Dental caries: an infectious and transmissible disease where have we been and where are we going?

Dental caries is often referred to as an infectious disease, which means different things to different people. Pediatric dentists may be tempted to liken caries to other childhood diseases they encounter yet treat caries from mostly a mechanical or surgical approach. On the other end of the spectrum are clinicians who view the oral cavity as a site teaming with bacteria and think that reducing the overall microbial mass using antimicrobial agents will somehow affect this infectious disease. Here, we argue for another view, that is, to combine the most effective portion of each therapy to yield

Using Probabilistic Cellular Automata to Model the Transmission of an Emerging Infectious Disease of Amphibians-A Preliminary Model.

Emerging infectious diseases are major threats to amphibian biodiversity. Ranaviruses (Family Iridoviridae) are a group of globally emerging infections that infect poikilothermic vertebrates. In the United Kingdom, the emergence of this group of viruses has been followed by the decline in populations of common frogs (Rana temporaria) across the country, especially in the southeast of England. Simple susceptible-infected (SI) and combinatorial models have been developed to explain the spread of ranavirus in populations of common frogs; here, we use those models as a basis for developing a simpl

Relevance of social contact definitions for use in infectious disease transmission modeling: a systematic review and recommendations.

BACKGROUND: Social mixing studies provide crucial evidence for parameterizing mathematical models of infectious disease transmission, and their validity for use in models is dependent on their study design and how well the contacts they capture map to the transmission routes of the pathogen of interest. This systematic review aims to catalogue contact definitions used in social mixing studies from 2005 to 2024 and evaluate their conceptual alignment to three archetypal pathogens. METHODS: We searched Ovid Medline, Embase, Scopus, and Global Index Medicus for studies of human-to-human interacti

Time variations in the generation time of an infectious disease: implications for sampling to appropriately quantify transmission potential.

Although the generation time of an infectious disease plays a key role in estimating its transmission potential, the impact of the sampling time of generation times on the estimation procedure has yet to be clarified. The present study defines the period and cohort generation times, both of which are time-inhomogeneous, as a function of the infection time of secondary and primary cases, respectively. By means of analytical and numerical approaches, it is shown that the period generation time increases with calendar time, whereas the cohort generation time decreases as the incidence increases.

HIV/AIDS and other infectious diseases among correctional inmates: transmission, burden, and an appropriate response.

Correctional inmates engage in drug-related and sexual risk behaviors, and the transmission of HIV, hepatitis, and sexually transmitted diseases occurs in correctional facilities. However, there is uncertainty about the extent of transmission, and hyperbolic descriptions of its extent may further stigmatize inmates and elicit punitive responses. Whether infection was acquired within or outside correctional facilities, the prevalence of HIV and other infectious diseases is much higher among inmates than among those in the general community, and the burden of disease among inmates and releasees

Assessing parameter identifiability in compartmental dynamic models using a computational approach: application to infectious disease transmission models.

BACKGROUND: Mathematical modeling is now frequently used in outbreak investigations to understand underlying mechanisms of infectious disease dynamics, assess patterns in epidemiological data, and forecast the trajectory of epidemics. However, the successful application of mathematical models to guide public health interventions lies in the ability to reliably estimate model parameters and their corresponding uncertainty. Here, we present and illustrate a simple computational method for assessing parameter identifiability in compartmental epidemic models. METHODS: We describe a parametric boo

Rural revitalization, carbon emissions, and infectious disease transmission in China: nonlinear dynamics, spatial spillovers, and moderating effect.

Atmospheric pollution, particularly carbon dioxide (CO2) emissions driven by rapid economic development, poses significant threats to public health. In China, rural revitalization policies have transformed rural environments and socioeconomic structures, with implications for infectious disease transmission. This study examines the relationship between carbon emissions, rural revitalization, and infectious diseases using panel data from 31 provinces (2013-2023) within a spatial econometric framework. Results show that carbon emissions significantly increase disease transmission. Rural Revitali

How do I interpret transfusion transmissible infectious disease testing in a low-risk donor population?

