Post-craniotomy emergence hypertension: A scoping review.

INTRODUCTION: Post-craniotomy emergence hypertension (PCEH) is a phenomenon of transient increase in blood pressure during and after emergence from anesthesia for a craniotomy. It is frequently encountered in neurosurgical and neuroanesthesia practice. Clinical definitions and reported prevalence of PCEH vary significantly, so this review aims to characterize existing literature on the diagnosis and management of this clinical entity. METHODS: A scoping review of the literature was conducted using six electronic databases: PubMed, Embase, CINAHL, Web of Science, Cochrane, and Scopus. Title an

Brainstem anesthesia during awake craniotomy: illustrative case.

BACKGROUND: Awake craniotomy (AC) is performed to remove the lesions near or in eloquent areas, during which the patients are alert and without any airway instrument. Apnea is a severe complication in AC. Here, the authors describe a case of sudden apnea induced by unexpected local anesthesia of the brainstem during AC. OBSERVATIONS: A 42-year-old male underwent AC for a large, recurrent, bilateral frontal lobe mass and experienced transient apnea and loss of brainstem reflexes during the surgery. The patient recovered spontaneous breath rhythm just a few minutes after the removal of a lidoca

Cavernous Sinus Meningioma Resection Through Orbitozygomatic Craniotomy.

Meningiomas of the cavernous sinus (CS) present a neurosurgical challenge. The anatomic complexity of the area can create a potential for injury to neurovascular structures. This has traditionally limited total tumor removal. A thorough understanding of the cavernous sinus surgical anatomy can help overcome this problem. Patients with CS meningiomas generally present with cranial nerve deficits (III-VI), proptosis, and visual disturbances. Management of CS tumors is controversial and includes observation, stereotactic radiosurgery, and surgical resection. We present the case of a 43-year-old r

Ischemic stroke following elective craniotomy in children.

OBJECTIVE An ischemic stroke following an elective craniotomy in a child is perceived to be a rare event. However, to date there are few papers on this topic. The purpose of this study was to investigate the occurrence of stroke following elective intracranial surgery at a children’s hospital. METHODS The authors performed a retrospective review of all patients who developed a perioperative stroke following an elective craniotomy from 2010 through 2017. Data were collected using an institutional database that contained demographic, medical, radiological, and outcome variables. RESULTS A total

Occipital Artery Pseudoaneurysm After Posterior Fossa Craniotomy.

BACKGROUND: Pseudoaneurysms of scalp arteries have been reported in rare cases after iatrogenic injury; however, they are far more commonly seen after traumatic injuries. They are usually associated with the superficial temporal artery; however, there have been a few reports of psuedoaneurysms of the occipital artery (OA). CASE DESCRIPTION: We present a unique case of an OA pseudoaneurysm presenting with delayed postoperative hemorrhage after a retrosigmoid craniotomy. The pseudoaneurysm was treated by coil embolization. CONCLUSIONS: The patient recovered fully after endovascular embolizatio

Hydrocephalus after decompressive craniotomy: a case series.

BACKGROUND: Post-craniectomy hydrocephalus in patients with intracranial hypertension is becoming a major concern for neurosurgeons because of the increasing number of hospital admissions for head trauma, stroke and other lesions which may lead to severe brain oedema requiring decompressive craniectomy. METHODS: We collected records of all the paediatric patients who developed hydrocephalus following decompressive craniotomy from October 2011 to October 2013 and analysed their clinical profiles. RESULTS: We had 3 patients in this group, ranging in age from 6 to 18 years; 1 patient died and t

Intraorbital meningioma: resection through modified orbitozygomatic craniotomy.

Intraorbital meningiomas are challenging lesions to excise because of their location and the restricted surgical corridor available due to the presence of important neighboring structures. Lesions located in the posterior one-third of the orbit require skull base approaches for their exposure and safe resection. Frontoorbital and modified orbitozygomatic (OZ) craniotomies may facilitate the exposure and resection of masses in the posterior intraorbital space. Specifically, the one-piece modified OZ craniotomy provides many advantages of the "full" OZ craniotomy (which includes a more extensive

[Osteoplastic decompressive craniotomy--indication and surgical technique].

Decompressive craniotomy is usually carried out using decompressive craniectomy (osteoclastic decompressive craniotomy) when the bone flap is removed. In situations when the level of expansion does not call for decomopressive craniectomy, we do not remove the bone flap and we perform osteoplastic decompressive craniotomy. The indication is based on assessment and cross correlation of the following parameters: intracranial pressure, midline shift and the number of pathologies on CT, actual influence of antiedematous therapy, expected cerebral oedema progression and especially according to the si

Sudden hearing loss following non-petrous craniotomy.

We present two patients with known otosclerosis undergoing craniotomy for conditions unrelated to the temporal bone who experienced sudden sensorineural hearing loss. In both patients, the hearing loss was noted immediately post-operatively and there was no subsequent recovery. Sudden hearing loss is a rare complication of non-otologic, non-cardiopulmonary bypass surgery. To our knowledge it has not been described in patients with otosclerosis undergoing craniotomy. This is a rare event that may occur in patients with vulnerable ears, such as those with otosclerosis or pre-existing sensorineur

Outer table craniotomy for frontal sinus mucocele.

