[Cystic congenital dilatation of main biliary duct].

A case of cystic congenital dilatation of hepatocholedoch, in a patient aged 27, is presented. The malformation affects the entire biliary duct, being classified as Todani Ic type. The diagnosis was intraoperatively established and the surgical solution was the quasitotal extirpation of the hepatocholedoch followed by the reconstruction of the biliary transit through hepaticojejunostomy on Y loop Roux. The postoperative evolution was favourable and the result is still good after 13 years.

Cystic Duct Remnant Leading to Stump Cholelithiasis.

A 42-year-old male presented with a two-month history of right upper quadrant abdominal pain, worsening over four days. Six months earlier, he had undergone a laparoscopic cholecystectomy. Examination revealed no jaundice or tenderness, and liver function tests were normal. The patient's basic inflammatory markers were within the normal range. Ultrasound revealed a residual gallbladder stump or dilated cystic duct with a calculus, and magnetic resonance cholangiopancreatography (MRCP) demonstrated a 3 × 1 cm stump with an 8 mm hypointense focus. Endoscopic retrograde cholangiopancreatography (

Primary tuberculosis of cystic duct lymph node.

Tuberculosis (TC) is very common and significant cause of morbidity and mortality worldwide. Isolated cystic duct lymph node TC cases without involvement of gallbladder are exceedingly rare. It is difficult to diagnose preoperatively because of lack of characteristic signs and symptoms of TC. We report a man aged 45 years who presented with right upper abdominal pain since 1week. It was associated with nausea and postprandial fullness. There was no evidence of jaundice and lymphadenopathy. Abdominal examination showed moderate right upper quadrant tenderness with positive Murphy's sign and spl

Early cystic duct carcinoma of new classification.

INTRODUCTION: Classically defined cystic duct carcinoma is extremely rare owing to its strict diagnostic criteria, which are not suitable in actual clinical settings. Recently, several new classifications of cystic duct carcinoma were reported, which defined it as a tumor with its center located in the cystic duct. On the other hand, the incidence of cystic duct carcinoma, based on the new classifications, is not rare, but all reported cases are advanced. PRESENTATION OF CASE: A 77-year-old man with dilatation of the common bile duct, a stricture at the level of the cystic duct junction, and

[The cystic duct stump after laparoscopic cholecystectomy].

The aim of this study was to evaluate the length of cystic-duct stumps after laparoscopic cholecystectomy. 113 patients underwent intravenous cholangiography 2 to 3 months postoperatively, whereby a cystic-duct remnant of up to 1 cm was found in 34.5% (n = 39) and between 1 and 2 cm in 36.3% (n = 41). In 24.8% (n = 28) a stump measuring 2 to 3 cm was registered and in 4.4% (n = 5) the cystic-duct remnant was more than 3 cm. Possible consequences with regard to the development of the postcholecystectomy syndrome are discussed. In our experience laparoscopic cholangiography seems to be a useful

Acute calculous cholecystitis with patent cystic duct.

In the diagnosis of acute cholecystitis, the interpretation of cholescintigraphy is usually not difficult. It is conceivable, however, that with unusual location of obstructing calculi atypical images could result. Three cases were studied with cholescintigraphy and one with cholecystography in which more distal locations of obstructing calculi resulted in partial visualization of the gallbladder and erroneous interpretations were possible. Nuclear medicine radiologists should, therefore, be aware that atypical gallbladder images may be the result of unexpected location of obstructing calculi.

Mid-to-long-term follow-up study on the application of cystic duct patency test and cystic duct exploration in laparoscopic cholecystectomy for calculous cholecystitis: Xinjiang-Gu laparoscopic cystic duct exploration.

BACKGROUND: Laparoscopic cholecystectomy (LC) is standard for gallstones, but residual cystic duct and common bile duct (CBD) stones occur due to lack of systematic intraoperative cystic duct exploration. This study evaluated the mid-to-long-term efficacy and safety of a modified procedure-Xinjiang-Gu Laparoscopic Cystic Duct Exploration and Lithotomy-in reducing residual biliary stones. METHODS: This retrospective cohort study enrolled patients undergoing LC for gallstones (08/2021-09/2023). EXCLUSION CRITERIA: biliary malignancy, intraoperative CBD exploration, prior bilioenteric anastomos

[Squamous cell carcinoma of the gallbladder complicating a cystic dilation of the cystic duct and common bile duct: a case report].

Congenital cystic disease of the biliary tree is a known risk factor for gallbladder cancer. We here report a case of squamous cell carcinoma of the bile duct (BD) complicating a cystic dilatation of the bile ducts in a 54-year-old woman hospitalized for biliary pancreatitis. Abdominal scanner showed nodular thickening of the fundus of the gallbladder and fusiform dilation of the cystic duct and the main bile duct (VBP) with lesion of the tail of the pancreas, initially suggesting mucinous cystadenoma. Extended cholecystectomy involving the gallbladder fossa with resection of the distal biliar

Low insertion of cystic duct increases risk for common bile duct stone recurrence.

