Managing Dermatologic Delusional Disorders: A US Psychodermatology Perspective for Clinicians.

Dermatological delusional disorders, such as delusional infestation (DI), are a unique type of psychosis in which the patient experiences specific delusions, such as parasites or inorganic materials that come out of their skin. Originally, this type of "encapsulated psychosis" was thought to be unresponsive to antipsychotic agents. However, over the past half a century, several medications with antipsychotic properties have been found to be effective in the treatment of these conditions in the USA, especially pimozide and risperidone. Despite effective treatments, several challenges may arise

Drug-induced Delusional Infestation.

It is considered that certain drugs might induce delusional infestation, yet, to date, no studies have been performed to identify the pharmacodynamics associated with these treatments. The aim of this review is to summarize current available knowledge of drug-induced delusional infestation. A literature search was performed for primary studies on suspected drugs reported to induce delusional infestation. Included articles were evaluated systematically using the Naranjo criteria. In addition, drug mechanisms of action were compared. The final selection included 31 studies, in which a total of 2

Bugs Bugging the Body and the Brain: A Case of a Bed Bug Infestation Progressing to Delusions of Parasitosis Treated With Clozapine.

Delusions of parasitosis, also known as delusional parasitosis or Ekbom syndrome, is a rare psychiatric disorder characterized by a fixed belief in a bug infestation, despite the absence of clinical evidence. It often co-occurs with other psychiatric conditions such as depression, anxiety, and schizophrenia. In this case, a 50-year-old man with bipolar-type schizoaffective disorder developed delusional parasitosis after a bed bug infestation in his group home. His inadequate adherence to prescribed medications likely exacerbated his symptoms. Treatment involved the consistent administration of

Kyrle's Disease in a Patient with Delusions of Parasitosis.

Acquired perforating disorders are a group of uncommon skin conditions characterized by transepidermal extrusion of altered dermal material, most often associated with diabetes mellitus and chronic kidney failure. Delusional parasitosis is a primary psychiatric disorder in which affected patients have fixed, false beliefs that their skin is infested by parasites, in the absence of any evidence supporting their statements. A 69 year old malepatient addressed the Dermatology Department for a skin eruption consisting of multiple umbilicated keratotic papules with a generalized distribution. The p

Consensus recommendations for the diagnosis and management of delusional infestation: A Delphi study.

BACKGROUND: Delusional infestation (DI) is a psychiatric disorder defined as a fixed false belief of being infested with animate or inanimate material in the absence of objective medical evidence. It presents frequently to dermatology and psychiatry and poses diagnostic and communication challenges. OBJECTIVES: To develop a Delphi consensus on the definition, diagnostic work-up and management of DI. METHODS: Under the auspices of the European Society for Dermatology and Psychiatry (ESDaP), a core expert group drafted 22 statements based on prior survey data and the literature. International

Aripiprazole Treatment in Delusional Infestation: Resolving Atypical Presentations of Body Fluid Leakage.

BACKGROUND Delusional infestation is a rare psychiatric disorder characterized by the fixed belief of being infested with parasites or other unseen organisms. Atypical presentations, such as delusions of infectious body fluid leakage, can pose diagnostic challenges. This report describes 2 cases of women with such atypical delusions, highlighting the importance of recognizing this unusual manifestation of delusional infestation. CASE REPORT Two women, ages 57 and 41 years, presented with persistent skin lesions and intense itching, initially misdiagnosed as dermatological conditions. Detailed

Uncommon and/or bizarre features of dementia.

This study aimed at describing uncommon or bizarre symptoms observed in patients suffering from dementia. Medline and Google scholar searches were conducted for relevant articles, chapters, and books published since 1967. Search terms used included uncommon presentation, behavioural and psychological symptoms, dementia, Alzheimer's disease, and fronto-temporal dementia. Publications found through this indexed search were reviewed for further relevant references. The uncommon symptoms are described as case-reports and there are no systematic investigations. Bizarre behaviours arising late in li

Can pimozide kill parasites? Surprisingly, the most honest answer is 'yes'.

Purpose: One of the most well-known medications for treating delusional infestation (DI) is pimozide. Many patients may be reluctant to initiate treatment unless a medication has anti-pathogenic properties, as they feel otherwise it does not address their concerns regarding infestation. In this article, we explore the evidence that pimozide has a range of antipathogenic effects and how this fact can aid in patient care. Materials and methods: A scoping literature review was performed using The National Library of Medicine (PubMed). The search terms used were pimozide AND anti-microbial OR anti

From empiric anti-scabietic treatments to delusional infestation: A case report.

