with ulcerative colitis (UC) have a high incidence of colorectal neoplasia. Kudo's pit pattern is useful for assessing the depth of colorectal neoplasia in the general population; however, its application in UC-associated neoplasia is limited. Inaccurate preoperative depth diagnosis may lead to positive resection margins and perforation. We aimed to determine whether evaluating lesion-border findings can help identify cases in which pit pattern analysis is useful for depth diagnosis in patients with UC. METHODS: This single-center, retrospective study analyzed 15

Activation of MTCH2 by Momordin Ic Prevents Colitis and Colitis-Associated Colorectal Cancer Through Rescuing the Mitochondrial Dysfunction of Macrophage.

Inflammation is associated with various cancers, including colorectal cancer. Momordin Ic (MIc) has anti-tumor and anti-inflammatory properties, but its effects on colitis-associated colorectal cancer (CAC) are not well understood. This study explores MIc's influence on macrophage mitochondrial dysfunction, macrophage polarization, and tumor development in CAC. Mouse colitis and CAC models were established to assess MIc's therapeutic potential. H&E staining and immunohistochemistry evaluated inflammation and mucosal damage in colonic tissues. Flow cytometry analyzed immune cell proportions, wh

Identification of the M2 Macrophage-associated Gene THBS2 as a Predictive Marker for Inflammatory Cancer Transformation.

Ulcerative colitis (UC)-induced colitis-associated colorectal cancer (CAC) has a worse prognosis than sporadic colorectal cancer. And with the incidence of ulcerative colitis on the rise, it is critical to identify new therapeutic targets in time to stop the progression of inflammation to cancer. Through immunohistochemistry (IHC) and Gene Expression Omnibus (GEO) database analysis, we acquired the gene M2DEG, which is differentially expressed in M2 macrophages. The impact of M2DEG on the immune environment and clinical variables was confirmed through various data sets and actual tissue sample

Natural shikonin and acetyl-shikonin improve intestinal microbial and protein composition to alleviate colitis-associated colorectal cancer.

BACKGROUND: Colorectal cancer (CRC) and inflammatory bowel disease (IBD) are the most common diseases of human digestive system. Nowadays, the influence of the inflammatory microenvironment on tumorigenesis has become a new direction, and the exploration of relative molecular mechanism will facilitate the discovery and identification of novel potential anti-cancer molecules. METHODS: Natural shikonin (SK) and acetyl-shikonin (acetyl-SK) was administered to azoxymethane (AOM)/dextran sodium sulphate (DSS)-induced colitis-associated colorectal cancer (CAC) mice model by gavage to investigate th

Targeting Inflammation and Immune Regulation in Chronic Inflammation Associated Cancers.

Chronic inflammation is a fundamental driver of cancer development, linking persistent immune activation to genomic instability, microenvironmental remodeling, and malignant transformation. In inflammation-associated malignancies such as colitis-associated cancer (CAC) and metabolic dysfunction-associated fatty liver disease-related hepatocellular carcinoma (MAFLD-HCC), sustained inflammatory signaling integrates tissue injury with oncogenic pathways to promote tumor initiation and progression. Importantly, these cancers arise through prolonged preneoplastic stages, during which dysregulated i

[A case of multiple carcinomas of the colon and rectum associated with ulcerative colitis].

The present paper describes a patient with four independent carcinomas associated with ulcerative colitis. Subtotal colectomy with lymph node dissection was done on June 28, 1978. The operative specimen revealed a carcinoma at the descending colon, two independent cancerous lesions and papillary polyps at the sigmoid colon and a scirrhous, flat elevated neoplasm at the rectum. These carcinomas were surrounded by lesions of various degrees of epithelial dysplasia, suggesting precancerous change. Carcinomas and precancerous lesions were stained for carcinoembryonic antigen (CEA) by the CEA perox

Needs for Increased Awareness of Gastrointestinal Manifestations in Patients With Human Inborn Errors of Immunity.

The gastrointestinal (GI) tract is frequently affected by inborn errors of immunity (IEI), and GI manifestations can be present in IEI patients before a diagnosis is confirmed. We aimed to investigate clinical features, endoscopic and histopathologic findings in IEI patients. This was a retrospective cohort study conducted from 1995 to 2020. Eligible patients were diagnosed with IEI and had GI manifestations that were enough to require endoscopies. IEI was classified according to the International Union of Immunological Societies classification. Of 165 patients with IEI, 55 (33.3%) had GI mani

Total biopsy via endoscopic submucosal dissection is useful for the diagnosis of neoplastic lesions in patients with ulcerative colitis.

