Personalizing therapy in early gestational diabetes.

PURPOSE OF REVIEW: Early gestational diabetes mellitus (eGDM) is being increasingly detected on the background of rising obesity rates and widespread early pregnancy screening. However, diagnostic criteria and management strategies remain uncertain. This review summarizes current evidence on diagnostic thresholds, maternal and fetal outcomes, and the impact of early treatment, with an emphasis on balancing benefits and risks. RECENT FINDINGS: While observational studies link eGDM to adverse maternal and fetal outcomes, evidence from intervention studies remains limited. The Treatment of Booki

Epidemiology and management of gestational diabetes.

Gestational diabetes is defined as hyperglycaemia first detected during pregnancy at glucose concentrations that are less than those of overt diabetes. Around 14% of pregnancies globally are affected by gestational diabetes; its prevalence varies with differences in risk factors and approaches to screening and diagnosis; and it is increasing in parallel with obesity and type 2 diabetes. Gestational diabetes direct costs are US$1·6 billion in the USA alone, largely due to complications including hypertensive disorders, preterm delivery, and neonatal metabolic and respiratory consequences. Gestational Diabetes Mellitus Prevention: A Commentary.

Gestational diabetes mellitus (GDM) poses a significant global health challenge, with a prevalence ranging from 10-25%, making it one of the most prevalent complications of pregnancy. The rates vary across countries, ethnicities, and diagnostic thresholds. In the UK alone, GDM affects approximately 35,000 pregnancies annually, reflecting an escalating trend. Elevated blood glucose levels in pregnancy exhibit a linear correlation with adverse maternal and neonatal outcomes, contributing to complications such as large for gestational age babies, increased caesarean section rates, shoulder dystoc

Early Gestational Diabetes Mellitus: An Update.

Erythroderma is a dermatological emergency presenting with generalized erythema and scaling involving >90% of the body surface.1 There are various causes of erythroderma, such as psoriasis, atopic dermatitis, phytophotodermatitis, pityriasis rubra pilaris, pemphigus foliaceus, cutaneous T-cell lymphoma, and drug eruptions.2 However, erythroderma induced by dermatophytosis is encountered rarely. We, here, describe a case of erythroderma developed secondary to extensive dermatophytosis.

Early Gestational Diabetes Mellitus: An Update.

Hyperglycemia occurring in pregnancy is a growing burden worldwide. It is now standard of care to screen all women during pregnancy, both to detect preexisting diabetes as well as gestational diabetes mellitus (GDM). Traditionally, GDM was diagnosed at 24-28 weeks. However, with many international bodies recommending screening at first contact or booking, we are now diagnosing GDM earlier on in pregnancy. Based on the time of gestation at which it is diagnosed, GDM can be classified as conventional gestational diabetes mellitus (cGDM) or early gestational diabetes mellitus (eGDM). The cGDM is

Incretin Hypersecretion in Gestational Diabetes Mellitus.

CONTEXT: Incretins are crucial stimulators of insulin secretion following food intake. Data on incretin secretion and action during pregnancy are sparse. OBJECTIVE: The aim of the study was to investigate the incretin response during an oral glucose tolerance test (OGTT) in pregnant women with and without gestational diabetes mellitus (GDM). DESIGN: We analyzed data from the ongoing observational PREG study (NCT04270578). SETTING: The study was conducted at the University Hospital Tübingen. PARTICIPANTS: We examined 167 women (33 with GDM) during gestational week 27 ± 2.2. INTERVENTION: S

Pre-Gestational Diabetes and Pregnancy Outcomes.

INTRODUCTION: Pre-gestational, type 1 and type 2 diabetes are associated with adverse neonatal outcomes and increased rates of emergency caesarean sections. METHODS: We studied pregnancy outcomes associated with pre-gestational diabetes in 174 women who attended the National Maternity Hospital in Dublin, Ireland, between 2015 and 2017. RESULTS: Fifty women (28.6%) had type 2 diabetes mellitus, and 124 women (71.4%) had type 1 diabetes mellitus. Women with type 2 diabetes mellitus were older (36 vs. 34 years, p 0.02) and had a higher BMI (32.6 vs. 26.2 kg/m2, p 0.00). Duration of diabetes mel

Gestational diabetes mellitus screening and outcomes.

