Does the posterior-only approach provide adequate treatment for cervical spine metastases with severe vertebral body compromise?

PURPOSE: To assess posterior-only surgery for cervical spine metastases with severe vertebral compromise, characterized by severe compression fracture, ventral epidural compression, and extensive osteolytic destruction. METHODS: We retrospectively analyzed 31 patients with C1-T1 metastases meeting severe compromise criteria: compression fracture with > 50% height collapse or local kyphosis, severe ventral compression with the epidural spinal cord compression scale > 1b, or extensive osteolytic destruction involving > 50% of the vertebral body. Comprehensive clinical assessments included pain

[Nerve compression syndromes of the extremities].

Peripheral entrapment neuropathies occur in high frequency and present clinically with a wide range of variations. They need to be recognized early enough in order to initiate correct therapy and so to obviate serious nerve lesions and possible neurological sequelae. This paper overviews the essentials of the compression neuropathies as they are encountered in both upper and lower extremities. Pathomechanisms , pathogenesis, evaluation considerations as well as differential diagnosis and basic treatment algorithms are emphasized.

Capnography and chest-wall impedance algorithms for ventilation detection during cardiopulmonary resuscitation.

OBJECTIVE: Hyperventilation is both common and detrimental during cardiopulmonary resuscitation (CPR). Chest-wall impedance algorithms have been developed to detect ventilations during CPR. However, impedance signals are challenged by noise artifact from multiple sources, including chest compressions. Capnography has been proposed as an alternate method to measure ventilations. We sought to assess and compare the adequacy of these two approaches. METHODS: Continuous chest-wall impedance and capnography were recorded during consecutive in-hospital cardiac arrests. Algorithms utilizing each of

Compressive streak microscopy for fast sampling of fluorescent reporters of neural activity.

SIGNIFICANCE: In vivo one-photon fluorescence imaging of calcium and voltage indicators expressed in neurons enables noninvasive recordings of neural activity with submillisecond precision. However, data acquisition speed is limited by the frame rate of cameras. AIM: We developed a compressive streak fluorescence microscope to record fluorescence in individual neurons at high speeds ( ≥ 200 frames per second) exceeding the nominal frame rate of the camera by trading off spatial pixels for temporal resolution. APPROACH: Our microscope leverages a digital micromirror device for targeted illum

Current challenges in diagnostic imaging of venous thromboembolism.

Because the clinical diagnosis of deep-vein thrombosis and pulmonary embolism is nonspecific, integrated diagnostic approaches for patients with suspected venous thromboembolism have been developed over the years, involving both non-invasive bedside tools (clinical decision rules and D-dimer blood tests) for patients with low pretest probability and diagnostic techniques (compression ultrasound for deep-vein thrombosis and computed tomography pulmonary angiography for pulmonary embolism) for those with a high pretest probability. This combination has led to standardized diagnostic algorithms w

Isolated spontaneous posterior interosseous nerve palsy: a review of aetiology and management.

Isolated posterior interosseous nerve palsy is an uncommon condition and its management is controversial. Existing literature is sparse and a treatment algorithm based on existing best evidence is absent. A comprehensive review was undertaken to elucidate the causes of spontaneous posterior interosseous nerve palsy and suggest a management strategy based on the current evidence. Posterior interosseous nerve palsy can be broadly categorized as compressive and non-compressive, and the existing evidence supports surgical intervention for compressive palsy. For posterior interosseous nerve patholo

Lossless compression of CGH via resolution-controlled sub-hologram.

Holographic displays require submicron pixel pitches to achieve wide viewing angles, resulting in extremely large data sizes. Inspired by foveated rendering, we propose a method that selectively reduces the resolution of peripheral regions by controlling the resolving power between two point light sources based on the Rayleigh criterion. This approach allows resolution-controlled sub-holograms to be generated using a single-panel spatial light modulator without hardware modifications. The characteristics of hologram data generated by this method were analyzed, and five lossless compression alg

Prediction and monitoring the therapeutic response of chronic dermal wounds.

