A comparison of the effects of three chelating agents on the root canals of extracted human teeth.

The effects of EDTA were compared in vitro with the effects of two other chelating agents: succimer and trientine HCl. Thirty extracted human teeth with single canals were used. Groups of 10 were treated by the three agents and then examined under the scanning electron microscope. The results showed that EDTA widened the dentinal tubules. Trientine HCl also produced widening of the dentinal tubules very similar to EDTA, whereas succimer produced more extensive widening.

In vitro determination and quantification of 30% hydrogen peroxide penetration through dentin and cementum during bleaching.

Bleaching pulpless teeth with 30% hydrogen peroxide was reported to cause external cervical root resorption. It was hypothesized that H2O2 penetrated open dentin tubules to initiate an inflammatory response that could result in root resorption. No direct experimental evidence was provided to support this theory. An in vitro model for determination and quantification of H2O2 penetration during bleaching procedures is suggested. Artificial defects of the cementum covering the cementoenamel junction were performed in 22 extracted human premolars. After 15 minutes of thermocatalytic bleaching, H2O

Tubular permeability to calcium hydroxide and to bleaching agents.

Recent reports of clinical cases suggest that cervical root resorption may follow bleaching of endodontically treated teeth. Inflammatory root resorption may be arrested by placement of calcium hydroxide into the root canal. The dentinal tubules are assumed to be a possible route of action for both agents. pH Values of the medium surrounding the tooth after placement of bleaching agents and calcium hydroxide within the root canals were determined in this study. Thirty extracted single-rooted human teeth were divided into three equal groups. The pulp cavity of the experimental teeth was filled

Effect of desensitizing agents on the hydraulic conductance of human dentin subjected to different surface pre-treatments--an in vitro study.

OBJECTIVES: The aim of this study was to evaluate the variation of fluid filtration across dentin treated with different formulations of potassium oxalate gels and one gel of acidified sodium fluorophosphate, under five different pre-conditioned dentin surfaces. METHODS: The methodology used for the measurement of hydraulic conductance of dentin in the present study was based on the model suggested by Pashley, Stewart and Galloway [Arch Oral Biol 29 (1984)379]. Two hundred dentin discs 1 mm in thickness, obtained from human extracted third molars, were divided into 20 groups of ten specimens

Low versus high pressure for in vitro determination of hydraulic conductance of human dentine.

The purpose of this study was to evaluate the effect of applied pressure and measurement time on the in vitro measurement of hydraulic conductance of human dentine. Dentine slices were prepared from 50 third molars. Water was forced through the slices under a constant hydrostatic pressure. Five pressures were tested: 1.3 kPa (n = 10), 13.3 kPa (n = 10), 26.6 kPa (n = 10), 40 kPa (n = 10) and 53.3 kPa (n = 10). The volume that went through the slices was recorded every 10 min for 3 h. The volume, the fluid flow rate and the hydraulic conductance under the five pressures were compared. The volum

A comparison between the occluding effects of the Nd:YAG laser and the desensitising agent sensodyne on permeation through exposed dentinal tubules of endodontically treated teeth: an in vitro study.

OBJECTIVE: Dentine hypersensitivity may occur due to loss of covering enamel and/or loss of cementum after gingival recession, resulting in exposure of cervical dentine and patency of dentinal tubules. The effect of thermally cooled pulsed neodymium:yttrium-aluminium-garnet (Nd:YAG) laser on the permeability and structural appearance of the root canal wall was investigated in vitro. The aim of this study was to compare the occluding effect of Nd:YAG laser with that of sensodyne on exposed dentine. Mechanism of action of a desensitizing fluoride toothpaste delivering calcium and phosphate ingredients in the treatment of dental hypersensitivity. Part II: comparison with a professional treatment for tooth hypersensitivity.

Tooth hypersensitivity can occur when gum recession causes exposure of dentin. Tiny tubules, which permeate dentin, provide open passageways from the mouth to the intradental nerve in the pulpal cavity. Under such circumstances, stimuli in the mouth can cause pressure on the intradental nerve, leading to pain. Sealing the outside of the tubules with an impermeable substance can effectively treat hypersensitivity. One such clinically proven composition is a professionally applied tooth desensitizer, which has been shown to initially produce a layer of amorphous calcium phosphate (ACP) on the su

Resin bonding to cervical sclerotic dentin: a review.