Chronic pruritus accounts for approximately 1 % of all physician visits, and providers are often confronted with how to treat patients with pruritus. Scabies is oftentimes a concern by both patients and providers, and anti-scabietic treatments are generally considered safe and effective. However, the pruritus will continue if there is an alternate etiology. We present a case of a patient empirically treated for scabies with multiple courses of anti-scabietic therapy without diagnostic confirmation of scabies. Her pruritus failed to improve, and she ultimately developed delusions of infestation

[Psychodermatology: current state of the problem].

An analysis of comorbidity of psychiatric and dermatological pathology in historical, epidemiological and clinical aspects is presented. Psychocutaneous disorders (delusional parasitosis, hypochondria circumscripta, obsessive-compulsive disorders with self-mutilations, pathomimia) play a central role in systematics elaborated in the present study. The authors suggest that delusional parasitosis is a subtype of paranoiac psychosis (paranoia, paranoiac schizophrenia). Psychiatric disorders triggered by dermatological pathology were specified as nosogenous reactions, depressive reactions with soc

Delusions of parasitosis and delusions of parasitosis by proxy in a mother and her children.

Delusions of parasitosis by proxy is an uncommon entity wherein an individual projects a delusional belief onto another person who lacks capacity to hold the same belief. We report a case of delusions of parasitosis by proxy in a mother who believed that she was infested with scabies and projected her fixed, delusional belief of scabies infestation onto her children. She subjected her children to numerous home remedies and medical treatments as well as removing them from school in an effort to cure them of the supposed infestation. Child maltreatment can be a concern in such cases with a low t

[Psychiatric and parasitologic aspects of dermatozoon delusion].

Patients suffering from delusional parasitosis are not only seen by psychiatrists, but more often by dermatologists and parasitologists. Previous results published by researchers of differing specialities led to somewhat contradictory opinions concerning these patients. The main goals of the present study were, on the one hand, to examine the various and partly controversial hypotheses, which were mostly based on single case observations in a substantial sample of 107 patients with delusional parasitosis and, on the other hand, to investigate possible differences between patients who consulted

A Case Series of Delusional Infestation in Huntington's Disease.

Huntington's disease (HD) is an autosomal dominant disorder that affects the basal ganglia, caused by CAG repeats in the huntingtin gene. Delusional infestation (DI) is a rare psychotic manifestation of the disease. This report presents two cases of HD patients with DI, both middle-aged females. The first patient achieved remission of DI with olanzapine, later cross-tapered to risperidone, but had spontaneous relapses. The second experienced gradual resolution of DI with risperidone in the setting of iron repletion and amantadine discontinuation, although her other psychotic symptoms remained.

Delusional Infestation and Chronic Pruritus: A Review.

The literature on chronic pruritus, paresthesia and delusional infestation indicates that a wide variety of conditions ranging from AIDS to vitamin deficiencies may cause these symptoms. In many, or perhaps most of these cases, activation of itch pathways seems to be the underlying cause of the skin sensations and perhaps even the visual hallucinations characteristic of delusional infestation. The principle difference between diagnoses of chronic pruritus and delusional infestation appears to lie in the patient's interpretation of the cause of the symptoms, rather than underlying physiological

[Diagnosis and therapy of paranoid syndromes in the elderly].

Paranoid syndromes in the elderly are nosologically unspecific. Delusional disorder is characterized by the presence of delusions, typically persecutory, commonly in their homes. Frequent risk factors are discriminating, insulting or threatening life events, social isolation as well as sensory impairments. Atypical antipsychotics are considered as first line treatment. Delusion of parasitosis and erotomania are presented as subtypes of delusional disorders.

Psychogenic parasitosis. A case series and literature review.

About 100 years ago, psychogenic parasitosis was first described in the literature. This peculiar symptom may complicate medical and psychiatric conditions or indicate the presence of a delusional disorder (somatic type). By using the authors' case series of 12 patients, which are reported in the article, and a review of the literature, an historical perspective and the authors' clinical orientation to evaluation and treatment of psychogenic parasitosis are presented.

A combined tropical medicine and psychiatry approach to patients with possible delusional infestation.

BACKGROUND: Delusional infestation (DI) is a well-recognized delusional disorder presenting as the persisting belief of being infested. Combined clinics have been run by dermatology and psychiatry in a small number of centres. In this article we focus on our Liverpool University Hospitals NHS Foundation Trust clinic hosted at the Liverpool School of Tropical Medicine, UK, where we run a specialist clinic for DI. METHODS: We describe the specific set-up and approach of our clinic as a guide for clinicians working in specialties likely to see patients with DI (including tropical medicine, infec