OBJECTIVE: To verify the usefulness of the World Health Organization criteria for the diagnosis of gestational diabetes mellitus in pregnant women and its effectiveness in the prevention of maternal and neonatal adverse results in women younger than 35 years without apparent risk factors for gestational diabetes mellitus. MATERIAL AND METHODS: This is a retrospective study based on population involving 1360 pregnant women who delivered and who were followed-up in a university hospital in Istanbul. All women underwent the 75-g oral glucose tolerance test screening, usually in between the 24(th

[Predisposition and phenotypes of gestational diabetes].

Gestational diabetes (GDM) is defined as glucose intolerance first diagnosed with a 75 gram oral glucose tolerance test based on IADPSG criteria which had been recently adopted by WHO. In industrial countries GDM is one of the most frequent pregnancy complications. In 2012, in Germany GDM had been diagnosed in 4,3 % of all births, overall 27,700 cases. GDM has to be considered as a preliminary stage of type 2 diabetes with insulin resistance and inadequate β-cell-compensation. Additionally, adverse metabolic profile, associations with inflammatory parameters, with D vitamin metabolism, and ins

Screening and diagnosing gestational diabetes mellitus.

BACKGROUND: There is uncertainty as to the optimal approach for screening and diagnosis of gestational diabetes mellitus (GDM). Based on systematic reviews published in 2003 and 2008, the U.S. Preventive Services Task Force concluded that there was insufficient evidence upon which to make a recommendation regarding routine screening of all pregnant women. OBJECTIVES: (1) Identify properties of screening tests for GDM, (2) evaluate benefits and harms of screening for GDM, (3) assess the effects of different screening and diagnostic thresholds on outcomes for mothers and their offspring, and (4

[Breastfeeding in women with gestational diabetes].

BACKGROUND AND OBJECTIVE: Children born to mothers with gestational diabetes are at an increased risk of developing obesity. Breastfeeding is acknowledged as beneficial for child development and it is suggested that breastfeeding protects against becoming obese. The aim of this study was to document breastfeeding habits of women with gestational diabetes and to identify factors that affect breastfeeding habits. METHODS: Breastfeeding habits (breastfeeding of any duration) were recorded of 257 mothers with gestational diabetes (mean age 31.4 +/- 4.8 years) who participated in a prospective pos

Periodontal pathogens and gestational diabetes mellitus.

In previous cross-sectional or case-control studies, clinical periodontal disease has been associated with gestational diabetes mellitus. To test the hypothesis that, in comparison with women who do not develop gestational diabetes mellitus, those who do develop it will have had a greater exposure to clinical and other periodontal parameters, we measured clinical, bacteriological (in plaque and cervico-vaginal samples), immunological, and inflammatory mediator parameters 7 weeks before the diagnosis of gestational diabetes mellitus in 265 predominantly Hispanic (83%) women in New York. Twenty-

WITHDRAWN: Dietary regulation for 'gestational diabetes'.

BACKGROUND: Impaired glucose metabolism in pregnancy may be associated with adverse pregnancy outcomes. Primary dietary therapy is used in the management of diabetes, including gestational diabetes. OBJECTIVES: The objective of this review was to assess the effects of primary dietary therapy in women identified as having gestational diabetes on fetal growth and neonatal outcomes. SEARCH STRATEGY: I searched the Cochrane Pregnancy and Childbirth Group trials register. SELECTION CRITERIA: Randomised trials of primary dietary therapy compared with no specific treatment in pregnant women with a

Neonatal morbidities in gestational diabetes mellitus.

The currently accepted definition of gestational diabetes mellitus (GDM) is rather broad. One might expect that fetal and neonatal complications that may occur in GDM pregnancy would be similar to those in pregestational diabetic pregnancy. Comparative evaluation of reported data on morbidity in GDM are often hampered by confounding variables (maternal age, parity, obesity) as well as the influence of factors such as ethnic origin, diagnostic criteria, and intervention during pregnancy. Recent observations indicate that GDM may be associated with increased incidence of fetal malformation and p

[Screening for gestational diabetes: who? How?].

Despite many international conferences, there is no consensus on gestational diabetes mellitus and many groups only screen those women who present risk factors or who present during pregnancy complications attributable to this condition. Nevertheless, complications secondary to gestational diabetes are equally frequent whether the mother presents risk factor or not, and these risk factors are only present in 30 to 45% of gestational diabetes. Therefore, all pregnant women should be screened ... or none at all. The type of screening test to be used is just as controversial. The test proposed by
