A review was made of 15 cases of septic arthritis in children. 13 of these cases corresponded to purulent arthritis, one to tuberculous arthritis and one to brucellar arthritis. All of them received diagnostic punction. Five joints received arthrotomy, by a washed-aspiration system. The antibiotic treatment was given approximately during for six weeks. We had two complications, die to a delayed diagnosis: one coxa valga and one capital femoral necrosis. It is important the insist on a early diagnosis and decompressive arthrotomy, specially in those cases in which the hip is the joint affected.

We report a boy and girl with a "new" multiple congenital anomalies/mental retardation syndrome which resemblances Sjögren-Larsson syndrome. Both patients had a concordant pattern of anomalies consisting of congenital lamellar ichthyosis with spontaneous improvement, moderate mental retardation, mild pyramidal involvement, telecanthus, flat facies, stubby long bones, and coxa valga. Severe myopia, ventriculoseptal defect, and postaxial polydactyly were present in the girl who had more severe bone involvement with dense, enlarged metaphyses, vertebral dysplasia, and advanced skeletal maturation

Twenty-three of 36 patients (64%) with Rett syndrome showed orthopedic problems at an average age of 8 years 7 months. Sixteen patients had spinal deformities (45%), six had scoliosis, one had sagittal deformity, and nine had scoliosis with sagittal deformity. Eight patients with scoliosis have shown progression, with two patients requiring surgery. Thirteen patients (36%) exhibited joint contractures; three of them have required surgery. Ten fractures occurred in five patients (14%). Coxa valga was present in all 14 hips. Skeletal ages in four preadolescent patients were normal.

The results of treatment with Pavlik harness in 168 hip joints have been reviewed. The average follow-up period was 8 years 6 months. In total 114 hip joints were treated only by Pavlik harness. Four hip joints were treated by surgical methods after conservative treatment. The results were very good in 81.0%. Subluxated hips were in 8.9%. Coxa valga et antetorta in 6.0% and deformed head in 4.1%. In a high percentage of our cases radiologically very good results were found. Joint development must be further followed up.

BACKGROUND: Varus or valgus deviations of the femoral neck in osteogenesis imperfecta have been an ignored chapter because the classic correction procedures were applied in medical practice with unsatisfying results. Until the use of telescopic rods, coronal deviations remained unsolved and the distal configuration of the proximal femoral extremity remained uncorrected or partially corrected, which required an extensive use of the wheel chair or bed immobilization of the patient. The concomitant correction of the complex deformities, coxa vara/valga and femoral integrated configuration, have b

PURPOSE OF THE STUDY: The aim of this retrospective study was to assess the results of varus osteotomy of the proximal femur in adults with coxa valga after developmental dysplasia of the hip (DDH) and to evaluate the efficacy of this method.
MATERIAL AND METHODS: Thirty hips in 28 patients treated by proximal femoral varus osteotomy in the period from 1983 to 1990 were evaluated. The indication for surgery involved coxa valga (145°-168°) with grade I- III of osteoarthritis and mild acetabular dysplasia. The patient group comprised twenty six women and two men with an average age of 28 years

BACKGROUND: Fibrous dysplasia of bone is a skeletal dysplasia with a propensity to affect the femur in its polyostotic form, leading to deformity, fracture, and pain. The proximal femur is most commonly involved with a tendency to distal progression, thereby producing the typical shepherd's crook deformity. However, there are few data on the spectrum and progression of femoral deformities in polyostotic fibrous dysplasia.
QUESTIONS/PURPOSES: The purposes of this study were (1) to develop a radiographic classification for polyostotic fibrous dysplasia; (2) to test this classification's intra-

In approximately 25% of patients with hereditary multiple exostosis, there is an abnormal osteochondral formation localized in the femoral proximal metaphysis. This formation often causes a mechanically progressive insufficiency of the acetabular cavity, a true developmental hip dysplasia, that together with a coxa valga deformity, which is also present, causes a gradual deterioration in the relations of this joint. This malformation has a poor prognosis and is difficult to manage. Although this entity is rather frequent and quite severe, it is rarely found in the medical literature. The autho

Pauwel's osteotomies unquestionably have a place in the treatment of the osteoarthritic consequences of subluxated dysplastic hips. They provide excellent and lasting results when the indication is correct and the surgical procedure is carried out properly. We recommend a varus osteotomy for coxa valga with subluxation presenting with a dense triangle at the edge of the socket and congruent joint surfaces in abduction; a valgus osteotomy when the femoral head and the acetabulum are considerably deformed, with a dense triangle at the edge of the socket and with the joint surfaces congruent in a