Several reports have indicated that resin bond strengths to noncarious sclerotic cervical dentine are lower than bonds made to normal dentine. This is thought to be due to tubule occlusion by mineral salts, preventing resin tag formation. The purpose of this review was to critically examine what is known about the structure of this type of dentine. Recent transmission electron microscopy revealed that in addition to occlusion of the tubules by mineral crystals, many parts of wedge-shaped cervical lesions contain a hypermineralised surface that resists the etching action of both self-etching pr

Intracoronal bleaching: concerns and considerations.

The history of intracoronal bleaching and the development of the walking bleach technique are reviewed. External cervical resorption associated with intracoronal bleaching is a serious sequela that has been reported in the literature. One explanation given for this phenomenon is that it results from an inflammatory process initiated by the presence of bleaching agents in the attachment apparatus. The basis for this conclusion is discussed. A review of published case reports on external cervical resorption associated with intracoronal bleaching revealed that: 100 per cent of the affected teeth

Pulp therapy in the primary dentition.

Several treatment options for pulp therapy in primary teeth are reviewed. Conservative treatments are recommended for primary teeth whose pulps have the potential to recover once the irritation has been removed. The role of dentin permeability and microleakage is emphasized when protective basis and indirect pulp treatment are discussed. Alternative dressings for formocresol pulpotomies such as 6.25% glutaraldehyde solution, antigen-extracted allogeneic dentin matrix, and crude bone morphogenetic protein are reported with promising results. Pulpectomy is recommended for teeth with evidence of

Effect of trichloracetic acid on the microhardness and surface morphology of human dentin and enamel.

Trichloracetic acid is recommended for the treatment of external cervical root resorption. The present study examined the effect of 90% trichloracetic acid on the microhardness and surface morphology of human dentin and enamel. Intact extracted human teeth were sectioned and embedded in acrylic resin. Each tooth was grinded and highly polished exposing a flat surface of dentin and enamel. The teeth were treated with 90% trichloracetic acid for 30, 60 and 90 s. Vicker's microhardness of the dentin and enamel was assessed for each tooth before and after each treatment. In addition the surface mo

Effect of biomimetic remineralization on the dynamic nanomechanical properties of dentin hybrid layers.

The mineral and organic phases of mineralized dentin contribute co-operatively to its strength and toughness. This study tested the null hypothesis that there is no difference in nano-dynamic mechanical behavior (complex modulus-E*; loss modulus-E''; storage modulus-E'; in GPa) of dentin hybrid layers (baseline: E*, 3.86 ± 0.24; E'', 0.23 ± 0.05; E', 3.85 ± 0.24) created by an etch-and-rinse adhesive in the presence or absence of biomimetic remineralization after in vitro aging. Using scanning probe microscopy and nano-dynamic mechanical analysis, we demonstrated that biomimetic remineralizati

How can nanoleakage occur in self-etching adhesive systems that demineralize and infiltrate simultaneously?

PURPOSE: Single-step adhesives which etch and prime simultaneously and are not rinsed should not exhibit areas of incomplete infiltration within hybrid layers produced in sound dentin. This study examined the extent of silver uptake using ammoniacal silver nitrate in three two-step, self-etching primers (Imperva Fluoro Bond, Shofu; UniFil Bond, GC, ABF system, Kuraray) and one single-step, self-etching adhesive (AQ Bond, Sun Medical) bonded to dentin and four poly(HEMA) resins used as controls. Pulpal response to adhesive resin systems applied to acid-etched vital dentin: damp versus dry primer application.

A number of studies have reported that acid etching of dentin is toxic to the cells of the odontoblastic layer and dental pulp. Other studies report that pulpal inflammation is a consequence of bacterial microleakage. The purpose of this study was to observe the degree of pulpal healing after pretreatment of vital dentin prior to placement of All-Bond and Scotch-bond 2 composite resin adhesives. Zinc oxide-eugenol cement and an acidic cement were employed as controls. One hundred twelve Class V nonexposed cavity preparations were placed throughout the dentitions of five healthy adult rhesus mo

Dentinal hypersensitivity: a review.

Dentinal hypersensitivity is generally reported by the patient after experiencing a sharp pain caused by one of several different stimuli. The pain response varies substantially from one person to another. The condition generally involves the facial surfaces of teeth near the cervical aspect and is very common in premolars and canines. The most widely accepted theory of how the pain occurs is Brannstrom's hydrodynamic theory, fluid movement within the dentinal tubules. The dental professional, using a variety of diagnostic techniques, will discern the condition from other conditions that may c