A significant proportion of chronic wounds fail to heal in response to treatment of underlying pathologies combined with good wound care practice. Current prognostic tests to identify these wounds rely on the use of algorithms based on clinically measurable parameters such as wound dimensions and wound duration. Venous leg ulcers may be stratified into healing/non healing at 24 weeks of compression therapy and diabetic foot ulcer treatment outcome assessed using a 3-parameter algorithm. Accurate and reproducible measurement of wound area is required for these algorithms to have clinical utilit

Analysis and classification of errors made by teams during neonatal resuscitation.

AIM: The Neonatal Resuscitation Program (NRP) algorithm serves as a guide to healthcare professionals caring for neonates transitioning to extrauterine life. Despite this, adherence to the algorithm is challenging, and errors are frequent. Information-dense, high-risk fields such as air traffic control have proven that formal classification of errors facilitates recognition and remediation. This study was performed to determine and characterize common deviations from the NRP algorithm during neonatal resuscitation. METHODS: Audiovisual recordings of 250 real neonatal resuscitations were obtai

A Systematic Review of Non-Traumatic Spinal Cord Injuries in Sub-Saharan Africa and a Proposed Diagnostic Algorithm for Resource-Limited Settings.

BACKGROUND: Non-traumatic myelopathy is common in Africa and there are geographic differences in etiology. Clinical management is challenging due to the broad differential diagnosis and the lack of diagnostics. The objective of this systematic review is to determine the most common etiologies of non-traumatic myelopathy in sub-Saharan Africa to inform a regionally appropriate diagnostic algorithm. METHODS: We conducted a systemic review searching Medline and Embase databases using the following search terms: "Non traumatic spinal cord injury" or "myelopathy" with limitations to epidemiology o

Controversies in treating nutcracker syndrome.

Nutcracker syndrome (NCS) is a relatively uncommon vascular condition characterized by compression of the left renal vein (LRV), resulting in a variable spectrum of nonspecific symptoms, including hematuria, flank pain, varicocele, and pelvic congestion syndrome. NCS can be classified into anterior and posterior types regarding the origin of LRV compression: anterior NCS occurs when LRV is compressed between the aorta and superior mesenteric artery, whereas posterior NCS involves LRV compression between the aorta and the spine. Despite advancements in diagnostic modalities, including Doppler u

Dictionary Learning Methods for Brain Activity Mapping with MEG Data.

A central goal in many brain studies is the identification of those brain regions that are activated during an observation window that may correspond to a motor task, a stimulus, or simply a resting state. While functional MRI is currently the most commonly employed modality for such task, methods based on the electromagnetic activity of the brain are valuable alternatives because of their excellent time resolution and of the fact that the measured signals are directly related to brain activation and not to a secondary effect such as the hemodynamic response. In this work we focus on the MEG m

Controlled Compression Nailing for At Risk Humeral Shaft Fractures.

Compression techniques seem to be the primary factor in determining the success of both plating and nailing techniques for the management of acute fractures and for delayed and nonunion management of these fractures. An intramedullary nail that can provide continual compression (like a plate) and mechanical manipulation of the callous throughout the course of treatment is an ideal device that provides all the advantages of plating and nailing and avoids the noted limitations of both. The UNYTE compression humeral nail is based on the PRECICE intramedullary limb lengthening system. This nail pr

Is vertebral augmentation the right choice for cancer patients with painful vertebral compression fractures?

Cancer-related fractures of the spine are different from osteoporotic ones, not only in pathogenesis but also in natural history and treatment. Higher class evidence now supports offering balloon kyphoplasty to a patient with cancer, provided that the pain is significant in intensity, has a positional character, and correlates to the area of the fractured vertebrae. Absence of clinical spinal cord compression and overt instability are paramount. Because of the frequent disruption of the posterior vertebral body cortex in these patients, the procedure should be performed by experienced operator
