Linear intracerebral calcification after depth electrode implantation.

We report linear calcifications along the trajectories of previously implanted depth electrodes in 2 patients. A 20-year-old man and a 38-year-old woman with medically intractable complex partial seizures (CPS), underwent bilateral frontal and mesiotemporal depth electrode implantation as part of their epilepsy surgery workup. Brain computed tomography (CT) at that time was normal (except for cerebrellar atrophy in one case). One patient had a left anterotemporal lobectomy (ATL), and the other declined operation. Subsequent CT scans showed linear calcifications 1-2 cm long in the occipital lob

Use of RPNIs and Implanted Electrodes for Prosthetic Wrist and Multi-Grip Hand Control During Functional Tasks: A Case Study.

OBJECTIVE: Surface electromyography (sEMG) pattern recognition prosthetic systems are unintuitive and cannot control many movements reliably. Intramuscular electrodes and regenerative peripheral nerve interfaces (RPNIs) can improve signal quality and provide additional control signals for controlling more degrees of freedom. This case study compared functional, biomechanical, and cognitive outcomes between control approaches incorporating active wrist rotation using sEMG and implanted intramuscular EMG (imEMG) signals from RPNIs and residual muscles. We also explored whether combining the EMG

Impact of Frequency Mismatch and Modiolar Proximity on Speech Perception in Single-sided Deafness Patients Implanted With Perimodiolar Cochlear Electrodes.

INTRODUCTION: This study evaluated the impact of frequency mismatch and electrode proximity to the modiolus on speech perception in patients with single-sided deafness (SSD) implanted with perimodiolar electrodes (PMEs). We aimed to determine whether frequency mismatch or electrode positioning more strongly predicts speech outcomes, and whether these associations change over time. METHODS: A retrospective cohort of 34 SSD patients implanted at the Royal Victorian Eye and Ear Hospital between 2014 and 2022 was analyzed. Frequency mismatch was calculated using the Stakhovskaya spiral ganglion m

Displacement of the deep brain stimulation electrodes implanted in the anterior nucleus of the thalamus due to chronic subdural hematoma.

Anterior nucleus of the thalamus deep brain stimulation (ANT DBS) offers a treatment option for patients with drug-resistant epilepsy (DRE). This treatment modality is particularly useful in cases of multifocal DRE originating from the frontal and temporal lobes. Here we present a 36-year-old epileptic patient who underwent ANT DBS implantation. The incidence of seizures dropped significantly after surgery, though the patient still experienced occasional seizure episodes. After 9 months, the patient sustained a head injury due to bilateral tonic-clonic seizures (BTCS), leading to the developme

The Active Electrode in the Living Brain: The Response of the Brain Parenchyma to Chronically Implanted Deep Brain Stimulation Electrodes.

BACKGROUND: Deep brain stimulation is an established symptomatic surgical therapy for Parkinson disease, essential tremor, and a number of other movement and neuropsychiatric disorders. The well-established foreign body response around implanted electrodes is marked by gliosis, neuroinflammation, and neurodegeneration. However, how this response changes with the application of chronic stimulation is less well-understood. OBJECTIVE: To integrate the most recent evidence from basic science, patient, and postmortem studies on the effect of such an "active" electrode on the parenchyma of the livi

Electroacupuncture via chronically implanted electrodes improves gastric dysmotility mediated by autonomic-cholinergic mechanisms in a rodent model of functional dyspepsia.

BACKGROUND: Electroacupuncture (EA) has been shown to be effective in reducing symptoms in patients with functional dyspepsia (FD). However, its mechanisms remain largely unknown. The aim of this study was to investigate mechanisms of the prokinetic effects of EA in a rodent model of FD. METHODS: A FD model was established by neonatal treatment of iodoacetamide (IA). Eight weeks later, the rats were implanted with electrodes in the stomach for the measurement of gastric slow waves (GSW) and electrodes into acupoints ST36 for EA. Autonomic functions were assessed by the spectral analysis of he

Implanted High Density Cuff Electrodes Functionally Activate Human Tibial and Peroneal Motor Units Without Chronic Detriment to Peripheral Nerve Health.

OBJECTIVE: Peripheral nerve stimulation via multi-contact nerve cuff electrodes (NCEs) has proved effective in restoring function to individuals with lower-extremity paralysis. This study investigates clinical measures of nerve health over one year post-implantation of a composite flat-interface nerve electrode (C-FINE) on the tibial and peroneal nerves above the knee in a human volunteer. This represents the first deployment of a novel NCE on new neural targets in a uniquely challenging location prone to prolonged externally applied forces, making acute and chronic postoperative observation c

Non-invasive electrocologram: correlation between the electrical activity of the dog colonic muscle wall recorded by cutaneous and implanted electrodes.

Chronic experiments were performed on dogs with silver, bipolar, ball-shaped electrodes implanted on the colonic and of the gastric muscle wall. Cutaneous electrodes were placed on the abdominal wall, along the projection of descending colon and also of the stomach, at the beginning of each experiment. Slow waves were led off in the electrocolomyogram (EColMG), corresponding to low-amplitude waves in the non-invasive electrocologram (EColG) with a frequency of 6.39 +/- 0.58 cpm (type A). When bursts of spike potentials appeared with the slow waves in the EColMG, increase of the amplitude of th

Early Deformation of Deep Brain Stimulation Electrodes Following Surgical Implantation: Intracranial, Brain, and Electrode Mechanics.