Variants of the external osteoplastic flap procedure or endoscopy have been used to approach the frontal sinus mucocele. The authors introduce a modified external approach for radical resection of the mucocele. Using the bicoronal skin incision, the skin flap exposed the right upper orbital rim. The outer table craniotomy was then performed to expose the frontal sinus cavity while carefully preserving the inner table, with radical removal of the mucocele mucosa. The sinus cavity was irrigated with hyper-oxide solution to ensure adequate destruction of possible residual mucosa. The fascia of th

Granuloma caused by oxidized cellulose following craniotomy.

Three cases are reported in which large granulomas developed 13 to 21 months respectively after removal of intracranial meningiomas (2 cases) and after operation for an anterior communicating artery aneurysm. In each of the cases oxidized cellulose had been used and left in place for haemostasis. All of the granulomas acted as space-occupying lesions. They could be removed operatively with good result. All of them consisted of mononuclear phagocytic and multi-nuclear giant cells and contained remnants of oxidized cellulose. No comparable cases have been reported in the literature. Diagnosis, t

Persistent Post-craniotomy Headache Following Adult Craniotomy: A Propensity Score-Matched Analysis of Scalp Block and Surgical Risk Factors.

OBJECTIVE: Persistent post-craniotomy headache (PCH) is a recognized complication affecting functional recovery. While scalp nerve blocks improve acute postoperative analgesia, their association with persistent headache remains uncertain. This study evaluated persistent PCH incidence and its association with scalp blocks after adult craniotomy. METHODS: This retrospective study included adults undergoing craniotomy at University Malaya Medical Centre between June 2018 and June 2022. PCH was defined using International Classification of Headache Disorders criteria. To reduce selection bias, an

Standard Orbitozygomatic Craniotomy versus Mini Orbitozygomatic Craniotomy via an Eyebrow Incision for Microsurgical Treatment of Anterior Communicating Artery Aneurysms.

Background  Skull base approaches are utilized to improve microsurgical treatment of cerebral aneurysms. Advantages include early proximal and distal control, increased visualization, and minimal brain retraction. Orbitozygomatic (OZ) craniotomies via pterional incision are commonly used for the treatment of anterior communicating artery (ACoA) aneurysms. A smaller, less invasive OZ craniotomy performed through an eyebrow incision may provide several advantages over a standard OZ approach. Objective  We compare surgical outcomes of the standard and eyebrow OZ for the treatment of ACoA aneurysm

Minimally invasive burr hole craniotomy versus drill hole craniotomy for the management of chronic subdural hematoma: a randomized clinical trial.

OBJECTIVE: Chronic subdural hematoma (cSDH) is a frequently treated entity for neurosurgeons worldwide. Because this pathology is common in older patients and those with multiple comorbidities, the demographic shift increases pressure on healthcare systems, which already face major economic challenges. Various surgical procedures are used, although the high recurrence rate leads to the even greater importance of standardizing the medical approach. Therefore, the aim of this study was to compare bedside cSDH evacuation using hollow screws (HSs) under local anesthesia with evacuation using enlar

Brain Sag: An Under-recognized Complication of Craniotomy.

PURPOSE OF REVIEW: The review aims to summarize postoperative disturbances in cerebrospinal fluid (CSF) dynamics, including the pathophysiology, risk factors, and clinical implications of brain sag after craniotomy. RECENT FINDINGS: The literature on CSF hypovolemia and brain sag after craniotomy is comprised of single center retrospective studies and case reports with variable definitions. Brain sag, downward displacement of the brain due to CSF hypovolemia and loss of buoyancy, is an under-recognized cause of neurological deterioration after neurosurgery. This occurs most often in the contex

Iatrogenic Pseudo-Duane Retraction Syndrome Following Orbitozygomatic Craniotomy.

Restrictive strabismus is a known complication of orbitozygomatic craniotomy. However, a pseudo-Duane syndrome has not been described following this procedure. The authors describe a 58-year-old woman who after craniotomy developed incomitant left exotropia with an adduction deficit; the globe retracted and palpebral fissure narrowed with attempted ocular adduction. [J Pediatr Ophthalmol Strabismus. 2024;61(1):e7-e10.].

An Unusual Case of Severe Trismus After Craniotomy.

Myositis ossificans Traumatica (MOT) is a non-neoplastic, heterotrophic ossifying disease process in muscle and soft tissue rarely involving the craniofacial area. Treatment depends on the extent of ossification and functional limitations. We present the first case of MOT of the temporalis muscle following neurosurgical intervention. This is the case of a 28-year-old male patient who underwent numerous neurosurgical interventions and reported progressive trismus following these interventions. Imaging demonstrated a right temporal region radio-opacity consistent with ossification of the tempora