BACKGROUND: Common bile duct (CBD) stone is one of the most frequent biliary diseases. Recurrence after the complete removal of CBD stones is high, and we aim to evaluate the rate and risk factors for symptomatic recurrence of CBD stones after endoscopic retrograde cholangiopancreatography (ERCP). METHODS: We, retrospectively, reviewed the database of patients who underwent ERCP for CBD stones and subsequent cholecystectomy between January 2015 and December 2017 at a tertiary hospital. The recurrence of symptomatic CBD stones was defined as the presence of a CBD stone with related symptoms at

Aberrant right hepatic duct draining into the cystic duct: clinical outcomes and management.

Background. Aberrant right hepatic duct (ARHD) draining into cystic duct (CD) is relatively rare but clinically important because of its susceptibility to injuries during cholecystectomy. These injuries are often-times missed or diagnosed late and as a result can develop serious complications. Methods. Four consecutive patients diagnosed with ARHD draining into CD were identified for inclusion. Results. The mean age of patients was 42.5 years. The diagnosis in one of the patient was incidental during a routine endoscopic retrograde cholangiopancreatography (ERCP). Other three patients were dia

Does routine cystic duct cholangiogram during laparoscopic cholecystectomy prevent common bile duct injury?

Between November 1989 and September 1992, the author performed 1,000 laparoscopic cholecystectomies. Three common bile duct injuries occurred (0.3%). Selective cystic duct cholangiograms were performed for diagnosis and management of common bile duct stones. A total of 102 cholangiograms (10%) were done. Only eight of these cholangiograms were done for anatomical verification in the face of severe chronic or acute cholecystitis. Five were cystic duct cholangiograms, resulting in two common bile duct injuries. To avoid the problem of common bile duct injury in cases of obscure anatomy where cla

[Anomalous junction of pancreaticobiliary duct in congenital bile-duct cystic and cylindrical dilatation].

OBJECTIVE: To study the relationship between anomalous junction of pancreaticobiliary duct (APBD) and congenital bile-duct cystic as well as cylindrical dilatation. METHOD: Cholangiography was performed in 51 patients with bile-duct dilatation. RESULTS: APBD was diagnosed in 43 patients. In 8 patients (72%), the bile duct was drained into the pancreatic duct (B-P) in the cystic group, whereas in 14 patients (78%), the pancreatic duct was drained into the bile duct (P-B) in cylindrical group. There were significant differences in the internal diameter of the distal bile duct (P < 0.01), the m

Cystic Salivary Duct Carcinoma Penetrated by Facial Nerve.

AIM: Understanding the physical and physiological demands of female youth basketball is essential for optimizing training and performance monitoring. However, evidence describing match demands in elite U-19 female players, particularly in African contexts, remains limited. Existing profiles are largely derived from male or adult cohorts and may not accurately reflect youth competition. This study aimed to examine the physical and physiological demands of elite North African U-19 female basketball players, considering differences by playing position (guards, forwards, centers) and competitive l

Cystic duct leaks after laparoendoscopic single-site cholecystectomy.

BACKGROUND: Leakage from the clipped cystic duct stump (cystic duct stump leak [CDSL]) as a cause of biliary peritonitis has not been emphasized enough. It deserves special mention because it is not an uncommon cause and it is easier to treat. With the advent of laparoendoscopic single-site (LESS) cholecystectomy, its occurrence in relation to other causes of biliary peritonitis needs reexamination. SUBJECTS AND METHODS: Details of 756 patients undergoing LESS cholecystectomy were analyzed, and patients presenting with biliary peritonitis were identified. The investigative profile included an

Cystic duct remnant and the 'post-cholecystectomy syndrome'.

Post-cholecystectomy syndrome refers to a wide spectrum of conditions that pose a challenging diagnostic dilemma. Cystic duct remnant, defined as a residual duct greater than 1 cm in length, may, in the presence of stones, cause post-cholecystectomy syndrome. In this report, 4 patients with post-cholecystectomy syndrome due to cystic duct remnant are described. All underwent laparoscopic cholecystectomy and one was converted to open. The patients presented with pain 10 months to 9 years post-cholecystectomy and investigations demonstrated cystic duct remnant. All patients underwent successful

Biliary-colonic fistula through a cystic duct stump.

BACKGROUND AND METHODS: Biliary-colonic fistulas are a known, but unusual, complication of gallstone disease. Fistulas occurring after cholecystectomy between cystic duct stump (CDS) and the colon are extremely rare; only two cases have been previously reported in the literature. We report a third case, and discuss the diagnostic work-up and management of this entity. RESULTS: Biliary-colonic fistulas usually present with biliary and septic complications. ERCP and barium enema play an important diagnostic role. Treatment is mainly surgical, with division of the fistula and excision of the CDS