BACKGROUND: Blood donors represent a unique population. Pre-donation screening questions, donor self-deferral, and temporary deferral and re-testing of repeat reactive donors result in lower prevalence of infectious disease compared to the general population. Prevalence directly affects the clinical performance of assay systems apart from the analytical sensitivity and specificity of the assay. STUDY DESIGN AND METHODS: It is important that blood operators understand what situations frequently elicit false reactive and false nonreactive cases, and how to mitigate the effect of these. RESULTS

Social contact patterns and implications for infectious disease transmission - a systematic review and meta-analysis of contact surveys.

BACKGROUND: Transmission of respiratory pathogens such as SARS-CoV-2 depends on patterns of contact and mixing across populations. Understanding this is crucial to predict pathogen spread and the effectiveness of control efforts. Most analyses of contact patterns to date have focused on high-income settings. METHODS: Here, we conduct a systematic review and individual-participant meta-analysis of surveys carried out in low- and middle-income countries and compare patterns of contact in these settings to surveys previously carried out in high-income countries. Using individual-level data from

Control of mother-to-child transmission of infectious diseases in Brazil: progress in HIV/AIDS and failure in congenital syphilis.

In Brazil, syphilis and HIV infection are considered serious public health problems. However, in practice, epidemiological surveillance, prevention measures, and prenatal care seem to be more effective in the control of mother-to-child transmission of the HIV than in the control of transmission of the Treponema pallidum. Here we discuss the differences in surveillance, prenatal care, and care of the newborn. Important differences were identified. It is concluded that there is an urgent need to establish prevention of mother-to-child transmission of syphilis as a public health priority, using a

American Academy of Pediatrics. Technical report: transmissible spongiform encephalopathies: A review for pediatricians. Committee on Infectious Diseases.

Transmissible spongiform encephalopathies (TSEs) are a family of rare, slowly progressive, and universally fatal neurodegenerative syndromes affecting animals and humans. Until recently, TSEs were of little interest to pediatricians. However, since the outbreak in adolescents and the association of TSEs with new-variant Creutzfeldt-Jakob disease (nvCJD), interest among pediatricians and the general public has increased. Even before bovine spongiform encephalopathy and nvCJD were linked, the recognition that iatrogenic Creutzfeldt-Jakob disease (CJD) had been acquired from administration of cad

Marburg Virus Disease in Rwanda: The Role of Infection Prevention and Control in Reducing Transmission of Infectious Disease Outbreaks among Healthcare Professionals.

Rwanda's first Marburg Virus Disease (MVD) outbreak resulted in 58 infections and 13 deaths within the first two weeks. Over 70% of cases occurred among healthcare professionals, highlighting the vulnerability of frontline workers and exposing critical gaps in the country's healthcare system, particularly in infection prevention and control (IPC) practices. Healthcare workers are essential to sustaining functional healthcare systems. However, they face a higher risk of infection and death at the onset of outbreaks, potentially due to lapses in IPC practices, thereby weakening the healthcare sy

Impact of population mixing between vaccinated and unvaccinated subpopulations on infectious disease dynamics: implications for SARS-CoV-2 transmission.

BACKGROUND: The speed of vaccine development has been a singular achievement during the COVID-19 pandemic, although uptake has not been universal. Vaccine opponents often frame their opposition in terms of the rights of the unvaccinated. We sought to explore the impact of mixing of vaccinated and unvaccinated populations on risk of SARS-CoV-2 infection among vaccinated people. METHODS: We constructed a simple susceptible-infectious-recovered compartmental model of a respiratory infectious disease with 2 connected subpopulations: people who were vaccinated and those who were unvaccinated. We s

Characterizing spatiotemporal spread of infectious diseases using ellipse-shaped transmission hotspots: application to dengue virus outbreaks.

BACKGROUND: Identifying transmission hotspots associated with micro-clustering patterns at the early stages of epidemics is helpful to characterize spatiotemporal spread of infectious diseases. However, standard methods with statistical validation to establish a dynamic warning system for emerging infectious diseases are lacking. We therefore aimed to integrate a geographic information system-based surveillance system and data-driven methods to identify transmission hotspots, thereby assisting decision-makers to implement appropriate policies at the early stages of epidemics.