A case presenting a peculiar type of spondylo-epiphyseal dysplasia was studied. Clinically, the normal height was striking. The X-rays showed large epiphysis and wide metaphysis. Bilateral coxa valga with very large femoral necks was present. The height of the vertebral bodies was slightly reduced. The study of the upper tibial growth cartilage showed glycoprotein inclusions in the chondrocytes and large dilatations of the rough endoplasmic reticulum. The gel electrophoresis of the non collagenous proteins extracted from the growth cartilage had an abnormal densitometric tracing. The type of i

CASE: Bilateral valgus slipped capital femoral epiphysis (SCFE) is a rare condition with few cases reported in the literature. However, there are no reports of bilateral unstable valgus SCFE. We report a unique case of bilateral acute-on-chronic unstable valgus SCFE with severe coxa valga in a 9-year-old girl. She was treated with bilateral capital realignment procedure along with subtrochanteric osteotomy and had a good clinical outcome.
CONCLUSION: We hitherto report a rare case of bilateral unstable valgus SCFE which was successfully managed. This report also highlights the importance of t

We report on an Egyptian girl with phenotypic abnormalities of cerebro-oculofacio-skeletal syndrome. She had microcephaly, bilateral congenital cataract, nystagmus, long ear pinnae, camptodactyly, prominent heels, coxa valga, kyphosis and flexure contracture of the elbows and knees. CT scan showed bilateral symmetrical intracranial calcifications. In addition, she had an apparently balanced translocation: 46,XX,t(1;16)(q23;q13) in all cells transmitted from a phenotypically normal mother with a similar balanced translocation mosaicism. We suggest that genes for COFS syndrome could be located o

We report a patient with a sporadic case of oto-palato-digital (OPD) syndrome type II. Parents and five previous sibs are normal. At 26 years of age the patient had conductive hearing impairment, cleft palate, a prominent forehead, a flat facies, and a broad nasal base resulting in the characteristic "pugilistic" appearance. Extension and supination were limited at the elbows; thumbs and halluces were broad. Many radiological abnormalities were noted: malformations of the cervical spine, pelvic abnormalities, bilateral coxa valga, genu valgum, small fibulae, pes equino varus, and 15 carpal bon

The cases of 2 patients with valgus slipped capital femoral epiphysis are presented. Additional imaging studies provide support for true posterolateral epiphyseal displacement. Increased femoral anteversion and coxa valga contribute to the pathogenesis of valgus slipped capital femoral epiphysis. In situ pin fixation is recommended for stable valgus slipped capital femoral epiphysis. The importance of valgus slipped capital femoral epiphysis lies in its recognition and appropriate screw placement when internal fixation is used. The percutaneous technique should be used with caution. A limited

From 657 children who suffered from fractures of the femoral shaft during the years 1965-1980 504 could be followed-up in 13 hospitals of the GDR clinically and roentgenologically. 313 had undergone conservative treatment, 191 surgery with different methods. In all surgically treated cases there was overgrowth of the broken femoral shaft there was a shortening only in a few cases. In most cases the rotational deformity of the distal fragment persisted. Dislocation ad latus always had disappeared, whereas varus and expecially valgus deformities as well as antecurvation had improved partially. C

Developmental dysplasia of the hip (DDH) is a common condition predisposing to osteoarthritis (OA). Especially since DDH is best identified and treated in infancy before bones ossify, there is surprisingly a near-complete absence of literature examining mechanical behavior of infant dysplastic hips. We sought to identify current practice in finite element modeling (FEM) of DDH, to inform future modeling of infant dysplastic hips. We performed multi-database systematic review using PRISMA criteria. Abstracts (n = 126) fulfilling inclusion criteria were screened for methodological quality, and r

Pelizaeus-Merzbacher disease (PMD) is a degenerative leukodystrophy of the central nervous system resulting in progressive spasticity and neurologic deterioration. Seitelberger (13) divided this rare disease into six types. Five patients with the type 1 and two patients with the type II form of PMD have been treated at the Children's Hospital of Eastern Ontario. Our study sought to identify the orthopaedic manifestations of PMD and to develop a common orthopaedic profile for these patients. All children with types I and II PMD developed spastic quadriparesis, truncal hypotonia, thoracolumbar s

Acetabuloplasty with lateral osteotomy of the pelvis in combination with intertrochanteric derotation-varus osteotomy was performed in our hospital between 1970 and 1976. Ninety hip joints have been reexamined recently after cessation of growth. Measurements of the acetabula show normal results in more than 80% of the cases. Measurements of the femoral neck and head, however, were normal in many fewer cases. Revalgization to a "head-in-neck position" or "subcapital coxa valga" was found in 40% of the joints, while the position of the acetabuloplasty remained without significant change if the t