OBJECTIVE: Ulcerative colitis-associated neoplasia (UCAN) and sporadic neoplasia require different treatment strategies; their differential diagnosis is based on endoscopic findings and biopsy results. However, accurate diagnosis is sometimes difficult. Similarly, it is often challenging to evaluate invasion depth. Recently, their diagnosis has become possible pathologically, following endoscopic UCAN resection. This study investigated whether endoscopic submucosal dissection (ESD) is useful as a treatment or diagnostic (total biopsy) modality for neoplastic lesions within the inflammatory reg

Cortex periplocae modulates the gut microbiota to restrict colitis and colitis-associated colorectal cancer via suppression of pathogenic Th17 cells.

Aberrant microbe-immune cell interaction is a predisposing factor in inflammatory bowel disease (IBD) and colitis-associated cancer (CAC). Cortex Periplocae is a famous traditional Chinese medicine with putative anti-rheumatoid arthritis and anti-dyspepsia effects. Here, we show that the Periploca sepium periplosides (PePs), a cardiac glycosides-free pregnane glycosides extract from root bark of Cortex Periplocae, alleviates colon inflammation, improves intestinal epithelial barrier function, and prevents colitis-associated tumorigenesis in mice with colitis and CAC. Mechanistically, PePs trea

The anti-inflammatory drug dimethyl itaconate protects against colitis-associated colorectal cancer.

Colorectal cancer (CRC) is the third most common diagnosed cancer of which risk factors include unhealthy diet, smoking, and chronic inflammation. Weakening the inflammatory response emerges as an effective therapeutic strategy to prevent the progression of CRC. Inflammatory macrophages produce substantial amounts of immunoregulatory metabolite itaconate, which is synthesized by the immune response gene 1 (Irg1). In this study, we use a membrane-permeable itaconate derivative, dimethyl itaconate (DI), for the protection against CRC in mouse model. DI decreased the high inflammatory state of ul

Colitis-related rat colon carcinogenesis induced by 1-hydroxy-anthraquinone and methylazoxymethanol acetate (Review).

Patients with long-standing ulcerative colitis (UC) have an increased risk for developing colorectal cancer (CRC) compared to the general population. For investigation of the mechanisms and prevention of UC and UC-related CRC, establishment of a promising animal model for such disease is important. 1-hydroxyanthraquinone (1-HAQ) present in certain medicinal plants such as Rubia tinctorum L. is a genotoxic and rodent colon carcinogen. Long-term feeding of 1-HAQ induced hyper-cell proliferation in rat colonic crypts with ulcerative changes, crypt abscess, severe inflammation and erosion before t

Gut microbiota and macrophage crosstalk: implications for colitis-associated colorectal cancer.

Colitis-associated colorectal cancer (CAC) is one of the most severe complications associated with inflammatory bowel disease (IBD). Within the global landscape of cancer epidemiology, colorectal cancer (CRC) ranks among the leading malignancies in terms of both incidence and mortality. CAC, which arises in the context of IBD-including Crohn's disease and ulcerative colitis-represents a distinct subtype of CRC that is closely linked to chronic intestinal inflammation. The pathogenesis of CAC is driven by the complex intestinal microenvironment, characterized by dynamic interactions among immun

Rebalancing the inflammatory trajectory from inflammatory bowel disease to colitis-associated colorectal cancer via artemisinin-based multitarget therapy.

Inflammatory bowel disease (IBD) comprises Crohn's disease and ulcerative colitis, and that is a major risk factor for colitis-associated colorectal cancer (CAC), a distinct and aggressive malignancy driven by chronic intestinal inflammation. Artemisinins, a group of sesquiterpene lactones derived from Artemisia annua, have emerged as promising therapeutic candidates for IBD due to their potent anti-inflammatory and anticancer properties. In this review, we summarize the current evidence that artemisinins exert diverse pharmacological actions including modulation of immune responses, reduction

TRIM34 attenuates colon inflammation and tumorigenesis by sustaining barrier integrity.

Loss of the colonic inner mucus layer leads to spontaneously severe colitis and colorectal cancer. However, key host factors that may control the generation of the inner mucus layer are rarely reported. Here, we identify a novel function of TRIM34 in goblet cells (GCs) in controlling inner mucus layer generation. Upon DSS treatment, TRIM34 deficiency led to a reduction in Muc2 secretion by GCs and subsequent defects in the inner mucus layer. This outcome rendered TRIM34-deficient mice more susceptible to DSS-induced colitis and colitis-associated colorectal cancer. Mechanistic experiments demo