INTRODUCTION: Although deep brain stimulation is nowadays performed worldwide, the biomechanical aspects of electrode implantation received little attention, mainly as physicians focused on the medical aspects, such as the optimal indication of the surgical procedure, the positive and adverse effects, and the long-term follow-up. We aimed to describe electrode deformations and brain shift immediately after implantation, as it may highlight our comprehension of intracranial and intracerebral mechanics. MATERIALS AND METHODS: Sixty electrodes of 30 patients suffering from severe symptoms of Par

The Brain is Not Flat: Conformal Electrode Arrays Diminish Complications of Subdural Electrode Implantation, A Series of 117 Cases.

BACKGROUND: Intracranial recordings are integral to evaluating patients with pharmacoresistant epilepsy whom noninvasive testing fails to localize seizure focus. Although stereo-electroencephalography is the preferred method of intracranial recordings in most centers, subdural electrode (SDE) implantation is necessary in selected cases. OBJECTIVE: To identify imaging correlates that predict SDE complications (extra-axial fluid collections [EFCs]), and determine if modifications that diminish stiffness of electrode sheets reduce complications. METHODS: A prospective epilepsy surgery database

Conduction studies in peripheral cat nerve using implanted electrodes: II. The effects of prolonged constriction on regeneration of crushed nerve fibers.

Arrays of chronically implanted electrodes were used to examine the time course of elongation and maturation of peripheral nerve fibers in the cat after crush of the tibial nerve in the proximal calf. Regeneration after crush alone was compared with crush 5 mm proximal to a tight constriction of the nerve. Regeneration was monitored by the progression of excitability along the electrode arrays on the tibial and plantar nerves. The sensitivity was sufficient to record the averaged activity in single nerve fibers allowing detection of the earliest regeneration. The diameters of the fastest regen

[Effects of afloqualone, a centrally acting muscle relaxant, on the sleep-wakefulness cycle in cats with chronically implanted electrodes (author's transl)].

The present study was carried out to elucidate whether or whether not afloqualone has a hypnotic action because of its similarity in chemical structure to methaqualone. In the sleep-wakefulness cycles during the 8-hour observation period (9:00-17:00), afloqualone increased the percentages of resting (REST) and slow wave light sleep (SWLS) stages at a dose of 25 mg/kg (p.o.), producing a moderate muscle relaxation. Even at a dose of 50 mg/kg (p.o.) where a marked muscle relaxation was produced, afloqualone had no influence on the percentage of slow wave deep sleep (SWDS) stage, though it increa

Effects of Electrode Position on Vestibular Implant Performance in Rhesus Macaque.

PURPOSE: Vestibular implants that target the three semicircular canal branches of the vestibular nerve can partially restore the 3-dimensional vestibulo-ocular reflex (3D VOR) in individuals disabled by bilateral vestibular hypofunction. A key goal of implant design is optimizing the number, spacing, and placement of stimulating and return electrodes to maximize response strength and selectivity. While computational models provide initial insights, empirical data are essential to validate performance. METHODS: We unilaterally implanted stimulating electrodes in each semicircular canal and pos

Determining ideal electrode insertion depth in paediatric cochlear implantation.

BACKGROUND: Speech and language outcomes following early cochlear implantation vary widely among patients. Identifying modifiable, device-specific surgical parameters contributing to this variability is essential to optimise therapeutic outcomes. OBJECTIVE: This study investigates the association between electrode angular depth of insertion (aDOI) and speech perception outcomes in a homogeneous cohort of children implanted with a single lateral wall electrode design. METHODS: A cohort of 25 prelingually deaf children, aged 4 to 5 years at implantation, received unilateral Oticon Neuro ZTI co

Frequency-to-Place Mismatch and Cochlear Implant Outcomes by Electrode Type.

IMPORTANCE: Speech recognition outcomes with a cochlear implant (CI) are highly variable. One factor suggested to correlate with CI-aided speech recognition is frequency-to-place mismatch, or the discrepancy between the natural tonotopic organization of the cochlea and the electric frequency allocation of the CI electrodes within the patient's cochlea. OBJECTIVE: To evaluate the association between frequency-to-place mismatch and speech recognition outcomes in a large cohort of postlingually deafened adult CI users, while controlling for various clinical factors known to be associated with th

Electrocochleography Latency: Correlation With Electrode Position During Cochlear Implantation.

OBJECTIVES: Cochlear implant (CI) candidates increasingly exhibit some degree of residual hearing, which should be preserved despite the implantation. Today, cochlear health is monitored during CI surgery by tracking the cochlear microphonic (CM) amplitude from intracochlear electrocochleography (ECochG) measurements. However, recent studies indicate that the insertion depth of the measuring electrode must be considered to accurately interpret these signals. The acoustic path from the cochlear base to the apex induces excitation delays in deeper regions, which should be reflected in the CM mea

Development and Characterization of a Micromagnetic Alternative to Cochlear Implant Electrode Arrays.

To stimulate the auditory nerve, cochlear implants directly inject electrical current into surrounding tissue via an implanted electrode array. While many cochlear implant users achieve strong speech perception scores, there remains significant variability. Since cochlear implant electrode arrays are surrounded by a conductive fluid, perilymph, a spread of excitation occurs. The functionality of the cochlea is spatially dependent, and a wider area of excitation negatively affects the hearing of the user. Importantly, magnetic fields are unaffected by the material properties of biological compo

Shift in electrocorticography electrode locations after surgical implantation in children.

Interpreting electrocorticography (ECoG) in the context of neuroimaging requires that multimodal information be integrated accurately. However, the implantation of ECoG electrodes can shift the brain impacting the spatial interpretation of electrode locations in the context of pre-implant imaging. We characterized the amount of shift in ECoG electrode locations immediately after implant in a pediatric population. Electrode-shift was quantified as the difference in the electrode locations immediately after surgery (via post-operation CT) compared to the brain surface before the operation (pre-i
